Project description:Various degree of remnant tissues exists following anterior cruciate ligament (ACL) injury. Making use of these tissues may be helpful for the reconstructed ACL from many aspects. There are many methods of remnant preservation and use, as well as many types of combined ACL reconstruction. However, the most effective methods of remnant reuse as well as ACL reconstruction are still being pursued. We introduce an anatomical double-bundle transtibial ACL reconstruction with in-between remnant preservation technique named sandwich-style ACL reconstruction. The indication of this technique is complete ACL tear with a large volume of connectable remnant. The main tricks of this technique are proper ligation of the remnant, proper location of the anteromedial-bundle tibial tunnels, and passing the anteromedial bundle through the shallow side of the remnant. We believe the introduction of this technique will provide more options for remnant preservation and ACL reconstruction.

Project description:We describe the all-anterior approach for arthroscopic posterior cruciate ligament (PCL) reconstruction with remnant preservation. In this technique, PCL is reconstructed through 3 anterior portals: anterolateral portal, anteromedial portal, and inferior anterolateral portal, whereas the PCL remnant is preserved with great possibility. With the PCL elevator and wire catcher (Smith & Nephew), a self-made wire loop is introduced into the joint through the tibial tunnel, and is used to pass the graft in an all-inside-out fashion. In the transtibial PCL reconstruction technique, posterior portals are routinely adopted and the PCL remnant is often removed for good vision and adequate operative space, which can prolong surgery time and increase the potential risk of iatrogenic injury. In comparison, this technique not only simplifies the surgical procedure and reduces surgical trauma, but also preserves the remnant for our patients.

Project description:The awareness of anterior cruciate ligament (ACL) injuries and their treatment is increasing among athletes and within the general population, so patients report early to orthopaedic surgeons. Because we encounter a thick ACL stump during arthroscopic ACL reconstruction, an attempt is made to preserve this stump. Remnant preservation-although promising in terms of graft healing because it enhances cell proliferation, revascularization, and regeneration of the proprioceptive organs in the reconstructed ACL-does not come without complications, such as cyclops lesions, impingement, and extension loss. These problems can be detrimental to an athlete's return to his or her preinjury level after ACL reconstruction. Therefore, we describe a technique in which the ACL is reconstructed using hamstring autograft and suture passes are made through the remnant with an antegrade suture-passing device. After anatomic tunnel drilling, the remnant is tensioned, along with the reconstructed ACL, at the femoral end without the use of an extra implant. Femoral-side fixation is achieved with an adjustable-loop button (Procinch; Stryker) and a bio-composite interference screw (Biosure Regenesorb; Smith & Nephew) on the tibial end. As the sutured remnant is tensioned and knots are made over the button, the remnant envelopes the graft in the orientation of the native ACL, which can be helpful for early and better ligamentization of the graft. The potential advantages of this technique are as follows: orientation of the remnant along the collagen of the ACL graft; no loose ACL stump in the notch, thus preventing cyclops lesions; retention of the proprioceptive organs in the ligament; and no extra implant.

Project description:Anterior cruciate ligament (ACL) reconstruction with preservation of either the remnant or the tibial stump is performed with the hope of improving the vascularization and proprioceptive function of the graft. Remnant preservation is technically difficult because it hinders the visualization of the intra-articular tunnel site. Taking a cue from the concept of tibial stump preservation, we have modified our ACL reconstruction technique to preserve a sleeve of the soft tissue and ACL stump attached to the femoral condyle, in addition to tibial stump preservation, while still allowing adequate visualization of the femoral ACL insertion site. We describe our modification in this article and hypothesize that this should further improve graft vascularization and ligamentization.

Project description:This report describes a modified anatomic single-bundle anterior cruciate ligament (ACL) reconstruction technique using the FlipCutter guide pin (Arthrex, Naples, FL) as a retrograde drill and a cortical suspensory fixation device (TightRope; Arthrex) with an adjustable graft loop length. Preservation of the ACL remnant as a biological sleeve for the graft is an important issue from the viewpoints of acceleration of revascularization and ligamentization, preservation of the proprioceptive nerve fibers, enhancement of the biological environment for healing, and maintenance of the anchor point at the native tibial attachment, in addition to yielding a lower incidence of tibial bone tunnel enlargement. The goal of our technique is to obtain some advantages of the remnant-preserving technique through an anatomic single-bundle ACL reconstruction, which is performed to minimize damage to the ACL tibial remnant.

Project description:The number of anterior cruciate ligament (ACL) ruptures treated arthroscopically has increased. In most cases, the rupture of the ligament occurs in the femoral attachment, leaving a robust stump attached to the tibia. ACL reconstruction with stump preservation might have some advantages: rapid reinnervation and revascularization of the graft. Most techniques have used an interference screw in tibial fixation, which could be more difficult to achieve the graft diameter within 8 to 10 mm in most situations. Therefore, we describe a technique for modified "all-inside" ACL reconstruction with remnant preservation using an appendiceal tibial tunnel to optimize the graft size. After tunnel drilling in anatomic positions, the ACL stump is bored centrally up to the diameter of the graft to be passed. After passing the graft from the tibial tunnel to the remnant to the femoral tunnel, adjustable loops are used to fix the femoral side and the tibial side by an appendiceal tibial tunnel.

Project description:In anterior cruciate ligament (ACL) reconstruction, the remnant ACL is often debrided in an effort to improve visualization of patient anatomy and reduce surgical difficulty. However, retention of the tibial remnant of a torn ACL theoretically portends several advantages. These include preservation of knee proprioception and mechanoreceptors, an enhanced biological environment for healing, and accelerated revascularization and ligamentization of the graft. Despite these advantages, it is not a widely adopted practice and the optimal technique is still subject to debate. The purpose of this technical note is to describe our preferred technique for performing hybrid ACL reconstruction with remnant incorporation.

To improve the clinical outcomes of PCL reconstruction, preservation of the remnant has long been a consideration. However, how to make the remnant-preservation technique simple and more effective is of concern. We describe a single-bundle anatomical PCL reconstruction technique with remnant preservation in which the posteromedial and posterolateral portals are used, the graft is placed at the lateral side of the remnant, and pulleys are used to facilitate graft passage at the 2 killer turns of the grafting routes. We consider introduction of this technique will provide reasonable choices in PCL reconstruction.

Project description:The clinical results of posterior cruciate ligament reconstruction (PCL) still leave much room for improvement. To get better knee stability, the double-bundle reconstruction, the use of strong grafts, and the preservation of the remnant have been applied. However, the combination of the 3 measures has seldom been reported. Thus I introduce a 4-tunnel double-bundle PCL reconstruction technique, in which I combine the use of strong grafts and preservation of the remnant. The critical point of this technique is manipulating skillfully across the femoral notch with the preservation, as well as the hindrance to operation of the PCL remnant. My clinical experience indicates this technique is somewhat complicated but most effective. I believe the introduction of this technique will provide an additional choice when PCL reconstruction is to be performed.

Project description:Anterior cruciate ligament (ACL) rupture remains a debilitating orthopaedic pathology with a substantial economic and psychological burden on patients, especially athletes. The purpose of ACL reconstruction is to attain maximum joint stability and functionality, allowing patients to resume their previous level of activity. Several graft options and techniques are available for ACL reconstruction. The all-inside remnant-preservation technique is a minimally invasive approach aiming for improved proprioception, better graft integration, and increased graft strength via ACL augmentation by suture approximation with an optimal anatomic reconstruction. ACL augmentation is associated with a decreased risk of rerupture. Moreover, enhancement of knee proprioception via the presented technique allows an early return to activity by patients because weight bearing (with a brace) can be initiated as early as day 1 postoperatively. Patients can resume running activities by 2 months postoperatively and return to pivot sports by 3 months postoperatively. Despite this surgical procedure being technically demanding, it is associated with improved clinical outcomes and functional capacities. Patients are also found to better tolerate the postoperative rehabilitation protocol.
