DCF versus doublet chemotherapy as first-line treatment of advanced squamous anal cell carcinoma: a multicenter propensity score-matching study.

ABSTRACT: Triplet DCF (docetaxel, cisplatin and 5-flurouracil) and doublet CP/CF (carboplatin and paclitaxel/cisplatin and 5-fluorouracil) regimens were prospectively evaluated in advanced squamous anal cell carcinoma (SCCA), and validated as standard treatments. Even though the high efficacy and good tolerance of DCF regimen were confirmed in 3 independent prospective trials, doublet CP regimen is still recommended in several guidelines based in its better safety profile with similar efficacy compared to CF regimen. We performed a propensity score-adjusted method with inverse probability of treatment weighted (IPTW) and matched case control (MCC) comparison among patients with metastatic or non-resectable locally advanced recurrent SCCA, treated with chemotherapy as first line regimen. The primary endpoint was the overall survival (OS), and the secondary endpoint was the progression-free survival (PFS). 247 patients were included for analysis. 154 patients received DCF and 93 patients received a doublet regimen. The median OS was 32.3 months with DCF and 18.3 months with doublet regimens (HR 0.53, 95%CI 0.38-0.74; p = 0.0001), and the median PFS was 11.2 months with DCF versus 7.6 months with doublet regimens (HR 0.53, 95%CI 0.39-0.73; p < 0.0001). The hazard ratios by IPTW and MCC analyses were 0.411 (95% CI, 0.324-0.521; p < 0.0001) and 0.406 (95% CI, 0.261-0.632; p < 0.0001) for OS, and 0.466 (95% CI, 0.376-0.576; p < 0.0001) and 0.438 (95% CI, 0.298-0.644; P < 0.0001) for PFS. The triplet DCF regimen provides a high and significant benefit in OS and PFS over doublet regimens, and should be considered as upfront treatment for eligible patients with advanced SCCA.

Project description:BackgroundObstetric anal sphincter injuries are a notable adverse outcome of vaginal deliveries, with incidence rates ranging from 0.25% to 6%. Key risk factors for these injuries include primiparity and operative vaginal deliveries. In recent decades, the use of episiotomy as a preventive measure for obstetric anal sphincter injuries has been subject to extensive scrutiny. The objective of this study was to investigate the role of mediolateral episiotomy in preventing obstetric anal sphincter injuries during vaginal deliveries or vacuum-assisted deliveries within the context of selective use.MethodsRetrospective study of all nulliparous women with a singleton gestation who underwent term vaginal delivery or vacuum-assisted delivery. A propensity score matching with a 1:1 ratio was employed to adjust for confounders. Primary outcome was obstetric anal sphincter injury rate, and the secondary outcome was a composite neonatal outcome (umbilical artery pH < 7.1, neonatal intensive care unit admission, and 5-min Apgar score < 7).ResultsThe study cohort comprised 22,738 deliveries: 77.0% vaginal deliveries (17,518/22,738) and 23.0% vacuum-assisted deliveries (5,220/22,738) with episiotomy rates of 38.5% (6,751/17,518) and 75.8% (3,959/5,220), respectively. Separate analyses were conducted for the matched vaginal delivery group (n = 13,500) and the matched vacuum-assisted delivery group (n = 2,518). No significant differences in obstetric anal sphincter injury rates were observed in the vaginal delivery (OR 1.15, 95% CI 0.78-1.69) or vacuum-assisted delivery (OR 0.58, 95% CI 0.31-1.05) groups. However, in the vacuum-assisted delivery group, episiotomy demonstrated a borderline protective effect against obstetric anal sphincter injuries. Further adjustment for the duration of the second stage of labor duration in vacuum-assisted deliveries revealed a significant protective effect of episiotomy against obstetric anal sphincter injuries, with a number needed to treat of 28 (95% CI 15-224) in cases of prolonged second stages of labor. Episiotomy in vaginal deliveries was associated with a higher rate of composite neonatal outcome (OR 1.23, 95% CI 1.04-1.45), while vacuum-assisted deliveries showed no such association (OR 1.11, 95% CI 0.86-1.44).ConclusionsIn context of selective use, mediolateral episiotomy has no significant protective effect on obstetric anal sphincter injuries in women undergoing vaginal delivery. However, there is a possible benefit in vacuum-assisted delivery, especially when considering the duration of the second stage of labor.

Project description:Background and objectiveAn adjusted indirect comparison was conducted to assess efficacy outcomes, particularly overall survival (OS), of osimertinib versus platinum-based doublet chemotherapy in patients with epidermal growth factor receptor-mutated (EGFRm) T790M mutation-positive non-small-cell lung cancer (NSCLC) who had progressed following an EGFR tyrosine kinase inhibitor (TKI). Analysis of treatment effect from two separate trials had the potential to more accurately estimate the magnitude of OS benefit due to absence of confounding due to treatment switching from the control arm to the osimertinib arm of the ongoing randomized control trial, AURA3.MethodsTwo non-randomized individual datasets were compared: pooled patients from the AURA extension and AURA2 trials (osimertinib 80 mg, n = 405, with a confirmed T790M mutation using tissue samples), and patients from the control arm of the IMPRESS study (platinum-based doublet chemotherapy, n = 61, with a confirmed T790M mutation using plasma circulating tumour DNA [ctDNA]). A propensity score-based approach was used to account for differences in baseline demographics and disease characteristics.ResultsAfter adjustment for baseline differences between the two groups, osimertinib demonstrated a statistically significant improvement in progression-free survival (PFS) versus platinum-based doublet chemotherapy (hazard ratio [HR] = 0.278, 95% confidence interval [CI] 0.188-0.409, p < 0.0001; median PFS 10.9 vs. 5.3 months). Improvements were also observed for objective response rate (ORR) and disease control rate (DCR) (ORR: 64.3 vs. 33.3%; odds ratio [OR] = 5.31, 95% CI 2.47-11.40, p < 0.001; DCR: 92.1 vs. 75.0%; OR = 4.72, 95% CI 1.92-11.58, p < 0.001). Similar results were obtained for patients who received osimertinib as second-line treatment only. A statistically significant improvement in OS was observed for the osimertinib group (HR = 0.412, 95% CI 0.273-0.622, p < 0.0001). Median OS for osimertinib was not reached.ConclusionsIn this indirect comparison, osimertinib showed a statistically significant improvement in efficacy outcomes versus platinum-based doublet chemotherapy in patients with EGFRm T790M NSCLC who had progressed after EGFR-TKI therapy.

Project description:AimTo compare the efficacy and safety of radiotherapy in combination with immunotherapy after achieving disease control from the first-line combination therapy of platinum-based chemotherapy and immunotherapy for advanced lung squamous cell carcinoma (LUSC).MethodsThis study retrospectively evaluated the patients with advanced LUSC treated with the combination of radiotherapy with immunotherapy and chemotherapy (ICRT group, n = 52) or immunotherapy and chemotherapy (ICT group, n = 63) as the first-line treatment from April 2018 to April 2022. Using propensity score matching (PSM), 50 pairs were created, while the confounders and bias were controlled. The objective response rate (ORR), duration of overall response (DOR), progression-free survival (PFS), overall survival (OS), and adverse events were analyzed in the two groups. The PFS and OS were re-analyzed separately for patients treated with thoracic radiotherapy.ResultsAfter PSM, the median PFS (12.23 vs. 7.43 months; P <0.001) and median OS (19.7 vs. 12.9 months; P <0.001) were significantly longer in the ICRT group than those in the ICT group. Both the PFS and OS rates were also significantly higher in the ICRT group than those in the ICT group, except for the OS rates in the 6th and 12th months. The mDOR of the ICRT group patients (17.10 vs. 8.27 months; P <0.001) was significantly higher than that of the ICT group patients. The median PFS, median OS, and local control rate were significantly longer in the thoracic radiotherapy group than in the control group. Radiation pneumonia was the most common adverse effect after radiotherapy; however, no treatment-related deaths occurred. The Cox regression analysis showed that ECOG scores 0-1, presence of necrosis in the tumor, radiotherapy, and optimal efficacy better than the stable disease (SD) were independent factors, affecting the PFS, while the patients with recurrent post-operative, pre-treatment NLR, radiotherapy, and optimal efficacy better than SD were the independent factors, affecting the OS.ConclusionsThe combination of radiotherapy with systematic immunotherapy and chemotherapy for the advanced LUSC was effective with tolerable adverse effects.
