Association of Maternal Plasma Manganese with the Risk of Spontaneous Preterm Birth: A Nested Case-Control Study Based on the Beijing Birth Cohort Study (BBCS) in China.

ABSTRACT: We performed this study to clarify the dynamic changes in maternal manganese (Mn) concentration during pregnancy and its association with spontaneous preterm birth (SPB). A nested case-control study was conducted based on the Beijing Birth Cohort Study (BBCS) from 2018 to 2020. Singleton pregnancy women aged 18-44 (n = 488) were involved in the study, including 244 cases of SPB and 244 controls. All of the participants provided blood samples twice (in their first and third trimesters). Inductively coupled plasma mass spectrometry (ICP-MS) was used for the laboratory analysis, and unconditional logistic regression was used for the statistical analysis. We found that the maternal Mn levels were significantly higher in the third trimester than those in the first trimester (median: 1.23 vs. 0.81 ng/mL). The SPB risk was increased to 1.65 (95% CI: 1.04-2.62, p = 0.035) in the highest Mn level (third tertile) in the third trimester, especially in normal-weight women (OR: 2.07, 95% CI: 1.18-3.61, p = 0.011) or non-premature rupture of membrane (PROM) women (OR: 3.93, 95% CI: 2.00-7.74, p < 0.001). Moreover, a dose-dependent relationship exists between the SPB risk and maternal Mn concentration in non-PROM women (P trend < 0.001). In conclusion, dynamic monitoring of maternal Mn level during pregnancy would be helpful for SPB prevention, especially in normal-weight and non-PROM women.

Project description:BACKGROUND: Spontaneous preterm birth (SPB, before 37 gestational weeks) is a major cause of perinatal mortality and morbidity, but its pathogenesis remains unclear. Studies on SPB have been hampered by the limited availability of markers for SPB in predelivery clinical samples that can be easily compared with gestational age-matched normal controls. We hypothesize that SPB involves aberrant placental RNA expression, and that such RNA transcripts can be detected in predelivery maternal plasma samples, which can be compared with gestational age-matched controls. PRINCIPAL FINDINGS: Using gene expression microarray to profile essentially all human genes, we observed that 426 probe signals were changed by >2.9-fold in the SPB placentas, compared with the spontaneous term birth (STB) placentas. Among the genes represented by those probes, we observed an over-representation of functions in RNA stabilization, extracellular matrix binding, and acute inflammatory response. Using RT-quantitative PCR, we observed differences in the RNA concentrations of certain genes only between the SPB and STB placentas, but not between the STB and term elective cesarean delivery placentas. Notably, 36 RNA transcripts were observed at placental microarray signals higher than a threshold, which indicated the possibility of their detection in maternal plasma. Among them, the IL1RL1 mRNA was tested in plasma samples taken from 37 women. It was detected in 6 of 10 (60%) plasma samples collected during the presentation of preterm labor (?32.9 weeks) in women eventually giving SPB, but was detected in only 1 of 27 (4%) samples collected during matched gestational weeks from women with no preterm labor (Fisher exact test, p?=?0.00056). CONCLUSION: We have identified 36 SPB-associated RNA transcripts, which are possibly detectable in maternal plasma. We have illustrated that the IL1RL1 mRNA was more frequently detected in predelivery maternal plasma samples collected from women resulting in SPB than the gestational-age matched controls.

Project description:BackgroundThe relationship between prenatal phthalate exposure and preterm birth from previous studies has been inconsistent. Meanwhile, few studies have explored the relationship between spontaneous preterm birth (SPTB) and genetic polymorphisms of metabolic enzyme genes or gene-phthalate interactions. The aim of this study is to evaluate the association of maternal phthalate exposure, genetic polymorphisms, and their interactions with SPTB.MethodsA total of 182 cases with SPTB and 321 controls with full-term delivery were enrolled. Nine phthalate metabolites in maternal second trimester urine samples were measured by ultra-high performance liquid chromatography coupled with tandem mass spectrometry. Genotyping was performed on twenty-six single nucleotide polymorphisms (SNPs) of metabolic enzyme genes, including CYP2C9, CYP2C19, UGT1A7, UGT2B7 and UGT2B15 genes. The associations between maternal phthalate exposure or genetic polymorphisms and SPTB were estimated by multivariable logistic regression analysis. The impact of interactions between gene-gene and gene-phthalate exposure on SPTB were analyzed via generalized multifactor dimensionality reduction.ResultsThere were no significant differences in the concentrations of phthalate metabolites between the two groups. No statistically significant associations were observed between maternal phthalate exposure and SPTB. The rs4244285 polymorphism of CYP2C19 gene was associated with decreased odds of SPTB under the log-additive (aOR = 0.73, 95% CI: 0.55-0.98) and recessive model (aOR = 0.37, 95% CI: 0.18-0.74). Two SNP loci of UGT2B15 were associated with increased odds of SPTB under the recessive genetic model (aOR = 3.85, 95% CI: 1.31-11.35 for rs3100, and aOR = 3.85, 95% CI: 1.31-11.35 for rs4148269). However, these associations were not significant after the false discovery rate correction. No significant gene-gene or gene-phthalate metabolites interactions for SPTB were observed.ConclusionsMaternal phthalate exposure in the present subjects and genetic polymorphisms of metabolic enzyme genes were not associated with SPTB. Moreover, there were no significant gene-gene or gene-phthalates interactions for SPTB.

Project description:The newborn's microbiota composition at birth seems to be influenced by maternal microbiota. Maternal vaginal microbiota can be a determining factor of spontaneous Preterm Birth (SPPTB), the leading cause of perinatal mortality. The aim of the study is to investigate the likelihood of a causal relationship between the maternal vaginal microbiota composition and neonatal lung and intestinal microbiota profile at birth, in cases of SPPTB. The association between the lung and/or meconium microbiota with the subsequent development of bronchopulmonary dysplasia (BPD) was also investigated. Maternal vaginal swabs, newborns' bronchoalveolar lavage fluid (BALF) (1st, 3rd, 7th day of life) and first meconium samples were collected from 20 women and 23 preterm newborns with gestational age ≤ 30 weeks (12 = SPPTB; 11 = Medically Indicated Preterm Birth-MIPTB). All the samples were analyzed for culture examination and for microbiota profiling using metagenomic analysis based on the Next Generation Sequencing (NGS) technique of the bacterial 16S rRNA gene amplicons. No significant differences in alpha e beta diversity were found between the neonatal BALF samples of SPPTB group and the MIPTB group. The vaginal microbiota of mothers with SPPTB showed a significant difference in alpha diversity with a decrease in Lactobacillus and an increase in Proteobacteria abundance. No association was found between BALF and meconium microbiota with the development of BPD. Vaginal colonization by Ureaplasma bacteria was associated with increased risk of both SPPTB and newborns' BPD occurrence. In conclusion, an increase in α-diversity values and a consequent fall in Lactobacillus in vaginal environment could be associated to a higher risk of SPPTB. We could identify neither a specific neonatal lung or meconium microbiota profiles in preterm infants born by SPPTB nor a microbiota at birth suggestive of subsequent BPD development. Although a strict match has not been revealed between microbiota of SPPTB mother-infant couples, a relationship cannot be excluded. To figure out the reciprocal influence of the maternal-neonatal microbiota and its potential role in the pathogenesis of SPPTB and BPD further research is needed.
