Microdissected Prefabricated Flap: An Evolution in Flap Prefabrication.

ABSTRACT: When traditional flap techniques are not feasible, we apply flap prefabrication, which is more complicated and sophisticated but supplies large and thin flaps. There are some disadvantages to the technique that require improvement, such as venous congestion after flap transfer, which requires months for neoangiogenesis and necessitates a vascular carrier. Here, the author presents a new technique, called as 'microdissected prefabricated flap,' to successfully produce a safe, large, and thin flap. This technique is based on the microdissection of the perforators to the greatest extent possible, spreading them out into the subdermal level and using them as a carrier. The details and the application of this technique are presented and reported.

Research on the tripartite evolution strategy of prefabricated building promotion based on the deepening of demand-side interests.

Project description:Under construction industry upgrading and environmental protection requirements, promoting prefabricated buildings is very important, but its development varies significantly in different regions. For example, it only accounts for 20% of the new construction area in China, and the overall development level is low. The promotion of prefabricated buildings involves multiple interests. Then, how to encourage all stakeholders to work together to promote its development is a crucial issue. This study obtains an evolutionary game model among the government, developers, and purchasers. Then the stability strategies of the stakeholders are given conditions. Finally, numerical simulation is used to validate theoretical findings and determine the sensitivity of key parameters to a subject's behavior. The results show that: (1) There are two key factors that restrict the cooperation of stakeholders to promote prefabricated buildings, including the lack of government subsidies and the weak demand of the purchasers; (2) The government's overall subsidy should not be too high, exceeding the government's expenditure on environmental protection, and the subsidies for developers and purchasers must be reasonably allocated to effectively compensate for development benefits and purchase costs under limited expenditures. (3) Improving the residential environment of prefabricated buildings can significantly enhance the vested interests of purchasers, make up for their purchase costs, increase their willingness to purchase, and thus reduce the sales risk of developers.

The lateral port control pharyngeal flap: a thirty-year evolution and followup.

Project description:In 1971, Micheal Hogan introduced the Lateral Port Control Pharyngeal Flap (LPCPF) which obtained good results with elimination of VPI. However, there was a high incidence of hyponasality and OSA. We hypothesized that preoperative assessment with videofluoroscopy and nasal endoscopy would enable modification and customization of the LPCPF and result in improvement in the result in both hyponasality and obstructive apnea while still maintaining results in VPI. Thirty consecutive patients underwent customized LPCPF. All patients had preoperative diagnosis of VPI resulting from cleft palate. Patient underwent either videofluoroscopy or nasal endoscopy prior to the planning of surgery. Based on preoperative velar and pharyngeal movement, patients were assigned to wide, medium, or narrow port designs. Patients with significant lateral motion were given wide ports while patients with minimal movement were given narrow ports. There was a 96.66% success rate in the treatment of VPI with one patient with persistent VPI (3.33%). Six patients had mild hyponasality (20 %). Two patients had initial OSA (6.67%), one of which had OSA which lasted longer than six months (3.33%). The modifications of the original flap description have allowed for success in treatment of VPI along with an acceptably low rate of hyponasality and OSA.

Marine Spongin: Naturally Prefabricated 3D Scaffold-Based Biomaterial.

Project description:The biosynthesis, chemistry, structural features and functionality of spongin as a halogenated scleroprotein of keratosan demosponges are still paradigms. This review has the principal goal of providing thorough and comprehensive coverage of spongin as a naturally prefabricated 3D biomaterial with multifaceted applications. The history of spongin's discovery and use in the form of commercial sponges, including their marine farming strategies, have been analyzed and are discussed here. Physicochemical and material properties of spongin-based scaffolds are also presented. The review also focuses on prospects and trends in applications of spongin for technology, materials science and biomedicine. Special attention is paid to applications in tissue engineering, adsorption of dyes and extreme biomimetics.

Minimally Invasive Diastema Restoration with Prefabricated Sectional Veneers.

Project description:This case report presents a new technique for sectional veneer fabrication and diastema restoration with a prefabricated composite veneer. For the purpose of diastema restoration, a prefabricated sectional veneer provides the same benefits of a traditional ceramic sectional veneer (highly esthetic restoration with no need for tooth preparation) but involves a less technically demanding and time-consuming clinical procedure and a less delicate restoration with a reduced risk of accidental breakage and post-bonding crack formation. The technique presented in this case report bridges the gap between a direct and indirect technique for diastema restoration and introduces a new treatment option to close anterior spacing with a highly esthetic sectional veneer in a predictable and timely manner.

Production scheduling of prefabricated components considering delivery methods.

Project description:To address the processing scheduling problem involving multiple molds, components, and floors, we propose the Genetic Grey Wolf Optimizer (GGA) as a means to optimize the production scheduling of components in a production line. This approach combines the Grey Wolf algorithm with the genetic algorithm. Previous methods have overlooked the storage requirements arising from the delivery characteristics of prefabricated components, often resulting in unnecessary storage costs. Intelligent algorithms have been demonstrated to be effective in production scheduling, and thus, to enhance the efficiency of prefabricated component production scheduling, our study presents a model incorporating a production objective function. This model takes into account production resources and delivery characteristics constraints. Subsequently, we develop a hybrid algorithm, combining the grey wolf algorithm with the genetic algorithm, to search for the optimal solution with a minimal storage cost. We validate the model using a case study, and the experimental results demonstrate that GAGWO successfully identifies the best precast production schedule. Furthermore, the precast production plan, considering the delivery method, is found to be reasonable.

Evolution of drug resistance drives destabilization of flap region dynamics in HIV-1 protease.

Project description:The HIV-1 protease is one of several common key targets of combination drug therapies for human immunodeficiency virus infection and acquired immunodeficiency syndrome. During the progression of the disease, some individual patients acquire drug resistance due to mutational hotspots on the viral proteins targeted by combination drug therapies. It has recently been discovered that drug-resistant mutations accumulate on the "flap region" of the HIV-1 protease, which is a critical dynamic region involved in nonspecific polypeptide binding during invasion and infection of the host cell. In this study, we utilize machine learning-assisted comparative molecular dynamics, conducted at single amino acid site resolution, to investigate the dynamic changes that occur during functional dimerization and drug binding of wild-type and common drug-resistant versions of the main protease. We also use a multiagent machine learning model to identify conserved dynamics of the HIV-1 main protease that are preserved across simian and feline protease orthologs. We find that a key conserved functional site in the flap region, a solvent-exposed isoleucine (Ile50) that controls flap dynamics is functionally targeted by drug resistance mutations, leading to amplified molecular dynamics affecting the functional ability of the flap region to hold the drugs. We conclude that better long-term patient outcomes may be achieved by designing drugs that target protease regions that are less dependent upon single sites with large functional binding effects.

Performance of a prestressed efficiently prefabricated beam-column connection.

Project description:Analyzing the seismic performance and flexural capacity of beam-column joints is crucial in structural design phase. The purpose of this paper is to investigate the seismic performance and flexural capacity of precast prestressed efficiently fabricated frame (PPEFF) joints. Reverse cyclic load tests and flexural capacity analysis are conducted. The damage modes, hysteresis curves, skeleton curves, stiffness degradation, ductility, and energy dissipation capacity of five PPEFF joint specimens with different reinforcement rates of the energy-dissipating bars and shear reinforcement are obtained. The results show that the damage pattern of the specimen is ideal, i.e., the plastic hinge region at the end of the beam is severely damaged, whereas the remainder of the beam is slightly damaged. Increasing the reinforcement rate of the energy-consuming steel bars enhances the load capacity, energy dissipation capacity, and initial stiffness of the joint but reduces the ductility performance. The maximum change in ductility was 5.31 for the reinforcement rate of energy-consuming steel bars ranging from 0.38% to 0.59%. In addition, the flexural capacity of the PPEFF joint is evaluated, considering the influence of the shear steel on the yielding and ultimate states. An equation of the flexural capacity is derived. A good agreement is observed between the experimental and calculation results, verifying the correctness of the proposed flexural capacity equation.

High-throughput sequencing of microdissected chromosomal regions.

Project description:The linkage of disease gene mapping with DNA sequencing is an essential strategy for defining the genetic basis of a disease. New massively parallel sequencing procedures will greatly facilitate this process, although enrichment for the target region before sequencing remains necessary. For this step, various DNA capture approaches have been described that rely on sequence-defined probe sets. To avoid making assumptions on the sequences present in the targeted region, we accessed specific cytogenetic regions in preparation for next-generation sequencing. We directly microdissected the target region in metaphase chromosomes, amplified it by degenerate oligonucleotide-primed PCR, and obtained sufficient material of high quality for high-throughput sequencing. Sequence reads could be obtained from as few as six chromosomal fragments. The power of cytogenetic enrichment followed by next-generation sequencing is that it does not depend on earlier knowledge of sequences in the region being studied. Accordingly, this method is uniquely suited for situations in which the sequence of a reference region of the genome is not available, including population-specific or tumor rearrangements, as well as previously unsequenced genomic regions such as centromeres.

The flip-flap puzzle flap: Another recycling option.

Project description:Post-traumatic soft tissue defects sometimes require sequential flap coverage to achieve complete healing. In the era of propeller flaps, which were developed to reduce donor site morbidity, Feng et al. introduced the concept of the free-style puzzle flap, in which a previously harvested flap becomes its own donor site by recycling the perforator. However, when a perforator cannot be found with a Doppler device, we suggest performing a new type of flap, the flip-flap puzzle flap, which combines two concepts: the free-style puzzle flap and the flip-flap flap described by Voche et al. in the 1990s. We present the cases of three patients who achieved complete healing through this procedure.

Epigenetic analysis of laser capture microdissected fetal epithelia.

Project description:Laser capture microdissection (LCM) is a superior method for nondestructive collection of specific cell populations from tissue sections. Although DNA, RNA, and protein have been analyzed from LCM-procured samples, epigenetic analyses, particularly of fetal, highly hydrated tissue, have not been attempted. A standardized protocol with quality assurance measures was established to procure cells by LCM of the medial edge epithelia (MEE) of the fetal palatal processes for isolation of intact microRNA for expression analyses and genomic DNA (gDNA) for CpG methylation analyses. MicroRNA preparations, obtained using the RNAqueous Micro kit (Life Technologies), exhibited better yields and higher quality than those obtained using the Arcturus PicoPure RNA Isolation kit (Life Technologies). The approach was validated using real-time polymerase chain reaction (PCR) to determine expression of selected microRNAs (miR-99a and miR-200b) and pyrosequencing to determine CpG methylation status of selected genes (Aph1a and Dkk4) in the MEE. These studies describe an optimized approach for employing LCM of epithelial cells from fresh frozen fetal tissue that enables quantitative analyses of microRNA expression levels and CpG methylation.
