Project description:OBJECTIVE:The aim of this study was to assess the occurrence of malaria outbreak and investigate contracting factors of malaria in Simada District, Northwest Ethiopia. A single observation original research. RESULTS:Among the total 54 cases, 44 (81.5%) of them were confirmed malaria cases. The average attack rate was 20 per 100 and slide positivity rate was 81.5%. People in the age group of 5-14 years were most affected with an attack rate of 37%. Presence of water bodies for mosquito breeding inside less than 1 km radius (AOR?=?3.32, 95% CI 1.18-9.34), no knowledge on transmission, prevention and control mechanisms of malaria (AOR?=?4.36, 95% CI 1.64, 12.23), not using Insecticide Treated Bed Net (AOR?=?5.85, 95% CI 1.94, 17.54) and absence of environmental control (AOR?=?10.01, 95% CI 2.94, 33.33) were factors associated with malaria outbreak.

Project description:ObjectiveScabies is an infection of the skin, which caused by human itch mite Sarcoptes scabiei. It is a common health problem in Ethiopia, especially during disasters, poor sanitation and overcrowded living condition. However, investigation on scabies outbreak and associated factors was absent or scarce in the country in general and in the study area in particular. Hence, this study was intended to investigate scabies outbreak, identify risk factors, and recommend preventive measures in Kechabira district, Kembata Tembaro zone, Southern Ethiopia.ResultWe identified a total of 243 scabies cases line listed with overall prevalence of 2.5% and attack rate of (AR) 20.5 per 1000 populations and no death was reported. Of the suspected cases 126 (51.9%) were males and 117 (48.1%) were females. The median age was 24 years with inter-quartile range (IQR) of 22 years. The highest cases were seen in children aged 5-14 (50.6%) years. The cases were seen in three villages and the highest incidence was in Burchana, 23.9 per 1000 population. Identified determinant factors for scabies outbreak were sharing clothes with scabies patients (AOR = 6.08, 95% CI [1.54-23.92], and households having greater than six family members AOR = 38.755, 95% CI [8.084-185.787].

Project description:BackgroundGlobally in 2015 about 214 million malaria cases and 438,000 deaths were reported with 75% were from Sub-Saharan Africa. Malaria transmission in Ethiopia is unstable, and outbreaks are considered public health emergencies. Understanding the trigger for outbreaks in low-transmission areas can help facilitate malaria elimination. On July 8th malaria outbreak was reported from Laelay Adyabo district. The objective was to investigate the magnitude and associated factors with malaria outbreak.MethodsWe defined a case as confirmed malaria using microscopy or a rapid diagnostic test for Plasmodium parasites in a resident of Laelay-Adyabo District from July 9-28, 2017. We identified cases by reviewing health facility records and conducted a case-control study using randomly-selected cases from a line list, and two neighborhood controls per case. A pretested semi-structured questionnaire adapted from WHO malaria guidelines was used to collect data from case-patients and controls. We calculated crude (COR) and adjusted (AOR) odds ratios to identify factors associated with malaria.ResultA total of 145 confirmed malaria cases (57.9% males) were identified with village attack rate (AR) of 12.1/1000. The AR was higher among males than females (14.1 verses 10.1/1000), children aged 5-14 years (12.9/1000), and in Zelazle Kebelle (13.6/1000 population). Wearing protective clothing (AOR = 0.27, 95% CI 0.11-0.66), having good knowledge of malaria transmission (AOR = 0.25, 95% CI 0.08-0.75), having waste collection material at home (AOR = 0.25 95% CI 0.11-0.61), availability of mosquito breeding sites around home (AOR = 9.08, 95% CI 3.6-22.93), and staying outdoor overnight (AOR = 3.7, 95% CI 1.44-9.56) were independently associated with malaria.ConclusionThe overall attack rate for malaria during this outbreak was high affecting > 1% of the population. Wearing protective clothing at night, knowing about malaria transmission, having mosquito breeding sites around the home, staying outdoors overnight, and having waste collection material in their house were predictors of the infection. Laelay Adyabo district health office should provide health education on malaria transmission and prevention measures and how to clear mosquito breeding sites.

Project description:An investigation of a foot-and-mouth disease (FMD) outbreak was conducted between late October and mid-December 2019 in Tigray region. The outbreak investigation team collected epidemiological data from the six villages of Kafta Humera and Seharti Samre districts, including morbidity proportions, mortality proportions, and clinical signs, and cattle management and vaccination history were collected via participatory methods, including interviews and group discussions with local experts and farmers in Kafta Humera and reports from the district veterinarians in Seharti Samre. Twenty-two tissue samples were collected for laboratory confirmation. Overall, 4,299/9,811 (43.8%) and 13,654/16,921 (80.6%) cattle showed clinical signs for FMD in Kafta Humera and Seharti Samre, respectively. In Kafta Humera, the highest morbidity proportion was found in adult cows and heifers (48.1%), followed by 27.8% in oxen and 15.9% in calves. In Seharti Samre, the morbidity proportion was similar in all age groups at ~81%. No death of FMD-suspected cattle was reported throughout the outbreak. The serotype of foot-and-mouth disease virus (FMDV) identified by laboratory analysis differed between the two districts (serotype O in Kafta Humera and serotype A in Seharti Samre). We, therefore, suggest that the outbreaks in the two districts occurred independently from each other. Experts and farmers were interviewed and believed that the outbreak in Kafta Humera was most likely caused by interaction between cattle and wildlife from the surrounding Kafta Sheraro National Park, which share common grazing land. This outbreak investigation showed that FMD can cause devastating cattle morbidity. A regular vaccination program against the identified circulating FMDV serotypes with sufficient coverage is required to avoid future outbreaks.

Project description:BackgroundApproximately 16 million teenagers aged 15-19 years and 2 million teenagers under the age of 15 years give birth annually, with 95% of these births occurring in developing countries. Ethiopia has one of the highest teenage fertility rates in Sub-Saharan Africa; however determinants of teenage pregnancy are not well studied. Therefore, this study aimed to identify determinants of teenage pregnancy among female teenagers in Degua Tembien district, Tigray, Northern Ethiopia, in 2015.MethodsA community-based case-control study was conducted in Degua' Tembien district from February 01, 2015 to March 15, 2015 with a randomly selected total sample size of 414 females (with a ratio of 1:2 case to control, 138 and 276 respectively). Data were entered in to Epi-Info and analyzed using SPSS software. Multivariable logistic regression was used to assess predictors of the outcome variable; variables with a p-value <0.25 in bivariable analysis were included in the model. Statistically significance was considered at a p-value ≤0.05 in both bivariable and multivariable logistic regression analyses.ResultThe mean ages (plus or minus standard deviation (±SD)) of cases and controls were 18.47 (0.72) and 17.09 (1.2) years, respectively. After adjustment for other variables, predictors of teenage pregnancy included: lower monthly income below ~$25 and ~$25-50 (adjusted odds ratio (AOR) = 23.96; 95% confidence interval (95%CI) 4.89-117.29 and AOR = 4.91; 95%CI 1.64-14.66, respectively); aged 18-19 years (AOR = 16.75; 95%CI 6.45-43.47); being married (AOR = 15.91; 95%CI 7.43-34.04); not communicating with parents on reproductive health issues (AOR = 6.52; 95%CI 3.12-13.64) and having a history of maternal teenage pregnancy (AOR = 4.14; 95%CI 1.84-9.33).ConclusionThe factors associated with teenage pregnancy in our study were lower family monthly income, being married, being in the 18-19 year age group, not communicating with parents on reproductive health issues and having a maternal history of teenage pregnancy. Programs that encourage parent-teenage communication of reproductive health issues, starting from early adolescence, in order to build skills to prevent pregnancy in the late teenage years, are very important. In addition, multi-pronged activities across sectors that encourage delayed marriage and improve health service utilizations for girls are essential.

Project description:BackgroundPertussis is a highly contagious respiratory illness caused by Bordetella pertussis. It is one of the most common vaccine-preventable bacterial infections that affects all susceptible individuals, regardless of age. Investigation was done to verify the existence of an outbreak and to identify associated risk factors contributed for the occurrence of an outbreak in Tork and Warkaye villages of Mekdela district.MethodsUnmatched community based case control and descriptive cross sectional investigation were conducted with one to two ratios. We used structured questionnaire to collect data from cases and controls.ResultsA total of 215 cases and eight deaths were identified with an overall attack rate of 1.3 per 1000 population. The mean age of the cases was 3.7 years which was ranged from 3 months to 45 years. The more affected groups were females. On multivariate logistic regression analysis, the risk factor that remained independently statically significant associated with developing pertussis was presence of infected person in the family AOR (adjusted odds ratio): 5.859, (95% CI 2.526-13.589). But previously sick with pertussis AOR: 0.053, (95% CI 0.006-0.460) and receiving full dose of vaccine AOR: 0.256, (95% CI 0.080-0.818) were remained as protective factors from pertussis infection.ConclusionsThe suspected pertussis outbreak was occurred in remote pocket villages/kebeles of Mekdela district. Routine immunization was not given regularly and functional refrigerators were not available in the health posts. Routine immunization services and treatment of infected patients with appropriate antibiotics should be intensified.

Project description:BackgroundHypertensive disorders of pregnancy are a global public health concern both in developed and developing countries. However, evidences regarding the risk factors of hypertensive disorders of pregnancy are limited particularly in Ethiopia. The aim of the study was to assess risk factors associated with hypertensive disorders of pregnancy among mothers in public hospitals of Tigray.MethodsThe study was conducted in seven public hospitals of Tigray region, Ethiopia from June 2017 to November 2017. A facility based matched case-control study was employed to select 110 cases and 220 controls who were pregnant women. Cases and controls were matched by parity status. A case was a mother diagnosed to have hypertensive disorders of pregnancy by an obstetrician in the antenatal period while a control was a mother who did not have a diagnosis of hypertensive disorders of pregnancy. Data were collected by face to face interview technique using a pretested questionnaire and a checklist. Conditional logistic regression analysis was used to identify the independent predictor variables. Adjusted matched odds ratio with its corresponding 95% confidence interval was used and significance was claimed at P-value less than 0.05. Overall findings were presented in texts and tables.ResultsRural residents were at greater odds of suffering from hypertensive disorders (OR?=?3.7, 95% CI; 1.9, 7.1). Similarly, mothers who consume less amount of fruits in their diet had 5 times higher odds of developing hypertensive disorders than those who consume fruits regularly (OR?=?5.1, 95% CI; 2.4, 11.15). Overweight (BMI?>?25 Kg/m2) mothers were also at risk of developing hypertensive disorders of pregnancy as compared with the normal and underweight mothers (AOR?=?5.5 95% CI; 1.12, 27.6). The risk of developing hypertensive disorders of pregnancy was 5.4 times higher among diabetic mothers.ConclusionRural residence, less fruit consumption, multiple pregnancy, presence of gestational diabetes mellitus and pre-pregnancy overweight were identified as independent risk factors in this study. It is recommended that health care givers may use these factors as a screening tool for the prediction, early diagnoses as well as timely interventions of hypertensive disorders of pregnancy.

Project description:BackgroundNinety percent of the global annual malaria mortality cases emanate from the African region. About 80-90% of malaria transmissions in sub-Saharan Africa occur indoors during the night. In Zimbabwe, 79% of the population are at risk of contracting the disease. Although the country has made significant progress towards malaria elimination, isolated seasonal outbreaks persistently resurface. In 2017, Beitbridge District was experiencing a second malaria outbreak within 12 months prompting the need for investigating the outbreak.MethodsAn unmatched 1:1 case-control study was conducted to establish the risk factors associated with contracting malaria in Ward 6 of Beitbridge District from week 36 to week 44 of 2017. The sample size constituted of 75 randomly selected cases and 75 purposively selected controls. Data were collected using an interviewer-administered questionnaire and Epi Info version 7.2.1.0 was used to conduct descriptive, bivariate and multivariate analyses of the factors associated with contracting malaria.ResultsFifty-two percent of the cases were females and the mean age of cases was 29?±?13 years. Cases were diagnosed using rapid diagnostic tests. Sleeping in a house with open eaves (OR: 2.97; 95% CI 1.44-6.16; p?<?0.01), spending the evenings outdoors (OR: 2.24; 95% CI 1.04-4.85; p?=?0.037) and sleeping in a poorly constructed house (OR: 4.33; 95% CI 1.97-9.51; p?<?0.01) were significantly associated with contracting malaria while closing eaves was protective (OR: 0.45; 95% CI 0.20-1.02; p?=?0.055). After using backward stepwise logistic regression, sleeping in a poorly constructed house was associated with five-fold odds of getting sick from malaria (AOR: 8.40; 95% CI 1.69-41.66; p?=?0.009). Those who had mosquito nets did not use them consistently. The district health team and the rural health centre were well prepared to response despite having limited human resources.ConclusionHealth promotion messages should emphasize the importance of closing the entry points of the malaria vector, and the construction of better houses in the future. Residents had to be educated in the importance of consistent use of mosquito nets. The district had to improve malaria preventive measures like distribution of mosquito nets and lobby for more human resources to assist with malaria surveillance thus, curbing the recurrence of malaria outbreaks.

Project description:BackgroundHepatic venous-occlusive disease is blockage of microscopic veins in the liver causing 20-50% mortality. Ingestion of pyrrolizidine alkaloid plant, radiation therapy, and post-bone-marrow-transplant reactions are the commonest causes. In Ethiopia, a venous-occlusive disease outbreak was identified in 2002 in Tahtay Koraro district, Tigray. Suspected due to ingestion of the toxic pyrrolizidine alkaloid plant Ageratum conyzoids, found throughout the district. We aimed to describe the surveillance data of venous-occlusive disease from September 2006 to August 2016 in Tahtay koraro district, Ethiopia, 2017.MethodologyWe defined a possible Hepatic venous-occlusive disease case as any patient with abdominal pain for at least 2 weeks, abdominal distention, and hepato-splenomegaly during September 2006-August 2016. We reviewed previous district line lists, weekly reports, and clinical records to identify and describe cases. Agricultural interventions were obtained from the agricultural offices of the district.ResultWe identified 179 possible cases with 83 deaths with a case-fatality rate of 46.3%. Among cases, 110 (61.5%) were males and 113 (63%) were >15 years. In total, 164 (91.6%) cases were from one village (Kelakil). The pick number of cases of VOD in this village was during 2008/09 which was 1076. The highest incidence (86/100,000) occurred in 2008. During the study period, 2,746 years of potential life were lost due to Hepatic venous-occlusive disease. Mechanical removal of the Ageratum started in 2011.ConclusionHepatic venous-occlusive disease was an ongoing problem in Tahtay Koraro; However, the problem has largely been alleviated by displacing people from the affected area and removing the causative weed. More research is needed to understand why Kelakil village was more affected despite the widespread presence of the weed. Chemical and mechanical removal of the Ageratum could strengthen intervention activities.

Project description:BackgroundIncidence and mortality of cervical cancer is the leading cancer among women in Ethiopia. Absence of effective detection methods and treatment strategies is a major reason for the sharply rising cervical cancer rates in developing countries.ObjectiveTo determine factors affecting utilization of cervical cancer screening services among women attending public hospitals in Tigray region in 2018.MethodsHospital based unmatched case control study was applied with sample size of 312 cases and 312 controls. Data was entered to Epi data version 3.1 and exported to SPSS version 20. The odds ratio with their 95% confidence interval, two-tailed P value was calculated. Variables with P value ≤ 0.05 in the bivariate analysis were included in the multivariate logistic regression model.ResultsBeing in the age group of 30-39 and 40-49 years were two and four times more likely to utilize cervical cancer screening than those who were 21-29 years (AOR = 2.15 95%CI:1.11, 4.17 and AOR = 3.86 95%CI:1.48, 10.06) respectively. Current occupation with governmental and private employee were four and three times more likely to utilize the screening service than those housewife respectively (AOR = 3.85 95%CI: 1.87, 7.92 and AOR = 3.17 95%CI: 1.31, 7.66). Having ever given birth and history of multiple sexual partners were more likely to utilize the screening service (AOR = 2.57 95%CI: 1.02, 6.50) and (AOR = 2.65 95%CI: 1.10, 6.40) respectively.ConclusionsThere is a need to strengthen policy and guidelines on cervical cancer screening among women particularly with regarding age group of 21-29, current occupation with housewife, single sexual partner and null parity. All stakeholders should give priority on the promotion and initiation of women to acquired good knowledge and attitude on cervical cancer screening.
