Project description:In India, under the National Tuberculosis Elimination Programme, the government provides free treatment for multidrug-resistant tuberculosis; however, many patients seek care elsewhere, which is costly. To determine those out-of-pocket expenses, we interviewed 40 presumptive patients and found that they spent more than their median annual income before registering for the government program.

Project description:Treatment of drug-resistant tuberculosis (TB), which is usually less successful than that of drug-susceptible TB, represents a challenge for TB control and elimination. We aimed to evaluate treatment outcomes and to identify the factors associated with death among patients with MDR and XDR-TB in Portugal. We assessed MDR-TB cases reported for the period 2000-2016, using the national TB Surveillance System. Treatment outcomes were defined according to WHO recommendations. We identified the factors associated with death using logistic regression. We evaluated treatment outcomes of 294 MDR- and 142 XDR-TB patients. The treatment success rate was 73.8% among MDR- and 62.7% among XDR-TB patients (p = 0.023). The case-fatality rate was 18.4% among MDR- and 23.9% among XDR-TB patients. HIV infection (OR 4.55; 95% CI 2.31-8.99; p < 0.001) and resistance to one or more second-line injectable drugs (OR 2.73; 95% CI 1.26-5.92; p = 0.011) were independently associated with death among MDR-TB patients. HIV infection, injectable drug use, past imprisonment, comorbidities, and alcohol abuse are conditions that were associated with death early on and during treatment. Early diagnosis of MDR-TB and further monitoring of these patients are necessary to improve treatment outcome.

Project description:Antimicrobial resistance is a major global concern. Tuberculosis (TB) strains resistant to rifampicin and other TB medicines challenge patient survival and public health. The World Health Organization (WHO) has published treatment guidelines for drug-resistant TB since 1997 and last updated them in 2016 based on reviews of aggregated and individual patient data from published and unpublished studies. An international expert panel formulated recommendations following the GRADE approach. The new WHO guidelines recommend a standardised 9-12?months shorter treatment regimen as first choice in patients with multidrug- or rifampicin-resistant TB (MDR/RR-TB) strains not resistant to fluoroquinolones or second-line injectable agents; resistance to these two classes of core second-line medicines is rapidly detectable with molecular diagnostics also approved by WHO in 2016. The composition of longer regimens for patients ineligible for the shorter regimen was modified. A first-ever meta-analysis of individual paediatric patient data allowed treatment recommendations for childhood MDR/RR-TB to be made. Delamanid is now also recommended in patients aged 6-17?years. Partial lung resection is a recommended option in MDR/RR-TB care. The 2016 revision highlighted the continued shortage of high-quality evidence and implementation research, and reiterated the need for clinical trials and best-practice studies to improve MDR/RR-TB patient treatment outcomes and strengthen policy.

Project description:Macrolide-resistant Bordetella pertussis emerged in Vietnam during 2016-2017. Direct analyses of swab samples from 10 patients with pertussis revealed a macrolide-resistant mutation, A2047G, in the 23S rRNA. We identified the MT104 genotype of macrolide-resistant B. pertussis (which is prevalent in mainland China) and its variants in these patients.

Project description:Introducción. La tuberculosis extremadamente resistente a los medicamentos (TB-XDR) es el resultado de deficiencias en la administración del tratamiento y en la prevención de la transmisión de la enfermedad; además, es un reto para los programas nacionales de control de la tuberculosis. Objetivo. Describir las características clínicas y epidemiológicas de los casos de TB-XDR diagnosticados en Colombia. Materiales y métodos. Se trata de un estudio de serie de casos, de pacientes con diagnóstico de TB-XDR, de 2006 a 2016 en Colombia. Las fuentes de información fueron el formato único de vigilancia y la base de datos del Laboratorio Nacional de Referencia. Se analizaron las variables: entidad territorial de procedencia, sexo, edad, régimen de afiliación, forma de tuberculosis, coinfección tuberculosis-HIV, patrón de sensibilidad a los fármacos de segunda línea y resultado (vivo o muerto). Resultados. En el período de estudio, se diagnosticaron 51 casos de TB-XDR, 28 hombres y 23 mujeres, con un promedio anual de cinco casos. En los departamentos de Antioquia, Valle del Cauca y Atlántico, se presentaron 46 (90 %) de los casos. El rango de edad fue de 5 a 81 años y, la mediana, de 40 años. De los 51 pacientes, 32 (63 %) estaban afiliados al régimen subsidiado de salud y 46 (90 %) presentaron tuberculosis pulmonar; de los 22 a quienes se les practicó la prueba para HIV, en tres (13,6 %) había coinfección con HIV, y 29 (57 %) murieron. Los fármacos a los cuales hubo mayor resistencia, fueron ofloxacina en 45 (88 %) casos y amikacina en 43 (84 %). Conclusión. La TB-XDR se presentó principalmente en formas pulmonares, lo cual aumenta la probabilidad de transmisión en la comunidad y se refleja en la aparición de tuberculosis resistente en menores de 15 años. La población más afectada es la económicamente activa, con una gran mortalidad. El Programa Nacional de Prevención y Control de la Tuberculosis debe generar estrategias para evitar la propagación de cepas resistentes.

Project description:Highlights • Diagnosis of DRTB was delayed by the initial wrong and ineffective treatment that was provided to about 70% of patients, impacting negatively on treatment outcomes• Being <6 months from previous TB treatment was associated with unfavourable treatment outcomes.• Strengthening access to drug sensitivity testing at diagnosis is important in DR-TB identification and control. Objective To investigate determinants of drug resistance and treatment outcomes among patients with drug-resistant tuberculosis (DR-TB). Design This was a cross-sectional study on patients diagnosed with DR-TB in Bulawayo and Matabeleland South provinces, 2015. Results A total of 129 participants were identified. DR-TB patients were 3.4 times more likely to have been treated previously for sensitive TB (95% confidence interval 1.3–9.2). Approximately 88.5% of DR-TB patients were diagnosed before completing the sensitive TB course and another 82.1% developed DR-TB within 6 months of completing sensitive TB treatment. The likelihood diminished with increasing time interval, becoming less likely at >12 months post-treatment. Most DR-TB patients (87.5%) were likely to have resided outside Zimbabwe and to have fallen ill there (85.2%). Overall, hearing loss was the most prevalent (70%) medication side effect experienced. Unfavourable interim treatment outcomes were high for patients <6 months on treatment (prevalence odds ratio 2.7, 95% CI 1.2–6.1), becoming 44% less likely after 18 months (95% CI 1.2–11.4). Conclusions The majority of DR-TB patients were diagnosed during sensitive TB treatment, suggesting missed DR-TB diagnosis or inadequate treatment. Delays in starting effective TB regimens negatively affect treatment outcomes. Drug sensitivity testing at diagnosis, patient monitoring, and enhanced adherence counselling are recommended.

Project description:Background:Drug-resistant tuberculosis (TB) is a global public health issue. To monitor this in Canada, surveillance systems have been in place for the last 20 years. Objective:To describe drug resistance patterns among TB isolates in Canada in 2017 by type of resistance as well as geographic location, demographic data and origin and to compare current data to those of the previous 10 years. Methods:Data were derived and analyzed from two sources. The Canadian Tuberculosis Laboratory Surveillance System (CTBLSS) is an isolate-based laboratory surveillance system and was used to obtain information on the results of drug susceptibility testing (DST) as well as province or territory, sex and age of the individual from which the sample originated. The Canadian Tuberculosis Reporting System (CTBRS) is a case-based surveillance system with information on active and retreatment TB cases in Canada and was used to derive origin data, which is defined as either foreign-born, Canadian-born Indigenous or Canadian-born non-Indigenous. Analysis was descriptive and compared with data from these two sources for 2007-2016. Results:In 2017, 1,515 TB isolates were tested for resistance to anti-TB drugs, with 123 (8.1%) demonstrating resistance to any first-line anti-TB drug. Of these, 103 were monoresistant, six were polyresistant and 14 were multidrug-resistant tuberculosis (MDR-TB). No extensively drug-resistant tuberculosis (XDR-TB) isolates were reported. Drug resistance was reported in seven provinces/territories (British Columbia, Alberta, Saskatchewan, Manitoba, Ontario, Quebec and New Brunswick). There were 63 isolates from females with drug resistance (9.5%) and 60 isolates from males with drug resistance (7.0%). Drug resistance was found in a greater percentage of isolates among those aged 25-34 (n=29, 23.6%). By origin, 1,072 (11%) foreign-born TB cases reported between 2005 and 2015 were drug-resistant. Among the Canadian-born non-Indigenous and Canadian-born Indigenous TB cases, 143 (9%) and 54 (2%) were drug-resistant, respectively. Compared with previous years, the number of isolates tested increased slightly (from 1,267 to 1,515); however, there was a decrease in the percentage of isolates with reported drug resistance (from 10.5% in 2007 to 8.1% in 2017). Conclusion:In 2017, TB drug resistance rates remained low in Canada.

New patients were more likely to be mono SM-resistant and less likely to be INH- and RIF-resistant. The most common mutations were katG (S315T), rpoB (S450L), rpsL (K43R), and embB (M306V).ConclusionsThe proportion of mono-SM-resistant TB among new patients was higher.
