Binaural Interaction Component of Middle Latency Response in Children Suspected to Central Auditory Processing Disorder.

ABSTRACT: Binaural processing disorder is an important deficit in children with (C)APD so binaural processing evaluations are crucial. There are subjective and objective tests for assessing binaural processing. Subjective tests require patient attention and active so objective evaluation of binaural processing is important. The aim of present study was investigating binaural interaction component (BIC) of middle latency response (MLR) in children suspected to (C)APD. Sixty 8-12 year-old children suspected to (C)APD and sixty normal children were selected based on inclusion criteria. Both groups were matched in terms of sex (40 boys and 20 girls) and age (9.05 ± 1.25 years old). MLR test (monaural right ear, monaural left ear and binaural) was performed in all the cases and BIC was calculated by subtracting binaural response from summed monaural responses. Independent t test showed that latency of Pa and Na (ms), Pa-Na amplitude (µv), BIC latency (ms) and amplitude (µv) were significantly different from normal subjects (p value ?0.001). Present study showed that MLR and BIC of MLR are clinically available and objective tests that can be used to determining children suspected to (C)APD. These tests might have the potential to separating normal children from children with (C)APD objectively.

Project description:Background:Some children appear to not hear well in class despite normal hearing sensitivity. These children may be referred for auditory processing disorder (APD) assessment but can also have attention, language, and/or reading disorders. Despite presenting with similar concerns regarding hearing difficulties in difficult listening conditions, the overall profile of deficits can vary in children with suspected or confirmed APD. The current study used cluster analysis to determine whether subprofiles of difficulties could be identified within a cohort of children presenting for auditory processing assessment. Methods:Ninety school-aged children (7-13 years old) with suspected APDs were included in a cluster analysis. All children had their reading, language, cognition and auditory processing assessed. Parents also completed the Children's Auditory Performance Scale (CHAPS). Cluster analysis was based on tasks where age-norms were available, including word reading (Castles and Coltheart irregular and non-words test), phonological awareness (Queensland University Inventory of Literacy), language [Comprehensive Language of Assessment-4, Comprehensive Assessment of Spoken Language (CASL)], sustained attention (Continuous Performance Test), working memory (digits forward and backward), and auditory processing [Frequency Pattern Test (FPT), Dichotic Digits Test (DDT)]. Hierarchical cluster analysis was undertaken to determine the optimal number of clusters for the data, followed by a k-means cluster analysis. Results:Hierarchical cluster analysis suggested a four-group solution. The four subgroups can be summarized as follows: children with (1) global deficits, n = 35; (2) poor auditory processing with good word reading and phonological awareness skills, n = 22; (3) poor auditory processing with poor attention and memory but good language skills, n = 15; and (4) poor auditory processing and attention with good memory skills, n = 18. Conclusion:The cluster analysis identified distinct subgroups of children. These subgroups display the variation in areas of difficulty observed across different studies in the literature (e.g., not every child with APD has an attention deficit), highlighting the heterogeneous nature of APD and the need to assess a range of skills in children with suspected APD. It would be valuable for future studies to independently verify these subgroups and to determine whether interventions can be optimized based on these subgroups.

Project description:: The purpose of this study was to investigate the relationship between electrophysiologic measures of the binaural interaction component (BIC) of the electrically evoked auditory brainstem response and psychophysical measures of interaural pitch comparisons in Nucleus bilateral cochlear implant users.: Data were collected for 10 postlingually deafened adult cochlear implant users. Each subject conducted an interaural pitch-comparison task using a biphasic pulse train with a pulse rate of 1000 pulses per second (pps) at high stimulation levels. Stimuli were presented in a two-interval, two-alternative forced-choice procedure with roving current variations. A subgroup of four subjects repeated the task at low stimulation levels. BICs were measured using loudness balanced, biphasic current pulses presented at a rate of 19.9 pps for each subject by pairing the electrode 12 (out of 22 intracochlear electrodes) in the right ear with each of 11 electrodes spaced across the electrode array in the left ear. The BIC was measured at high stimulation levels in 10 subjects and at low stimulation levels in 7 subjects. Because of differences in stimulation rate used in BIC measures and interaural pitch comparisons, the actual stimulation levels were different in these two measures. The relationship between BIC responses and results of interaural pitch comparisons was evaluated for each of the individual subjects and at the group level. Evaluation was carried out separately for results obtained at high and low stimulation levels.: There was no significant correlation between results of BIC measures and interaural pitch comparisons on either the individual or group levels. Lower stimulation level did not improve the relationship between these two measures.: No significant correlations between psychophysical measures of interaural pitch comparisons and electrophysiologic measures of the BIC of the electrically evoked auditory brainstem response were found. The lack of correlation may be attributed to methods used to quantify the data, small number of subjects retested at low stimulation levels, and central processing components involved in the interaural pitch-comparison task.
