Seroprevalence of hepatitis B virus among antenatal clinic attendees in Gamawa Local Government Area, Bauchi State, Nigeria.

ABSTRACT: BACKGROUND:Hepatitis B virus disease is a potentially life-threatening liver infection and a major global health problem. It causes chronic infection and puts people at high risk of death from cirrhosis and liver cancer. WHO estimated 257 million people are living with hepatitis B virus (HBV) infection and in 2015 alone HBV resulted in to 887,000 deaths globally. We determined the prevalence and associated factors of hepatitis B virus infection among Antenatal Care (ANC) attendees in Gamawa Local Government Area, Bauchi State. METHODS:We conducted a descriptive cross-sectional, health facility-based study between March and April 2018. We used systematic random sampling technique to recruit 210 pregnant women aged 15-49?years. With a structured questionnaire, we interviewed the respondents and collected blood sample to test for hepatitis B surface antigen. We calculated frequencies, means, proportions, and tested for associations using Epi Info 7.2 and Microsoft Excel. RESULTS:The mean age of respondents was 24.5?±?6.0?years; 112 (53%) of whom were younger than 25?years. All were married, 183 (87%) had no formal education and up to 190 (90%) were employed. Overall, 14 (6.7%) tested positive for HBsAg; women aged ?35?years had the highest prevalence (10%). None with tertiary education tested positive and women married before 18?years had 13 (6.2%) prevalence. CONCLUSIONS:The prevalence of HBsAg among pregnant women in Gamawa LGA was 6.7% which is quite lower than the national prevalence reported. We recommended improved surveillance of HBV infection and screening of women attending ANC.

Project description:Foot and mouth disease (FMD) is an important transboundary viral disease of both domestic and wild cloven-hoofed animals characterized by high morbidity with devastating consequence on the livestock worldwide. Despite the endemic nature of FMD in Nigeria, little is known about the epidemiology of the disease at the wildlife-livestock interface level. To address this gap, blood samples were collected between 2013 and 2015 from some wildlife and cattle, respectively, within and around the Yankari Game Reserve and Sumu Wildlife Park in Bauchi State, Nigeria. Wild animals were immobilized using a combination of etorphine hydrochloride (M99® Krüger-Med South Africa) at 0.5-2?mg/kg and azaperone (Stresnil®, Janssen Pharmaceuticals (Pty.) Ltd., South Africa) at 0.1?mg/kg using a Dan-Inject® rifle (Dan-Inject APS, Sellerup Skovvej, Denmark) fitted with a 3?ml dart syringe and for reversal, naltrexone (Trexonil® Kruger-Med South Africa) at 1.5?mg IM was used, and cattle were restrained by the owners for blood collection. Harvested sera from blood were screened for presence of antibodies against the foot and mouth disease virus (FMDV) using the PrioCHECK® 3ABC NSP ELISA kit, and positive samples were serotyped using solid-phase competitive ELISA, (IZSLER Brescia, Italy). Out of the 353 sera collected from cattle and wildlife 197 (65.7%) and 13 (24.5%) (P < 0.05), respectively, tested positive for antibodies to the highly conserved nonstructural 3ABC protein of FMDV by the FMDV-NS blocking ELISA. Classification of cattle into breed and sex showed that detectable antibodies to FMDV were higher (P < 0.05) in White Fulani 157 (72.8%) than in Red Bororo 23 (39.7%) and Sokoto Gudali 17 (33.3%) breeds of cattle, whereas in females, detectable FMDV antibodies were higher (P < 0.05) 150 (72.8%) than in males 47 (50.0%). In the wildlife species, antibodies to FMDV were detected in the waterbucks 2 (28.6%), elephant 1 (25.0%), wildebeests 4 (33.3%), and elands 6 (25.0%). Four serotypes of FMDV: O, A, SAT 1, and SAT 2 were detected from the 3ABC positive reactors in waterbucks, elephants, wildebeests, and elands. The results showed presence of antibodies to FMDV in some wildlife and cattle and suggested that wildlife could equally play an important role in the overall epidemiology of FMD in Nigeria. FMD surveillance system, control, and prevention program should be intensified in the study area.

Project description:Culex species are the most widespread mosquito species across the world and are known to be highly opportunistic, feeding on humans and livestock. They are known to acquire the potential to transmit zoonotic diseases, including Rift Valley Fever (RVF). However, despite their public health significance, they remain understudied in North-western Nigeria, compared to Anophelines. This study was therefore aimed at determining the relative abundance and Multiplex polymerase chain reaction (Multiplex PCR) identification of members of the Culex pipiens complex, in Kura Local Government Area (LGA), North-western, Nigeria. Adult mosquitoes were collected using Center for Disease Control (CDC) miniature light traps from August to October 2019. Mosquitoes were identified using morphological identification keys. Members of the Culex pipiens complex were further identified using Multiplex PCR to assess the presence of sibling species. A total of 413 mosquitoes, belonging to 3 genera, Culex, Anopheles and Aedes were collected. Of this figure, 120 Culex spp. females were collected. Homes with livestock had the highest occurrence of mosquitoes, 123 (61.19%) compared to those without livestock, 78 (38.81%). There was no statistical difference among the two (2) categories of homes (P ≥ 0.005). Culicoides spp. were the most common with 130 collected (65.38%). Again, homes with livestock had the highest occurrence, 85 whilst homes without livestock had 45 of the other flies caught. Multiplex- PCR revealed no expected bands for Cx. quinquefasciatus and Cx. pipiens from the DNA obtained from field collected mosquitoes as confirmed by using genomic DNA of an insectary Culex quinquefasciatus as control. Cx. spp. is presently regarded as a biting nuisance having no significant epidemiological importance. Efforts at its control should be intensified before it is too late. This study provides useful information on the occurrence and multiplex PCR of Culex spp in Kura Local Government Area, North-western Nigeria. These results have implications for the control of Culex spp. mosquito populations and the spread of human, livestock and avian diseases.

Project description:BACKGROUND:Access to antiretroviral therapy (ART) has increased dramatically in Sub-Saharan Africa. In Kenya, 560,000 people had access to ART by the end of 2011. This scaling up of ART has raised challenges to the Kenyan health system due to emergence of drug resistant viruses among those on treatment and possible onward transmission. To counter this, and come up with an effective treatment strategy, it has become vital to determine baseline mutations associated with drug resistance among the circulating strains of HIV-1 in Kenya. METHODS:The prevalence of mutations associated with drug resistance in HIV-1 protease (PR) and reverse transcriptase (RT) regions from 188 HIV-1 infected treatment-naïve pregnant women was investigated in Kapsabet, Nandi Hills and Kitale district hospitals of Kenya. Blood samples were collected between April 2005 and June 2006. The HIV-1 pol gene was amplified using primers for HIV-1 PR and RT and sequenced using the BigDye chemistry. The mutations were analyzed based on the IAS algorithm as well as the Stanford University HIV Drug Resistance Database. RESULTS:Based on the PR and RT sequences, HIV-1 subtypes A1 (n=117, 62.2%), A2 (n=2, 1.1%), D (n=27, 14.4%), C (n=13, 6.9%), G (n=3, 1.6%), and possible recombinants (n=26, 13.8%) were detected. Mutations associated with nucleoside reverse transcriptase inhibitors (NRTI) and non-nucleoside RTI (NNRTI)-resistance were detected in 1.6% (3 of 188) and 1.1% (2 of 188), respectively. Mutations associated with PI resistance were detected in 0.5% (1 of 188) of the study population. CONCLUSION:The prevalence of drug resistance among drug-naïve pregnant women in rural North Rift, Kenya in 2006 was 3.2%. Major drug resistance mutations associated with PIs, NRTIs and NNRTIs do exist among treatment-naïve pregnant women in North Rift, Kenya. There is a need for consistent follow-up of drug-naïve individuals in this region to determine the impact of mutations on treatment outcomes.

Project description:Schistosomiasis is a parasitic disease and its distribution, in space and time, can be influenced by environmental factors such as rivers, elevation, slope, land surface temperature, land use/cover and rainfall. The aim of this study is to identify the areas with suitable conditions for schistosomiasis transmission on the basis of physical and environmental factors derived from satellite imagery and spatial analysis for Akure North Local Government Area (LGA) of Ondo State. Nigeria. This was done through methodology multicriteria evaluation (MCE) using Saaty's analytical hierarchy process (AHP). AHP is a multi-criteria decision method that uses hierarchical structures to represent a problem and makes decisions based on priority scales. In this research AHP was used to obtain the mapping weight or importance of each individual schistosomiasis risk factor. For the purpose of identifying areas of schistosomiasis risk, this study focused on temperature, drainage, elevation, rainfall, slope and land use/land cover as the factors controlling schistosomiasis incidence in the study area. It is by reclassifying and overlaying these factors that areas vulnerable to schistosomiasis were identified. The weighted overlay analysis was done after each factor was given the appropriate weight derived through the analytical hierarchical process. The prevalence of urinary schistosomiasis in the study area was also determined by parasitological analysis of urine samples collected through random sampling. The results showed varying risk of schistosomiasis with a larger portion of the area (82%) falling under the high and very high risk category. The study also showed that one community (Oba Ile) had the lowest risk of schistosomiasis while the risk increased in the four remaining communities (Iju, Igoba, Ita Ogbolu and Ogbese). The predictions made by the model correlated strongly with observations from field study. The high risk zones corresponded to known endemic communities. This study revealed that environmental factors can be used in identifying and predicting the transmission of schistosomiasis as well as effective monitoring of disease risk in newly established rural and agricultural communities.
