Project description:To assess the function of peripheral blood derived monocytes in patently infected filaria patients (either Wuchereria bancrofti, Mansonella perstans, or both), we investigated the cytokine production and gene expression profile of these cells in filaria infected patients and compared them with those from uninfected normal blood bank donors. Monocytes from infected individuals were studded with intracellular microfilarial antigens. In addition, Staphylococcus aureus Cowan I bacteria (SAC)/IFN γ activated monocytes from the infected individuals produced less IL 8, Exodus II, MIP 1a, MIP 1b, eotaxin, RANTES, IL 1a, and MIP 3b compared with normal patients. Microarray analysis of ex vivo isolated monocytes demonstrated that multiple genes involved in signal transduction, apoptosis, and adhesion were expressed to a greater degree in filaria infected patient monocytes compared with those of uninfected normal individuals. Eight months following treatment with a single dose of ivermectin/albendazole, monocytes of W. bancrofti infected individuals were capable of producing more IL 8, IL 1a, MIP 1a, and IL 10 in response to SAC/IFN γ compared with their pretreatment levels. Further, when monocyte global expression was compared before and after treatment of the W. bancrofti, there was a marked increase in mRNA expression of genes associated with protein metabolism, and in HSP most particularly, compared with pretreatment expression. These data suggest that the function and gene expression of monocytes in filaria infected patients are altered; however, this dysfunction can be reveresed to a degree following treatment with a single dose of ivermection/albendazole.

Project description:Lymphatic filariasis (LF) threatens nearly 20% of the world’s population and has handicapped one-third of the 120 million people currently infected. Current control and eventual elimination of LF rely on mass drug administration (MDA) programs with three drugs: ivermectin (IVM), albendazole (ALB), and diethylcarbamazine (DEC). Only the mechanism of action of albendazole is well-understood. . To gain a better insight into antifilarial drug action, we treated gerbils with patent B. malayi infections with 6 mg/kg DEC, 1 mg/kg ALB, or 0.15 mg/kg IVM to mirror the doses used in human MDA programs. These treatments had no effect on the numbers of worms present in the peritoneal cavity of infected animals. Adults and Mf were collected 1 and 7 days post-treatment and RNA was isolated for transcriptomic analysis. The experiment was repeated three times. Preliminary analysis of the effects of IVM, ALB, and DEC on adult males and females revealed treatment-specific changes in transcripts related to collagen expression, reproduction, embryogenesis, larval development, and lifespan determination. Similar data are currently being collected for the microfilariae.

Project description:Soil-transmitted helminths and Mycobacterium tuberculosis frequently coincide geographically and it is hypothesized that gastrointestinal helminth infection may exacerbate tuberculosis (TB) disease by suppression of Th1 and Th17 responses. However, few studies have focused on latent TB (LTBI) infection, which predominates globally. We performed a large observational study of healthy adults migrating from Nepal to the UK (n=645). Individuals were screened for LTBI and gastrointestinal parasite (GIP) infections. A significant negative association between hookworm (HW) and LTBI-positivity was seen (OR=0.221; p=0.039). HW treatment did not effect LTBI conversions. Blood from individuals with HW had a significantly greater ability to control virulent mycobacterial growth in vitro than from those without, which was lost following HW treatment. There was a significant negative relationship between mycobacterial growth and eosinophil counts. We also analysed differential gene expression among individuals with and without HW infection, pre- and post- anti-helminth treatment. Eosinophil-associated differential gene expression characterised the whole blood transcriptome of hookworm infection and correlated with improved mycobacterial control. These data provide a potential alternative explanation for the reduced prevalence of LTBI among individuals with HW infection, and possibly an anti-mycobacterial role for helminth-induced eosinophils.

Project description:The study objective was to propose molecular mechanisms of action of the histone deacetylase inhibitor vorinostat. In the PRAVO phase 1 study, patients that were scheduled to receive pelvic palliative radiation to 30 Gy in 3-Gy fractions for gastrointestinal carcinoma, were enrolled onto four sequential dose levels of vorinostat, starting at 100 mg daily with dose escalation in increments of 100 mg. Endpoints included treatment safety and tolerability, tumor response, and biological activity of vorinostat. For the purpose of identifying biomarkers of vorinostat action, peripheral blood mononuclear cells, representing normal tissue exposed to vorinostat, were used. The samples were collected, one at baseline and two on-treatment samples. The time points for sample collection were chosen based on our previous data from experimental colorectal carcinoma models exposed to vorinostat, demonstrating that the maximum tumor histone acetylation 2-3 hours after drug exposure was restored to baseline after 24 hours. In PRAVO study patients, tumor histone hyperacetylation was observed 3 hours after vorinostat administration. From the 17 patients enrolled onto the PRAVO study, a full set of three samples was obtained from 14 individuals: one baseline sample collected prior to commencement of vorinostat treatment (T0), and two on-treatment samples collected 2 and 24 hours after the patient had received the preceding daily dose of vorinostat (T2 and T24). Individual vorinostat dose levels were 100 mg (D100), 200 mg (D200), 300 mg (D300), or 400 mg (D400).

Project description:Combining venetoclax, a selective BCL-2 inhibitor, with low-dose navitoclax, a BCL-XL/BCL-2 inhibitor, may potentiate therapeutic BCL-2 and BCL XL inhibition without dose-limiting thrombocytopenia associated with navitoclax monotherapy. The safety and preliminary efficacy of venetoclax with low-dose navitoclax and chemotherapy was assessed in this phase 1 dose escalation study (NCT03181126) in pediatric and adult patients with relapsed/refractory acute lymphoblastic leukemia or lymphoblastic lymphoma. Forty-seven patients received treatment. A recommended phase 2 dose of 50 mg navitoclax for adults and 25 mg for patients <45 kg with 400 mg adult-equivalent venetoclax was identified. Delayed hematopoietic recovery was the primary safety finding. The complete remission rate was 60%, including responses in patients who had previously received hematopoietic cell transplantation or immunotherapy. Thirteen (28%) patients proceeded to transplantation or CAR T-cell therapy on study. Venetoclax with navitoclax and chemotherapy was well tolerated and had promising efficacy in this heavily pretreated patient population.

Plasma samples were drawn from 9 male Sinclair swine at baseline, post-trauma, and 0.25, 2, and 4 hourspost resuscitation with 500 mL of hydroxyethyl starch. Swine were randomized to receive either saline, low-dose VitC (50 mg/kg), or high-dose VitC (200 mg/kg) during volume resuscitation (N = 3/treatment). High performance LC-MS/MS proteomics was then used to quantitatively measure plasma protein fluctations over time as a function of treatment.
