Detection of incorrect manufacturer labelling of hip components.

ABSTRACT: We describe the case of a 53-year-old man who underwent a left metal-on-metal hip resurfacing in 2015. Component size mismatch (CSM) was suspected because of the patient's immediate post-operative mechanical symptoms and high metal ion levels. Surgical notes indicated the appropriate combinations of implants were used. However, we detected a mismatch using computed tomography. Revision was performed and subsequent measurements of explanted components confirmed the mismatch. To our knowledge, this case is the first report of a CT method being used in a patient to pre-operatively identify CSM.

DNA barcoding reveals incorrect labelling of insects sold as food in the UK.

Project description:BackgroundInsects form an established part of the diet in many parts of the world and insect food products are emerging into the European and North American marketplaces. Consumer confidence in product is key in developing this market, and accurate labelling of content identity is an important component of this. We used DNA barcoding to assess the accuracy of insect food products sold in the UK.MethodsWe purchased insects sold for human consumption from online retailers in the UK and compared the identity of the material ascertained from DNA barcoding to that stated on the product packaging. To this end, the COI sequence of mitochondrial DNA was amplified and sequenced, and compared the sequences produced to reference sequences in NCBI and the Barcode of Life Data System (BOLD).ResultsThe barcode identity of all insects that were farmed was consistent with the packaging label. In contrast, disparity between barcode identity and package contents was revealed in two cases of foraged material (mopane worm and winged termites). One case of very broad family-level description was also highlighted, where material described as grasshopper was identified as Locusta migratoria from DNA barcode.ConclusionOverall these data indicate the need to establish tight protocols to validate product identity in this developing market. Maintaining biosafety and consumer confidence rely on accurate and consistent product labelling that provides a clear chain of information from producer to consumer.

High Rates of Aseptic Loosening in Modern Posterior-stabilized Femoral Components From a Single Manufacturer

Project description:Background Total knee arthroplasty (TKA) is one of the most commonly performed surgical procedures in the United States. There are concerns and reports of early aseptic loosening due to debonding at the cement-implant interface in a specific posterior-stabilized femoral design. This study describes failure at the cement-implant interface with 2 femoral implant designs produced by a single manufacturer. Methods This is a review of failed primary total knee arthroplasties performed with Optetrak Logic and Truliant posterior-stabilized femoral components between 2010 and 2020. Cases with revision surgery performed for femoral component loosening were reviewed. Results Seventeen of 896 knees were revised for femoral component loosening. The mean time from index arthroplasty to revision was 73 months. Patients presented with knee pain and recurrent aseptic effusions. More than half of patients had no evidence of component loosening on plain radiographs. Bone scans were positive in 77% of patients with loosening. Femoral components were easily disimpacted from the cement mantle at the time of revision surgery. Patients were treated with femoral only revision or femoral and tibial revision. Two complications occurred in this series. Conclusions Providers should maintain vigilance in the surveillance of patients with these implants. Clinicians should have a low threshold for additional diagnostic testing if these patients develop pain and/or recurrent effusions. Isolated femoral or complete revision both seem to be acceptable treatment strategies for this failure mechanism.

Multithreaded two-pass connected components labelling and particle analysis in ImageJ.

Project description:Sequential region labelling, also known as connected components labelling, is a standard image segmentation problem that joins contiguous foreground pixels into blobs. Despite its long development history and widespread use across diverse domains such as bone biology, materials science and geology, connected components labelling can still form a bottleneck in image processing pipelines. Here, I describe a multithreaded implementation of classical two-pass sequential region labelling and introduce an efficient collision resolution step, 'bucket fountain'. Code was validated on test images and against commercial software (Avizo). It was performance tested on images from 2 MB (161 particles) to 6.5 GB (437 508 particles) to determine whether theoretical linear scaling (O(n)) had been achieved, and on 1-40 CPU threads to measure speed improvements due to multithreading. The new implementation achieves linear scaling (b = 0.905-1.052, time ∝ pixelsb ; R 2 = 0.985-0.996), which improves with increasing thread number up to 8-16 threads, suggesting that it is memory bandwidth limited. This new implementation of sequential region labelling reduces the time required from hours to a few tens of seconds for images of several GB, and is limited only by hardware scale. It is available open source and free of charge in BoneJ.

Constrained components for the unstable hip following total hip arthroplasty: a literature review.

Project description:Patients with chronic instability or late dislocation following total hip arthroplasty often require operative management. Unfortunately, there is an increased risk of recurrent dislocation following revision in these patients. Over the past decade the use of constrained devices for patients with chronic instability has gained increased interest; however, there is a paucity of studies available in the literature regarding the use of these devices. The purpose of this study was to analyze the available literature over the past 15 years, focusing on larger, long-term studies, to obtain recommendations from the respective articles for indications and contraindications for the use of constrained devices. Our review of eight reports included 1,199 hips in 1,148 patients with a total mean follow-up of 51 months (range, 24 to 124 months). The mean rate of dislocation following revision with a constrained liner was 10% and the mean re-operation rate for reasons other than dislocation was 4%. We concluded that constrained liners are an option for patients who have failed management of instability with other implants, those with instability of unclear etiology, those with cognitive problems who are unable to follow dislocation precautions, those with deficient abductors, and elderly or low-demand individuals with well-positioned implants requiring revision.

The Availability and Quality of Food Labelling Components in the Canadian E-Grocery Retail Environment.

Project description:Although packaged foods sold in retail stores must follow food labelling regulations, there are no e-grocery food labelling regulations to mandate and standardize the availability and presentation of product information. Therefore, the objective of the study was to evaluate the availability and quality of food labelling components in the Canadian e-grocery retail environment. A sample of fresh and pre-packaged products was identified on eight leading grocery retail websites in Canada, to assess the availability and quality of food labelling components. Out of 555 product searches, all products were accompanied by product images with front-of-pack images more readily available (96.0%) than back-of-pack (12.4%) and other side panel images (3.1%). The following mandatory nutrition information was available for 61.1% of the products: nutrition facts table (68.8%), ingredient (73.9%), and allergen (53.8%) information. The majority of the nutrition information was available after scrolling down, clicking additionally on the description page, or viewing only as an image. Date markings were not available; packaging material information was available for 2.0% of the products. There was wide variability and inconsistencies in the presentation of food labelling components in the e-grocery retail environment, which can be barriers in enabling Canadians to make informed purchasing decisions.

Resiliency of Mutualistic Supplier-Manufacturer Networks.

Project description:Current Supplier-Manufacturer (SM) networks are highly complex and susceptible to local and global disruptions, due to connectivity and interdependency among suppliers and manufacturers. Resiliency of supply chains is critical for organizations to remain operational in the face of disruptive events. Existing quantitative analyses oversimplify the mutualistic nature of SM networks, in which failure of individual entities affects not only the directly connected entities but also those connected indirectly. In this work we investigate resiliency of SM networks using the quantitative methods employed to study mutualistic ecological systems. Much like in ecological systems, catastrophic failures of SM networks are difficult to predict due to high dimensionality of their interactive space. To address this, first we create a bipartite representation and generate a multidimensional nonlinear model that captures the dynamics of a SM network. We transform the multidimensional model into a two-dimensional model without sacrificing the model's ability to predict the point of collapse. We extensively validate the model using real-world global automotive SM networks. We observe that the resiliency of a SM network depends on both the network structure and parameters. The current work offers a means for designing resilient supply chains that can remain robust to local and global perturbations.

Simultaneous detection of multiple DNA damage types by multi-colour fluorescent labelling.

Project description:Herein we present an assay allowing concurrent detection of oxidative DNA damage and photoproducts. We apply DNA repair enzymes specific for each lesion type to incorporate spectrally distinct fluorescent nucleotides, enabling simultaneous quantification of the lesions on individual DNA molecules. We follow the repair of both damage types in skin cells exposed to artificial sunlight.

Application of stable-isotope labelling techniques for the detection of active diazotrophs.

Project description:Investigating active participants in the fixation of dinitrogen gas is vital as N is often a limiting factor for primary production. Biological nitrogen fixation is performed by a diverse guild of bacteria and archaea (diazotrophs), which can be free-living or symbionts. Free-living diazotrophs are widely distributed in the environment, yet our knowledge about their identity and ecophysiology is still limited. A major challenge in investigating this guild is inferring activity from genetic data as this process is highly regulated. To address this challenge, we evaluated and improved several 15 N-based methods for detecting N2 fixation activity (with a focus on soil samples) and studying active diazotrophs. We compared the acetylene reduction assay and the 15 N2 tracer method and demonstrated that the latter is more sensitive in samples with low activity. Additionally, tracing 15 N into microbial RNA provides much higher sensitivity compared to bulk soil analysis. Active soil diazotrophs were identified with a 15 N-RNA-SIP approach optimized for environmental samples and benchmarked to 15 N-DNA-SIP. Lastly, we investigated the feasibility of using SIP-Raman microspectroscopy for detecting 15 N-labelled cells. Taken together, these tools allow identifying and investigating active free-living diazotrophs in a highly sensitive manner in diverse environments, from bulk to the single-cell level.

Direct detection of circulating microRNA-122 using dynamic chemical labelling with single molecule detection overcomes stability and isomiR challenges for biomarker qualification

Project description:Circulating microRNAs are biomarkers reported to be stable and translational acrossspecies. miR-122 (miR-122-5p) is a hepatocyte-specific microRNA biomarker for drug-induced liver injury (DILI). Our objective was to develop an extraction-free andamplification-free detection method for measuring miR-122 that has translationalutility in context of DILI. We developed a single molecule dynamic chemical labelling(DCL) assay based on miR-122 hybridization to an abasic peptide nucleic acid probethat contained a reactive amine instead of a nucleotide at a specific position in thesequence. The single molecule DCL assay specifically measured miR-122 directly from10 μL of serum or plasma without any extraction steps, with a fit-for-purpose limit ofdetection of 1.32 pM. In 192 human serum samples, DCL accurately identified patientsat risk of DILI (area under ROC curve 0.98 (95%CI 0.96-1), P<0.0001). The miR-122assay also quantified liver injury in rats and dogs. When DCL beads were added toserum, the miR-122 signal was stabilised (no loss of signal after 14 days at roomtemperature). By contrast, there was substantial degradation of miR-122 in the absenceof beads (≈60% lost in 1 day). RNA sequencing demonstrated the presence of multiplemiR-122 isomiRs with DILI that were at low concentration or not present in healthypatient serum. Sample degradation over time produced more isomiRs, particularlyrapidly with DILI. PCR was inaccurate when analysing miR-122 isomiRs, whereas theDCL assay demonstrated accurate quantification. In summary, the DCL assay canaccurately measure miR-122 directly from serum and plasma to diagnose liver injury inhumans and other species, and can overcome important microRNA biomarker analyticaland biological challenges.

Tet-mediated covalent labelling of 5-methylcytosine for its genome-wide detection and sequencing.

Project description:5-methylcytosine is an epigenetic mark that affects a broad range of biological functions in mammals. The chemically inert methyl group prevents direct labelling for subsequent affinity purification and detection. Therefore, most current approaches for the analysis of 5-methylcytosine still have limitations of being either density-biased, lacking in robustness and consistency, or incapable of analysing 5-methylcytosine specifically. Here we present an approach, TAmC-Seq, which selectively tags 5-methylcytosine with an azide functionality that can be further labelled with a biotin for affinity purification, detection and genome-wide mapping. Using this covalent labelling approach, we demonstrate high sensitivity and specificity for known methylated loci, as well as increased CpG dinucleotide coverage at lower sequencing depth as compared with antibody-based enrichment, providing an improved efficiency in the 5-methylcytosine enrichment and genome-wide profiling.
