The role of the cerebellum in schizophrenia.

ABSTRACT: For many years the cerebellum has been considered to serve as a coordinator of motor function. Likewise, for many years schizophrenia has been considered to be a disease that primarily affects the cerebrum. This review summarizes recent evidence that both these views must be revised in the light of emerging evidence about cerebellar function and the mechanisms of schizophrenia. Evidence indicating that the cerebellum plays a role in higher cortical functions is summarized. Evidence indicating that cerebellar abnormalities occur in schizophrenia is also reviewed. These suggest interesting directions for future research.

The role of the cerebellum in schizophrenia: from cognition to molecular pathways.

Project description:Beside its role in motor coordination, the cerebellum is involved in cognitive function such as attention, working memory, verbal learning, and sensory discrimination. In schizophrenia, a disturbed prefronto-thalamo-cerebellar circuit has been proposed to play a role in the pathophysiology. In addition, a deficit in the glutamatergic N-methyl-D-aspartate (NMDAf) receptor has been hypothesized. The risk gene neuregulin 1 may play a major role in this process. We demonstrated a higher expression of the NMDA receptor subunit 2D in the right cerebellar regions of schizophrenia patients, which may be a secondary upregulation due to a dysfunctional receptor. In contrast, the neuregulin 1 risk variant containing at least one C-allele was associated with decreased expression of NMDA receptor subunit 2C, leading to a dysfunction of the NMDA receptor, which in turn may lead to a dysfunction of the gamma amino butyric acid (GABA) system. Accordingly, from post-mortem studies, there is accumulating evidence that GABAergic signaling is decreased in the cerebellum of schizophrenia patients. As patients in these studies are treated with antipsychotics long term, we evaluated the effect of long-term haloperidol and clozapine treatment in an animal model. We showed that clozapine may be superior to haloperidol in restoring a deficit in NMDA receptor subunit 2C expression in the cerebellum. We discuss the molecular findings in the light of the role of the cerebellum in attention and cognitive deficits in schizophrenia.

PROTEOMIC SIGNATURE OF THE CEREBELLUM IN SCHIZOPHRENIA

Project description:Malfunction of multiple regions in distributed brain circuits has been suggested to underlie the diversity of symptoms and cognitive impairments in schizophrenia. The cerebellum, as part of this circuit, may contribute to these symptoms for this reason we undertook a proteomic profile of this region. Here, we design a quantitative exploratory proteomic analysis in pooled grey matter of the cerebellum from schizophrenia and control individuals with the aim of discovering consistently altered biomarker proteins in schizophrenia.

Calcium-binding proteins are altered in the cerebellum in schizophrenia.

Project description:Alterations in the cortico-cerebellar-thalamic-cortical circuit might underlie the diversity of symptoms in schizophrenia. However, molecular changes in cerebellar neuronal circuits, part of this network, have not yet been fully determined. Using LC-MS/MS, we screened altered candidates in pooled grey matter of cerebellum from schizophrenia subjects who committed suicide (n = 4) and healthy individuals (n = 4). Further validation by immunoblotting of three selected candidates was performed in two cohorts comprising schizophrenia (n = 20), non-schizophrenia suicide (n = 6) and healthy controls (n = 21). We found 99 significantly altered proteins, 31 of them previously reported in other brain areas by proteomic studies. Transport function was the most enriched category, while cell communication was the most prevalent function. For validation, we selected the vacuolar proton pump subunit 1 (VPP1), from transport, and two EF-hand calcium-binding proteins, calmodulin and parvalbumin, from cell communication. All candidates showed significant changes in schizophrenia (n = 7) compared to controls (n = 7). VPP1 was altered in the non-schizophrenia suicide group and increased levels of parvalbumin were linked to antipsychotics. Further validation in an independent cohort of non-suicidal chronic schizophrenia subjects (n = 13) and non-psychiatric controls (n = 14) showed that parvalbumin was increased, while calmodulin was decreased in schizophrenia. Our findings provide evidence of calcium-binding protein dysregulation in the cerebellum in schizophrenia, suggesting an impact on normal calcium-dependent synaptic functioning of cerebellar circuits. Our study also links VPP1 to suicide behaviours, suggesting a possible impairment in vesicle neurotransmitter refilling and release in these phenotypes.

Disrupted modular architecture of cerebellum in schizophrenia: a graph theoretic analysis.

Project description:Recent studies of schizophrenia have revealed cognitive and memory deficits that are accompanied by disruptions of neuronal connectivity in cortical and subcortical brain regions. More recently, alterations of topological organization of structural networks in schizophrenia are also being identified using graph theoretical analysis. However, the role of the cerebellum in this network structure remains largely unknown. In this study, global network measures obtained from diffusion tensor imaging were computed in the cerebella of 25 patients with schizophrenia and 36 healthy volunteers. While cerebellar global network characteristics were slightly altered in schizophrenia patients compared with healthy controls, the patients showed a retained small-world network organization. The modular architecture, however, was changed mainly in crus II. Furthermore, schizophrenia patients had reduced correlations between modularity and microstructural integrity, as measured by fractional anisotropy (FA) in lobules I-IV and X. Finally, FA alterations were significantly correlated with the Positive and Negative Syndrome Scale symptom scores in schizophrenia patients. Taken together, our data suggest that schizophrenia patients have altered network architecture in the cerebellum with reduced local microstructural connectivity and that cerebellar structural abnormalities are associated symptoms of the disorder.

Dysfunctional Activation of the Cerebellum in Schizophrenia: A Functional Neuroimaging Meta-Analysis.

Project description:The cognitive dysmetria framework postulates that the deficits seen in schizophrenia are due to underlying cerebello-thalamo-cortical dysfunction. The cerebellum is thought to be crucial in the formation of internal models for both motor and cognitive behaviors. In healthy individuals there is a functional topography within the cerebellum. Alterations in the functional topography and activation of the cerebellum in schizophrenia patients may be indicative of altered internal models, providing support for this framework. Using state-of-the-art neuroimaging meta-analysis, we investigated cerebellar activation across a variety of task domains affected in schizophrenia and in comparison to healthy controls. Our results indicate an altered functional topography in patients. This was especially apparent for emotion and working memory tasks, and may be related to deficits in these domains. Results suggest that an altered cerebellar functional topography in schizophrenia may be contributing to the many deficits associated with the disease, perhaps due to dysfunctional internal models.

Questioning the role of rebound firing in the cerebellum.

Project description:A key component of recent theories on cerebellar function is rebound firing in neurons of the deep cerebellar nuclei (DCN). Despite the robustness of this phenomenon in vitro, in vivo studies have provided little evidence for its prevalence. We found that intact mouse or rat DCN neurons rarely showed rebound firing under physiological conditions in vitro or in vivo. These observations necessitate a critical re-evaluation of recent cerebellar models.

Consensus paper: pathological role of the cerebellum in autism.

Project description:There has been significant advancement in various aspects of scientific knowledge concerning the role of cerebellum in the etiopathogenesis of autism. In the current consensus paper, we will observe the diversity of opinions regarding the involvement of this important site in the pathology of autism. Recent emergent findings in literature related to cerebellar involvement in autism are discussed, including: cerebellar pathology, cerebellar imaging and symptom expression in autism, cerebellar genetics, cerebellar immune function, oxidative stress and mitochondrial dysfunction, GABAergic and glutamatergic systems, cholinergic, dopaminergic, serotonergic, and oxytocin-related changes in autism, motor control and cognitive deficits, cerebellar coordination of movements and cognition, gene-environment interactions, therapeutics in autism, and relevant animal models of autism. Points of consensus include presence of abnormal cerebellar anatomy, abnormal neurotransmitter systems, oxidative stress, cerebellar motor and cognitive deficits, and neuroinflammation in subjects with autism. Undefined areas or areas requiring further investigation include lack of treatment options for core symptoms of autism, vermal hypoplasia, and other vermal abnormalities as a consistent feature of autism, mechanisms underlying cerebellar contributions to cognition, and unknown mechanisms underlying neuroinflammation.

Gene expression of NMDA receptor subunits in the cerebellum of elderly patients with schizophrenia.

Project description:To determine if NMDA receptor alterations are present in the cerebellum in schizophrenia, we measured NMDA receptor binding and gene expression of the NMDA receptor subunits in a post-mortem study of elderly patients with schizophrenia and non-affected subjects. Furthermore, we assessed influence of genetic variation in the candidate gene neuregulin-1 (NRG1) on the expression of the NMDA receptor in an exploratory study. Post-mortem samples from the cerebellar cortex of ten schizophrenic patients were compared with nine normal subjects. We investigated NMDA receptor binding by receptor autoradiography and gene expression of the NMDA receptor subunits NR1, NR2A, NR2B, NR2C and NR2D by in situ hybridization. For the genetic study, we genotyped the NRG1 polymorphism rs35753505 (SNP8NRG221533). Additionally, we treated rats with the antipsychotics haloperidol or clozapine and assessed cerebellar NMDA receptor binding and gene expression of subunits to examine the effects of antipsychotic treatment. Gene expression of the NR2D subunit was increased in the right cerebellum of schizophrenic patients compared to controls. Individuals carrying at least one C allele of rs35753505 (SNP8NRG221533) showed decreased expression of the NR2C subunit in the right cerebellum, compared to individuals homozygous for the T allele. Correlation with medication parameters and the animal model revealed no treatment effects. In conclusion, increased NR2D expression results in a hyperexcitable NMDA receptor suggesting an adaptive effect due to receptor hypofunction. The decreased NR2C expression in NRG1 risk variant may cause a deficit in NMDA receptor function. This supports the hypothesis of an abnormal glutamatergic neurotransmission in the right cerebellum in the pathophysiology of schizophrenia.

Role of barhl1 in amphbian cerebellum

Project description:Cerebellar granule neuron progenitors (GNPs) originate from the upper rhombic lip (URL), a germinative niche in which developmental defects produce human diseases. T-cell factor (TCF) responsiveness and Notch dependence are hallmarks of self-renewal in neural stem cells. TCF activity, together with transcripts encoding proneural gene repressors hairy and enhancer of split (Hes/Hey), are detected in the URL; however, their functions and regulatory modes are undeciphered. Here, we established amphibian as a pertinent model for studying vertebrate URL development. The amphibian long-lived URL is TCF active, whereas the external granular layer (EGL) is non-proliferative and expresses hes4 and hes5 genes. Using functional and transcriptomic approaches, we show that TCF activity is necessary for URL emergence and maintenance. We establish that the transcription factor Barhl1 controls GNP exit from the URL, acting partly through direct TCF inhibition. Identification of Barhl1 target genes suggests that, besides TCF, Barhl1 inhibits transcription of hes5 genes independently of Notch signaling. Observations in amniotes suggest a conserved role for Barhl in maintenance of the URL and/or EGL via co-regulation of TCF, Hes and Hey genes.
