A systematic review of community readiness tool applications: implications for reporting.

ABSTRACT:

Background

A systematic review characterised and synthesised applications of the Community Readiness Tool (CRT) and synthesised quantitative results for readiness applications at multiple time points.

Methods

Eleven databases in OvidSP and EBSCHOhost were searched to retrieve CRT applications. Information from primary studies was extracted independently by two researchers.

Results

Forty applications of the CRT met inclusion criteria focussing on 14 different health and social issues. The community of interest was most often defined solely on the basis of its geographical location (52.5%). Most studies used the CRT to plan (85%) and/or evaluate programs (40%). The CRT protocol was modified in 40% of studies. Six applications evaluated readiness at multiple time points, however limited reporting in primary studies precluded any synthesis of results. Applications identified methodological rigour, contextual information and community engagement as strengths, and time and resource costs as limitations.

Conclusions

The CRT is well suited for planning and evaluating complex community health interventions given its flexibility to accommodate diverse definitions of community and issues. CRT applications would benefit from improved reporting; reporting recommendations for use of the CRT are outlined.

Project description:BackgroundStandards for the reporting of factorial randomised trials remain to be established. We aimed to review the quality of reporting of methodological aspects of published factorial trials of complex interventions in community settings.MethodsWe searched MEDLINE, EMBASE, PsychInfo and the Cochrane Controlled Trials Register to identify factorial randomised trials of complex interventions in community settings from January 2000 to August 2009. We also conducted a citation search of two review papers published in 2003. Data were extracted by two reviewers on 22 items relating to study design, analysis and presentation.ResultsWe identified 5941 unique titles, from which 116 full papers were obtained and 76 were included in the review. The included trials reflected a broad range of target conditions and types of intervention. The median sample size was 400 (interquartile range 191-1001). Most (88%) trials employed a 2 × 2 factorial design. Few trials (21%) explicitly stated the rationale for using a factorial design. Reporting of aspects of design, analysis or presentation specific to factorial trials was variable, but there was no evidence that reporting of these aspects was different for trials published before or after 2003. However, for CONSORT items that apply generally to the reporting of all trials, there was some evidence that later studies were more likely to report employing an intention-to-treat (ITT) approach (78% vs 52%), present appropriate between-group estimates of effect (88% vs 63%), and present standard errors or 95% confidence intervals for such estimates (78% vs 56%). Interactions between interventions and some measure of the precision associated with such effects were reported in only 14 (18%) trials.ConclusionsReports of factorial trials of complex interventions in community settings vary in the amount of information they provide regarding important methodological aspects of design and analysis. This variability supports the extension of CONSORT guidelines to include the specific reporting of factorial trials.

Project description:Many hospitals continue to use incident reporting systems (IRSs) as their primary patient safety data source. The information IRSs collect on the frequency of harm to patients [adverse events (AEs)] is generally of poor quality, and some incident types (e.g. diagnostic errors) are under-reported. Other methods of collecting patient safety information using medical record review, such as the Global Trigger Tool (GTT), have been developed. The aim of this study was to undertake a systematic review to empirically quantify the gap between the percentage of AEs detected using the GTT to those that are also detected via IRSs. The review was conducted in adherence to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. Studies published in English, which collected AE data using the GTT and IRSs, were included. In total, 14 studies met the inclusion criteria. All studies were undertaken in hospitals and were published between 2006 and 2022. The studies were conducted in six countries, mainly in the USA (nine studies). Studies reviewed 22 589 medical records using the GTT across 107 institutions finding 7166 AEs. The percentage of AEs detected using the GTT that were also detected in corresponding IRSs ranged from 0% to 37.4% with an average of 7.0% (SD 9.1; median 3.9 and IQR 5.2). Twelve of the fourteen studies found <10% of the AEs detected using the GTT were also found in corresponding IRSs. The >10-fold gap between the detection rates of the GTT and IRSs is strong evidence that the rate of AEs collected in IRSs in hospitals should not be used to measure or as a proxy for the level of safety of a hospital. IRSs should be recognized for their strengths which are to detect rare, serious, and new incident types and to enable analysis of contributing and contextual factors to develop preventive and corrective strategies. Health systems should use multiple patient safety data sources to prioritize interventions and promote a cycle of action and improvement based on data rather than merely just collecting and analysing information.

Project description:Smart manufacturing, Industry 4.0 and Smart Factory are phenomena regarded as a key necessity for Small, Medium and Micro Businesses (SMMEs) worldwide. Even though these 4th Industrial Revolution (4IR) phenomena are generally used interchangeably, this paper sought to identify how SMME readiness for smart manufacturing has been investigated through a systematic review. The systematic review was conducted through the lens of Nooteboom, and Tornatzky and Klein’s research on technological innovation in SMMEs based on Rogers’ diffusion of innovations theory. The findings reveal that there is little to no research on smart manufacturing in relation to SMMEs in low-income countries particularly the African continent. The results also show that smart manufacturing is still an emergent phenomenon with disparate definitional challenges. These definitional challenges make the adoption of smart manufacturing innovations a challenge in resource-constrained contexts; but similarly present an opportunity for new definitions and theories in such contexts. The little research often treats SMMEs homogenously and as such misses their important heterogeneous (sector or industry specific) nature. Few research studies investigate SMME awareness (adequate knowledge) or make explicit the benefits (relative advantage) of smart manufacturing. Even fewer studies are explicit on the smart manufacturing technologies that are relevant for different SMME sectors. Smart manufacturing is identified as incompatible with SMME characteristics, that is SMMEs lack expertise/skills to comprehend the complexity of smart manufacturing, and also lack financial and human resources to implement smart manufacturing. Given that awareness, relative advantage, complexity and compatibility are critical barriers for SMME smart manufacturing readiness/adoption, there is a critical need for research to focus on these factors in particular for the context of resource constrained low-income country environments.
