Project description:BackgroundThe os trigonum (OT)-the most common accessory bone of the foot-although usually asymptomatic, may cause posterior ankle impingement syndrome (PAIS), which may be a severely debilitating problem for recreational or competitive athletes. The aim of the present study was to evaluate effectiveness of posterior ankle arthroscopy and to assess the outcome in the treatment of PAIS secondary to OT impingement or OT fractures within a group of young athletes and their return to previous sports level.MethodsFrom 2011 to 2018, a retrospective review of 81 recreational athletes of mean age 27.8 years was performed. All patients were diagnosed with PAIS due to OT pathology and were operated on endoscopically with resection of the OT. Pre- and postoperative clinical evaluation were performed at 3 months, 1 year, and 2 years based on visual analog scale (VAS), ankle range of motion (ROM), American Orthopaedic Foot & Ankle Society (AOFAS) hindfoot score, and the Foot & Ankle Disability Index (FADI) scores, in a follow-up of at least 2 years.ResultsVAS score was significantly improved from an average of 7.5 (5-9) preoperatively to 1.9 (1-3) at 3 months postoperatively and to 0.6 (0-2) and 0.3 (0 -1) at 1 and 2 years postoperatively. Ankle ROM was significantly improved from an average of 24.8 (10-35) preoperatively to 58.0 (50-65) at 3 months postoperatively and to 64.0 (50-65) at 1 year and 64.7 (60-65) at 2 years postoperatively. AOFAS and FADI scores were significantly improved from 39.4 (18-55) and 49.7 (42.3-62.5) preoperatively to 85.2 (74-89) and 87.3 (81.7-88.5) postoperatively at 3 months to 97.7 (85-100) and 97.9 (93.3-100) postoperatively at 1 year, respectively (P < .001). Only 5 patients dropped to a lower activity level. There were 5 complications (4 transient).ConclusionEndoscopic treatment of PAIS due to OT pathology demonstrated excellent results. Posterior ankle arthroscopy was an effective treatment and allowed for a prompt return to a high activity level of their athletic performance.Level of evidenceLevel IV, therapeutic study / retrospective case series.

Project description:Subfibular impingement is a common cause of lateral heel pain after calcaneal fracture. It can be caused by calcaneofibular impingement, peroneal impingement, or a combination thereof. It may not be easy to differentiate bony impingement from soft-tissue impingement. Simultaneous lateral calcaneal ostectomy and peroneal tendon decompression may increase the chance of pain relief. The purpose of this Technical Note was to report the technical details of endoscopic lateral calcaneal ostectomy and peroneal tendon decompression with the patient in the prone position. This technique is indicated for symptomatic subfibular impingement recalcitrant to conservative treatment. It has the advantages of being minimally invasive surgery and allowing early mobilization of the foot and ankle.

Project description:Longitudinal flexor hallucis longus (FHL) tendon tears are sometimes complicated by posterior ankle impingement syndrome (PAIS), especially in ballet dancers. In recent years, PAIS has been treated endoscopically, but it is difficult to suture FHL tendon tears endoscopically. In this report, we describe how to suture the FHL tendon endoscopically with the Meniscal Viper Repair system (Arthrex, Naples, FL). Without our endoscopic technique, when a patient is found to have a longitudinal tear of the FHL under endoscopy, we must choose to either neglect the tear or convert to an open repair. Open tendon suture techniques have reportedly had relatively good results but require a longer skin incision than endoscopic surgery for PAIS. Compared with the open repair, the advantages of our technique include earlier recovery, less pain, a lower rate of soft tissue complications, and improved healing through better preservation of the blood supply. This technique is an attractive and useful option because it is an easy and safe method for longitudinal FHL tendon tears.

Project description:Background The classification of calcaneofibular ligament (CFL) injuries on magnetic resonance imaging (MRI) is time-consuming and subject to substantial interreader variability. This study explores the feasibility of classifying CFL injuries using deep learning methods by comparing them with the classifications of musculoskeletal (MSK) radiologists and further examines image cropping screening and calibration methods. Methods The imaging data of 1,074 patients who underwent ankle arthroscopy and MRI examinations in our hospital were retrospectively analyzed. According to the arthroscopic findings, patients were divided into normal (class 0, n=475); degeneration, strain, and partial tear (class 1, n=217); and complete tear (class 2, n=382) groups. All patients were divided into training, validation, and test sets at a ratio of 8:1:1. After preprocessing, the images were cropped using Mask region-based convolutional neural network (R-CNN), followed by the application of an attention algorithm for image screening and calibration and the implementation of LeNet-5 for CFL injury classification. The diagnostic effects of the axial, coronal, and combined models were compared, and the best method was selected for outgroup validation. The diagnostic results of the models in the intragroup and outgroup test sets were compared with those results of 4 MSK radiologists of different seniorities. Results The mean average precision (mAP) of the Mask R-CNN using the attention algorithm for the left and right image cropping of axial and coronal sequences was 0.90–0.96. The accuracy of LeNet-5 for classifying classes 0–2 was 0.92, 0.93, and 0.92, respectively, for the axial sequences and 0.89, 0.92, and 0.90, respectively, for the coronal sequences. After sequence combination, the classification accuracy for classes 0–2 was 0.95, 0.97, and 0.96, respectively. The mean accuracies of the 4 MSK radiologists in classifying the intragroup test set as classes 0–2 were 0.94, 0.91, 0.86, and 0.85, all of which were significantly different from the model. The mean accuracies of the MSK radiologists in classifying the outgroup test set as classes 0–2 were 0.92, 0.91, 0.87, and 0.85, with the 2 senior MSK radiologists demonstrating similar diagnostic performance to the model and the junior MSK radiologists demonstrating worse accuracy. Conclusions Deep learning can be used to classify CFL injuries at similar levels to those of MSK radiologists. Adding an attention algorithm after cropping is helpful for accurately cropping CFL images.

Project description:Posterior ankle endoscopy is a safe and effective approach for treatment of posterior ankle impingement. This is usually performed with the patient in prone position. The purpose of this technical note is to describe an arthroscopic approach of decompression of posterior ankle impingement with the patient in supine position. This is indicated if there is posterior ankle impingement together with other ankle pathology requiring anterior ankle arthroscopy. This approach allows treatment of both anterior ankle and posterior ankle pathology with the patient in the supine position. Concomitant anterior ankle arthroscopy can be performed with the usual orientation without the need of change of patient's position.

Project description:Malunion of calcaneal fractures is a common complication after improper nonoperative treatment. It may develop into posttraumatic subtalar osteoarthritis, lateral calcaneal wall prominence, and severe foot deformities. This Technical Note reports the technical aspects of lateral endoscopic calcaneoplasty with posterior arthroscopic subtalar arthrodesis for the treatment of malunited calcaneal fracture with lateral calcaneal wall prominence and posttraumatic subtalar osteoarthritis. This technique was chosen because it can reduce the risk of wound complication compared with a conventional lateral extensile approach. This Technical Note describes the use of the technique in a patient with prone position. This offers the advantages of easier access to the lateral surface of the calcaneus and posterior facet of the subtalar joint, and is less invasive to the soft tissue envelope on the lateral side of the foot. Moreover, the concomitant bone void area of the subtalar joint can be dealt with by an arthroscopic procedure.

Project description:BackgroundOs trigonum and Stieda process are common etiologies for posterior ankle impingement syndrome (PAIS), and diagnosis is typically made by radiographs, computed tomographic, or magnetic resonance imaging. However, these static tests may not detect associated soft tissue and bony pathologies. Posterior ankle and hindfoot arthroscopy (PAHA) is dynamic, providing at least ×8 magnification with full anatomical visualization. The primary aim of this study was to report the prevalence of associated conditions seen with trigonal impingement treated with PAHA.MethodsIn this retrospective comparative study, patients who underwent PAHA for PAIS due to trigonal impingement, from January 2011 to September 2016, were reviewed. Concomitant open posterior procedures and other indications for PAHA were excluded. Demographic data were collected with pre- and postoperative diagnosis, arthroscopic findings, type of impingement, location, associated procedures, and anatomical etiologies. Trigonal impingements were divided in os trigonal or Stieda and subgrouped as isolated, with flexor hallucis longus (FHL) disorders, with FHL plus other impingement, and with other impingement lesions.ResultsA total of 111 ankles were studied-74 os trigonum and 37 Stieda. Isolated trigonal disorders accounted for 15.3% of PAIS (n = 17). Cases having associated conditions had a mode of 3 additional pathologies. FHL disorders were found in 69.4%, subtalar impingement in 32.4%, posteromedial ankle synovitis in 25.2%, posterolateral ankle synovitis in 22.5%, and posterior inferior tibiofibular ligament impingement in 19.8% of cases. Associated pathologies were observed in 58.6% of cases when FHL was not considered. Significant differences were noted comparing os and Stieda (isolated: 20.3% to 5.4%, P = .040; FHL plus others: 35.1% to 59.5%, P = .015).ConclusionTrigonal bone (os trigonum or Stieda) was found to cause impingement in isolation in a small proportion of cases even when the FHL was considered part of the same disease spectrum. This should alert surgeons when considering removing trigonal impingement. Open approaches may limit the visualization and assessment of associated posterior ankle and subtalar pathoanatomy, thus possibly overlooking concomitant causes of PAIS.Level of evidenceLevel III, retrospective comparative study.

Project description:Accurate positioning of the total ankle arthroplasty implant components with the absence of any hindfoot deformity does not preclude the development of bony impingement. In cases of ankle stiffness after total ankle arthroplasty, the usual limitation is in dorsiflexion. If triceps surae contracture is excluded or persistent restriction remains in ankle dorsiflexion after gastrocnemius recession or tendo-Achilles lengthening, posterior ankle capsulectomy, debridement of posterior ankle gutter, and release of the deep posterior deltoid ligament and the posterior talofibular ligament are indicated. In this Technical Note, the technical details of endoscopic posterior ankle decompression and release after total ankle arthroplasty are described. Technique Video Video 1 Endoscopic posterior ankle decompression and release after total ankle arthroplasty of the left ankle. The patient is in the prone position. Posteromedial portal is the viewing portal and posterolateral portal is the working portal. The flexor hallucis longus (FHL) tendon is exposed. The arthroscope is switched to the posterolateral portal and the posterolateral ankle capsule is resected with arthroscopic instruments via the posteromedial portal. The FHL tendon is pushed medially by the instruments to facilitate resection of the more medial posterior ankle capsule. The arthroscope is switched back to the posteromedial portal and the most lateral part of the posterior ankle capsule is resected with arthroscopic shaver and radiofrequency wand via the posterolateral portal. Then, the FHL tendon is looped with a vascular sling via the posterolateral portal and the tendon is retracted laterally. The medial part of the posterior ankle capsule is resected with arthroscopic instruments via the posteromedial portal. The fibrous tissue of the posteromedial ankle gutter is also resected. The arthroscope is switched to the posteromedial portal and the fibrous tissue of the posterolateral ankle gutter is resected. The scarred posterolateral ankle ligamentous structure and fibrous band is released from its calcaneal insertion. The arthroscope is switched back to the posterolateral portal and the scarred posteromedial ankle ligamentous structure and fibrous band is carefully cut by a No. 15 scalpel with the cutting blade facing laterally.

Project description:Posterior ankle pain is a common foot and ankle problem faced by orthopaedic surgeons. It can have many causes, including posterior ankle joint pathologies (synovitis or osteochondral lesion), ankle impingement (posterior, posterolateral, or posteromedial), flexor hallucis longus pathologies, peroneal tendon problems, posterior tibial tendinopathy, Achilles tendon pathology, posterior subtalar tarsal coalition, or tarsal tunnel syndrome. Most of these pathologies can be managed effectively by an endoscopic approach. The classic endoscopic approach is the 2-portal posterior ankle endoscopy with the portals at the medial and lateral side of the Achilles tendon. This is performed with the patient in prone position. In this technical note, we describe the details of endoscopic approach to the posterior ankle via lateral portals. This allow flexibility of patient positioning (supine, prone, or lateral). It is particularly effective for the management of posteromedial ankle impingement without the risk of injury to the posterior tibial neurovascular bundle. Moreover, any concomitant peroneal tendon pathology can be managed via the same lateral portals.

Project description:BackgroundCalcaneofibular impingement is characterized by lateral hindfoot pain and is commonly resulting from calcaneal fracture malunion or severe flatfoot deformity. Lateral calcaneal wall decompression has been used successfully to relieve pain in patients who have calcaneofibular impingement. However, in cases of severe impingement and hindfoot valgus, lateral wall excision may leave only a small remnant of calcaneal bone for weightbearing and can lead to chronic heel pain. We describe a surgical technique using a medial displacement calcaneal osteotomy (MDCO) combined with a lateral wall exostectomy and report on the outcomes from our series of patients.MethodsRetrospective study of a single surgeon's patients was done from 2010 to 2020 who underwent medial slide calcaneal osteotomy and lateral wall exostectomy for calcaneofibular impingement. Descriptive statistics were used to summarize patient characteristics. Our study included 9 patients, 6 females and 3 males, with a mean age of 59 years (range: 19-77) and a mean follow-up of 62 weeks (range: 6-184).ResultsFive had an Achilles split approach, 2 had an oblique lateral approach, and 1 had an extensile lateral approach. Patients achieved radiographic relief of impingement and improvement in pain. Minor skin and soft tissue complications occurred in 3 patients, all of which were associated with laterally based incisions, and all resolved after a 10-day course of oral antibiotics. No major complications, emergency department visits, or readmissions occurred.ConclusionsMDCO and lateral wall exostectomy was a safe and effective treatment for severe calcaneofibular impingement.Level of evidenceLevel IV, retrospective case series.
