Project description:Knee joint pain is a common symptom in obese individuals and walking is often prescribed as part of management programs. Past studies in obese individuals have focused on standing alignment and kinematics in the sagittal and coronal planes. Investigation of 6 degree-of-freedom (6DOF) knee joint kinematics during standing and gait is important to thoroughly understand knee function in obese individuals with knee pain. This study aimed to investigate the 6DOF knee joint kinematics in standing and during gait in obese patients using a validated fluoroscopic imaging system. Ten individuals with obesity and knee pain were recruited. While standing, the knee was in 7.4±6.3°of hyperextension, 2.8±3.3° of abduction and 5.6±7.3° of external rotation. The femoral center was located 0.7±3.1mm anterior and 5.1±1.5mm medial to the tibial center. During treadmill gait, the sagittal plane motion, i.e., flexion/extension and anterior-posterior translation, showed a clear pattern. Specifically, obese individuals with knee pain maintained the knee in more flexion and more anterior tibial translation during most of the stance phase of the gait cycle and had a reduced total range of knee flexion when compared to a healthy non-obese group. In conclusion, obese individuals with knee pain used hyperextension knee posture while standing, but maintained the knee in more flexion during gait with reduced overall range of motion in the 6DOF analysis.

Project description:BackgroundObesity is a mechanical risk factor for osteoarthritis. In individuals with obesity, knee joint pain is prevalent. Weight loss reduces joint loads, and therefore potentially delays disease progression; however, how the knee joint responds to weight loss in individuals with obesity and knee pain is not clear.Research questionTo assess the effect of weight loss on knee joint kinematics during gait in individuals with obesity and knee pain.MethodsWe recruited individuals with obesity (BMI ≥ 35) and knee pain who were participating in a weight loss program which included bariatric surgery or medical management. At baseline and 1 year follow-up, participants walked on a treadmill, and their knee joint kinematics were assessed using a dual-fluoroscopic imaging system and subject-specific magnetic resonance imaging knee joint models. Gait changes were represented by change in range of tibiofemoral motion, i.e., excursions in flexion-extension, adduction-abduction, internal-external rotation, anterior-posterior translation, medial-lateral translation, and superior-inferior translation during gait.ResultsTwelve individuals with obesity and knee pain completed the gait analysis at baseline and 1 year follow-up. Participants lost on average 10.4% (standard deviation: 17.2%) of their baseline body weight. Reduction in body weight was associated with increased range of flexion-extension (r = -0.75, p < 0.01) and decreased range of adduction-abduction (r = 0.60, p = 0.04) during gait. The reduction in body weight was also associated with self-reported pain decrease (r = 0.62, p = 0.04); however, the change in pain was not significantly associated with kinematic changes.SignificanceWeight loss was associated with improved gait kinematics in the sagittal and frontal planes. The change in gait pattern in individuals with obesity and knee pain was not associated with the change in pain given a reduction in body weight.

Project description:It is critical to distinguish gait compensations from true abnormalities when planning interventions to improve gait in individuals with neuromuscular disorders.The aim of this study was to determine the effect of isolated ankle equinus on knee kinematics during the initial contact phase of gait.Ten healthy subjects (29?+?4.3 years) participated, and testing occurred in a motion analysis laboratory. This cross-sectional study investigated five gait conditions in each subject: shoe alone, shoe with unilateral ankle foot orthosis locked at neutral, 10°, 20°, and 30° of fixed ankle plantar flexion. Gait kinematics were recorded and calculated with 3D motion analysis. The difference between the shoe and each brace condition was analyzed by repeated-measures ANOVA. The primary outcome was knee flexion at initial contact.With greater than 10° simulated ankle equinus, the primary gait compensation pattern was increased knee flexion at initial contact. A significant degree of knee flexion occurred ranging from 7° to 22°.Our data suggests that observed knee flexion at initial contact may be a compensation pattern in individuals with >10° ankle equinus. However, in individuals with ?10° ankle equinus, observed knee flexion may represent a true gait deviation. This has clinical significance in the realm of cerebral palsy for treatment planning to improve gait.

Project description:Six kinematic parameters are needed to fully describe three-dimensional (3D) bone motion at a joint. At the knee, the relative movements of the femur and tibia are often represented by a 1-degree-of-freedom (1-DOF) model with a single flexion-extension axis or a 2-DOF model comprising a flexion-extension axis and an internal-external rotation axis. The primary aim of this study was to determine the accuracy with which 1-DOF and 2-DOF models predict the 3D movements of the femur, tibia and patella during daily activities. Each model was created by fitting polynomial functions to 3D tibiofemoral (TF) and patellofemoral (PF) kinematic data recorded from 10 healthy individuals performing 6 functional activities. Model cross-validation analyses showed that the 2-DOF model predicted 3D knee kinematics more accurately than the 1-DOF model. At the TF joint, mean root-mean-square (RMS) errors across all activities and all participants were 3.4°|mm (deg or mm) for the 1-DOF model and 2.4°|mm for the 2-DOF model. At the PF joint, mean RMS errors were 4.0°|mm and 3.9°|mm for the 1-DOF and 2-DOF models, respectively. These results indicate that a 2-DOF model with two rotations as inputs may be used with confidence to predict the full 3D motion of the knee-joint complex.

Project description:After knee arthroplasty (KA) surgery, patients experience abnormal kinematics and kinetics during numerous activities of daily living. Biomechanical investigations have focused primarily on level walking, whereas walking on sloped surfaces, which is stated to affect knee kinematics and kinetics considerably, has been neglected to this day. This study aimed to analyze over-ground walking on level and sloped surfaces with a special focus on transverse and frontal plane knee kinematics and kinetics in patients with KA. A three-dimensional (3D) motion analysis was performed by means of optoelectronic stereophogrammetry 1.8 ± 0.4 years following total knee arthroplasty (TKA) and unicompartmental arthroplasty surgery (UKA). AnyBody™ Modeling System was used to conduct inverse dynamics. The TKA group negotiated the decline walking task with reduced peak knee internal rotation angles compared with a healthy control group (CG). First-peak knee adduction moments were diminished by 27% (TKA group) and 22% (UKA group) compared with the CG during decline walking. No significant differences were detected between the TKA and UKA groups, regardless of the locomotion task. Decline walking exposed apparently more abnormal knee frontal and transverse plane adjustments in KA patients than level walking compared with the CG. Hence, walking on sloped surfaces should be included in further motion analysis studies investigating KA patients in order to detect potential deficits that might be not obvious during level walking.

Project description:This preliminary study investigated whether direct measurement of head rotation improves prediction of mild traumatic brain injury (mTBI). Although many studies have implicated rotation as a primary cause of mTBI, regulatory safety standards use 3 degree-of-freedom (3DOF) translation-only kinematic criteria to predict injury. Direct 6DOF measurements of human head rotation (3DOF) and translation (3DOF) have not been previously available to examine whether additional DOFs improve injury prediction. We measured head impacts in American football, boxing, and mixed martial arts using 6DOF instrumented mouthguards, and predicted clinician-diagnosed injury using 12 existing kinematic criteria and 6 existing brain finite element (FE) criteria. Among 513 measured impacts were the first two 6DOF measurements of clinically diagnosed mTBI. For this dataset, 6DOF criteria were the most predictive of injury, more than 3DOF translation-only and 3DOF rotation-only criteria. Peak principal strain in the corpus callosum, a 6DOF FE criteria, was the strongest predictor, followed by two criteria that included rotation measurements, peak rotational acceleration magnitude and Head Impact Power (HIP). These results suggest head rotation measurements may improve injury prediction. However, more 6DOF data is needed to confirm this evaluation of existing injury criteria, and to develop new criteria that considers directional sensitivity to injury.

Project description:Six-degree-of-freedom (6DoF) sensors measure translation along three axes and rotation around three axes. These collocated measurements make it possible to fully describe building motion without the need for an external reference point. This is an advantage for building health monitoring, which uses interstory drift and building eigenfrequencies to monitor stability. In this paper, IMU50 6DoF sensors are characterized to determine their suitability for building health monitoring. The sensors are calibrated using step table methods and by comparison with earth's rotation and gravity. These methods are found to be comparable. The sensor's self-noise is examined through the power spectral density and the Allan deviation of data recorded in a quiet environment. The effect of temperature variation is tested between 14 and 50 °C. It appears that the self-noise of the rotation components increases while the self-noise of the acceleration components decreases with temperature. The comparison of the sensor self-noise with ambient building signal and higher amplitude shaking shows that these sensors are in general not sensitive enough for ambient signal building health monitoring in the frequency domain, but could be useful for monitoring interstory drift and building motion during, for example, strong earthquake shaking in buildings similar to those examined here.

Project description:BackgroundIn 3D gait analysis, the knee joint is usually described by the Eulerian way. It consists in breaking down the motion between the articulating bones of the knee into three rotations around three axes: flexion/extension, abduction/adduction and internal/external rotation. However, the definition of these axes is prone to error, such as the "cross-talk" effect, due to difficult positioning of anatomical landmarks. This paper proposes a correction method, principal component analysis (PCA), based on an objective kinematic criterion for standardization, in order to improve knee joint kinematic analysis.MethodsThe method was applied to the 3D gait data of two different groups (twenty healthy subjects and four with knee osteoarthritis). Then, this method was evaluated with respect to three main criteria: (1) the deletion of knee joint angle cross-talk (2) the reduction of variance in the varus/valgus kinematic profile (3) the posture trial varus/valgus deformation matching the X-ray value for patients with knee osteoarthritis. The effect of the correction method was tested statistically on variabilities and cross-talk during gait.ResultsCross-talk was lower (p<0.05) after correction (the correlation between the flexion-extension and varus-valgus kinematic profiles being annihilated). Additionally, the variance in the kinematic profile for knee varus/valgus and knee flexion/extension was found to be lower and higher (p<0.05), respectively, after correction for both the left and right side. Moreover, after correction, the posture trial varus/valgus angles were much closer to x-ray grading.ConclusionThe results show that the PCA correction applied to the knee joint eliminates the cross-talk effect, and does not alter the radiological varus/valgus deformation for patients with knee osteoarthritis. These findings suggest that the proposed correction method produces new rotational axes that better fit true knee motion.

Project description:PurposeTo validate surface imaging (SI)-reported offsets using a six degree-of-freedom couch and an anthropomorphic phantom for commissioning and routine quality assurance of an SI system used for stereotactic radiosurgery (SRS).MethodsAn anthropomorphic phantom with a radiopaque ball bearing (BB) placed either anterior, midline, or posterior, was tracked with SI with a typical SRS region of interest. Couch motion in all six degrees of freedom was programmed and delivered on a linac. SI system logs were synchronized with linac trajectory logs. Ten random couch positions were selected at couch 0°, 45°, 90°, 270°, 315° with megavolt (MV) images taken to account for couch walkout. The SI residual error (ε), the difference between SI reported offset and MV or trajectory log position, was calculated. Residual errors were measured with and without one SI pod blocked.ResultsThe median [range] of magnitude of translational ε was 0.13 [0.07, 0.21], 0.16 [0.11, 0.26], 0.61 [0.50, 0.68], 0.49 [0.42, 0.55], 0.55 [0.38, 0.72] mm for couch rotations of 0°, 45°, 90°, 270°, 315°, respectively, for the midline BB and no pod blocked. The range of all translational ε from all couch angles (with and without pod block) at different BB positions is [0.05, 0.96] mm. The absolute range of difference when changing BB position when no pod is blocked in median translational ε is [0.01, 0.40] mm with the maximum at BB posterior. The absolute range of difference when not changing BB positions with and without pod block in median translational ε is [0.01, 0.37] mm with the maximum at BB posterior and couch 315°.ConclusionSI system and linac trajectory log analysis can be used to assess SI system performance with automated couch motion to validate SI accuracy.

Project description:Hemophilic arthropathy is the result of repetitive intra-articular bleeding and synovial inflammation. In people with hemophilic arthropathy (PWHA), very little is known about the neural control of individual muscles during movement. The aim of the present study was to assess if the neural control of individual muscles and coordination between antagonistic muscle pairs and joint kinematics during gait are affected in PWHA. Thirteen control subjects (CG) walked overground at their preferred and slow velocity (1 m/s), and 14 PWHA walked overground at the preferred velocity (1 m/s). Joint kinematics and temporal gait parameters were assessed using four inertial sensors. Surface electromyography (EMG) was collected from gluteus maximus (GMAX), gluteus medius (GMED), vastus medialis (VM), vastus lateralis (VL), rectus femoris (RF), medial gastrocnemius (MG), lateral gastrocnemius (LG), soleus (SOL), tibialis anterior (TA), semitendinosus (ST), and biceps femoris (BF). Waveforms were compared using the time-series analysis through statistical parametric mapping. In PWHA compared to CG, EMG amplitude during the stance phase was higher for LG (for both velocities of the CG), BF (slow velocity only), and ST (preferred velocity only) (p < 0.05). Co-contraction during the stance phase was higher for MG-TA, LG-TA, VL-BF, VM-ST, LG-VL, and MG-VM (both velocities) (p < 0.05). MG and LG were excited earlier (preferred velocity only) (p < 0.05). A later offset during the stance phase was found for VL, BF, and ST (both velocities), and BF and GMAX (preferred velocity only) (p < 0.05). In addition, the range of motion in knee and ankle joints was lower in PWHA (both velocities) and hip joint (preferred velocity only) (p < 0.05). In conclusion, the neural control of individual muscles and coordination between antagonistic muscles during gait in PWHA differs substantially from control subjects.
