Censored Quantile Instrumental Variable Estimates of the Price Elasticity of Expenditure on Medical Care.

ABSTRACT: Efforts to control medical care costs depend critically on how individuals respond to prices. I estimate the price elasticity of expenditure on medical care using a censored quantile instrumental variable (CQIV) estimator. CQIV allows estimates to vary across the conditional expenditure distribution, relaxes traditional censored model assumptions, and addresses endogeneity with an instrumental variable. My instrumental variable strategy uses a family member's injury to induce variation in an individual's own price. Across the conditional deciles of the expenditure distribution, I find elasticities that vary from -0.76 to -1.49, which are an order of magnitude larger than previous estimates.

Estimation of causal quantile effects with a binary instrumental variable and censored data.

Project description:The causal effect of a treatment is of fundamental interest in the social, biological, and health sciences. Instrumental variable (IV) methods are commonly used to determine causal treatment effects in the presence of unmeasured confounding. In this work, we study a new binary IV framework with randomly censored outcomes where we propose to quantify the causal treatment effect by the concept of complier quantile causal effect (CQCE). The CQCE is identifiable under weaker conditions than the complier average causal effect when outcomes are subject to censoring, and it can provide useful insight into the dynamics of the causal treatment effect. Employing the special characteristic of the binary IV and adapting the principle of conditional score, we uncover a simple weighting scheme that can be incorporated into the standard censored quantile regression procedure to estimate CQCE. We develop robust nonparametric estimation of the derived weights in the first stage, which permits stable implementation of the second stage estimation based on existing software. We establish rigorous asymptotic properties for the proposed estimator, and confirm its validity and satisfactory finite-sample performance via extensive simulations. The proposed method is applied to a bone marrow transplant dataset to evaluate the causal effect of rituximab in diffuse large B-cell lymphoma patients.

Assessing quantile prediction with censored quantile regression models.

Project description:An important goal of censored quantile regression is to provide reliable predictions of survival quantiles, which are often reported in practice to offer robust and comprehensive biomedical summaries. However, formal methods for evaluating and comparing working quantile regression models in terms of their performance in predicting survival quantiles have been lacking, especially when the working models are subject to model mis-specification. In this article, we proposes a sensible and rigorous framework to fill in this gap. We introduce and justify a predictive performance measure defined based on the check loss function. We derive estimators of the proposed predictive performance measure and study their distributional properties and the corresponding inference procedures. More importantly, we develop model comparison procedures that enable thorough evaluations of model predictive performance among nested or non-nested models. Our proposals properly accommodate random censoring to the survival outcome and the realistic complication of model mis-specification, and thus are generally applicable. Extensive simulations and a real data example demonstrate satisfactory performances of the proposed methods in real life settings.

The challenging interpretation of instrumental variable estimates under monotonicity.

Project description:BackgroundInstrumental variable (IV) methods are often used to identify 'local' causal effects in a subgroup of the population of interest. Such 'local' effects may not be ideal for informing clinical or policy decision making. When the instrument is non-causal, additional difficulties arise for interpreting 'local' effects. Little attention has been paid to these difficulties, even though commonly proposed instruments in epidemiology are non-causal (e.g. proxies for physician's preference; genetic variants in some Mendelian randomization studies).MethodsFor IV estimates obtained from both causal and non-causal instruments under monotonicity, we present results to help investigators pose four questions about the local effect estimates obtained in their studies. (1) To what subgroup of the population does the effect pertain? Can we (2) estimate the size of or (3) describe the characteristics of this subgroup relative to the study population? (4) Can the sensitivity of the effect estimate to deviations from monotonicity be quantified?ResultsWe show that the common interpretations and approaches for answering these four questions are generally only appropriate in the case of causal instruments.ConclusionsAppropriate interpretation of an IV estimate under monotonicity as a 'local' effect critically depends on whether the proposed instrument is causal or non-causal. The results and formal proofs presented here can help in the transparent reporting of IV results and in enhancing the use of IV estimates in informing decision-making efforts.

A Bayesian approach for instrumental variable analysis with censored time-to-event outcome.

Project description:Instrumental variable (IV) analysis has been widely used in economics, epidemiology, and other fields to estimate the causal effects of covariates on outcomes, in the presence of unobserved confounders and/or measurement errors in covariates. However, IV methods for time-to-event outcome with censored data remain underdeveloped. This paper proposes a Bayesian approach for IV analysis with censored time-to-event outcome by using a two-stage linear model. A Markov chain Monte Carlo sampling method is developed for parameter estimation for both normal and non-normal linear models with elliptically contoured error distributions. The performance of our method is examined by simulation studies. Our method largely reduces bias and greatly improves coverage probability of the estimated causal effect, compared with the method that ignores the unobserved confounders and measurement errors. We illustrate our method on the Women's Health Initiative Observational Study and the Atherosclerosis Risk in Communities Study.

HIGH DIMENSIONAL CENSORED QUANTILE REGRESSION.

Project description:Censored quantile regression (CQR) has emerged as a useful regression tool for survival analysis. Some commonly used CQR methods can be characterized by stochastic integral-based estimating equations in a sequential manner across quantile levels. In this paper, we analyze CQR in a high dimensional setting where the regression functions over a continuum of quantile levels are of interest. We propose a two-step penalization procedure, which accommodates stochastic integral based estimating equations and address the challenges due to the recursive nature of the procedure. We establish the uniform convergence rates for the proposed estimators, and investigate the properties on weak convergence and variable selection. We conduct numerical studies to confirm our theoretical findings and illustrate the practical utility of our proposals.

Quantile regression for doubly censored data.

Project description:Double censoring often occurs in registry studies when left censoring is present in addition to right censoring. In this work, we propose a new analysis strategy for such doubly censored data by adopting a quantile regression model. We develop computationally simple estimation and inference procedures by appropriately using the embedded martingale structure. Asymptotic properties, including the uniform consistency and weak convergence, are established for the resulting estimators. Moreover, we propose conditional inference to address the special identifiability issues attached to the double censoring setting. We further show that the proposed method can be readily adapted to handle left truncation. Simulation studies demonstrate good finite-sample performance of the new inferential procedures. The practical utility of our method is illustrated by an analysis of the onset of the most commonly investigated respiratory infection, Pseudomonas aeruginosa, in children with cystic fibrosis through the use of the U.S. Cystic Fibrosis Registry.

Comparison of variance estimators for meta-analysis of instrumental variable estimates.

Project description:Mendelian randomization studies perform instrumental variable (IV) analysis using genetic IVs. Results of individual Mendelian randomization studies can be pooled through meta-analysis. We explored how different variance estimators influence the meta-analysed IV estimate.Two versions of the delta method (IV before or after pooling), four bootstrap estimators, a jack-knife estimator and a heteroscedasticity-consistent (HC) variance estimator were compared using simulation. Two types of meta-analyses were compared, a two-stage meta-analysis pooling results, and a one-stage meta-analysis pooling datasets.Using a two-stage meta-analysis, coverage of the point estimate using bootstrapped estimators deviated from nominal levels at weak instrument settings and/or outcome probabilities ? 0.10. The jack-knife estimator was the least biased resampling method, the HC estimator often failed at outcome probabilities ? 0.50 and overall the delta method estimators were the least biased. In the presence of between-study heterogeneity, the delta method before meta-analysis performed best. Using a one-stage meta-analysis all methods performed equally well and better than two-stage meta-analysis of greater or equal size.In the presence of between-study heterogeneity, two-stage meta-analyses should preferentially use the delta method before meta-analysis. Weak instrument bias can be reduced by performing a one-stage meta-analysis.

Medical care price indexes for patients with employer-provided insurance: nationally representative estimates from MarketScan Data.

Project description:ObjectiveCommonly observed shifts in the utilization of medical care services to treat diseases may pose problems for official price indexes at the Bureau of Labor Statistics (BLS) that do not account for service shifts. We examine how these shifts may lead to different price estimates than those observed in official price statistics at the BLS.Data sourcesWe use a convenience sample of enrollees with employer-provided insurance from the MarketScan database for the years 2003 to 2007. Population weights that consider the age, sex, and geographic distribution of enrollees are assigned to construct representative estimates.Study designWe compare two types of price indexes: (1) a Service Price Index (SPI) that is similar to the BLS index, which holds services fixed and measures the prices of the underlying treatments; (2) a Medical Care Expenditure Index (MCE) that measures the cost of treating diseases and allows for utilization shifts.Principal findingsOver the entire period of study the CAGR of the SPI grows 0.7 percentage points faster than the preferred MCE index.ConclusionsOur findings suggest that the health component of inflation may be overstated by 0.7 percentage points per year, and real GDP growth may be understated by a similar amount. However, more work may be necessary to precisely replicate the indexes of the BLS to obtain a more accurate measure of these price differences.

Does cigarette demand respond to price increases in Uganda? Price elasticity estimates using the Uganda National Panel Survey and Deaton's method.

Project description:ObjectiveTo provide the first published estimates of the price elasticity of demand for cigarettes in Uganda and thereby contribute to growing the evidence base of the likely impact of excise taxes on cigarette consumption and tax revenues in Sub-Saharan Africa.MethodWe use a linear approximation of the Almost Ideal Demand System along with expenditure data from the Uganda National Panel Survey and exploit the fact that prices of cigarettes vary across geographical space in Uganda.ResultsWe find that cigarettes are price inelastic in Uganda with elasticity estimates ranging between -0.26 and -0.33. That is, we expect that cigarette demand will decline by between 2.6% and 3.3% every time cigarette prices rise by 10%. These elasticity estimates are in line with international evidence and are robust to outliers in the data.ConclusionOur estimates of the price elasticity of demand for cigarettes suggest that the authorities in Uganda can reduce cigarette consumption and simultaneously increase tax revenues by increasing the excise taxes on cigarettes.

Censored quantile regression with recursive partitioning-based weights.

Project description:Censored quantile regression provides a useful alternative to the Cox proportional hazards model for analyzing survival data. It directly models the conditional quantile of the survival time and hence is easy to interpret. Moreover, it relaxes the proportionality constraint on the hazard function associated with the popular Cox model and is natural for modeling heterogeneity of the data. Recently, Wang and Wang (2009. Locally weighted censored quantile regression. Journal of the American Statistical Association 103, 1117-1128) proposed a locally weighted censored quantile regression approach that allows for covariate-dependent censoring and is less restrictive than other censored quantile regression methods. However, their kernel smoothing-based weighting scheme requires all covariates to be continuous and encounters practical difficulty with even a moderate number of covariates. We propose a new weighting approach that uses recursive partitioning, e.g. survival trees, that offers greater flexibility in handling covariate-dependent censoring in moderately high dimensions and can incorporate both continuous and discrete covariates. We prove that this new weighting scheme leads to consistent estimation of the quantile regression coefficients and demonstrate its effectiveness via Monte Carlo simulations. We also illustrate the new method using a widely recognized data set from a clinical trial on primary biliary cirrhosis.
