Project description:The risk difference scale is often of primary interest when evaluating public health impacts of interventions on binary outcomes. However, few investigators report findings in terms of additive interaction, probably because the models typically used for binary outcomes implicitly measure interaction on the multiplicative scale. One measure with which to assess additive interaction from multiplicative models is the relative excess risk due to interaction (RERI). The RERI measure has been applied in many contexts, but one limitation of previous approaches is that clustering in data has rarely been considered. We evaluated the RERI metric for the setting of clustered data using both population-averaged and cluster-conditional models. In simulation studies, we found that estimation and inference for the RERI using population-averaged models was straightforward. However, frequentist implementations of cluster-conditional models including random intercepts often failed to converge or produced degenerate variance estimates. We developed a Bayesian implementation of log binomial random-intercept models, which represents an attractive alternative for estimating the RERI in cluster-conditional models. We applied the methods to an observational study of adverse birth outcomes in mothers with human immunodeficiency virus, in which mothers were clustered within clinical research sites.

Project description:The purpose of the data was to determine excess lifetime cancer risk (ELCR) and risk of lung cancer from inhalation of radon in radiotherapy staff at Tehran radiotherapy Centers in 2015.The concentration of radon gas was extracted from a study done at Tehran radiotherapy centers, and then ELCR and risk of lung cancer were calculated in all centers by standard equations. The excess lifetime cancer risk and risk of lung cancer were 1.89 and 8.46 cases per 100,000 people in radiotherapy centers in Tehran City. The data indicate that the excess lifetime cancer risk and risk of lung cancer in radiotherapy centers are lower than the standard values which presented by UNSCEAR 2000.

Project description:We critically examined existing approaches for the estimation of the excess familial risk of cancer that can be attributed to identified common genetic risk variants and propose an alternative, more straightforward approach for calculating this proportion using well-established epidemiological methodology. We applied the underlying equations of the traditional approaches and the new epidemiological approach for colorectal cancer (CRC) in a large population-based case-control study in Germany with 4,447 cases and 3,480 controls, who were recruited from 2003 to 2016 and for whom interview, medical and genomic data were available. Having a family history of CRC (FH) was associated with a 1.77-fold risk increase in our study population (95% CI 1.52-2.07). Traditional approaches yielded estimates of the FH-associated risk explained by 97 common genetics variants from 9.6% to 23.1%, depending on various assumptions. Our alternative approach resulted in smaller and more consistent estimates of this proportion, ranging from 5.4% to 14.3%. Commonly employed methods may lead to strongly divergent and possibly exaggerated estimates of excess familial risk of cancer explained by associated known common genetic variants. Our results suggest that familial risk and risk associated with known common genetic variants might reflect two complementary major sources of risk.

Project description:The technology development, population growth, development of metropolises and subsequent pollution are serious threats to the environment and public health. Therefore, monitoring and evaluation of various emissions and their sources, and also providing practical strategies of pollution reduction, are necessary to solve these problems. In this regard, the use of modern methods to predict the concentration of pollutants can improve decision-making and provide appropriate solutions. Tehran has been ranked as one of the most polluted cities in Iran. In this study, the meteorological monthly data were employed to achieve potent models based on a Box-Jenkins method for the modelling of concentration level of five major air pollutants in Tehran such as NO2, PM10, O3, SO2, CO, and Pollutant Standard Index. The best models were selected using goodness of fit criteria such as Akaike Information Criterion (AIC) and Schwartz Bayesian Criterion (SBC) and least prediction error. Prediction of concentrations of those pollutants can be a powerful tool in order to take preventive measures, such as the reduction of emissions and alerting the affected population. The results indicated that the concentration of pollutants in each period was influenced by their level and shocks they received during previous periods, which is mainly explained by special climatic and geographic conditions of Tehran that accumulates the pollution over time.Supplementary informationThe online version contains supplementary material available at 10.1007/s40201-020-00582-w.

Project description:The aim of the data was to measure the absorbed dose of gamma radiation in order to estimate the excessive risk of cancer-induced gamma radiation during the lifetime of Bojnurd residents. In this descriptive cross-sectional study, gamma radiations in 30 places was measured in Bojnurd City during four seasons in 2015. A dosimeter was stacked on a tripod at 1 m from the ground for 50 minutes, and then, the absorbed dose of gamma radiation was recorded in the checklist. Ultimately, the effective dose and the excessive lifetime risk of cancer were determined. The mean ± SE of absorbed dose of gamma radiation in spring, summer, autumn, winter was 134.25 ± 1.45; 139.89 ± 1.64; 134.40 ± 1.25; 143.80 ± 1.73 nGy, respectively. The average annual effective dose by residents in open space was estimated at an average of 0.167 mSv. Also, the excessive risk of cancer-induced gamma radiation was equal to 0.67 × 10-3. The annual effective dose and the excessive risk of cancer-induced gamma radiation during the lifetime of Bojnurd residents are higher than the global average.

Project description:Outbreaks of arbovirus infections vectored by invasive Aedes albopictus have already occurred and are predicted to become increasingly frequent in Southern Europe. We present a probabilistic model to assess risk of arbovirus outbreaks based on incident cases worldwide, on the probability of arrival of infected travelers, and on the abundance of the vector species. Our results show a significant risk of Chikungunya outbreak in Rome from mid June to October in simulations with high human biting rates (i.e. when ≥50% of the population is bitten every day). The outbreak risk is predicted to be highest for Chikungunya and null for Zika. Simulated increase of incident cases in selected endemic countries has no major impact on the outbreak risk. The model correctly estimated the number of imported cases and can be easily adapted to other urban areas where Ae. albopictus is the only potential vector present.

Project description:BackgroundAge estimation from panoramic radiographs is a fundamental task in forensic sciences. Previous age assessment studies mainly focused on juvenile rather than elderly populations (> 25 years old). Most proposed studies were statistical or scoring-based, requiring wet-lab experiments and professional skills, and suffering from low reliability.ResultBased on Soft Stagewise Regression Network (SSR-Net), we developed DENSEN to estimate the chronological age for both juvenile and older adults, based on their orthopantomograms (OPTs, also known as orthopantomographs, pantomograms, or panoramic radiographs). We collected 1903 clinical panoramic radiographs of individuals between 3 and 85 years old to train and validate the model. We evaluated the model by the mean absolute error (MAE) between the estimated age and ground truth. For different age groups, 3-11 (children), 12-18 (teens), 19-25 (young adults), and 25+ (adults), DENSEN produced MAEs as 0.6885, 0.7615, 1.3502, and 2.8770, respectively. Our results imply that the model works in situations where genders are unknown. Moreover, DENSEN has lower errors for the adult group (> 25 years) than other methods. The proposed model is memory compact, consuming about 1.0 MB of memory overhead.ConclusionsWe introduced a novel deep learning approach DENSEN to estimate a subject's age from a panoramic radiograph for the first time. Our approach required less laboratory work compared with existing methods. The package we developed is an open-source tool and applies to all different age groups.

Project description:BackgroundTrends in cause-specific mortality in most African countries are currently estimated from epidemiological models because the coverage of the civil registration system is low and national statistics on causes of death are unreliable at the national level. We aim to evaluate the performance of the death notification system in Antananarivo, the capital city of Madagascar, to inform cause-of-death statistics.MethodsInformation on the sex of the deceased, dates of birth and death, and underlying cause of death were transcribed from death registers maintained in Antananarivo. Causes of death were coded in ICD-9 and mapped to cause categories from the Global Burden of Disease 2016 Study (GBD). The performance of the notification system was assessed based on the Vital Statistics Performance Index, including six dimensions: completeness of death registration, quality of cause of death reporting, quality of age and sex reporting, internal consistency, level of cause-specific detail, and data availability and timeliness. We redistributed garbage codes and compared cause-specific mortality fractions in death records and estimates from the GBD with concordance correlation coefficients.ResultsThe death notification system in Antananarivo performed well on most dimensions, although 31% of all deaths registered over the period 1976-2015 were assigned to ICD codes considered as "major garbage codes" in the GBD 2016. The completeness of death notification, estimated with indirect demographic techniques, was higher than 90% in the period 1975-1993, and recent under-five mortality rates were consistent with estimates from Demographic and Health Surveys referring to the capital city. After redistributing garbage codes, cause-specific mortality fractions derived from death notification data were consistent with GBD 2016 for the whole country in the 1990s, with concordance correlation coefficients higher than 90%. There were larger deviations in recent years, with concordance correlation coefficients in 2015 at 0.74 (95% CI 0.66-0.81) for men and 0.81 (95% CI 0.74-0.86) for women.ConclusionsDeath notification in Antananarivo is a low-cost data source allowing real-time mortality monitoring, with a potential to improve disease burden estimates. Further efforts should be directed towards evaluating data quality in urban centers in Madagascar and other African countries to fill important data gaps on causes of death.

Project description:Particulate matter with an aerodynamic diameter <2.5 μm (PM2.5) represents a severe environmental problem and is of negative impact on human health. Xi'an City, with a population of 6.5 million, is among the highest concentrations of PM2.5 in China. In 2013, in total, there were 191 days in Xi'an City on which PM2.5 concentrations were greater than 100 μg/m3. Recently, a few studies have explored the potential causes of high PM2.5 concentration using remote sensing data such as the MODIS aerosol optical thickness (AOT) product. Linear regression is a commonly used method to find statistical relationships among PM2.5 concentrations and other pollutants, including CO, NO2, SO2, and O3, which can be indicative of emission sources. The relationships of these variables, however, are usually complicated and non-linear. Therefore, a generalized additive model (GAM) is used to estimate the statistical relationships between potential variables and PM2.5 concentrations. This model contains linear functions of SO2 and CO, univariate smoothing non-linear functions of NO2, O3, AOT and temperature, and bivariate smoothing non-linear functions of location and wind variables. The model can explain 69.50% of PM2.5 concentrations, with R2 = 0.691, which improves the result of a stepwise linear regression (R2 = 0.582) by 18.73%. The two most significant variables, CO concentration and AOT, represent 20.65% and 19.54% of the deviance, respectively, while the three other gas-phase concentrations, SO2, NO2, and O3 account for 10.88% of the total deviance. These results show that in Xi'an City, the traffic and other industrial emissions are the primary source of PM2.5. Temperature, location, and wind variables also non-linearly related with PM2.5.

Project description:BackgroundDe-identification is a common way to protect patient privacy when disclosing clinical data for secondary purposes, such as research. One type of attack that de-identification protects against is linking the disclosed patient data with public and semi-public registries. Uniqueness is a commonly used measure of re-identification risk under this attack. If uniqueness can be measured accurately then the risk from this kind of attack can be managed. In practice, it is often not possible to measure uniqueness directly, therefore it must be estimated.MethodsWe evaluated the accuracy of uniqueness estimators on clinically relevant data sets. Four candidate estimators were identified because they were evaluated in the past and found to have good accuracy or because they were new and not evaluated comparatively before: the Zayatz estimator, slide negative binomial estimator, Pitman's estimator, and mu-argus. A Monte Carlo simulation was performed to evaluate the uniqueness estimators on six clinically relevant data sets. We varied the sampling fraction and the uniqueness in the population (the value being estimated). The median relative error and inter-quartile range of the uniqueness estimates was measured across 1000 runs.ResultsThere was no single estimator that performed well across all of the conditions. We developed a decision rule which selected between the Pitman, slide negative binomial and Zayatz estimators depending on the sampling fraction and the difference between estimates. This decision rule had the best consistent median relative error across multiple conditions and data sets.ConclusionThis study identified an accurate decision rule that can be used by health privacy researchers and disclosure control professionals to estimate uniqueness in clinical data sets. The decision rule provides a reliable way to measure re-identification risk.
