The Big Picture: Evidence Base and Current Trials in Cardiac CT.

ABSTRACT: Cardiac computed tomography angiography (CCTA) has technically matured into a robust imaging modality for various cardiac disorders. Whereas early trials focused on assessment of the efficacy of CCTA in comparison with established recommended methods, current research efforts focus on the effectiveness of the technique in specific clinical scenarios. In this article, we provide an overview of recent technology advances, describe major clinical scenarios in which CCTA has been evaluated, and detail pertinent evidence from completed or ongoing clinical trials, including its use to investigate acute chest pain, its use among patients with stable chest pain syndrome, and its prognostic value for the occurrence of cardiovascular events.

Fooling the Big Picture in Classification Tasks

Project description:Minimally perturbed adversarial examples were shown to drastically reduce the performance of one-stage classifiers while being imperceptible. This paper investigates the susceptibility of hierarchical classifiers, which use fine and coarse level output categories, to adversarial attacks. We formulate a program that encodes minimax constraints to induce misclassification of the coarse class of a hierarchical classifier (e.g., changing the prediction of a ‘monkey’ to a ‘vehicle’ instead of some ‘animal’). Subsequently, we develop solutions based on convex relaxations of said program. An algorithm is obtained using the alternating direction method of multipliers with competitive performance in comparison with state-of-the-art solvers. We show the ability of our approach to fool the coarse classification through a set of measures such as the relative loss in coarse classification accuracy and imperceptibility factors. In comparison with perturbations generated for one-stage classifiers, we show that fooling a classifier about the ‘big picture’ requires higher perturbation levels which results in lower imperceptibility. We also examine the impact of different label groupings on the performance of the proposed attacks. The Epidemiology of Obesity: A Big Picture.

Project description:The epidemic of overweight and obesity presents a major challenge to chronic disease prevention and health across the life course around the world. Fueled by economic growth, industrialization, mechanized transport, urbanization, an increasingly sedentary lifestyle, and a nutritional transition to processed foods and high-calorie diets over the last 30 years, many countries have witnessed the prevalence of obesity in its citizens double and even quadruple. A rising prevalence of childhood obesity, in particular, forebodes a staggering burden of disease in individuals and healthcare systems in the decades to come. A complex, multifactorial disease, with genetic, behavioral, socioeconomic, and environmental origins, obesity raises the risk of debilitating morbidity and mortality. Relying primarily on epidemiologic evidence published within the last decade, this non-exhaustive review discusses the extent of the obesity epidemic, its risk factors-known and novel-, sequelae, and economic impact across the globe.

Functional outcome in bipolar disorder: the big picture.

Project description:Previous research on functional outcome in bipolar disorder (BD) has uncovered various factors that exacerbate psychosocial disability over the course of illness, including genetics, illness severity, stress, anxiety, and cognitive impairment. This paper presents an integrated view of these findings that accounts for the precipitous decline in psychosocial functioning after illness onset. The proposed model highlights a number of reciprocal pathways among previously studied factors that trap people in a powerful cycle of ailing forces. The paper discusses implications to patient care as well as the larger social changes required for shifting the functional trajectory of people with BD from psychosocial decline to growth.

Neoadjuvant treatment of locally advanced esophageal and junctional cancer: the evidence-base, current key questions and clinical trials.

Project description:Recent trials, including CROSS, MAGIC, ACCORD, and OEO2, have established neoadjuvant therapy as standard of care for locally advanced (cT2-3NanyM0) esophageal and junctional cancer compared with surgery alone. The CROSS trial in particular defines a new benchmark for outcomes from multimodal therapy, with a 5 year survival rate of 47%, a median survival of 47 months, a pathologic complete response rate (pCR) of 29% and an R0 resection rate of 92%. Several key questions remain, in particular whether CROSS-regimen chemoradiotherapy is superior to neoadjuvant chemotherapy alone for esophageal cancer, in particular adenocarcinoma. Second, with respect to neoadjuvant chemoradiation, whether an apparent complete clinical response can justify a "watch and wait" surveillance policy, with salvage surgery reserved for where relapse occurs. Third, whether with modern staging, predicted node negative cT2 tumors merit neoadjuvant therapy as standard. Finally, with the enormous interest in the application of targeted and immune-based therapies, and positive leads from other cancers, whether such approaches can improve outcomes in patients undergoing treatment with curative intent. We review herein a brief overview of the existing evidence-base and current active trials addressing these key questions.

Are UX Evaluation Methods Providing the Same Big Picture?

Project description:The success of a software application is related to users' willingness to keep using it. In this sense, evaluating User eXperience (UX) became an important part of the software development process. Researchers have been carrying out studies by employing various methods to evaluate the UX of software products. Some studies reported varied and even contradictory results when applying different UX evaluation methods, making it difficult for practitioners to identify which results to rely upon. However, these works did not evaluate the developers' perspectives and their impacts on the decision process. Moreover, such studies focused on one-shot evaluations, which cannot assess whether the methods provide the same big picture of the experience (i.e., deteriorating, improving, or stable). This paper presents a longitudinal study in which 68 students evaluated the UX of an online judge system by employing AttrakDiff, UEQ, and Sentence Completion methods at three moments along a semester. This study reveals contrasting results between the methods, which affected developers' decisions and interpretations. With this work, we intend to draw the HCI community's attention to the contrast between different UX evaluation methods and the impact of their outcomes in the software development process.

Detailed close-ups and the big picture of spliceosomes.

Project description:The spliceosome is the huge macromolecular assembly responsible for the removal of introns from pre-mRNA transcripts. The size and complexity of this dynamic cellular machine dictate that structural analysis of the spliceosome is best served by a combination of techniques. Electron microscopy is providing a more global, albeit less detailed, view of spliceosome assemblies. X-ray crystallographers and NMR spectroscopists are steadily reporting more atomic resolution structures of individual spliceosome components and fragments. Increasingly, structures of these individual pieces in complex with binding partners are yielding insights into the interfaces that hold the entire spliceosome assembly together. Although the information arising from the various structural studies of splicing machinery has not yet fully converged into a complete model, we can expect that a detailed understanding of spliceosome structure will arise at the juncture of structural and computational modeling methods.

The Big Picture of Neurodegeneration: A Meta Study to Extract the Essential Evidence on Neurodegenerative Diseases in a Network-Based Approach.

Project description:The common features of all neurodegenerative diseases, including Alzheimer's disease, Parkinson's disease, Amyotrophic Lateral Sclerosis (ALS), and Huntington's disease, are the accumulation of aggregated and misfolded proteins and the progressive loss of neurons, leading to cognitive decline and locomotive dysfunction. Still, they differ in their ultimate manifestation, the affected brain region, and the kind of proteinopathy. In the last decades, a vast number of processes have been described as associated with neurodegenerative diseases, making it increasingly harder to keep an overview of the big picture forming from all those data. In this meta-study, we analyzed genomic, transcriptomic, proteomic, and epigenomic data of the aforementioned diseases using the data of 234 studies in a network-based approach to study significant general coherences but also specific processes in individual diseases or omics levels. In the analysis part, we focus on only some of the emerging findings, but trust that the meta-study provided here will be a valuable resource for various other researchers focusing on specific processes or genes contributing to the development of neurodegeneration.

Missing the big picture: impaired development of global shape processing in autism.

Project description:Individuals with autism exhibit hypersensitivity to local elements of the input, which may interfere with the ability to group visual elements perceptually. We investigated the development of perceptual grouping abilities in high-functioning individuals with autism (HFA) across a wide age range (8-30 years) using a classic compound letter global/local (GL) task and a more fine-grained microgenetic prime paradigm (MPP), including both few- and many-element hierarchical displays. In the GL task, contrary to the typically developing (TD) controls, HFA participants did not develop an increasing sensitivity to the global information with age. In the MPP, like the TD controls, individuals with autism at all three age groups evinced a bias to individuate the few-element displays. However, contrary to the TD controls, the HFA group failed to show age-related improvements in the ability to encode the global shape of the many-element displays. In fact, across the age range, the HFA group was consistently faster than the TD controls at perceiving the local elements in these displays. These results indicate that in autism the full process of garnering shape information from perceptual grouping, which is essential for the ability to do fast and efficient object recognition and identification, never matures, and this is especially evident in adolescence when this ability begins to improve in TD individuals. The atypical development of these perceptual organizational abilities may disrupt processing of visually presented objects, which may, in turn, fundamentally impede the development of major aspects of the social and emotional behaviors in individuals with autism.
