Stroboscopic light effects during electronic dance music festivals and photosensitive epilepsy: a cohort study and case report.

ABSTRACT:

Objectives

Electronic dance music (EDM) concerts are becoming increasingly popular. Strong stroboscopic light effects are commonly part of these shows, and may provoke seizures in individuals with photosensitive epilepsy. This study aims to examine the risk of epileptic seizures during EDM concerts.

Setting

28 EDM concerts taking place in The Netherlands.

Participants

We describe a young man who experienced a seizure during an EDM concert, and who later showed a positive electroencephalographic provocation test during exposure to video footage of the same concert. Subsequently, we performed a cohort study of 400 343 visitors to EDM concerts, divided in those exposed (concert occurring in darkness) versus unexposed (concert in daylight) to stroboscopic light effects.

Results

In total, 400 343 EDM concert visitors were included: 241 543 (representing 2 222 196 person hours) in the exposed group and 158 800 (representing 2 334 360 person hours) in the control group. The incidence density ratio of epileptic seizures in exposed versus unexposed individuals was 3.5 (95% CI: 1.7 to 7.8; p<0.0005). Less than one-third of cases occurred during use of ecstasy or similar stimulant drugs.

Conclusion

Stroboscopic light effects during EDM concerts occurring in darkness probably more than triple the risk of epileptic seizures. Concert organisers and audience should warn against the risk of seizures and promote precautionary measures in susceptible individuals.

Project description:Background and objectivesElectronic dance music (EDM) party attendees are at high risk for drug use. However, little is known regarding the extent of underreporting of drug use in this population, in part, because use of synthetic drugs is often associated with unknown exposure to adulterant drugs. We estimated the extent of underreported drug use in this population by comparing self-reported use to hair toxicology results.MethodsTime-space sampling was used to survey adults entering EDM events at nightclubs and dance festivals in New York City from January through August of 2019. Seven hundred ninety-four adults were surveyed and 141 provided analyzable hair samples. We queried past-year use of >90 drugs and tested hair samples using ultra-high performance liquid chromatography-tandem mass spectrometry. We compared hair test results to past-year self-reported use and adjusted prevalence estimates by defining use as reporting use or testing positive. Correlates of discordant reporting, defined as testing positive after not reporting use, were estimated.ResultsPrevalence of drug use increased when considering positive hair tests in estimates, with 43.8% of participants testing positive for at least one drug after not reporting use. For example, based on self-report, cocaine use prevalence was 51.1%, and increased by a factor of 1.6 to a prevalence of 80.0% when adding hair test results to self-report. Younger adults (ages 18-25), black and other/mixed race participants, those reporting "other" sexuality, and those with a college degree were at significantly higher risk for testing positive for drugs not reportedly used. Those who self-reported using more types of drugs were less likely to test positive after not reporting use (adjusted prevalence ratio = 0.53, 95% confidence interval = 0.41-0.68).ConclusionsWe detected underreporting of drug use, particularly cocaine and ketamine. More research is needed to determine whether this is driven by intentional underreporting or unknown exposure through adulterants.

Project description:Certain visual images, even in the absence of motion or flicker, can trigger seizures in patients with photosensitive epilepsy. As of yet, there is no systematic explanation as to why some static images are likely to provoke seizures, while others pose little or no risk. Here, we examined the neurophysiology literature to assess whether the pattern of neural responses in healthy visual cortex is predictive of the pathological responses in photosensitive epilepsy. Previous studies have suggested that gamma oscillations (30-80 Hz) measured in human visual cortex may play a role in seizure generation [1,2]. Recently, we and others have shown that increases in gamma band power can come from two very different cortical signals, one that is oscillatory (with a narrow peak between 30 Hz and 80 Hz), and another that is broadband[3]. The oscillatory signal arises from neuronal synchrony in the local population, while the broadband signal reflects the level of asynchronous neuronal activity, and is correlated with multiunit spiking [4]. These two responses have different biological origins and different selectivity for image properties. Here, we followed up on the previous proposals [1,2] to ask whether the image features that increase seizure likelihood in photosensitive epilepsy are linked to narrowband gamma oscillations specifically, or are associated with any kind of increase in visual activity. Based on published work, we compared pairs of image classes on a number of dimensions, and show that the type of image that elicits larger narrowband gamma oscillations in healthy visual cortex is also more likely to provoke seizures or pre-seizure activity in patients with photosensitive epilepsy. In contrast, images that elicit larger broadband, multiunit, or fMRI responses are much less predictive of seizure activity. We propose that a risk factor for seizures in patients with photosensitive epilepsy is engagement of the circuitry that produces gamma oscillations.

Project description:Dance and music often co-occur as evidenced when viewing choreographed dances or singers moving while performing. This study investigated how the viewing of dance motions shapes sound perception. Previous research has shown that dance reflects the temporal structure of its accompanying music, communicating musical meter (i.e. a hierarchical organization of beats) via coordinated movement patterns that indicate where strong and weak beats occur. Experiments here investigated the effects of dance cues on meter perception, hypothesizing that dance could embody the musical meter, thereby shaping participant reaction times (RTs) to sound targets occurring at different metrical positions.In experiment 1, participants viewed a video with dance choreography indicating 4/4 meter (dance condition) or a series of color changes repeated in sequences of four to indicate 4/4 meter (picture condition). A sound track accompanied these videos and participants reacted to timbre targets at different metrical positions. Participants had the slowest RT's at the strongest beats in the dance condition only. In experiment 2, participants viewed the choreography of the horse-riding dance from Psy's "Gangnam Style" in order to examine how a familiar dance might affect meter perception. Moreover, participants in this experiment were divided into a group with experience dancing this choreography and a group without experience. Results again showed slower RTs to stronger metrical positions and the group with experience demonstrated a more refined perception of metrical hierarchy. Results likely stem from the temporally selective division of attention between auditory and visual domains. This study has implications for understanding: 1) the impact of splitting attention among different sensory modalities, and 2) the impact of embodiment, on perception of musical meter. Viewing dance may interfere with sound processing, particularly at critical metrical positions, but embodied familiarity with dance choreography may facilitate meter awareness. Results shed light on the processing of multimedia environments.
