ABSTRACT: Background: If the majority of antibody mediated rejections (AMRs) of the renal allograft are due to anti-HLA antibodies, non anti-HLA antibodies have also been suspected. The occurrence of non anti-HLA-associated AMRs remains associated with unresolved diagnostic and therapeutic issues. Methods: To better understand the pathomechanisms of these rejections, we identified, though a nation-wide study, kidney transplant recipients (KTRs), without anti-HLA donor specific antibodies, experiencing acute graft dysfunction within the first 3 months after transplantation and severe microvascular injury on biopsy (called acute microvascular rejections, AMVR).
