Arthroscopic Transseptal Single-Tunnel Posterior Cruciate Ligament Refixation.

ABSTRACT: Posterior cruciate ligament avulsions are relatively rare and often go undiagnosed. However, they need to be fixed to restore knee biomechanics. Fixation techniques vary from open to arthroscopic with comparable results. Arthroscopic techniques are less invasive; however, they are technically demanding. This Technical Note describes one such relatively low-cost arthroscopic suture tape pull-out technique using both an anterior and transseptal portals to fix the posterior cruciate ligament avulsion fragment.

Project description:The preservation of posterior cruciate ligament (PCL) remnant augmentation was recently proposed as a technique for PCL reconstruction. The technique achieved isometry and anatomic position of the PCL graft, but it was technically difficult. The present technique is a modified transseptal technique with visualization from both posteromedial and posterolateral portals. Moreover, this rod-pulley technique could prevent the killer turn effectively. The result was anatomic remnant augmented PCL graft.

Adjustable Button Devices for All-Arthroscopic Posterior Cruciate Ligament Reconstruction Using the Hamstrings Tendons and the "Forgotten" Transseptal Approach.

Project description:Posterior cruciate ligament (PCL) ruptures account for nearly 20% of all ligamentous knee injuries. These may be either isolated or in the setting of a more complex knee trauma. Isolated tears with moderate posterior laxity (grades I or II) are commonly treated conservatively; nevertheless, symptomatic grade III injuries frequently require surgical intervention. PCL reconstruction remains a challenging surgery for multiple reasons like the neurovascular structures' proximity, the difficult passage of the graft with the "killer turn" angle, or the risk of poor graft fixation. We describe an all-inside operative technique using hamstrings tendon autografts with tibial and femoral adjustable buttons cortical fixation and the visualization of the posterior transseptal portal.

Arthroscopic Anterior Cruciate Ligament Femoral Tunnel Visualization for Button Fixation.

Project description:Arthroscopic reconstruction of the anterior cruciate ligament (ACL) remains one of the most commonly performed procedures in orthopaedic surgery. We describe a technique to visualize the button being advanced through the femoral tunnel using an arthroscope placed in the anteromedial portal. Looking into the femoral tunnel in line with the sutures, this technique allows the surgeon to directly visualize the femoral button as it traverses the femoral tunnel and confirms that it is engaged over the femoral cortex. Certain complications can arise, however, with the use of a suspensory fixation with a button on the femoral cortex. This method can decrease operative time and complication rates.

Arthroscopic Primary Posterior Cruciate Ligament Repair With Suture Augmentation.

Project description:Isolated posterior cruciate ligament (PCL) injuries are relatively rare and PCL injuries most commonly occur in the setting of multiligamentous knee injuries. PCL injuries can be treated with primary repair, which has the advantages of preserving the native tissue, maintaining proprioception, and minimal invasive surgery when compared with reconstruction surgery. Historically, primary repair of PCL injuries was performed in all tear types using an open approach, and, although the subjective outcomes were relatively good, patients often had residual laxity. Modern advances and increasing knowledge could improve the outcomes of PCL repair. With magnetic resonance imaging patients with proximal tears and sufficient tissue quality can be selected, and with arthroscopy and suture anchors minimal invasive surgery with direct fixation can be performed. Furthermore, with suture augmentation the healing of the repaired PCL can be protected and the residual laxity can be prevented. In this Technical Note, we describe the surgical technique of arthroscopic primary repair of proximal PCL tears with suture anchors and suture augmentation. The goal of arthroscopic primary repair is the preservation of the native PCL using a minimally invasive method and subsequent protection of this repair using suture augmentation.

Quad Tendon Autograft for Posterior Cruciate Ligament Reconstruction Using Transseptal and Posteromedial Portals.

Project description:We describe a surgical technique for reconstruction of the posterior cruciate ligament with quad tendon autograft using transseptal portal. We place the guide for the tibial socket through the posteromedial portal instead of transnotch, which is the most common practice. The use of the transseptal portal allows good visualization during the drilling of the tibial socket to protect the neurovascular bundle while avoiding the use of fluoroscopy. The advantage of using the posteromedial approach is the easy placement of the drill guide and to option to pull the graft once through the posteromedial portal and a second time through the notch, which helps passing the "killer turn." The quad tendon is harvested with a bone block that is placed in the tibial socket and fixed with screws in the tibial and femoral side.

Combined Anterior Cruciate Ligament, Medial Collateral Ligament, and Posterior Oblique Ligament Reconstruction Through Single Tibial Tunnel Using Hamstring Tendon Autografts.

Project description:Combined anterior cruciate ligament (ACL) and medial collateral ligament (MCL) injuries are the most common type of combined ligamentous injury of the knee. The optimal treatment for these combined injuries is controversial. Combined ACL and MCL-posterior oblique ligament (POL) reconstruction avoids late anteromedial rotatory instability and chronic valgus instability of the knee and decreases the increased stress on the ACL graft. Graft choice (hamstring tendon autograft, quadriceps bone-patellar tendon-bone autograft, or Achilles tendon allograft) and anatomic restoration of the medial and posteromedial corner of the knee are challenges of this combined reconstruction. This article describes a technique that allows combined ACL and MCL-POL reconstruction. The hamstring tendons from the contralateral limb are tripled and used as the ACL graft. The gracilis tendon from the ipsilateral limb is doubled and used as the MCL-POL graft. The semitendinosus tendon of the ipsilateral limb is preserved. After ACL reconstruction, the MCL-POL graft is suspended on the ACL graft at the distal end of the tibial tunnel and the graft limbs are used for open reconstruction of the MCL and POL. Three interference screws (Arthrex, Naples, FL) and 1 metal staple are used for graft fixation of this combined reconstruction.

All-Arthroscopic Posterior Cruciate Ligament Distal Reattachment With Extracortical Fixation.

Project description:Avulsion fractures of the posterior cruciate ligament (PCL) are a rare, but serious, knee pathology. Early surgical treatment is regarded as necessary to maintain knee stability. Recommended management involves open reduction with internal fixation through a posterior approach. However, open surgeries are associated with a greater risk of complications. Current data suggest excellent outcomes for arthroscopic-assisted fixation, with a low complication rate. The purpose of this technical note is to present an all-arthroscopic PCL distal reattachment with extracortical fixation technique. This technique provides precise fracture reduction, is easily reproducible, and is relatively safe to treat PCL tibial avulsion fractures.

Four-Tunnel Double-Bundle Posterior Cruciate Ligament Reconstruction With Remnant Preservation.

Project description:The clinical results of posterior cruciate ligament reconstruction (PCL) still leave much room for improvement. To get better knee stability, the double-bundle reconstruction, the use of strong grafts, and the preservation of the remnant have been applied. However, the combination of the 3 measures has seldom been reported. Thus I introduce a 4-tunnel double-bundle PCL reconstruction technique, in which I combine the use of strong grafts and preservation of the remnant. The critical point of this technique is manipulating skillfully across the femoral notch with the preservation, as well as the hindrance to operation of the PCL remnant. My clinical experience indicates this technique is somewhat complicated but most effective. I believe the introduction of this technique will provide an additional choice when PCL reconstruction is to be performed.

Project description:Combined posterior cruciate ligament (PCL) and medial collateral ligament (MCL) injuries represent a complex pathology that requires a thorough clinical and radiographic examination to diagnose and identify all injured structures. Anatomic reconstruction of the injured ligaments is recommended, including double-bundle PCL reconstruction and superficial MCL augmentation. In the setting of this complex reconstruction, several technical aspects require consideration and preoperative planning, including the risk of femoral tunnel convergence on the medial aspect of the femoral condyle. This article details our technique for combined anatomic double-bundle PCL reconstruction and superficial MCL augmentation to avoid tunnel convergence. Level I (knee); level II (PCL).

Arthroscopic Repair of Proximal Posterior Cruciate Ligament Injuries in Pediatric Patients.

Project description:A renewed interest in arthroscopic knee ligament repair is emerging as a result of diagnostic and technical improvements. In pediatric patients with posterior cruciate ligament (PCL) injury, surgical reconstruction is rarely considered as an option because of the risk of iatrogenic physeal injury. In this Technical Note, we describe an arthroscopic surgical repair technique of PCL proximal avulsions in pediatric patients. The main reasons to consider arthroscopic PCL repair in this population include minimal surgical morbidity, preservation of the complex biomechanical properties of the native ligament, the small diameter of the bone tunnels, the physeal respecting nature of the procedure, the absence of graft harvesting, and the absence of fixation devices. The indications for this technique are limited to patients with an acute proximal PCL avulsion.
