Efficacy of the Monte Carlo method and dose reduction strategies in paediatric panoramic radiography.

ABSTRACT: Monte Carlo (MC) simulation is a simpler radiation dose assessment method than the conventional method, thermoluminescent dosimetry (TLD). MC simulation and TLD were compared as tools to evaluate the effective dose from paediatric panoramic radiography. Various exposure conditions and machine geometries were simulated using the MC method to investigate factors resulting in effective dose reduction. The effective dose of paediatric panoramic radiography was obtained using an MC simulation and its reliability was verified by a comparison with the value obtained using TLD. Next, 7 factors determining the effective dose in the MC simulation were input with 6 equally-spaced values, and a total of 36 simulations were performed to obtain effective dose values. The correlations between each dose-determining factor and the resulting effective dose were evaluated using linear regression analysis. The TLD-measured dose was 3.850?µSv, while the MC simulation yielded a dose of 3.474?µSv. Beam height was the factor that most strongly influenced the effective dose, while rotation angle and focus-to-patient distance were the least influential factors. MC simulation is comparable to TLD for obtaining effective dose values in paediatric panoramic radiography. Obtaining panoramic radiography with a short beam height can effectively reduce the dose in paediatric patients.

Project description:PurposeTo demonstrate potential of correlated sampling Monte Carlo (CMC) simulation to improve the calculation efficiency for permanent seed brachytherapy (PSB) implants without loss of accuracy.MethodsCMC was implemented within an in-house MC code family (PTRAN) and used to compute 3D dose distributions for two patient cases: a clinical PSB postimplant prostate CT imaging study and a simulated post lumpectomy breast PSB implant planned on a screening dedicated breast cone-beam CT patient exam. CMC tallies the dose difference, ΔD, between highly correlated histories in homogeneous and heterogeneous geometries. The heterogeneous geometry histories were derived from photon collisions sampled in a geometrically identical but purely homogeneous medium geometry, by altering their particle weights to correct for bias. The prostate case consisted of 78 Model-6711 (125)I seeds. The breast case consisted of 87 Model-200 (103)Pd seeds embedded around a simulated lumpectomy cavity. Systematic and random errors in CMC were unfolded using low-uncertainty uncorrelated MC (UMC) as the benchmark. CMC efficiency gains, relative to UMC, were computed for all voxels, and the mean was classified in regions that received minimum doses greater than 20%, 50%, and 90% of D(90), as well as for various anatomical regions.ResultsSystematic errors in CMC relative to UMC were less than 0.6% for 99% of the voxels and 0.04% for 100% of the voxels for the prostate and breast cases, respectively. For a 1 × 1 × 1 mm(3) dose grid, efficiency gains were realized in all structures with 38.1- and 59.8-fold average gains within the prostate and breast clinical target volumes (CTVs), respectively. Greater than 99% of the voxels within the prostate and breast CTVs experienced an efficiency gain. Additionally, it was shown that efficiency losses were confined to low dose regions while the largest gains were located where little difference exists between the homogeneous and heterogeneous doses. On an AMD 1090T processor, computing times of 38 and 21 sec were required to achieve an average statistical uncertainty of 2% within the prostate (1 × 1 × 1 mm(3)) and breast (0.67 × 0.67 × 0.8 mm(3)) CTVs, respectively.ConclusionsCMC supports an additional average 38-60 fold improvement in average efficiency relative to conventional uncorrelated MC techniques, although some voxels experience no gain or even efficiency losses. However, for the two investigated case studies, the maximum variance within clinically significant structures was always reduced (on average by a factor of 6) in the therapeutic dose range generally. CMC takes only seconds to produce an accurate, high-resolution, low-uncertainly dose distribution for the low-energy PSB implants investigated in this study.

Project description:A recently introduced brachytherapy system for partial breast irradiation, MammoSite, consists of a balloon applicator filled with contrast solution and a catheter for insertion of an 192Ir high-dose-rate (HDR) source. In using this system, the treatment dose is typically prescribed to be delivered 1 cm from the balloon's surface. Most treatment-planning systems currently in use for brachytherapy procedures use water-based dosimetry with no correction for heterogeneity. Therefore, these systems assume that full scatter exists regardless of the amount of tissue beyond the prescription line. This assumption might not be a reasonable one, especially when the tissue beyond the prescription line is thin. In such a case, the resulting limited scatter could cause an underdose to be delivered along the prescription line. We used Monte Carlo simulations to investigate how the thickness of the tissue between the surface of the balloon and the skin or lung affected the treatment dose delivery. Calculations were based on a spherical water phantom with a diameter of 30 cm and balloons with diameters of 4 cm, 5 cm, and 6 cm. The dose modification factor is defined as the ratio of the dose rate at the typical prescription distance of 1 cm from the balloon's surface with full scatter obtained using the water phantom to the dose rate with a finite tissue thickness (from 0 cm to 10 cm) beyond the prescription line. The dose modification factor was found to be dependent on the balloon diameter and was 1.098 for the 4-cm balloon and 1.132 for the 6-cm balloon with no tissue beyond the prescription distance at the breast-skin interface. The dose modification factor at the breast-lung interface was 1.067 for the 4-cm balloon and 1.096 for the 6-cm balloon. Even 5 cm of tissue beyond the prescription distance could not result in full scatter. Thus, we found that considering the effect of diminished scatter is important to accurate dosimetry. Not accounting for the dose modification factor may result in delivering a lower dose than is prescribed.

Project description:The variety of cone-beam computed tomography (CBCT) machines and their applications has rapidly increased in recent years, making the dose evaluation of individual devices an important issue. Patient doses from CBCT were assessed with two different methods: optically stimulated luminescence dosimeter (OSLD) measurements and Monte Carlo (MC) simulations, in four different examination modes. Based on an analysis of the measurement process and the obtained values, a recommendation is made regarding which method is more practical and efficient for acquiring the effective dose of CBCT. Twenty-two OSLDs were calibrated and equipped in human phantoms of head and neck organs. They were exposed to radiation from two CBCT units-CS9300 (Carestream Dental LLC, Atlanta, Georgia) and RAYSCAN α+ (Ray Co. Ltd, Hwaseong-si, Korea)-using two different examination modes. The dose recorded using the OSLDs was used to calculate the organ dose and the effective dose for each unit in each examination mode. These values were also calculated using MC software, PCXMC (STUK, Helsinki, Finland). The organ doses and effective doses obtained using both methods were compared for each examination mode of the individual units. The OSLD-measured effective dose value was higher than that obtained using the MC method for each examination mode, except the dual jaw mode of CS9300. The percent difference of the effective dose between the two methods ranged from 4.0% to 14.3%. The dose difference between the methods decreased as the field of view became smaller. The organ dose values varied according to the method, although the overall trend was similar for both methods. The organs showing high doses were mostly consistent for both methods. In this study, the effective dose obtained by OSLD measurements and MC simulations were compared, and both methods were described in detail. As a relatively efficient and easy-to-perform method, we cautiously suggest using MC simulations for dose evaluations in the future.
