Three vanishing masses in the heart.

ABSTRACT: A 26 year primigravida, 8 days after delivering a still born baby presented with gradually progressive dyspnea from New York Heart Association (NYHA) class II to class III over a period of 1 week. 2D-Echocardiography showed severely compromised left ventricular systolic function with evidence of thrombi in three different chambers of the heart. All the clots disappeared after an effective anticoagulation therapy without any embolic complications. The hypercoagulable state of the peripartum period and the severe biventricular dysfunction led to the formation of clots. .

Anterior Mediastinal Masses in the Framingham Heart Study: Prevalence and CT Image Characteristics.

Project description:PURPOSE:To investigate the prevalence and CT image characteristics of anterior mediastinal masses in a population-based cohort and their association with the demographics of the participants. MATERIALS AND METHODS:Chest CT scans of 2571 Framingham Heart Study participants (mean age 58.9 years, 51% female) were evaluated by two board-certified radiologists with expertise in thoracic imaging for the presence of anterior mediastinal masses, their shape, contour, location, invasion of adjacent structures, fat content, and calcification. For participants with anterior mediastinal masses, a previous cardiac CT scan was reviewed for interval size change of the masses, when available. The demographics of the participants were studied for any association with the presence of anterior mediastinal masses. RESULTS:Of 2571, 23 participants (0.9%, 95% CI: 0.6 to 1.3) had anterior mediastinal masses on CT. The most common CT characteristics were oval shape, lobular contour, and midline location, showing soft tissue density (median 32.1 HU). Fat content was detected in a few cases (9%, 2/23). Six out of eight masses with available prior cardiac CT scans demonstrated an interval growth over a median period of 6.5 years. No risk factors for anterior mediastinal masses were detected among participants' demographics, including age, sex, BMI, and cigarette smoking. CONCLUSIONS:The prevalence of anterior mediastinal masses is 0.9% in the Framingham Heart Study. Those masses may increase in size when observed over 5-7 years. Investigation of clinical significance in incidentally found anterior mediastinal masses with a longer period of follow-up would be necessary.

Programmable Vanishing Multifunctional Optics.

Project description:Physically transient optics, a form of optics that can physically disappear with precisely controlled degradation behaviors, has widespread applications including information security, drug release, and degradable implants. Here, a set of silk-based programmable vanishing, biologically functional, multichromatic diffractive optical elements (MC-DOEs) is reported. Silk proteins produced by silkworms and spiders are mechanically robust, biocompatible, biodegradable, and importantly, optically transparent, which open up new opportunities for a set of fully degradable transient optical devices with no need of metallic or semiconductor components. Compared with monochromatic DOEs, MC-DOEs carry out richer information for more practical applications such as encryption and decryption of multilevel information, quantitative sensing/monitoring of chemical/biological cascade reactions, and effective treatment of infections caused by multiple pathogens.

Vanishing native American dog lineages.

Project description:BackgroundDogs were an important element in many native American cultures at the time Europeans arrived. Although previous ancient DNA studies revealed the existence of unique native American mitochondrial sequences, these have not been found in modern dogs, mainly purebred, studied so far.ResultsWe identified many previously undescribed mitochondrial control region sequences in 400 dogs from rural and isolated areas as well as street dogs from across the Americas. However, sequences of native American origin proved to be exceedingly rare, and we estimate that the native population contributed only a minor fraction of the gene pool that constitutes the modern population.ConclusionsThe high number of previously unidentified haplotypes in our sample suggests that a lot of unsampled genetic variation exists in non-breed dogs. Our results also suggest that the arrival of European colonists to the Americas may have led to an extensive replacement of the native American dog population by the dogs of the invaders.

Natural History of Vanishing White Matter.

Project description:ObjectiveTo comprehensively describe the natural history of vanishing white matter (VWM), aiming at improving counseling of patients/families and providing natural history data for future therapeutic trials.MethodsWe performed a longitudinal multicenter study among 296 genetically confirmed VWM patients. Clinical information was obtained via disease-specific clinical questionnaire, Health Utilities Index and Guy's Neurological Disability Scale assessments, and chart review.ResultsFirst disease signs occurred at a median age of 3 years (mode = 2 years, range = before birth to 54 years); 60% of patients were symptomatic before the age of 4 years. The nature of the first signs varied for different ages of onset. Overall, motor problems were the most common presenting sign, especially in children. Adolescent and adult onset patients were more likely to exhibit cognitive problems early after disease onset. One hundred two patients were deceased. Multivariate Cox regression analysis revealed a positive relation between age at onset and both preservation of ambulation and survival. Absence of stress-provoked episodes and absence of seizures predicted more favorable outcome. In patients with onset before 4 years, earlier onset was associated with more severe disability and higher mortality. For onset from 4 years on, disease course was generally milder, with a wide variation in severity. There were no significant differences for sex or for the 5 eIF2B gene groups. The results confirm the presence of a genotype-phenotype correlation.InterpretationThe VWM disease spectrum consists of a continuum with extremely wide variability. Age at onset is a strong predictor for disease course. Ann Neurol 2018;84:274-288.

Phase-vanishing halolactonization of neat substrates.

Project description:Phase-vanishing reactions are triphasic reactions which involve a reagent, a liquid perfluoroalkane as a phase screen and a substrate. The perfluoroalkane does not dissolve any of the reactants and is used to separate them. Halolactonization of neat substrates under phase-vanishing conditions avoids use of both solvents and basic reaction conditions. Both gamma,delta-alkenoic acids as well as the corresponding methyl esters are suitable substrates for phase-vanishing halolactonizations. The reaction works well both on solid and liquid substrates and the products are obtained in good to excellent yields, particularly in the case of rigid bicyclic systems. Bromine (Br(2)) and iodine monochloride (ICl) are suitable electrophiles for bromolactonization and iodolactonization, respectively. Although in some cases iodine gave satisfactory yields of the corresponding iodolactone, it is generally inferior to iodine monochloride.
