Time-of-arrival mapping at three-dimensional time-resolved contrast-enhanced MR angiography.

ABSTRACT: This study was HIPAA compliant and institutional review board approved, and informed consent was obtained from all volunteers. The authors describe a method for generating a time-of-arrival (TOA) map of intravenously administered contrast material, as observed in a time series of three-dimensional (3D) contrast material-enhanced magnetic resonance (MR) angiograms. The method may enable visualization and interpretation, on one 3D image, of the temporal enhancement patterns that occur in the vasculature. Colorization of TOA values may further aid interpretation. The quality of the results depends not only on the adequacy of the frame rate, spatial resolution, and signal-to-noise ratio of the MR image acquisition method but also on the accuracy and clarity with which the leading edge of the contrast material bolus is depicted. The criteria for optimizing these parameters are described. The TOA mapping technique is demonstrated by using vascular studies of the hands, brain, and lower leg regions.

Project description:Background and purposeThe diagnosis of spinal vascular malformations may be challenging on conventional MR imaging because neither the location of the signal abnormality in the spinal cord nor the level of the abnormal flow voids correlates with the level of the fistula. We conducted a retrospective evaluation of the utility of using a time-resolved imaging of contrast kinetics sequence in the diagnosis, characterization, and localization of spinal vascular malformations, comparing it with the criterion standard of spinal DSA.Materials and methodsFifty-five consecutive patients with a suspected diagnosis of spinal vascular malformation underwent time-resolved imaging of contrast kinetics followed by spinal DSA. All scans were performed on a 1.5T scanner by using a standard 8-channel spine coil and were reported by a neuroradiologist before the DSA was performed.ResultsForty-seven lesions were confirmed on time-resolved imaging of contrast kinetics and classified as spinal dural arteriovenous fistulas (n = 33, with 1 patient having a type Ib fistula), perimedullary spinal cord arteriovenous fistulas (n = 10), and intramedullary arteriovenous malformations (n = 3). One patient had an extradural spinal vascular malformation. Time-resolved imaging of contrast kinetics identified the location of the arterial feeder to within 1 vertebral level in 27/33 patients (81.8%) with spinal dural arteriovenous fistulas and correctly predicted the side in 22/33 (66.6%) patients. Perimedullary spinal cord arteriovenous fistulas were erroneously considered to represent spinal dural arteriovenous fistulas before spinal DSA. The anatomy of the arterial supply to intramedullary arteriovenous malformations was also poorly characterized on time-resolved contrast-enhanced MR angiography.ConclusionsIt has been our experience that time-resolved imaging of contrast kinetics is a useful confirmatory tool when a spinal vascular malformation is suspected on the basis of clinical and conventional MR imaging findings. As experience with the technique grows and sequences are refined, it may be possible to rely on time-resolved imaging of contrast kinetics as a screening tool for the diagnosis of spinal vascular malformations.

Project description:PurposeTo prospectively evaluate the feasibility of performing high-spatial-resolution (1-mm isotropic) time-resolved three-dimensional (3D) contrast material-enhanced magnetic resonance (MR) angiography of the peripheral vasculature with Cartesian acquisition with projection-reconstruction-like sampling (CAPR) and eightfold accelerated two-dimensional (2D) sensitivity encoding (SENSE).Materials and methodsAll studies were approved by the institutional review board and were HIPAA compliant; written informed consent was obtained from all participants. There were 13 volunteers (mean age, 41.9; range, 27-53 years). The CAPR sequence was adapted to provide 1-mm isotropic spatial resolution and a 5-second frame time. Use of different receiver coil element sizes for those placed on the anterior-to-posterior versus left-to-right sides of the field of view reduced signal-to-noise ratio loss due to acceleration. Results from eight volunteers were rated independently by two radiologists according to prominence of artifact, arterial to venous separation, vessel sharpness, continuity of arterial signal intensity in major arteries (anterior and posterior tibial, peroneal), demarcation of origin of major arteries, and overall diagnostic image quality. MR angiographic results in two patients with peripheral vascular disease were compared with their results at computed tomographic angiography.ResultsThe sequence exhibited no image artifact adversely affecting diagnostic image quality. Temporal resolution was evaluated to be sufficient in all cases, even with known rapid arterial to venous transit. The vessels were graded to have excellent sharpness, continuity, and demarcation of the origins of the major arteries. Distal muscular branches and the communicating and perforating arteries were routinely seen. Excellent diagnostic quality rating was given for 15 (94%) of 16 evaluations.ConclusionThe feasibility of performing high-diagnostic-quality time-resolved 3D contrast-enhanced MR angiography of the peripheral vasculature by using CAPR and eightfold accelerated 2D SENSE has been demonstrated.

Project description:PurposeTo determine the feasibility of using real-time fluoroscopic tracking for bolus-chase magnetic resonance (MR) angiography of peripheral vasculature to image three stations from the aortoiliac bifurcation to the pedal arteries.Materials and methodsThis prospective study was institutional review board approved and HIPAA compliant. Eight healthy volunteers (three men; mean age, 48 years; age range, 30-81 years) and 13 patients suspected of having peripheral arterial disease (five men; mean age, 67 years; age range, 47-81 years) were enrolled and provided informed consent. All subjects were imaged with the fluoroscopic tracking MR angiographic protocol. Ten patients also underwent a clinical computed tomographic (CT) angiographic runoff examination. Two readers scored the MR angiographic studies for vessel signal intensity and sharpness and presence of confounding artifacts and venous contamination at 35 arterial segments. Mean aggregate scores were assessed. The paired MR angiographic and CT angiographic studies also were scored for visualization of disease, reader confidence, and overall diagnostic quality and were compared by using a Wilcoxon signed rank test.ResultsReal-time fluoroscopic tracking performed well technically in all studies. Vessel segments were scored good to excellent in all but the following categories: For vessel signal intensity and sharpness, the abdominal aorta, iliac arteries, distal plantar arteries, and plantar arch were scored as fair to good; and for presence of confounding artifacts, the abdominal aorta and iliac arteries were scored as fair. The MR angiograms and CT angiograms did not differ significantly in any scoring category (reader 1: P = .50, .39, and .39; reader 2: P = .41, .61, and .33, respectively). CT scores were substantially better in 20% (four of 20) and 25% (five of 20) of the pooled evaluations for the visualization of disease and overall image quality categories, respectively, versus 5% (one of 20) for MR scores in both categories.ConclusionThree-station bolus-chase MR angiography with real-time fluoroscopic tracking provided high-spatial-resolution arteriograms of the peripheral vasculature, enabled precise triggering of table motion, and compared well with CT angiograms.

Project description:To prospectively compare a new three-dimensional (3D) radial phase-contrast magnetic resonance (MR) angiographic method with contrast material-enhanced MR angiography for anatomic assessment of the renal arteries.An institutional review board approved this prospective HIPAA-compliant study. Informed consent was obtained. Twenty-seven subjects (mean age, 52.6 years ± 20.5 [standard deviation]) were imaged with respiratory-gated phase-contrast vastly undersampled isotropic projection reconstruction (VIPR) prior to contrast-enhanced MR angiographic acquisition with a 3.0-T clinical system. The imaging duration for phase-contrast VIPR was 10 minutes and provided magnitude and complex difference ("angiographic") images with 3D volumetric (320 mm) coverage and isotropic high spatial resolution (1.25 mm(3)). Quantitative analysis consisted of comparing vessel diameters between the two techniques. Qualitative assessment included evaluation of the phase-contrast VIPR and contrast-enhanced MR angiographic techniques for artifacts, noise, and image quality. Bland-Altman analysis was used for comparison of quantitative measurements, and the Wilcoxon signed rank test was used for comparison of qualitative scores.Phase-contrast VIPR images were successfully acquired in all subjects. The vessel diameters measured with phase-contrast VIPR were slightly greater than those measured with contrast-enhanced MR angiography (mean bias = 0.09 mm). Differences in mean artifact, quality scores for the proximal renal arteries, and overall image quality scores between phase-contrast VIPR and contrast-enhanced MR angiographic techniques were not statistically significant (P = .31 and .29, .27 and .39, and .43 and .69 for readers 1 and 2, respectively). The quality scores for the segmental renal arteries were higher for phase-contrast VIPR than for contrast-enhanced MR angiography (P < .05). Although the noise scores were higher with phase-contrast VIPR than with contrast-enhanced MR angiography and were statistically significant (P < .05), the presence of noise did not interfere with the ability to interpret the images.Isotropic, high-spatial-resolution, unenhanced MR angiography of the renal arteries is feasible with 3D radial undersampling.
