Revisiting the theoretical and methodological foundations of depression measurement.

ABSTRACT: Depressive disorders are among the leading causes of global disease burden, but there has been limited progress in understanding the causes and treatments for these disorders. In this Perspective, we suggest that such progress crucially depends on our ability to measure depression. We review the many problems with depression measurement, including limited evidence of validity and reliability. These issues raise grave concerns about common uses of depression measures, such as diagnosis or tracking treatment progress. We argue that shortcomings arise because depression measurement rests on shaky methodological and theoretical foundations. Moving forward, we need to break with the field's tradition that has, for decades, divorced theories about depression from how we measure it. Instead, we suggest that epistemic iteration, an iterative exchange between theory and measurement, provides a crucial avenue for depression measurement to progress.

Narrative Review of the Theoretical-Methodological Foundations of the TREINI Program.

Project description:Scientific knowledge has advanced in the implementation of safe and beneficial interventions for children and adolescents with cerebral palsy (CP). Although the importance of interdisciplinary interventions that integrate all components of the International Classification of Functioning, Disability and Health (ICF) into family-centered practices is widely recognized, this approach is not yet widely adopted. Instead, many programs remain focused on isolated domains. This study presents the theoretical and methodological foundation of TREINI, an interdisciplinary and family-centered program developed for children and youth with CP and other neurodevelopmental disorders. TREINI incorporates intervention strategies that address all ICF domains. It is grounded in the biopsychosocial model of health and utilizes principles based on the best evidence in pediatric rehabilitation, including intensive training, task-oriented training, and a naturalistic learning environment. Unlike traditional rehabilitation approaches, the care provided by the TREINI program is delivered through an intensive and interdisciplinary approach, by a team working collaboratively in a single location. In addition to including evidence-based interventions, the TREINI program features two innovative components: the "City of Tomorrow", a naturalistic learning environment, and the "TREINI Exoflex" therapeutic suit, specifically designed to address deficiencies in the body functions and structures of children with CP and other neurodevelopmental disorders. This program has been carefully designed to support the process of neurological re-education and rehabilitation for children and adolescents with neuropsychomotor developmental delays.

Navigating the maze of active ageing measurement: untangling methodological and theoretical issues in the UJACAS questionnaire.

Project description:Active Ageing (AA) involves maintaining engagement in physical, social, and mental activities in older adulthood, reflecting societal values and political agendas. Despite various measurement tools, there is no universal standard. The University of Jyväskylä Active Ageing Scale (UJACAS) is increasingly used in Europe but faces inconsistencies from varying response scales across versions. This perspective paper examines these discrepancies and proposes a refined model for AA measurement, emphasizing the dimensions of motivation, ability, opportunity, and activity frequency. The goal is to improve the accuracy of AA assessments by adjusting for personal and environmental limitations, ensuring a fair evaluation that captures the complexities of ageing. The proposed adjustments to the UJACAS scoring model include recalibrating the equation to account for personal and environmental limitations, integrating motivation as a separate measure, and exploring adaptive methodologies to ensure fairer and more nuanced assessments of Active Ageing, particularly for individuals facing disabilities or obstacles. By addressing these issues, this perspective paper aims to enhance the effectiveness of AA tools for older adults. Moreover, adopting these methodological challenges has significant societal implications. Precise measurement of AA is essential for tailoring interventions to the needs of diverse older populations and for designing policies that promote inclusive, healthy and safe ageing. Enhancing the accuracy of AA assessments can inform strategies that empower older adults, mitigate disparities, and foster resilience in ageing communities. This study highlights the necessity of advancing AA measurement frameworks to support evidence-based decisions in public health and social policy.

Documenting and defining emergent phenomenology: theoretical foundations for an extensive research strategy.

Project description:Meditation, psychedelics, and other similar practices or induction methods that can modulate conscious experience, are becoming increasingly popular in clinical and non-clinical settings. The phenomenology associated with such practices or modalities is vast. Many similar effects and experiences are also reported to occur spontaneously. We argue that this experiential range is still not fully described or understood in the contemporary literature, and that there is an ethical mandate to research it more extensively, starting with comprehensive documentation and definition. We review 50 recent clinical or scientific publications to assess the range of phenomena, experiences, effects, after-effects, and impacts associated with a broad variety of psychoactive compounds, meditative practices, and other modalities or events. This results in a large inventory synthesizing the reports of over 30,000 individual subjects. We then critically discuss various terms and concepts that have been used in recent literature to designate all or parts of the range this inventory covers. We make the case that specialized terminologies are needed to ground the nascent research field that is forming around this experiential domain. As a step in this direction, we propose the notion of "emergence" and some of its derivatives, such as "emergent phenomenology," as possibly foundational candidates.

A methodological review of resilience measurement scales.

Project description:BackgroundThe evaluation of interventions and policies designed to promote resilience, and research to understand the determinants and associations, require reliable and valid measures to ensure data quality. This paper systematically reviews the psychometric rigour of resilience measurement scales developed for use in general and clinical populations.MethodsEight electronic abstract databases and the internet were searched and reference lists of all identified papers were hand searched. The focus was to identify peer reviewed journal articles where resilience was a key focus and/or is assessed. Two authors independently extracted data and performed a quality assessment of the scale psychometric properties.ResultsNineteen resilience measures were reviewed; four of these were refinements of the original measure. All the measures had some missing information regarding the psychometric properties. Overall, the Connor-Davidson Resilience Scale, the Resilience Scale for Adults and the Brief Resilience Scale received the best psychometric ratings. The conceptual and theoretical adequacy of a number of the scales was questionable.ConclusionWe found no current 'gold standard' amongst 15 measures of resilience. A number of the scales are in the early stages of development, and all require further validation work. Given increasing interest in resilience from major international funders, key policy makers and practice, researchers are urged to report relevant validation statistics when using the measures.

Exploring the theoretical foundations of visual art programmes for people living with dementia.

Project description:Despite the growing international innovations for visual arts interventions in dementia care, limited attention has been paid to their theoretical basis. In response, this paper explores how and why visual art interventions in dementia care influence changes in outcomes. The theory building process consists of a realist review of primary research on visual art programmes. This aims to uncover what works, for whom, how, why and in what circumstances. We undertook a qualitative exploration of stakeholder perspectives of art programmes, and then synthesised these two pieces of work alongside broader theory to produce a conceptual framework for intervention development, further research and practice. This suggests effective programmes are realised through essential attributes of two key conditions (provocative and stimulating aesthetic experience; dynamic and responsive artistic practice). These conditions are important for cognitive, social and individual responses, leading to benefits for people with early to more advanced dementia. This work represents a starting point at identifying theories of change for arts interventions, and for further research to critically examine, refine and strengthen the evidence base for the arts in dementia care. Understanding the theoretical basis of interventions is important for service development, evaluation and implementation.

Cumulative culture in the laboratory: methodological and theoretical challenges.

Project description:In the last decade, cultural transmission experiments (transmission chains, replacement, closed groups and seeded groups) have become important experimental tools in investigating cultural evolution. However, these methods face important challenges, especially regarding the operationalization of theoretical claims. In this review, we focus on the study of cumulative cultural evolution, the process by which traditions are gradually modified and, for technological traditions in particular, improved upon over time. We identify several mismatches between theoretical definitions of cumulative culture and their implementation in cultural transmission experiments. We argue that observed performance increase can be the result of participants learning faster in a group context rather than effectively leading to a cumulative effect. We also show that in laboratory experiments, participants are asked to complete quite simple tasks, which can undermine the evidential value of the diagnostic criterion traditionally used for cumulative culture (i.e. that cumulative culture is a process that produces solutions that no single individual could have invented on their own). We show that the use of unidimensional metrics of cumulativeness drastically curtail the variation that may be observed, which raises specific issues in the interpretation of the experimental evidence. We suggest several solutions to these mismatches (learning times, task complexity and variation) and develop the use of design spaces in experimentally investigating old and new questions about cumulative culture.

Decision theoretical foundations of clinical practice guidelines: an extension of the ASH thrombophilia guidelines.

Project description:AbstractDecision analysis can play an essential role in informing practice guidelines. The American Society of Hematology (ASH) thrombophilia guidelines have made a significant step forward in demonstrating how decision modeling integrated within Grading of Recommendations Assessment, Developing, and Evaluation (GRADE) methodology can advance the field of guideline development. Although the ASH model was transparent and understandable, it does, however, suffer from certain limitations that may have generated potentially wrong recommendations. That is, the panel considered 2 models separately: after 3 to 6 months of index venous thromboembolism (VTE), the panel compared thrombophilia testing (A) vs discontinuing anticoagulants (B) and testing (A) vs recommending indefinite anticoagulation to all patients (C), instead of considering all relevant options simultaneously (A vs B vs C). Our study aimed to avoid what we refer to as the omitted choice bias by integrating 2 ASH models into a single unifying threshold decision model. We analyzed 6 ASH panel's recommendations related to the testing for thrombophilia in settings of "provoked" vs "unprovoked" VTE and low vs high bleeding risk (total 12 recommendations). Our model disagreed with the ASH guideline panels' recommendations in 4 of the 12 recommendations we considered. Considering all 3 options simultaneously, our model provided results that would have produced sounder recommendations for patient care. By revisiting the ASH guidelines methodology, we have not only improved the recommendations for thrombophilia but also provided a method that can be easily applied to other clinical problems and promises to improve the current guidelines' methodology.

Utilitarian redundancy in local medical systems - theoretical and methodological contributions.

Project description:The utilitarian redundancy model (URM) is one of the recent contributions to ethnobiology. We argue that URM can be applied to access use-pressure on plant species, the resilience of socioecological systems (e.g., local medical systems), cultural keystone species, and the role of exotic species in social-ecological systems. Based on previous URM studies, we also emphasize the need to differ practical (considering plants and uses that are currently employed) and theoretical (considering both currently employed and potentially employed plants and uses) redundancy. Based on the main applications of the URM, we propose a new index to access redundancy of a therapeutic indication: the Uredit, so that Uredit = NSp + CR, were Uredit is the Utilitarian Redundancy Index for the therapeutic indication; NSp is the total number of species mentioned for the indication, and CR is the species' contribution to redundancy (in terms of knowledge sharing). The maximum value that the Uredit could reach is twice the number of species employed for the therapeutic indication. We believe that this theoretical and methodological improvement in the model can improve comparisons of redundancy in different social-ecological systems. We also highlight some limitations of the URM (and our Uredit), and we believe that conscious reasons behind people's decisions should be incorporated into future studies on the subject.

An academic survey on theoretical foundations, common assumptions and the current state of consciousness science.

Project description:We report the results of an academic survey into the theoretical and methodological foundations, common assumptions, and the current state of the field of consciousness research. The survey consisted of 22 questions and was distributed on two different occasions of the annual meeting of the Association of the Scientific Study of Consciousness (2018 and 2019). We examined responses from 166 consciousness researchers with different backgrounds (e.g. philosophy, neuroscience, psychology, and computer science) and at various stages of their careers (e.g. junior/senior faculty and graduate/undergraduate students). The results reveal that there remains considerable discussion and debate between the surveyed researchers about the definition of consciousness and the way it should be studied. To highlight a few observations, a majority of respondents believe that machines could have consciousness, that consciousness is a gradual phenomenon in the animal kingdom, and that unconscious processing is extensive, encompassing both low-level and high-level cognitive functions. Further, we show which theories of consciousness are currently considered most promising by respondents and how supposedly different theories cluster together, which dependent measures are considered best to index the presence or absence of consciousness, and which neural measures are thought to be the most likely signatures of consciousness. These findings provide us with a snapshot of the current views of researchers in the field and may therefore help prioritize research and theoretical approaches to foster progress.

The ANEMONE: Theoretical Foundations for UX Evaluation of Action and Intention Recognition in Human-Robot Interaction.

Project description:The coexistence of robots and humans in shared physical and social spaces is expected to increase. A key enabler of high-quality interaction is a mutual understanding of each other's actions and intentions. In this paper, we motivate and present a systematic user experience (UX) evaluation framework of action and intention recognition between humans and robots from a UX perspective, because there is an identified lack of this kind of evaluation methodology. The evaluation framework is packaged into a methodological approach called ANEMONE (action and intention recognition in human robot interaction). ANEMONE has its foundation in cultural-historical activity theory (AT) as the theoretical lens, the seven stages of action model, and user experience (UX) evaluation methodology, which together are useful in motivating and framing the work presented in this paper. The proposed methodological approach of ANEMONE provides guidance on how to measure, assess, and evaluate the mutual recognition of actions and intentions between humans and robots for investigators of UX evaluation. The paper ends with a discussion, addresses future work, and some concluding remarks.
