National Prevalence and Effects of Multiple Chemical Sensitivities.

ABSTRACT: OBJECTIVE:The aim of this study was to assess the prevalence of multiple chemical sensitivities (MCS), its co-occurrence with asthma and fragrance sensitivity, and effects from exposure to fragranced consumer products. METHODS:A nationally representative cross-sectional population-based sample of adult Americans (n?=?1137) was surveyed in June 2016. RESULTS:Among the population, 12.8% report medically diagnosed MCS and 25.9% report chemical sensitivity. Of those with MCS, 86.2% experience health problems, such as migraine headaches, when exposed to fragranced consumer products; 71.0% are asthmatic; 70.3% cannot access places that use fragranced products such as air fresheners; and 60.7% lost workdays or a job in the past year due to fragranced products in the workplace. CONCLUSION:Prevalence of diagnosed MCS has increased over 300%, and self-reported chemical sensitivity over 200%, in the past decade. Reducing exposure to fragranced products could help reduce adverse health and societal effects.

Project description:Multiple chemical sensitivities (MCS) is a medical condition associated with exposure to common chemical pollutants. The aims of this study are to assess the prevalence of MCS, its overlaps with asthma and fragrance sensitivity, and its health and societal effects in Australia. Data were collected in June 2016 using an on-line survey with a representative national sample (N?=?1098) of adults (ages 18-65) in Australia. Results found that, across the country, 6.5% report medically diagnosed MCS, 18.9% report chemical sensitivity (being unusually sensitive to everyday chemicals and chemically formulated products), and 19.9% either or both. Among people with MCS, 74.6% also have diagnosed asthma or an asthma-like condition, and 91.5% have fragrance sensitivity, reporting health problems (such as migraine headaches) when exposed to fragranced consumer products (such as air fresheners and cleaning supplies). In addition, among people with MCS, 77.5% are prevented from access to places because of fragranced products, 52.1% lost workdays or a job in the past year due to fragranced product exposure in the workplace, and 55.4% report health effects considered potentially disabling. Results indicate that MCS is a widespread disease, affecting an estimated 1 million adult Australians, with chemical sensitivity affecting another 2 million. Reducing chemical exposure to problematic sources, such as fragranced consumer products, is critical to reduce adverse effects.

Diverse sensitivities of TRPA1 from different mosquito species to thermal and chemical stimuli.

Project description:Temperature and odors profoundly affect the behavior of animals. Transient receptor potential channel, subfamily A, member 1 (TRPA1) functions as a polymodal nociceptor for sensing both vital environmental cues in insects. Mosquitoes are recognized as disease vectors, and many efforts have been devoted to investigations of their host-seeking behaviors and repellents. However, the physiological characteristics of mosquito TRPA1 have not been systematically studied. We identified multiple alternative splice variants of the TrpA1 gene from Anopheles gambiae, Anopheles stephensi, Aedes aegypti and Culex pipiens pallens mosquitoes. And we performed comparative analyses of the responses of mosquito TRPA1s to heat or chemical stimuli with calcium-imaging and whole-cell patch-clamp methods. Comparison of TRPA1 among four mosquito species from different thermal niches revealed that TRPA1 of Culex pipiens pallens inhabiting the temperate zone had a lower temperature threshold for heat-evoked activation, which was supported by the in vivo heat-avoidance test. Notably, the chemosensitivity of mosquito TRPA1 channels revealed differences not only between variants but also among species. Moreover, we discovered three novel mosquito TRPA1 agonists. Thermal niches selection and evolutionary trajectories significantly affect the functional properties of mosquito TRPA1, which represents a hallmark of the behaviors that may permit the design of improved mosquito control methods.

Flexible mechanical metamaterials enabling soft tactile sensors with multiple sensitivities at multiple force sensing ranges.

Project description:The majority of existing tactile sensors are designed to measure a particular range of force with a fixed sensitivity. However, some applications require tactile sensors with multiple task-relevant sensitivities at multiple ranges of force sensing. Inspired by the human tactile sensing capability, this paper proposes a novel soft tactile sensor based on mechanical metamaterials which exhibits multiple sensitivity regimes due to the step-by-step locking behaviour of its heterogenous multi-layered structure. By tuning the geometrical design parameters of the collapsible layers, each layer experiences locking behaviour under different ranges of force which provides different sensitivity of the sensor at different force magnitude. The integration of a magnetic-based transduction method with the proposed structure results in high design degrees of freedom for realising the desired contact force sensitivities and corresponding force sensing ranges. A systematic design procedure is proposed to select appropriate design parameters to produce the desired characteristics. Two example designs of the sensor structure were fabricated using widely available benchtop 3D printers and tested for their performance. The results showed the capability of the sensor in providing the desired characteristics in terms of sensitivity and force range and being realised in different shapes, sizes and number of layers in a single structure. The proposed multi-sensitivity soft tactile sensor has a great potential to be used in a wide variety of applications where different sensitivities of force measurement is required at different ranges of force magnitudes, from robotic manipulation and human-machine interaction to biomedical engineering and health-monitoring.

Hepatitis C prevalence in Denmark in 2016-An updated estimate using multiple national registers.

Project description:BackgroundChronic hepatitis C (CHC) can be eliminated as a public health threat by meeting the WHO targets: 90% of patients diagnosed and 80% treated by 2030. To achieve and monitor progress towards elimination, an updated estimate of the size of the CHC population is needed, but Denmark has no complete national CHC register. By combining existing registers in 2007, we estimated the population living with CHC to be 16,888 (0.38% of the adult population).AimTo estimate the population living with diagnosed and undiagnosed CHC in Denmark on 31 December 2016. Among additional aims were to estimate the proportion of patients attending specialised clinical care.MethodsPeople with diagnosed CHC were identified from four national registers. The total diagnosed population was estimated by capture-recapture analysis. The undiagnosed population was estimated by comparing the register data with data from two cross-sectional surveys.ResultsThe population living with diagnosed CHC in Denmark was 7,581 persons (95%CI: 7,416-12,661) of which 6,116 (81%) were identified in the four registers. The estimated undiagnosed fraction was 24%, so the total CHC infected population was 9,975 corresponding to 0.21% of the adult population (95%CI: 9,758-16,659; 0.21%-0.36%). Only 48% of diagnosed patients had received specialised clinical care.ConclusionCHC prevalence in Denmark is declining and 76% of patients have been diagnosed. Linking diagnosed patients to care and increasing efforts to test people with former or current drug use will be necessary to achieve CHC elimination.

Comparison of the sensitivities of noroviruses and feline calicivirus to chemical disinfection under field-like conditions.

Project description:Noroviruses (NV), in the family Caliciviridae, are an important cause of gastroenteritis in humans worldwide. Measures for prevention and control of NV dissemination are therefore necessary to ensure public safety. The abilities of an organic acid (Venno Vet 1 Super), an aldehyde (Venno FF Super), a halogen compound (sodium hypochlorite solution), and a peroxide (Oxystrong FG) to inactivate feline calicivirus (FCV), a cultivable virus surrogate for NV, were studied. Molecular protocols were then used for the comparative evaluation of disinfectant efficacies against NV and FCV, which were tested by reproducing NV field conditions, using human fecal material as a protein load. Generally, disinfectant efficacy was strongly reduced by the organic impurities (feces) used during tests. All disinfectants, except the aldehyde, were effective on FCV, as measured by cell culture and reverse transcription-PCR (RT-PCR), with inactivation levels of >or=99.9%. The glutaraldehyde-based compound failed to adequately inactivate FCV according to RT-PCR results, although the infectivity in cell culture was completely abolished. Similar inactivation levels were achieved with NV, but generally NV appeared more resistant than FCV, and consequently, the suitability of FCV as a model for NV should be considered with caution. In conclusion, according to RT-PCR results, 5% Venno Vet 1 Super, 1% Oxystrong FG, and not less than 2% Venno FF Super, with a contact time of 1 h, and 1% sodium hypochlorite, with 6,000 ppm of free chlorine and a contact time of 15 min, are required for safe disinfection when a calicivirus-related outbreak is suspected.

Comparison of finite difference based methods to obtain sensitivities of stochastic chemical kinetic models.

Project description:Sensitivity analysis is a powerful tool in determining parameters to which the system output is most responsive, in assessing robustness of the system to extreme circumstances or unusual environmental conditions, in identifying rate limiting pathways as a candidate for drug delivery, and in parameter estimation for calculating the Hessian of the objective function. Anderson [SIAM J. Numer. Anal. 50, 2237 (2012)] shows the advantages of the newly developed coupled finite difference (CFD) estimator over the common reaction path (CRP) [M. Rathinam, P. W. Sheppard, and M. Khammash, J. Chem. Phys. 132, 034103 (2010)] estimator. In this paper, we demonstrate the superiority of the CFD estimator over the common random number (CRN) estimator in a number of scenarios not considered previously in the literature, including the sensitivity of a negative log likelihood function for parameter estimation, the sensitivity of being in a rare state, and a sensitivity with fast fluctuating species. In all examples considered, the superiority of CFD over CRN is demonstrated. We also provide an example in which the CRN method is superior to the CRP method, something not previously observed in the literature. These examples, along with Anderson's results, lead to the conclusion that CFD is currently the best estimator in the class of finite difference estimators of stochastic chemical kinetic models.

Prevalence and Incidence of Multiple Myeloma in Urban Area in China: A National Population-Based Analysis.

Project description:Multiple myeloma (MM) is the second most frequent malignancy of blood, and information on disease burden of MM is limited in developing countries. We aimed to estimate the prevalence and incidence of MM in China. We used data from the national urban employee and urban resident basic medical insurance from 2012 to 2016 in China. MM cases were based on the primary diagnosis (International Classification of Diseases (ICD) code, ICD for oncology, or text of diagnosis) of patients. The crude prevalence and incidence were 6.88 per 100,000 population (95% CI, 5.75-8.00) and 1.60 per 100,000 person-years (1.28-1.92), respectively. The standardized prevalence and incidence were 5.68 (5.64-5.72) and 1.15 (1.11-1.19), respectively. Overall, the rates were higher in males compared with females for prevalence (7.89 vs. 5.79, P < 0.05) and incidence (1.84 vs. 1.30, P < 0.05). Both rates increased with age, and the mean age (SD) of MM patients was 57.9 (14.4) years. Prevalence peaked between 55 and 74 years old for both genders. The incidence in women aged 55-59 had a significantly high incidence of 5.53 (4.98-6.11). The prevalence and incidence were significantly lower than those in North America, Australia, and Western Europe but were in the same range as those in Japan or Korea. MM should be one of the cancers in the spotlight from both medical and socioeconomic perspectives in low-resource but populous countries because of the incidence of more elderly MM patients in the next decade. Further research is warranted to examine the potential pathophysiologic mechanism.

A pathwise derivative approach to the computation of parameter sensitivities in discrete stochastic chemical systems.

Project description:Characterizing the sensitivity to infinitesimally small perturbations in parameters is a powerful tool for the analysis, modeling, and design of chemical reaction networks. Sensitivity analysis of networks modeled using stochastic chemical kinetics, in which a probabilistic description is used to characterize the inherent randomness of the system, is commonly performed using Monte Carlo methods. Monte Carlo methods require large numbers of stochastic simulations in order to generate accurate statistics, which is usually computationally demanding or in some cases altogether impractical due to the overwhelming computational cost. In this work, we address this problem by presenting the regularized pathwise derivative method for efficient sensitivity analysis. By considering a regularized sensitivity problem and using the random time change description for Markov processes, we are able to construct a sensitivity estimator based on pathwise differentiation (also known as infinitesimal perturbation analysis) that is valid for many problems in stochastic chemical kinetics. The theoretical justification for the method is discussed, and a numerical algorithm is provided to permit straightforward implementation of the method. We show using numerical examples that the new regularized pathwise derivative method (1) is able to accurately estimate the sensitivities for many realistic problems and path functionals, and (2) in many cases outperforms alternative sensitivity methods, including the Girsanov likelihood ratio estimator and common reaction path finite difference method. In fact, we observe that the variance reduction using the regularized pathwise derivative method can be as large as ten orders of magnitude in certain cases, permitting much more efficient sensitivity analysis than is possible using other methods.

Projecting National-Level Prevalence of General Obesity and Abdominal Obesity Among Chinese Adults With Aging Effects.

Project description:ObjectivesTo explore the impact of population aging on the projected prevalence of obesity among Chinese adults in 2030.MethodsIn total, 71450 observations were extracted from the China Health and Nutrition Survey between 1991 and 2015.Population was projected to 2030 using a Bayesian hierarchical modeling method. Two different approaches were adopted to estimate and project the national prevalence of overweight/obesity from 1991 to 2030. One method assumed a constant population at the base year, while the other allowed the age and gender distributions vary in each year.ResultsOur projection indicated that approximately two-thirds of Chinese adults would be affected by overweight/general obesity in 2030, and more than 60% of Chinese adults will suffer from abdominal obesity in 2030. Ignoring population aging led to an underestimation of overweight, general obesity and abdominal obesity for women by 3.81, 0.06, and 3.16 percentage points (pp), and overweight and abdominal obesity among men by 1.67 and 0.53 pp, respectively; but the prevalence of general obesity among men will be overestimated by 2.11 pp. Similar underestimations were detected in the estimation from 1991 to 2015.ConclusionsEstimating and projecting the national prevalence of obesity using a constant population structure at the base line would cause significant underestimation if countries are undergoing rapid population aging.
