Virtual immediate feedback with POCUS in Belize.

ABSTRACT: Point of care ultrasound (POCUS) is a portable and accessible tool that has immense potential in low- and middle-income countries (LMIC) for diagnostic accuracy and medical education. We implemented a hybrid in-person and virtual training curriculum to teach providers in Belize the basic techniques of lung ultrasound in the diagnosis of pneumonia. Between August 2021 and June 2022, a total of eleven lung scans were performed at Hillside Clinic for patients presenting with respiratory complaints. Deidentified images were shared via the ButterflyIQ web platform to POCUS experts in the United States. We found that training was solidified through virtual, immediate feedback using the common interfaces Butterfly iQ + and WhatsApp to share images and guide diagnostic reasoning. The aim of this review study is to share our experience and challenges in the implementation of a POCUS training curriculum in an LMIC, provide an example of training methodology that can be effective, and discuss how this can be implemented and modified for clinicians in similar settings.

Project description:It is well known that some survey respondents reduce the effort they invest in answering questions by taking mental shortcuts - survey satisficing. This is a concern because such shortcuts can reduce the quality of responses and, potentially, the accuracy of survey estimates. This article explores "speeding," an extreme type of satisficing, which we define as answering so quickly that respondents could not have given much, if any, thought to their answers. To reduce speeding among online respondents we implemented an interactive prompting technique. When respondents answered faster than a minimal response time threshold, they received a message encouraging them to answer carefully and take their time. Across six web survey experiments, this prompting technique reduced speeding on subsequent questions compared to a no prompt control. Prompting slowed response times whether the speeding that triggered the prompt occurred early or late in the questionnaire, in the first or later waves of a longitudinal survey, among respondents recruited from non-probability or probability panels, or whether the prompt was delivered on only the first or on all speeding episodes. In addition to reducing speeding, the prompts increased response accuracy on simple arithmetic questions for a key subgroup. Prompting also reduced later straightlining in one experiment, suggesting the benefits may generalize to other types of mental shortcuts. Although the prompting could have annoyed respondents, it was not accompanied by a noticeable increase in breakoffs. As an alternative technique, respondents in one experiment were asked to explicitly commit to responding carefully. This global approach complemented the more local, interactive prompting technique on several measures. Taken together, these results suggest that interactive interventions of this sort may be useful for increasing respondents' conscientiousness in online questionnaires, even though these questionnaires are self-administered.

Project description:IntroductionDental anxiety is common and causes symptomatic use of oral health services.ObjectivesThe aim was to study if a short-term virtual reality intervention reduced preoperative dental anxiety.MethodsA randomized controlled single-center trial was conducted with 2 parallel arms in a public oral health care unit: virtual reality relaxation (VRR) and treatment as usual (TAU). The VRR group received a 1- to 3.5-min 360° immersion video of a peaceful virtual landscape with audio features and sound supporting the experience. TAU groups remained seated for 3 min. Of the powered sample of 280 participants, 255 consented and had complete data. Total and secondary sex-specific mixed effects linear regression models were completed for posttest dental anxiety (Modified Dental Anxiety Scale [MDAS] total score) and its 2 factors (anticipatory and treatment-related dental anxiety) adjusted for baseline (pretest) MDAS total and factor scores and age, taking into account the effect of blocking.ResultsTotal and anticipatory dental anxiety decreased more in the VRR group than the TAU group (β = -0.75, P < .001, for MDAS total score; β = -0.43, P < .001, for anticipatory anxiety score) in patients of a primary dental care clinic. In women, dental anxiety decreased more in VRR than TAU for total MDAS score (β = -1.08, P < .001) and treatment-related dental anxiety (β = -0.597, P = .011). Anticipatory dental anxiety decreased more in VRR than TAU in both men (β = -0.217, P < .026) and women (β = -0.498, P < .001).ConclusionShort application of VRR is both feasible and effective to reduce preoperative dental anxiety in public dental care settings (ClinicalTrials.gov NCT03993080).Knowledge transfer statementDental anxiety, which is a common problem, can be reduced with short application of virtual reality relaxation applied preoperatively in the waiting room. Findings of this study indicate that it is a feasible and effective procedure to help patients with dental anxiety in normal public dental care settings.
