Project description:Social networks - or the web of relationships between individuals - may influence cardiovascular disease risk, particularly in low-income urban communities that suffer from a high prevalence of cardiovascular disease. Our objective was to describe the social networks of public housing residents - a low-income urban population - in Baltimore, MD and the association between these networks and blood pressure. We used cross-sectional survey data of randomly selected heads of household in two public housing complexes in Baltimore, MD (8/2014-8/2015). Respondents answered questions about 10 social network members, including attributes of their relationship and the frequency of interaction between members. We calculated measures of network composition (e.g., proportion of network members who were family members) and network structure (e.g., density), which we then dichotomized as "high" (upper quartile) and "low" (less than upper quartile). We used linear regression to test the association between network measures and mean systolic and diastolic blood pressure. The sample included 259 respondents (response rate: 46.6%). Mean age was 44.4 years, 85.7% were women, 95.4% Black, and 56.0% had a history of hypertension. A high proportion of older children (age 8-17 years) in the network (>30%) was associated with a 4.0% (95%CI [0.07, 8.07], p = 0.047) higher mean systolic blood pressure (~4.9 mmHg greater). Other network attributes had no association with blood pressure. Social network attributes, such as having a high proportion of older children in one's network, may have particular relevance to blood pressure among low-income public housing residents, reinforcing the potential importance of social relationships to cardiovascular health.

Project description:ObjectiveTo examine the relationship between public health system network density and organizational centrality in public health systems and public health governance, community size, and health status in three public health domains.Data sources/study settingDuring the fall and the winter of 2007-2008, primary data were collected on the organization and composition of eight rural public health systems.Study designMultivariate analysis and network graphical tools are used in a case comparative design to examine public health system network density and organizational centrality in the domains of adolescent health, senior health, and preparedness. Differences associated with public health governance (centralized, decentralized), urbanization (micropolitan, noncore), health status, public health domain, and collaboration area are described.Data collection/extraction methodsSite visit interviews with key informants from local organizations and a web-based survey administered to local stakeholders.Principal findingsGovernance, urbanization, public health domain, and health status are associated with public health system network structures. The centrality of local health departments (LHDs) varies across public health domains and urbanization. Collaboration is greater in assessment, assurance, and advocacy than in seeking funding.ConclusionsIf public health system organization is causally related to improved health status, studying individual system components such as LHDs will prove insufficient for studying the impact of public health systems.

Project description:Public housing provides affordable housing and, potentially, housing stability for low-income families. Housing stability may be associated with lower incidence or prevalence and better management of a range of health conditions through many mechanisms. We aimed to test the hypotheses that public housing residency is associated with both housing stability and reduced risk of diabetes incidence, and the relationship between public housing and diabetes risk varies by levels of housing stability. Using 2004-16 World Trade Center Health Registry data, we compared outcomes (housing stability measured by sequence analysis of addresses, self-reported diabetes diagnoses) between 730 New York City public housing residents without prevalent diabetes at baseline and 730 propensity score-matched non-public housing residents. Sequence analysis found 3 mobility patterns among all 1460 enrollees, including stable housing (65%), limited mobility (27%), and unstable housing patterns (8%). Public housing residency was associated with stable housing over 12 years. Diabetes risk was not associated with public housing residency; however, among those experiencing housing instability, a higher risk of diabetes was found among public housing versus non-public housing residents. Of those stably housed, the association remained insignificant. These findings provide important evidence for a health benefit of public housing via housing stability among people living in public housing.

Project description:Although stable, high-quality housing improves children's physical and social-emotional health, little is known about the health of children living in buildings financed by the federal government's primary tool for constructing and renovating affordable rental housing: the Low-Income Housing Tax Credit (LIHTC). Using data from the National Health Interview Survey (2004-16) linked to data on LIHTC properties (1987-2016), this study provides national estimates for health care access and health status among low-income children living in LIHTC properties compared with low-income children not living in LIHTC properties. Children living in LIHTC properties were more likely to have had a well-child visit in the past twelve months and a dental visit in the past six months. These children also had a higher likelihood of chronic school absenteeism and current asthma. These exploratory findings suggest that policy makers should consider features of LIHTC policy as possible mechanisms to improve low-income children's health care access and health status while addressing the shortage of affordable housing in the US.

Project description:Young adults are prone to psychological stress and anxiety induced by major transitions to adulthood. While employment has predominated in previous research on the social determinants of young people's mental health, this study examines the association between young people's housing problems and mental health in the context of an unaffordable housing market. Using the Survey on the Living Conditions and Welfare Needs of Youths (n = 1308) in Korea, the study found that perceived poor housing quality and material hardship are negatively associated with the mental health of young adults living independently. Specifically, while poor housing quality and material hardship induced by housing cost burden were negatively associated with single-person households' mental health, only poor housing quality was associated with non-single-person households' mental health. This study is one of the few studies examining the linkage between housing problems and mental health of young adults and informs the interventions aimed at promoting the psychological well-being of young adults in the transition from parents' homes to independent living.

Project description:Background and aimExposure to lead can harm a child's health, including damage to the nervous system, delayed growth, hearing loss, and many other adverse health effects, as well as implications for social, economic, educational and social well-being. Lead exposure in children is still a concern and cases require public health management to find the exposure source and interrupt the exposure pathway. Housing characteristics can indicate the presence of lead-contaminated paint and leaded water supply pipes. We aimed to explore the relationship between housing characteristics and elevated blood lead concentration (BLC) in children in England.MethodsWe used a retrospective cohort design and included all cases of lead exposure in children reported to the UK Health Security Agency between 2014 and 2020 via surveillance. A case was a child aged under 16 years, resident in England, BLC of ≥ 0.48 μmol/L (10 µg/dL) and referred for public health management. We collected case demographic details and housing characteristics (age and type). We explored associations between elevated BLC and risk factors, using generalised linear mixed effects models and compared cases' housing type to that expected nationally.ResultsTwo hundred and sixty-six out of 290 cases met the case definition. There was no difference in BLCs between genders, age groups, deprivation, and housing type. After adjusting for reporting source, housing age and type, cases residing in housing built pre-1976 had a BLC of 0.32 (95%CI 0.02, 0.63) µmols/L (6.63 (95%CI 0.42, 13.0) µg/dL) higher than cases living in housing built after this time. Cases were 1.68 times more likely to be living in terraced housing (housing adjoined to one another) than other children and less likely to live in apartments and detached properties.ConclusionThis study suggests an association between housing characteristics and BLC in children. Housing age and type may act as a proxy for lead exposure risk through exposure to leaded paint, lead water pipes, and lead contaminated dust from indoor and outdoor sources. Public health action should consider targeting families more at risk in older housing by raising awareness of the potential presence of lead pipes and paint. Interventions should include working with wider stakeholders including other housing and environmental professionals, the private sector, as well as parents and carers.

Project description:ObjectiveTo pilot public health interventions at women potentially interested in maternity care via campaigns on social media (Twitter), social networks (Facebook), and online search engines (Google Search).Data sources/study settingPrimary data from Twitter, Facebook, and Google Search on users of these platforms in Los Angeles between March and July 2014.Study designObservational study measuring the responses of targeted users of Twitter, Facebook, and Google Search exposed to our sponsored messages soliciting them to start an engagement process by clicking through to a study website containing information on maternity care quality information for the Los Angeles market.Principal findingsCampaigns reached a little more than 140,000 consumers each day across the three platforms, with a little more than 400 engagements each day. Facebook and Google search had broader reach, better engagement rates, and lower costs than Twitter. Costs to reach 1,000 targeted users were approximately in the same range as less well-targeted radio and TV advertisements, while initial engagements-a user clicking through an advertisement-cost less than $1 each.ConclusionsOur results suggest that commercially available online advertising platforms in wide use by other industries could play a role in targeted public health interventions.

Project description:The UK, and other high-income countries, are experiencing substantial increases in living costs. Several overlapping and intersecting economic crises threaten physical and mental health in the immediate and longer term. Policy responses may buffer against the worst effects (e.g. welfare support) or further undermine health (e.g. austerity). We explore fundamental causes underpinning the cost-of-living crisis, examine potential pathways by which the crisis could impact population health and use a case study to model potential impacts of one aspect of the crisis on a specific health outcome. Our modelling illustrates how policy approaches can substantially protect health and avoid exacerbating health inequalities. Targeting support at vulnerable households is likely to protect health most effectively. The current crisis is likely to be the first of many in era of political and climate uncertainty. More refined integrated economic and health modelling has the potential to inform policy integration, or 'health in all policies'.

Project description:Housing conditions have been an enduring focus for public health activity throughout the modern public health era. However, the nature of the housing and health challenge has changed in response to an evolution in the understanding of the diverse factors influencing public health. Today, the traditional public health emphasis on the type and quality of housing merges with other wider determinants of health. These include the neighbourhood, community, and "place" where a house is located, but also the policies which make access to a healthy house possible and affordable for everyone. Encouragingly, these approaches to policy and action on housing have the potential to contribute to the "triple win" of health and well-being, equity, and environmental sustainability. However, more effective housing policies (and in public health in general) that adopt more systemic approaches to addressing the complex interactions between health, housing, and wider environment are needed. This paper illustrates some of the key components of the housing and health challenge in developed countries, and presents a conceptual model to co-ordinate activities that can deliver the "triple win." This is achieved by offering a perspective on how to navigate more effectively, inclusively and across sectors when identifying sustainable housing interventions.

Project description:Objective: Given that positive psychological capital motivates citizens to actively participate in social affairs, this study aims to provide insight into food safety risk management in China by empirically determining which individual characteristics are associated with positive psychological capital for actively participating in social co-governance. Methods: A questionnaire-based survey was undertaken between December 5 and 10, 2020. The study participants were residents of Wuxi in China over the age of 18 years. A validated and pretested questionnaires was used to elicit responses with the participants. Student's t-test and one-way analysis of variance were performed to determine which individual characteristics are significantly correlated with the psychological capital of citizens who participate in co-governance. Post-hoc multiple comparisons were performed for each individual characteristic with a significant correlation to determine which categories of these characteristics yielded the significant differences. Study data were analyzed using IBM SPSS Statistics 24.0. Results: A total of 752 completed responses were received. Most respondents were females (52.39%), aged 26-45 (66.09%), married (70.48%), company employees (44.28%), and in good health (89.76%). Most had a household size of 3 (55.98%), a bachelor's degree (40.96%), a personal annual income of more than 100,000 yuan (26.46%), and no children aged under 18 (50.27%) or pregnant women (93.22%) in their households. Data analysis indicated that education, income, and health status significantly associate with the psychological capital of citizens to participate in co-governance. Citizens with high education, high income, and good health status have higher psychological capital to participate in co-governance. Conclusion: The present study suggested citizens are likely to actively participate in food safety social co-governance only when they have at least one of the following three characteristics: (1) higher than average income in their city of residence; (2) a bachelor's degree or higher education; or (3) good health. Therefore, motivating citizens to participate in co-governance is a long-term process in China. The fundamental strategy is to increase the income of citizens, especially among low-income groups, promote education to improve the food safety literacy of the public, and improve sanitation and public health.
