Project description:Foreign body reaction (FBR), initiated by adherence of macrophages to biomaterials, is associated with several complications. Searching for mechanisms potentially useful to overcome these complications, we have established the signaling role of macrophages in the development of FBR. This study profiles gene expression of in vitro fibrinogen activated macrophages as well as that of freshly isolated macrophages from 3-days implants, against a background of unactivated macrophages/monocytes. Total RNA was isolated from fibrinogen activated macrophages, FBR macrophages and macrophages from blood (control). Samples were processed in triplicate on Rat Genome 230 2.0 Array GeneChips, adding up to 9 arrays in total. Microarray processing was performed at DNAVision (Charleroi, Belgium). It is often difficult to detect and identify vaginal foreign bodies in pediatric cases without any symptoms. Therefore, detection of vaginal foreign bodies using noninvasive methods, like transabdominal ultrasonography, is important in pediatric patients.

In Vivo Disintegration and Bioresorption of a Nacre-Inspired Graphene-Silk Film Caused by the Foreign-Body Reaction.

Project description:Graphene-based substrates are emerging as a promising functional platform for biomedical applications. Although dispersible graphene sheets have been demonstrated to be biodegradable, their assembled macroscopic architectures are biopersistent because of strong π-π interactions. In this study, we developed a nacre-inspired graphene-silk nanocomposite film by vacuum filtration with a subsequent green chemical reduction procedure. The "brick-and-mortar" architecture not only ensures the mechanical and electrical properties of the film but also endows it with disintegrable and bioresorbable properties following rat subcutaneous implantation. Furthermore, covalent cross-linking leads to the formation of graphene with decreased interlayer spacing, which effectively prolongs the residence time in vivo. We found that enzymatic treatment created microcracks on the film surface and that the foreign-body reaction was involved in the deformation, delamination, disintegration, and phagocytosis processes of the nanocomposite films. This bioinspired strategy paves the way for the development of high-performance graphene-based macroscopic biomaterials with tunable bioresorbability.

Bio-inspired poly-DL-serine materials resist the foreign-body response.

Project description:Implantation-caused foreign-body response (FBR) is a commonly encountered issue and can result in failure of implants. The high L-serine content in low immunogenic silk sericin, and the high D-serine content as a neurotransmitter together inspire us to prepare poly-DL-serine (PSer) materials in mitigating the FBR. Here we report highly water soluble, biocompatible and easily accessible PSer hydrogels that cause negligible inflammatory response after subcutaneous implantation in mice for 1 week and 2 weeks. No obvious collagen capsulation is found surrounding the PSer hydrogels after 4 weeks, 3 months and 7 months post implantation. Histological analysis on inflammatory cytokines and RNA-seq assay both indicate that PSer hydrogels show low FBR, comparable to the Mock group. The anti-FBR performance of PSer hydrogels at all time points surpass the poly(ethyleneglycol) hydrogels that is widely utilized as bio-inert materials, implying the potent and wide application of PSer materials in implantable biomaterials and biomedical devices.

Role of Macrophages in the Foreign Body Response to Poly(ethylene glycol) Hydrogels

Project description:Macrophages are known to be the drivers of the Foreign Body Response (FBR) to implanted biomaterials. While each macrophage population may polarize into distinct phenotypes, the timescales of this process and their functional roles in the FBR are not known. The goal of this study was to investigate temporal changes in macrophage polarization over the course of the FBR to a PEG hydrogel implant. RNA-seq methods were applied to map temporal changes in the transcriptome of implant-associated monocytes, which represent newly arrived blood monocytes prior to their maturation into macrophages, and separately implant-associated macrophages, which represent both recruited and tissue resident macrophages. The transcriptomics analysis of implant-associated monocytes revealed that pro-inflammatory pathways were up-regulated early and continued to increase at day 14, but by day 28 began to subside. On the contrary, macrophages were initially in a pro-inflammatory state, but underwent a shift in their polarization state to an alternatively activated pro-fibrotic state by day 14, correlating with the emergence of the fibrous capsule. They retained this state through day 28, which correlated with the continued growth of the fibrous capsule around the hydrogel implant.

Delayed presentation of spinal foreign body - Case report and review of literature.

Project description:Although spinal cord injuries are frequent causes of myelopathy in young patients, stab wounds of the spinal cord rarely occur and are typically maximal symptomatic immediately after the trauma.A 31-year-old male developed delayed onset of symptoms 4 years after a stab wound to the cervical spinal cord attributed to a plant needle (plant called Mandacaru). Following removal of the foreign body and decompression/excision of scarring at the C34 level, the patient's symptoms resolved.Surgical excision should be encouraged to remove chronic penetrating foreign bodies to both decompress and untether the spinal cord.

PLGA microsphere/PVA hydrogel coatings suppress the foreign body reaction for 6 months.

Project description:The application of dexamethasone releasing poly (lactic-co-glycolic acid) (PLGA) microspheres embedded in a poly vinyl alcohol (PVA) hydrogel coatings have been successfully used in the suppression of the foreign body response (FBR) to implantable glucose sensors. In the current study, dexamethasone-loaded PLGA microspheres were prepared by blending two types of PLGA polymers (RG503H and DLG7E with MW of ca. 25 kDa and 113 kDa, respectively) to achieve long-term (6 months) inhibition of the FBR. The microsphere composition was optimized according to the in vitro drug release profiles. Microspheres with DLG7E/RG503H/dexamethasone = 70/13.3/16.7 wt% composition, when embedded in a PVA hydrogel, provided a continuous drug release for 6 months. By combining the aforementioned microspheres with microspheres composed solely of the DLG7E polymer within a similar PVA hydrogel realized an even longer (>7 months) in vitro drug release. A heat map was constructed to depict the daily in vitro drug released and elucidate possible lag phases that could affect the pharmacodynamic response. These drug-loaded implant coatings were investigated in vivo (rat model) and showed inhibition of the foreign body response for 6 months. These results suggest that the minimum effective daily dose to counter chronic inflammation is ca. 0.1 μg per mg of coating surrounding a 0.5 × 0.5 × 5 mm silicon implant (dummy sensor). Accordingly, these drug-eluting composite coatings can ensure long-term inflammation control for miniaturized implantable devices.

A pin detected by ultrasonography within the normal appendix: prior to surgery, an impressive use of ultrasonography to localize an ingested foreign body exactly.

Project description:We present a 1-year-old boy who was asymptomatic and brought to the emergency room on suspicion of his having swallowed a pin. Confirmation of ingestion of the pin and its passage through the gut was achieved with abdominal radiography. The pin, which was followed with serial abdominal radiographs, was expected to leave the gastrointestinal tract, but was fixed to the right lower quadrant. When the pin had not passed after 10 days, and with increasing concern about the likelihood of perforation, ultrasonography was used to locate its exact position and allow surgical removal. Only a few cases involving the use of ultrasonography to reveal the exact location of an ingested foreign body prior to surgery have been reported ın the literature. This case presents an impressive example of the use of ultrasonography to reveal the intra-appendiceal location of an ingested foreign body, and to facilitate its surgical removal.
