Adrenergic receptor antagonism induces neuroprotection and facilitates recovery from acute ischemic stroke.

Project description:Spontaneous waves of cortical spreading depolarization (CSD) are induced in the setting of acute focal ischemia. CSD is linked to a sharp increase of extracellular K+ that induces a long-lasting suppression of neural activity. Furthermore, CSD induces secondary irreversible damage in the ischemic brain, suggesting that K+ homeostasis might constitute a therapeutic strategy in ischemic stroke. Here we report that adrenergic receptor (AdR) antagonism accelerates normalization of extracellular K+, resulting in faster recovery of neural activity after photothrombotic stroke. Remarkably, systemic adrenergic blockade before or after stroke facilitated functional motor recovery and reduced infarct volume, paralleling the preservation of the water channel aquaporin-4 in astrocytes. Our observations suggest that AdR blockers promote cerebrospinal fluid exchange and rapid extracellular K+ clearance, representing a potent potential intervention for acute stroke.

MiRNA-seq in blood of myasthenia gravis (MG) patients responder and non-responder to immunosuppressive treatment

Project description:Purpose: to identify specific miRNAs as biomarkers associated with immunosuppressive treatment response in MG patients. Methods: Blood miRNome profiles of 15 responder and 15 non-responder MG patients by miRNA-seq using Illumina NextSeq 500 sequencing. Raw data were de-multiplexed and FASTQ files for each sample were generated using the bcl2fastq software. FASTQ data were checked using the FastQC tool. After mapping the data and counting to relevant entries in miRBase 20 (http://www.mirbase.org/), the numbers of known microRNAs was calculated.For statistical analysis, along with TPM normalization, the trimmed mean of M-values (TMM) normalization method was used. Validation was perfomed using TaqMan Advanced real-time PCR on microRNA array cards. Results: An average of 9.98 million reads was obtained per sample. We identified 41 statistically significant differentially expressed miRNAs (logFoldChange from +1.371 to -1.439, p < 0.05), including 18 miRNAs up-regulated and 23 miRNAs down-regulated in non-responder versus responder MG patients. 23 miRNAs were selected for validation. Among them, three miRNAs were significantly decreased in non-responder versus responder patients, whereas two showed an opposite trend. Conclusions: We identified a blood miRNA signature associated with drug responsiveness in MG patients, that could represent a valuable tool for therapeutic follow-up in MG.

Investigational therapies for ischemic stroke: neuroprotection and neurorecovery.

Project description:Stroke is one of the leading causes of death and disability worldwide. Current treatment strategies for ischemic stroke primarily focus on reducing the size of ischemic damage and rescuing dying cells early after occurrence. To date, intravenous recombinant tissue plasminogen activator is the only United States Food and Drug Administration approved therapy for acute ischemic stroke, but its use is limited by a narrow therapeutic window. The pathophysiology of stroke is complex and it involves excitotoxicity mechanisms, inflammatory pathways, oxidative damage, ionic imbalances, apoptosis, angiogenesis, neuroprotection, and neurorestoration. Regeneration of the brain after damage is still active days and even weeks after a stroke occurs, which might provide a second window for treatment. A huge number of neuroprotective agents have been designed to interrupt the ischemic cascade, but therapeutic trials of these agents have yet to show consistent benefit, despite successful preceding animal studies. Several agents of great promise are currently in the middle to late stages of the clinical trial setting and may emerge in routine practice in the near future. In this review, we highlight select pharmacologic and cell-based therapies that are currently in the clinical trial stage for stroke.
