Patient Engagement Activities and Patient Experience: Are Patients With a History of Depression the Canary in the Coal Mine?

ABSTRACT: Little is known about the effectiveness of primary care practices' efforts to engage patients in their health and health care. We examine the association between patient engagement efforts and patients' experiences of care. We found no association between an unweighted count of patient engagement activities and patient experience. Compared with the bottom quartile of practices, however, the top quartile had better performance on patient experience domains of communication, front-office staff, and organizational access (out of nine domains). Furthermore, patients reporting a diagnosis of depression have higher ratings across five domains of patient experience when in practices with higher levels of patient engagement activities measured using an unweighted scale. Future research is needed to understand how the benefits of patient engagement activities can accrue to more patient subgroups. These promising results suggest that payers and policy makers should continue to support implementation and benchmarking of patient engagement efforts across practices.

Project description:This assessment was designed to explore and characterize the airborne particles, especially for the sub-micrometer sizes, in an underground coal mine. Airborne particles present in the breathing zone were evaluated by using both (1) direct reading real-time instruments (RTIs) to measure real-time particle number concentrations in the workplaces and (2) gravimetric samplers to collect airborne particles to obtain mass concentrations and conduct further characterizations. Airborne coal mine particles were collected via three samplers: inhalable particle sampler (37 mm cassette with polyvinyl chloride (PVC) filter), respirable dust cyclone (10 mm nylon cyclone with 37 mm Zefon cassette and PVC filter), and a Tsai diffusion sampler (TDS). The TDS, a newly designed sampler, is for collecting particles in the nanometer and respirable size range with a polycarbonate filter and grid. The morphology and compositions of collected particles on the filters were characterized using electron microscopy (EM). RTIs reading showed that the belt entry had a greatly nine-times higher total particle number concentration in average (~ 34,700 particles/cm3) than those measured at both the underground entry and office building (~ 4630 particles/cm3). The belt entry exhibited not only the highest total particle number concentration, but it also had different particle size fractions, particularly in the submicron and smaller sizes. A high level of submicron and nanoparticles was found in the belt conveyor drift area (with concentrations ranging from 0.54 to 1.55 mg/m3 among three samplers). The data support that small particles less than 300 nm are present in the underground coal mine associated with dust generated from practical mining activities. The chemical composition of the air particles has been detected in the presence of Ca, Cu, Si, Al, Fe, and Co which were all found to be harmful to miners when inhaled.Supplementary informationThe online version contains supplementary material available at 10.1007/s42461-024-01140-w.

Project description:IntroductionDespite increasing evidence that Environmental Health Monitoring (EHM) is an effective method to perform rodent colony health surveillance, promotes the 3Rs as a Replacement, is comparable or reduces cost, and demonstrates labor benefits, many research institutions continue to use live Soiled Bedding Sentinels (SBS). Some veterinarians and others responsible for rodent colony health monitoring remain cautious of the evidence supporting this new technology. Therefore, our objective was to perform a formal systematic review to identify, summarize, and evaluate the research on the efficacy of EHM as compared to SBS. This information is essential to establishing recommendations for rodent health monitoring programs.MethodsWe systematically searched and evaluated all articles comparing EHM to SBS identified from PubMed, Web of Science, and CAB abstracts initially on November 12, 2021 with an update on Oct 15, 2023. Our inclusion criteria included publication in a peer-reviewed journal and collection of empirical data on rodent health monitoring alternatives to soiled bedding sentinels. Outcome data was extracted and analyzed via logistic regression and secondary descriptive statistics. Due to the nature of the included studies, no risk of bias assessment was performed. No specific funding was received for this review.ResultsForty-two peer-reviewed articles passed inclusion criteria for analysis. The design of studies varied substantially but included publications investigating exhaust dust testing (n = 27), sentinel-free soiled bedding (n = 8), and direct colony sampling (n = 24). Based on both logistical regression and descriptive criteria, all types of EHM appear to be superior to soiled bedding sentinels at detecting pathogens, regardless of their taxonomic classification.ConclusionBased on these findings, we conclude there is a strong evidence base supporting the replacement of SBS with EHM. Looking forward, we encourage continued research on the detection of existing and emerging pathogens with these technologies and development of best practices for routine rodent health monitoring programs.

Project description:Plain english summaryThe Alberta Depression Research Priority Setting Project aimed to meaningfully involve patients, families and clinicians in determining a research agenda aligned to the needs of Albertans who have experienced depression. The project was modeled after a process developed in the UK by the James Lind Alliance and adapted to fit the Alberta, Canada context. This study describes the processes used to ensure the voices of people with lived experience of depression were integrated throughout the project stages. The year long project culminated with a facilitated session to identify the top essential areas of depression research focus. People with lived experience were engaged as part of the project's Steering Committee, as survey participants and as workshop participants. It is hoped this process will guide future priority setting opportunities and advance depression research in Alberta.AbstractBackground The Depression Research Priority Setting (DRPS) project has the clear aim of describing the patient engagement process used to identify depression research priorities and to reflect on the successes of this engagement approach, positive impacts and opportunities for improvement. To help support patient-oriented depression research priority setting in Alberta, the Patient Engagement (PE) Platform of the Alberta Strategy for Patient Oriented Research Support for People and Patient-Oriented Research and Trials (SUPPORT) Unit designed, along with the support of their partners in addictions and mental health, an explit process to engage patients in the design and execution of the DRPS. Methods The UK's James Lind Alliance (JLA) Priority Setting Partnership (PSP) method was adapted into a six step process to ensure voices of "people with lived experience" (PWLE) with depression were included throughout the project stages. This study uses an explicit and parallel patient engagement process throughout each estage of the PSP designed by the PE Platform. Patient engagement was divided into a five step process: i) Awareness and relationship building; ii) Co-designing and co-developing a shared decision making process; iii) Collaborative communication; iv) Collective sensemaking; and v) Acknowledgement, celebration and recognition. A formative evaluation of the six PE processes was undertaken to explore the success of the parallel patient engagement process. Results This project was successful in engaging people with lived depression experience as partners in research priority setting, incorporating their voices into the discussions and decisions that led to the top 25 depression research questions. Conclusions The DRPS project has positively contributed to depression research in Canada by identifying the priorities of Albertans who have experienced depression for depression research. Dissemination activities to promote further knowledge exchange of prioritized research questions, with emphasis on the importance of process in engaging the voices of PWLE of depression are planned.
