Angiokeratoma circumscriptum in a young male.

ABSTRACT: A 20-year-old male presented with multiple eruptions on his right leg since birth; these bled and were painful on trivial trauma. Examination revealed dark brown, hyperkeratotic, indurated, verrucous linear plaques with irregular borders. Histopathological evidence of hyperkeratosis, acanthosis, and extensive vascular proliferation in papillary dermis confirmed clinical suspicion of angiokeratoma circumscriptum (AKC). Excision and skin grafting yielded a cosmetically favorable outcome. Angiokeratomas, first described by Mibeli in 1889, are a group of vascular ectasias involving the papillary dermis. Angiokeratomas are more common in males; however, AKC-the rarest of its five variants-exhibits a female preponderance (F:M:3:1). AKC is an extremely rare nevoid disorder, only 100 of its cases having been reported in the world literature until 2006. Herein, we have reported a typical case of AKC in a young male that was previously misdiagnosed, and the patient wrongly counseled about the likelihood of its spontaneous regression.

Protein acetylation in young versus midlife Drosophila male heads

Project description:Old age is associated with a progressive decline of mitochondrial function and changes in nuclear chromatin. However, little is known about how metabolic activity and epigenetic modifications change as organisms reach their midlife. Here, we assessed how protein acetylation changes during midlife in Drosophila melanogaster. Midlife flies show elevated acetyl-CoA levels and alterations in protein acetylation. Based on these observations, we decreased the activity of the acetyl-CoA-synthesizing enzyme ATP citrate lyase (ATPCL). We find that lower ATPCL activity alleviates the observed aging-associated changes and promote longevity. Our findings reveal a pathway that couples changes of intermediate metabolism during aging with the chromatin-mediated regulation of transcription and changes in the activity of associated enzymes that modulate organismal lifespan.

Expression data from the mouse brain after 90'-MCAO and sham-control surgery in: young male mice, young female mice, diabetic young male mice and aged male mice

Project description:Ischemic stroke is a major cause of death and disability worldwide. Translation into the clinical setting of neuroprotective agents showing promising results in pre-clinical studies has systematically failed. One of the reasons that could explain this failure is that the animal models used to test neuroprotectants do not represent properly the population affected by stroke, as the majority of pre-clinical studies are performed in healthy young male mice. In this regard, we aimed to determine if the response to cerebral ischemia differed depending on age, sex and the presence of comorbidities. Thus, we explored proteomic and transcriptomic changes triggered during the hyperacute phase of cerebral ischemia (by transient intraluminal middle cerebral artery occlusion) in the brain of: (1) young male mice, (2) young female mice, (3) aged male mice and (4) diabetic young male mice. Moreover, we compared the proteomic and transcriptomic changes of each group using an integrative enrichment pathways analysis to disclose key common and exclusive altered proteins, genes and pathways in the first stages of the disease. We found 61 differentially expressed genes (DEG) in male mice, 77 in females, 699 in diabetic and 24 in aged mice. Of these, only 14 were commonly dysregulated in all four groups. The enrichment pathways analysis revealed that the inflammatory response was the biological process with more DEG in all groups of animals, followed by hemopoiesis and lymphoid organ development. Our findings indicate that the response to cerebral ischemia regarding proteomic and transcriptomic changes differs depending on the sex, age and comorbidities, highlighting the importance of incorporating these groups of animals in future stroke research studies.

Unexpected papilledema in a young male with Type 1 diabetes.

Project description:In young patients with T1D, neurological manifestations of cerebral hypertension should suggest the possibility of a cerebral venous sinus thrombosis (CVST). In these patients an inherited prothrombotic risk factor, including factor V Leiden G1691A gene mutation, should be considered during an event of thrombosis. Improving the glycemic control is the first factor that should be controlled in a patient who carries a genetic prothrombotic risk factor. Anticoagulant treatment should be started as son as CVST has been diagnosed. Long-term antithrombotic treatment with tinzaparin 175 IU/kg/day, a low-molecular weight heparin (LMWH), could be reliable and well tolerated, although an indefinite special follow-up, including neurological controls, is advisable even in asymptomatic patients.

Male-Dominated Occupations and Substance Use Disorders in Young Adulthood.

Project description:This article examined associations between male-dominated occupations and substance use disorders in young adulthood, accounting for adolescent experiences of work intensity (more than 15 hr a week at 16 to 17 years of age) and substance use (i.e., smoking, heavy drinking, cannabis, and illicit drug use). The moderating effects of biological sex and coming from a family with a low socioeconomic status (SES) were also assessed. Data were from a 10-year prospective study of community-based youth aged 12-18 in 2003 (T1; N = 662; 48% male; Mage = 15.5, SD = 1.9). Their occupations at ages 22-29 were categorized so that higher scores indicated more male-dominated occupations. Young adults in male-dominated occupations (more than 75% males) had lower education, worked in less prestigious occupations, and earned higher hourly wages than those in the other gendered-occupation groups. Work intensity in high school was associated with substance use at ages 18-25 and substance use was also associated with alcohol- and cannabis-use disorder symptoms and illicit drug use in young adulthood (ages 22-29). Adding to these effects, employment in a male-dominated occupation was associated with more cannabis-use disorder symptoms for the low, but not the high SES group. Public health messages need greater focus on preventing substance use disorders among individuals employed in male-dominated jobs in young adulthood. Efforts to promote self-assessment of problematic substance use and motivation to change may be particularly important for young workers.

A secular trend toward earlier male sexual maturity: evidence from shifting ages of male young adult mortality.

Project description:This paper shows new evidence of a steady long-term decline in age of male sexual maturity since at least the mid-eighteenth century. A method for measuring the timing of male maturity is developed based on the age at which male young adult mortality accelerates. The method is applied to mortality data from Sweden, Denmark, Norway, the United Kingdom, and Italy. The secular trend toward earlier male sexual maturity parallels the trend toward earlier menarche for females, suggesting that common environmental cues influence the speed of both males' and females' sexual maturation.

Exceptional mortality risk among police-identified young black male gang members.

Project description:Gang membership is associated with many risky behaviors but is often overlooked as a source of mortality among young Americans. Gang Member-Linked Mortality Files (GM-LMFs) match St. Louis, Missouri gang members listed in a law enforcement gang database to mortality records in the National Death Index. We created three analytic samples composed of black males aged 15-35 years by merging cases of the GM-LMFs with National Vital Statistics System and Census data in years 1993-2016. Mortality rates standardized to the 15-35-year-old 2010 U.S. male population were estimated for all-cause (1477.4, 99% CI = 1451.5-1503.3), homicide (950.1, 99% CI = 932.2-967.9), non-homicide injury (314.0, 99% CI = 308.8-319.2), and non-injury (213.3, 99% CI = 202.3-224.4) deaths in the GM-LMFs. We fitted Poisson rate models to estimate mortality rate ratios (RR) between gang members and demographically-matched comparison groups. Black male gang members in St. Louis were at an elevated mortality risk from all causes of death, and homicides contributed substantially to this risk. Compared to black males in St. Louis, gang members experienced greater relative risk of all-cause (RR = 2.9, 99% CI = 2.4-3.5), homicide (RR = 3.2, 99% CI = 2.5-4.1), and non-homicide injury (RR = 4.0, 99% CI = 2.8-5.8) mortality between 1993 and 2016. Relative risk was greater when compared to black males in St. Louis MSA, Missouri, and the USA. These results identify a key source of excess mortality among young black Americans. Health policies and interventions may be most efficacious when they acknowledge, address, and incorporate information about and target high-risk populations, including gang members, that contribute to relatively high mortality risk in the USA.

In memory of tachycardia: A wide complex tachycardia in a young male

Project description:Abstract We present a case of a 24?year?old male with palpitations and a wide complex tachycardia. Baseline electrocardiogram (ECG) after termination of tachycardia demonstrates a normal rhythm but with inferior/anterolateral T?wave inversions (TWIs). Electrophysiologic study confirmed the diagnosis of posterior fascicular ventricular tachycardia successfully terminated by anatomic ablation of the left posterior fascicle. TWIs on the patient's baseline ECG were consistent with cardiac memory. Twenty?four?year?old male presents with palpitations and wide complex tachycardia on ECG. Baseline ECG after termination of tachycardia demonstrates a normal rhythm but with inferior/anterolateral T?wave inversions (TWIs).
