Metrics of the normal anterior sclera: imaging with optical coherence tomography.

ABSTRACT: To investigate anterior scleral thickness in a cohort of healthy subjects using enhanced depth imaging anterior segment optical coherence tomography.Observational case series. The mean scleral thickness in the inferonasal, inferotemporal, superotemporal, and superonasal quadrant was measured 2 mm from the scleral spur on optical coherence tomography in healthy volunteers.Fifty-three eyes of 53 Caucasian patients (25 male and 28 female) with an average age of 48.6 years (range: 18 to 92 years) were analysed. Project description:The technology of optical coherence tomography (OCT) has evolved rapidly from time-domain to spectral-domain and swept-source OCT over the recent years. OCT has become an important tool for assessment of the anterior chamber angle and detection of angle closure. Improvement in image resolution and scan speed of OCT has facilitated a more detailed and comprehensive analysis of the anterior chamber angle. It is now possible to examine Schwalbe's line and Schlemm's canal along with the scleral spur. High-speed imaging allows evaluation of the angle in 360°. With three-dimensional reconstruction, visualization of the iris profiles and the angle configurations is enhanced. This article summarizes the development and application of OCT for anterior chamber angle measurement, detection of angle closure, and investigation of the pathophysiology of primary angle closure.

Quantification metrics for telangiectasia using optical coherence tomography.

Project description:Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disorder that causes vascular malformations throughout the body. The most prevalent and accessible of these lesions are found throughout the skin and mucosa, and often rupture causing bleeding and anemia. A recent increase in potential HHT treatments have created a demand for quantitative metrics that can objectively measure the efficacy of new and developing treatments. We employ optical coherence tomography (OCT)-a high resolution, non-invasive imaging modality in a novel pipeline to image and quantitatively characterize dermal HHT lesion behavior over time or throughout the course of treatment. This study is aimed at detecting detailed morphological changes of dermal HHT lesions to understand the underlying dynamic processes of the disease. We present refined metrics tailored for HHT, developed from a pilot study using 3 HHT patients and 6 lesions over the course of multiple imaging dates, totalling to 26 lesion images. Preliminary results from these lesions are presented in this paper alongside representative OCT images. This study provides a new objective method to analyse and understand HHT lesions using a minimally invasive, accessible, cost-effective, and efficient imaging modality with quantitative metrics describing morphology and blood flow.

Dynamic imaging of accommodation by swept-source anterior segment optical coherence tomography.

Project description:PurposeTo study the accommodation process in normal eyes using a commercially available clinical system based on swept-source anterior segment optical coherence tomography (AS-OCT).SettingOphthalmology Department, University of Parma, Italy.DesignEvaluation of diagnostic technology.MethodsRight eyes were analyzed using swept-source AS-OCT (Casia SS-1000). The optical vergence of the internal coaxial fixation target was adjusted during imaging to obtain monocular accommodation stimuli with different amplitudes (0, 3.0, 6.0, and 9.0 diopters [D]). Overlapping of real and conjugate OCT images enabled imaging of all the anterior segment optical surfaces in a single frame. Central corneal thickness (CCT), anterior chamber depth (ACD), and lens thickness were extracted from the OCT scans acquired at different static accommodation stimulus amplitudes. The crystalline lens was analyzed dynamically during accommodation and disaccommodation by acquiring sequential OCT images of the anterior segment at a rate of 8 frames per second. The lens thickness was extracted from the temporal sequence of OCT images and plotted as a function of time.ResultsThe study analyzed 14 eyes of 14 subjects aged 18 to 46 years. During accommodation, the decrease in the ACD was statistically significant (P < .05), as were the increase in the lens thickness (P < .001) and the slight movement forward of the lens central point (P < .01). The CCT and anterior chamber width measurements did not change statistically significantly during accommodation. The lens thickness at 0 D was positively correlated with age (P < .01).ConclusionHigh-resolution real-time imaging and biometry of the accommodating anterior segment can be effectively performed using a commercially available swept-source AS-OCT clinical device.Financial disclosureNo author has a financial or proprietary interest in any material or method mentioned.

Comparison of optical coherence tomography angiography metrics in primary angle-closure glaucoma and normal-tension glaucoma.

Project description:To investigate the peripapillary vascular metrics in early normal tension glaucoma (NTG) and early primary angle closure glaucoma (PACG) eyes using optical coherence tomography angiography (OCT-A). One or both eyes of each subject were imaged for a 3 × 3 mm peripapillary region by swept-source OCT-A (DRI-OCT Triton, Topcon, Japan) and assessed by an automated MATLAB program. OCT-A metrics including circumpapillary vessel density (cpVD) and fractal dimension (cpFD) were compared. Their association with visual field (VF) parameters and retinal nerve fiber layer (RNFL) thickness were determined. Sixty-eight eyes of 51 PACG, 68 eyes of 48 NTG, and 68 eyes of 49 control subjects were cross-sectionally analyzed. NTG eyes had significantly lower global cpVD (52.369 ± 0.781%) compared with PACG eyes (55.389 ± 0.721%, P = 0.004) that had comparable disease severity and average RNFL thickness. Multivariable analysis revealed that, for PACG and NTG eyes, decreased cpVD ([PACG] β = -4.242; CI: -8.120, -0.363 vs [NTG] β = -5.531; CI: -9.472, -1.590) and cpFD ([PACG] β = -8.894;CI: -11.925, -5.864 vs [NTG] β = -12.064; CI: -17.095, -6.932) were associated with decreased RNFL thickness (all P ≤ 0.032); with a stronger association between decrease cpFD and decreased RNFL thickness in NTG eyes (P = 0.028). Decreased cpVD was associated with decrease mean deviation (MD) in NTG eyes (β = -0.707; CI: -1.090, -0.324; P ≤ 0.001) and not associated with the visual field parameters in PACG eyes. Early NTG had lower global cpVD compared with early PACG, despite similar disease severity and average RNFL thickness.

Conjunctival and Intrascleral Vasculatures Assessed Using Anterior Segment Optical Coherence Tomography Angiography in Normal Eyes.

Project description:PURPOSE:To investigate conjunctival and intrascleral vasculatures using anterior segment optical coherence tomography angiography (AS-OCTA) in normal eyes. DESIGN:Cross-sectional study. METHODS:AS-OCTA images of the corneal limbus were acquired circumferentially using a swept-source optical coherence tomography system in 10 eyes of 10 healthy subjects. AS-OCTA flow patterns with en face maximum projection were compared between the superficial (from the conjunctival epithelium to a depth of 200 ?m) and deep (from a depth of 200 ?m to 1000 ?m) layers. The OCTA images were also compared with fluorescein scleral angiography and indocyanine green aqueous angiography images. Quantitative parameters (vessel density, vessel length density, vessel diameter index, and fractal dimension) were compared among different locations. RESULTS:The OCTA vessel patterns differed between the superficial and deep layers. The superficial-layer flow signals showed centrifugal patterns from the limbus, whereas the deep-layer flow signals showed segmental patterns. The OCTA en face images with whole signals had a similar appearance to the scleral angiography images, whereas those in the deep layer showed a similar appearance to the aqueous angiography images. In the superficial layer, only the vessel diameter index was significantly different among the locations (P = .003). In the deep layer, all 4 parameters differed significantly among the locations (P < .001 to P = .003). CONCLUSIONS:OCTA is a promising tool for evaluating conjunctival and intrascleral vasculatures. It may also help in understanding ocular surface blood flow relevant to vascular and ocular surface diseases, as well as aqueous humor outflow.

Corneal Epithelial Mapping Characteristics in Normal Eyes Using Anterior Segment Spectral Domain Optical Coherence Tomography.

Project description:PurposeThe detailed mapping characteristics of the corneal epithelial thickness (CET) in normal eyes from a Middle Eastern population were investigated in relation to age, sex, intraocular pressure, and keratometric power (K).MethodsA retrospective cross-sectional and analytical study was conducted using spectral domain optical coherence tomography (OCT). We calculated the CET in 124 subjects in 17 zones within a 6 mm circle. Exclusion criteria included subjects with dry eyes, keratoconus, previous eye surgery, glaucoma, and irregular corneas.ResultsA total of 124 individuals was composed of 64 males and 60 females. The mean age of this population was 45.52, ranging from 18 to 79 years. The central CET was thicker in the central 2 mm than the other zones of the cornea except the nasal, inferior-nasal, inferior and inferior-temporal zones, respectively. Males have thicker CET than females in all zones except in the peripheral nasal zone. We found a positive and significant correlation between age and CET in the central, superior-peripheral, inferior-paracentral, and inferior-temporal paracentral zones. Additionally, a medium-positive correlation was detected between increasing age and the variability of epithelial spectral domain in different zones. No link between CET and intraocular pressure was found.ConclusionsThis study analyzed 17 CET zones within the central 6 mm, where the central epithelium is resistant to aging. The CET was thinner superiorly than inferiorly. This may help in decision-making in refractive procedures and in the prediction of corneal diseases.Translational relevanceOCT novel algorithms are noninvasive methods for measuring CET and have been demonstrated to be useful in refractive surgery planning and follow-up, as well as a robust tool for diagnosing potential corneal ectasia.

Update on Optical Coherence Tomography and Optical Coherence Tomography Angiography Imaging in Proliferative Diabetic Retinopathy.

Project description:Proliferative diabetic retinopathy (PDR) is a major cause of blindness in diabetic individuals. Optical coherence tomography (OCT) and OCT-angiography (OCTA) are noninvasive imaging techniques useful for the diagnosis and assessment of PDR. We aim to review several recent developments using OCT and discuss their present and potential future applications in the clinical setting. An electronic database search was performed so as to include all studies assessing OCT and/or OCTA findings in PDR patients published from 1 January 2020 to 31 May 2021. Thirty studies were included, and the most recently published data essentially focused on the higher detection rate of neovascularization obtained with widefield-OCT and/or OCTA (WF-OCT/OCTA) and on the increasing quality of retinal imaging with quality levels non-inferior to widefield-fluorescein angiography (WF-FA). There were also significant developments in the study of retinal nonperfusion areas (NPAs) using these techniques and research on the impact of PDR treatment on NPAs and on vascular density. It is becoming increasingly clear that it is critical to use adequate imaging protocols focused on optimized segmentation and maximized imaged retinal area, with ongoing technological development through artificial intelligence and deep learning. These latest findings emphasize the growing applicability and role of noninvasive imaging in managing PDR with the added benefit of avoiding the repetition of invasive conventional FA.

Full anterior segment biometry with extended imaging range spectral domain optical coherence tomography at 1340 nm.

Project description:We demonstrate an extended-imaging-range anterior-segment optical coherence tomography (eAS-OCT) system for the biometric assessment of full AS in human eye. This newly developed eAS-OCT operating at 1340-nm wavelength band is simultaneously capable of an imaging speed of 120 kHz A-line scan rate, an axial resolution of 7.2 ?m, and an extended imaging range of up to 16 mm in air. Imaging results from three healthy subjects and one subject with a narrow-angle demonstrate the instrument's utility. With this system, it can provide anatomical dimensions of AS, including central corneal thickness, anterior chamber width, anterior chamber depth, crystalline lens vault, crystalline lens thickness, angle opening distance (AOD500/AOD750), and the area described by the trabecular-iris space (TISA500/TISA750) at 500/750???m. We also use eAS-OCT to image and quantify dynamic functional changes of the AS in response to a light stimulus that induces physiological pupillary changes as well as accommodative efforts that induce lens changes. The results show that the described eAS-OCT is able to provide full anatomical biometry for AS and is useful for the studies where the dynamic response of AS compartment to certain stimulus is required.

Utility of anterior segment swept-source optical coherence tomography for imaging eyes with antecedent ocular trauma.

Project description:PurposeTo evaluate the utility of swept source optical coherence tomography (SS-OCT) for the analysis of anterior segment structures in cases with previous traumatic eye injuries.ObservationsWe report three eyes of three patients with anterior segment traumatic eye injury and highlight the role of SS-OCT in their evaluation and management. This technology enabled us to visualize the structural details of anterior segment of the eye and augment the clinical examination in our patients. Given that it is non-invasive and that there is no contact involved, it may be an ideal imaging modality for traumatic eye injuries for viewing the details before and after any clinical intervention especially in the sub-acute setting.ConclusionsThe anterior segment SS-OCT is a useful device allowing non-invasive, non-contact, real-time, cross-sectional anterior segment images of eyes with previous ocular trauma.

Determinants of Quantitative Optical Coherence Tomography Angiography Metrics in Patients with Diabetes.

Project description:Early microvascular damage in diabetes (e.g. capillary nonperfusion and ischemia) can now be assessed and quantified with optical coherence tomography-angiography (OCT-A). The morphology of vascular tissue is indeed affected by different factors; however, there is a paucity of data examining whether OCT-A metrics are influenced by ocular, systemic and demographic variables in subjects with diabetes. We conducted an observational cross-sectional study and included 434 eyes from 286 patients with diabetes. Foveal avascular zone (FAZ) area, FAZ circularity, total and parafoveal vessel density (VD), fractal dimension (FD), and vessel diameter index (VDI) from the superficial capillary plexus OCT-angiogram were measured by a customized automated image analysis program. We found that diabetic retinopathy (DR) severity was associated with increased FAZ area, decreased FAZ circularity, lower VD, lower FD, and increased VDI. Enlarged FAZ area was correlated with shorter axial length and thinner central subfield macular thickness. Decreased FAZ circularity was correlated with a reduction in visual function. Decreased VD was correlated with thinner macular ganglion-cell inner plexiform layer. Increased VDI was correlated with higher fasting glucose level. We concluded that the effects of ocular and systemic factors in diabetics should be taken into consideration when assessing microvascular alterations via OCT-A.
