All-Arthroscopic Posterior Cruciate Ligament Distal Reattachment With Extracortical Fixation.

ABSTRACT: Avulsion fractures of the posterior cruciate ligament (PCL) are a rare, but serious, knee pathology. Early surgical treatment is regarded as necessary to maintain knee stability. Recommended management involves open reduction with internal fixation through a posterior approach. However, open surgeries are associated with a greater risk of complications. Current data suggest excellent outcomes for arthroscopic-assisted fixation, with a low complication rate. The purpose of this technical note is to present an all-arthroscopic PCL distal reattachment with extracortical fixation technique. This technique provides precise fracture reduction, is easily reproducible, and is relatively safe to treat PCL tibial avulsion fractures.

Arthroscopic Primary Bundle-Specific Posterior Cruciate Ligament Repair with Transosseous Fixation.

Project description:Posterior cruciate ligament (PCL) injuries are most commonly associated with multiligamentous knee injuries. Isolated rupture can be treated nonoperatively with bracing, but with concomitant surgical injuries or high-grade instability, operative intervention may be warranted. While historically PCL injuries were surgically managed with open primary repair, contemporary surgical options include arthroscopic primary repair and reconstruction. Appropriate patient selection is critical in avoiding residual laxity following primary repair, and innovations in advanced imaging and arthroscopic technology now allow for identification of suitable patients. In this technical vote, we describe a method for anatomic bundle-specific primary PCL repair with transosseous fixation. The appropriate patient for this procedure has a femoral-sided avulsion of 1 or both PCL bundles, presents with an acute or subacute injury, and has adequate tissue quality for bundle reapproximation to the footprint. This allows for minimally invasive, anatomic restoration of tension for each bundle.

Arthroscopic Suture-to-Loop Fixation of Posterior Cruciate Ligament Tibial Avulsion Fracture.

Project description:Avulsion fracture of the tibial insertion of the posterior cruciate ligament (PCL) receives constant concern. Arthroscopic procedures have long been attempted because of their minimally invasive nature, and various related techniques have been reported. However, the best arthroscopic method is still being pursued. In this article, we introduce an arthroscopic suture ligation and backup adjustable-loop fixation technique for PCL tibial avulsion fracture. The critical points of this technique are proper ligation of the PCL, proper location of the 2 tibial tunnels to create pulleys for posterior-inferior reduction of the bone fragment, and additional backup suture loop fixation. Our experience indicates that this technique is efficient and relatively simple. We consider that the introduction of this technique will provide a reasonable choice in the treatment of PCL tibial avulsion fracture.

Femoral Fixation With Curve Cross-Pin System in Arthroscopic Posterior Cruciate Ligament Reconstruction.

Project description:Posterior cruciate ligament (PCL) ruptures account for 1% to 44% of all acute ligament injuries of the knee. In this paper we wanted to try out a system for femural fixation. Hamstring tendons are harvested and standard tibial tunnel is prepared using the transtibial PCL guide; by identifying the PCL footprint, the femoral half tunnel 27 to 30 mm with in-out technique is performed. The femoral rod of a curve cross-pin system is inserted into the anterolateral access within the femoral half tunnel. The guide block is placed 2.5 cm anterior (in a coronal plane) and 2.5 cm proximal to the lateral epicondyle. The arc attachment is assembled and the bone stock assessed with the bone gauge pin in contact with the cortex of the medial femoral condyle. Then the first sleeve over trocar is assembled, and the graft is passed through the tunnel and fixed on the femur with the pins and on the tibia with interferential screw. After biomechanical studies we obtained a maximum load at 930.95 N and maximum stiffness at 58.92 N/mm.

Dual Posteromedial Portal Technique: Arthroscopic Loop Suspensory Fixation-Posterior Cruciate Ligament Tibial Avulsion Fracture.

Project description:Posterior cruciate ligament avulsion fracture injuries have been associated with an increased risk of meniscal tears and premature patellofemoral/medial compartment osteoarthritis. Sports-related trauma is a common cause of posterior cruciate ligament avulsion fractures. Surgical management is recommended for displaced bony avulsion of the posterior cruciate ligament to stabilize the knee and prevent knee pain, nonunion, and osteoarthritis progression. This article discusses our preferred treatment using a loop suspensory fixation device through a dual posteromedial portal technique.

Arthroscopic Fixation Technique for Avulsion Fracture of the Posterior Cruciate Ligament From the Tibia.

Project description:This study describes an arthroscopic pullout fixation technique for small and comminuted avulsion fractures of the posterior cruciate ligament from the tibia. Intra-articular surgery required 3 arthroscopic portals, the anterolateral, anteromedial, and posteromedial portals. To simplify surgery, the posterolateral portal was omitted. A 2.4-mm K-wire was inserted through the anterior incision to the center of the bone fragment. This central guidewire was subsequently overdrilled with a 4.0-mm cannulated drill. The fixation material consisted of Pass Telos artificial ligaments inserted through the fiber loop of a fixed suspensory device such as RIGIDLOOP. The leading end of the thread of the RIGIDLOOP was pulled out through the anteromedial portal. The button of RIGIDLOOP was gradually advanced through the bone tunnel. The button was pulled out and flipped over the bony fragment. The artificial ligament was pulled distally to reduce the bony fragment, and fixed onto the tibia using a ligament button while applying anterior drawer force to the proximal tibia with the knee flexed at 90°. This minimally invasive procedure was successful in treating small and comminuted avulsion fracture of the tibial attachment of the posterior cruciate ligament.

This Technical Note describes one such relatively low-cost arthroscopic suture tape pull-out technique using both an anterior and transseptal portals to fix the posterior cruciate ligament avulsion fragment.

Arthroscopic Primary Posterior Cruciate Ligament Repair With Suture Augmentation.

Project description:Isolated posterior cruciate ligament (PCL) injuries are relatively rare and PCL injuries most commonly occur in the setting of multiligamentous knee injuries. PCL injuries can be treated with primary repair, which has the advantages of preserving the native tissue, maintaining proprioception, and minimal invasive surgery when compared with reconstruction surgery. Historically, primary repair of PCL injuries was performed in all tear types using an open approach, and, although the subjective outcomes were relatively good, patients often had residual laxity. Modern advances and increasing knowledge could improve the outcomes of PCL repair. With magnetic resonance imaging patients with proximal tears and sufficient tissue quality can be selected, and with arthroscopy and suture anchors minimal invasive surgery with direct fixation can be performed. Furthermore, with suture augmentation the healing of the repaired PCL can be protected and the residual laxity can be prevented. In this Technical Note, we describe the surgical technique of arthroscopic primary repair of proximal PCL tears with suture anchors and suture augmentation. The goal of arthroscopic primary repair is the preservation of the native PCL using a minimally invasive method and subsequent protection of this repair using suture augmentation.

Arthroscopic Reduction and Internal Fixation of Posterior Cruciate Ligament Avulsion Fracture Using an Adjustable-Length Loop Device.

Project description:A displaced avulsion fracture at the tibial attachment of the posterior cruciate ligament is considered an indication for surgical reduction and internal fixation because nonunion and remaining posterior instability of the knee are common consequences of conservative treatment. The problems with standard open surgical techniques are that they are relatively invasive despite the limited operative field and it is impossible to explore intra-articular lesions by the posterior approach. An arthroscopic procedure has the advantage of being minimally invasive and allowing the surgeon to detect and treat associated intra-articular injuries. We present an arthroscopic reduction-internal fixation technique using an adjustable-length loop device. A trans-septal portal is created to visualize the fracture fragment directly, and the fragment is reduced and penetrated with a cannulated drill under fluoroscopic guidance. An adjustable-length loop device is relayed from the posteromedial portal and pulled out through the fragment in an anterograde fashion, placing a button on top of the fragment. By tightening the loop, downward compression can be applied to the fragment. Overall, this technique provides good reduction and bone union, and excellent clinical outcomes, including posterior knee stability, can be achieved.

Arthroscopic Nontransosseous Suture Fixation of Anterior Cruciate Ligament Avulsion Fractures.

Project description:Few cases of anterior cruciate ligament (ACL) tibial avulsion injuries occur in adolescents. Bony ACL avulsion from the tibial side has been treated by various methods ranging from conservative management to a wide range of operative procedures. Various arthroscopic operative procedures are available to reduce and fix these fractures. We describe a technique of arthroscopic "8" knotted fixation without any tibial tunnels for tibial-sided ACL avulsion. This simple technique avoids the potential complications of hardware fixation within a joint. It represents an arthroscopic treatment option for ACL tibial avulsion injuries.
