Role of Bcl-2 -938 C>A polymorphism in susceptibility and prognosis of cancer: a meta-analysis.

ABSTRACT: The association between B-cell lymphoma 2 (Bcl-2) polymorphism and cancer is under debate and remains elusive. This meta-analysis was performed to evaluate the relationships of Bcl-2 -938 C>A polymorphism (rs2279115) with susceptibility and prognosis of cancer. Odds ratios (ORs) were used to measure the association between Bcl-2 polymorphisms and cancer risk. Hazard ratios (HRs) were used to measure the association between Bcl-2 polymorphisms and cancer prognosis. On the basis of 26 studies about Bcl-2 -938C>A polymorphism and cancer, we found Bcl-2 -938 C>A polymorphism was significantly associated with increased cancer risk in dominant model (OR = 1.12, 95%CI: 1.00-1.25, P = 0.04), recessive model (OR = 1.38, 95%CI: 1.11-1.71, P = 0.004), allelic model (OR = 1.15, 95%CI: 1.04-1.28, P = 0.007) and homozygote comparison(OR = 1.44, 95%CI: 1.11-1.87, P = 0.006). Furthermore, Bcl-2 -938 C>A polymorphism was significantly associated with increased cancer risk in Asians but not in Caucasians. Moreover, Bcl-2 -938 C>A polymorphism was not significantly associated with the prognosis of cancer (AA vs CA: OR = 0.99, 95%CI: 0.77-1.27, P = 0.93; AA vs CC: OR = 0.92, 95%CI: 0.65-1.30, P = 0.63; AC vs CC: OR = 0.94, 95%CI: 0.80-1.11, P = 0.48; CC vs AA+CA: OR = 1.21, 95%CI: 0.69-2.13, P = 0.50; AA vs CC+CA: OR = 0.99, 95%CI: 0.48-2.04, P = 0.97). Studies with larger samples and gene-environment interactions are needed to validate our findings.

Project description:Recent lung cancer studies have focused on identifying the effects of single nucleotide polymorphisms (SNPs) in candidate genes, among which DNA repair genes are increasingly being studied. Genetic variations in DNA repair genes are thought to modulate DNA repair capacity and are suggested to be related to lung cancer risk. In this study, we tried to assess reported studies of association between polymorphism of human 8-oxoguanine DNA glycosylase 1 (hOGG1) Ser326Cys and lung cancer. We conducted MEDLINE, Current Contents and Web of Science searches using "hOGG1", "lung cancer" and "polymorphism" as keywords to search for papers published (from January 1995 through August 2010). Data were combined using both a fixed effects (the inverse variance-weighted method) and a random effects (DerSimonian and Laird method) model. The Cochran Q test was used for the assessment of heterogeneity. Publication bias was assessed by both Begg's and Egger's tests. We identified 20 case-control studies in 21 different ethnic populations. As two studies were not in the Hardy-Weinberg equilibrium, 18 case-control studies in 19 different ethnic populations (7,792 cases and 9,358 controls) were included in our meta-analysis. Summary frequencies of the Cys allele among aucasians and Asians based on the random effects model were 20.9% (95% confidence interval (CI) = 18.9-22.9) and 46.1% (95% CI = 40.2-52.0), respectively. The distribution of the Cys allele was significantly different between Asians and Caucasians (P < 0.001). The Cys/Cys genotype was significantly associated with lung cancer risk in Asian populations (odds ratio = 1.27, 95% CI = 1.09-1.48) but not in Caucasian populations. This ethnic difference in lung cancer risk may be due to environmental factors such as cigarette smoking and dietary factors. Although the summary risk for developing lung cancer may not be large, lung cancer is such a common malignancy that even a small increase in risk can translate to a large number of excess lung cancer cases. As lung cancer is a multifactorial disease, further investigations of the gene-gene and gene-environment interactions on the hOGG1 polymorphism-associated lung cancer risk may help to better understand of the molecular pathogenesis of human lung cancer.

Project description:BackgroundAssociation between the single nucleotide polymorphism rs3218536 (known as Arg188His) located in the X-ray repair cross complementing group 2 (XRCC2) gene and cancer susceptibility has been widely investigated. However, results thus far have remained controversial. A meta-analysis was performed to identify the impact of this polymorphism on cancer susceptibility.MethodsPubMed and Embase databases were searched systematically until September 7, 2013 to obtain all the records evaluating the association between the XRCC2 Arg188His polymorphism and the risk of all types of cancers. We used the odds ratio (OR) as measure of effect, and pooled the data in a Mantel-Haenszel weighed random-effects meta-analysis to provide a summary estimate of the impact of this polymorphism on breast cancer, ovarian cancer and other cancers. All the analyses were carried out in STATA 12.0.ResultsWith 30868 cases and 38656 controls, a total of 45 case-control studies from 26 publications were eventually included in our meta-analysis. No significant association was observed between the XRCC2 Arg188His polymorphism and breast cancer susceptibility (dominant model: OR = 0.94, 95%CI = 0.86-1.04, P = 0.232). However, a significant impact of this polymorphism was detected on decreased ovarian cancer risk (dominant model: OR = 0.83, 95%CI = 0.73-0.95, P = 0.007). In addition, we found this polymorphism was associated with increased upper aerodigestive tract (UADT) cancer susceptibility (dominant model: OR = 1.51, 95%CI = 1.04-2.20, P = 0.032).ConclusionThe Arg188His polymorphism might play different roles in carcinogenesis of various cancer types. Current evidence did not suggest that this polymorphism was directly associated with breast cancer susceptibility. However, this polymorphism might contribute to decreased gynecological cancer risk and increased UADT cancer risk. More preclinical and epidemiological studies were still imperative for further evaluation.
