Project description:There is growing evidence in the epidemiologic literature of the relationship between air pollution and adverse health outcomes. Prediction of individual air pollution exposure in the Environmental Protection Agency (EPA) funded Multi-Ethnic Study of Atheroscelerosis and Air Pollution (MESA Air) study relies on a flexible spatio-temporal prediction model that integrates land-use regression with kriging to account for spatial dependence in pollutant concentrations. Temporal variability is captured using temporal trends estimated via modified singular value decomposition and temporally varying spatial residuals. This model utilizes monitoring data from existing regulatory networks and supplementary MESA Air monitoring data to predict concentrations for individual cohort members. In general, spatio-temporal models are limited in their efficacy for large data sets due to computational intractability. We develop reduced-rank versions of the MESA Air spatio-temporal model. To do so, we apply low-rank kriging to account for spatial variation in the mean process and discuss the limitations of this approach. As an alternative, we represent spatial variation using thin plate regression splines. We compare the performance of the outlined models using EPA and MESA Air monitoring data for predicting concentrations of oxides of nitrogen (NO x )-a pollutant of primary interest in MESA Air-in the Los Angeles metropolitan area via cross-validated R2. Our findings suggest that use of reduced-rank models can improve computational efficiency in certain cases. Low-rank kriging and thin plate regression splines were competitive across the formulations considered, although TPRS appeared to be more robust in some settings.

Project description:The sanitary measures implemented to control and prevent an increase in infections due to the COVID-19 pandemic have produced an improvement in the air quality of many urban areas around the world. We assessed air quality during the COVID-19 pandemic for particulate matter (PM2.5 and PM10), NO2 and O3 in in metropolitan area of Lima, Peru between pre-lockdown period (February 1 and March 15 of 2020), historical period (March 16 to April 30 2017-2019) and lockdown period (March 16 to April 30, 2020). The complete national lockdown that was implemented in Peru produced statistically significant reductions in the in-air pollutant (PM10 (-40% and -58%), PM2.5 (-31% and -43%) and NO2 (-46% and -48%)), as recorded by the by the ground-based air quality monitoring network throughout the metropolitan area, compared with the corresponding concentrations for the previous weeks and over the same period for 2017-2019. Analysis of the spatial Distribution of satellite data also show decreases in the concentrations of PM10, PM2.5 and NO2 as a result of the containment measures and suspension of activities implemented by the Peruvian government. The concentrations of O3 significantly increased (11% and 170%) as a result of the decrease in the concentration of NO2, confirming that the study area is a hydrocarbon-limited system, as previously reported. Project description:There is substantial observational evidence that long-term exposure to particulate air pollution is associated with premature death in urban populations. Estimates of the magnitude of these effects derive largely from cross-sectional comparisons of adjusted mortality rates among cities with varying pollution levels. Such estimates are potentially confounded by other differences among the populations correlated with air pollution, for example, socioeconomic factors. An alternative approach is to study covariation of particulate matter and mortality across time within a city, as has been done in investigations of short-term exposures. In either event, observational studies like these are subject to confounding by unmeasured variables. Therefore the ability to detect such confounding and to derive estimates less affected by confounding are a high priority. In this article, we describe and apply a method of decomposing the exposure variable into components with variation at distinct temporal, spatial, and time by space scales, here focusing on the components involving time. Starting from a proportional hazard model, we derive a Poisson regression model and estimate two regression coefficients: the "global" coefficient that measures the association between national trends in pollution and mortality; and the "local" coefficient, derived from space by time variation, that measures the association between location-specific trends in pollution and mortality adjusted by the national trends. Absent unmeasured confounders and given valid model assumptions, the scale-specific coefficients should be similar; substantial differences in these coefficients constitute a basis for questioning the model. We derive a backfitting algorithm to fit our model to very large spatio-temporal datasets. We apply our methods to the Medicare Cohort Air Pollution Study (MCAPS), which includes individual-level information on time of death and age on a population of 18.2 million for the period 2000-2006. Results based on the global coefficient indicate a large increase in the national life expectancy for reductions in the yearly national average of PM2.5. However, this coefficient based on national trends in PM2.5 and mortality is likely to be confounded by other variables trending on the national level. Confounding of the local coefficient by unmeasured factors is less likely, although it cannot be ruled out. Based on the local coefficient alone, we are not able to demonstrate any change in life expectancy for a reduction in PM2.5. We use additional survey data available for a subset of the data to investigate sensitivity of results to the inclusion of additional covariates, but both coefficients remain largely unchanged.

Project description:A resurgence of scarlet fever has caused many pediatric infections in East Asia and the United Kingdom. Although scarlet fever in Taiwan has not been a notifiable infectious disease since 2007, the comprehensive national health insurance data can still track its trend. Here, we used data from the open data portal of the Taiwan Centers for Disease Control. The scarlet fever trend was measured by outpatient and hospitalization rates from 2009 to 2017. In order to elucidate the spatio-temporal hotspots, we developed a new method named the spatio-temporal Gi* statistic, and applied Joinpoint regression to compute the annual percentage change (APC). The overall APCs in outpatient and hospitalization were 15.1% (95% CI: 10.3%-20.2%) and 7.7% (95%CI: 4.5% -10.9%). The major two infected groups were children aged 5-9 (outpatient: 0.138 scarlet fever diagnoses per 1,000 visits; inpatient: 2.579 per 1,000 visits) and aged 3-4 (outpatient: 0.084 per 1,000 visits; inpatient: 1.469 per 1,000 visits). We found the counties in eastern Taiwan and offshore counties had the most hotspots in the outpatient setting. In terms of hospitalization, the hotspots mostly occurred in offshore counties close to China. With the help of the spatio-temporal statistic, health workers can set up enhanced laboratory surveillance in those hotspots.

Project description:Sputum induction, bronchoalveolar lavage, or gastric aspiration are often needed to produce adequate diagnostic respiratory samples from people with HIV in whom tuberculosis is suspected. Since these procedures are rarely appropriate in less-developed countries, we compared the performances of a simple string test and the gold-standard sputum induction. 160 HIV-positive adults under investigation for tuberculosis, and 52 asymptomatic HIV-positive control patients underwent the string test followed by sputum induction. The string test detected tuberculosis in 14 patients in whom this disease was suspected; sputum induction detected only eight of them (McNemar's test, p=0.03). These preliminary data suggest that the string test is safe and effective for retrieval of useful clinical specimens for diagnosis of pulmonary tuberculosis, and is at least as sensitive as sputum induction.

Project description:This study was performed to investigate the role of dysglycemia on the genetic diversity of Mycobacterium tuberculosis (MTB) among pulmonary tuberculosis (TB) patients to build scientific evidence about the possible mechanisms of TB transmission. MTB isolates obtained of patients affected by pulmonary tuberculosis from health care facilities of North Lima-Peru, were analyzed using whole genome sequencing and 24-locus mycobacterial interspersed repetitive-unit -variable-number tandem repeats (MIRU-VNTR). Subsequently, clinical and epidemiological characteristics were associated with clustering, lineages and comorbid conditions. The analysis carried out 112 pulmonary TB patients from various health centers in North Lima, 17 (15%) had diabetes mellitus (DM) and 33 (29%) had pre-diabetes (PDM). Latin American-Mediterranean, Haarlem and Beijing were the most frequent MTB lineages found in those patients. Previous TB (adjusted odds ratio [aOR] = 3.65; 95%CI: 1.32-17.81), age (aOR = 1.12; 95%CI: 1.03-1.45) and Beijing lineage (aOR = 3.53; 95%CI: 1.08-13.2) were associated with TB-DM comorbidity. Alcoholism (aOR = 2.92; 95%CI: 1.10-8.28), age (aOR = 1.05; 95%CI: 1.03-1.12) and Haarlem lineage (aOR = 2.54; 95%CI: 1.04-6.51) were associated with TB-PDM comorbidity. Beijing and Haarlem lineages were independently associated with TB-DM and TB-PDM comorbidities, respectively. Although these findings may be surprising, we must be cautious to suggest that dysglycemia could be associated with a highly clustering and predisposition of MTB lineages related to a serious impact on the severity of TB disease, which requires further research.

Project description:BackgroundExposure to atmospheric particulate matter (PM) remains an important public health concern, although it remains difficult to quantify accurately across large geographic areas with sufficiently high spatial resolution. Recent epidemiologic analyses have demonstrated the importance of spatially- and temporally-resolved exposure estimates, which show larger PM-mediated health effects as compared to nearest monitor or county-specific ambient concentrations.MethodsWe developed generalized additive mixed models that describe regional and small-scale spatial and temporal gradients (and corresponding uncertainties) in monthly mass concentrations of fine (PM2.5), inhalable (PM10), and coarse mode particle mass (PM(2.5-10)) for the conterminous United States (U.S.). These models expand our previously developed models for the Northeastern and Midwestern U.S. by virtue of their larger spatial domain, their inclusion of an additional 5 years of PM data to develop predictions through 2007, and their use of refined geographic covariates for population density and point-source PM emissions. Covariate selection and model validation were performed using 10-fold cross-validation (CV).ResultsThe PM2.5 models had high predictive accuracy (CV R2=0.77 for both 1988-1998 and 1999-2007). While model performance remained strong, the predictive ability of models for PM10 (CV R2=0.58 for both 1988-1998 and 1999-2007) and PM(2.5-10) (CV R2=0.46 and 0.52 for 1988-1998 and 1999-2007, respectively) was somewhat lower. Regional variation was found in the effects of geographic and meteorological covariates. Models generally performed well in both urban and rural areas and across seasons, though predictive performance varied somewhat by region (CV R2=0.81, 0.81, 0.83, 0.72, 0.69, 0.50, and 0.60 for the Northeast, Midwest, Southeast, Southcentral, Southwest, Northwest, and Central Plains regions, respectively, for PM2.5 from 1999-2007).ConclusionsOur models provide estimates of monthly-average outdoor concentrations of PM2.5, PM10, and PM(2.5-10) with high spatial resolution and low bias. Thus, these models are suitable for estimating chronic exposures of populations living in the conterminous U.S. from 1988 to 2007.

Project description:The complex correlation between regions caused by the externality of air pollution increases the difficulty of its governance. Therefore, analysis of the spatio-temporal network of air pollution (STN-AP) holds great significance for the cross-regional coordinated governance of air pollution. Although the spatio-temporal distribution of air pollution has been analyzed, the structural characteristics of the STN-AP still need to be clarified. The STN-AP in the Yangtze River Delta urban agglomeration (YRDUA) is constructed based on the improved gravity model and is visualized by UCINET with data from 2012 to 2019. Then, its overall-individual-clustering characteristics are analyzed through social network analysis (SNA) method. The results show that the STN-AP in the YRDUA was overall stable, and the correlation level gradually improved. The centrality of every individual city is different in the STN-AP, which reveals the different state of their interactive mechanism. The STN-AP could be subdivided into the receptive block, overflow block, bidirectional block and intermediary block. Shanghai, Suzhou, Hangzhou and Wuxi could be key cities with an all above degree centrality, betweenness centrality and closeness centrality and located in the overflow block of the STN-AP. This showed that these cities had a greater impact on the STN-AP and caused a more pronounced air pollution spillovers. The influencing factors of the spatial correlation of air pollution are further determined through the quadratic assignment procedure (QAP) method. Among all factors, geographical proximity has the strongest impact and deserves to be paid attention in order to prevent the cross-regional overflow of air pollution. Furthermore, several suggestions are proposed to promote coordinated governance of air pollution in the YRDUA.

Project description:The lockdown and related measures implemented by many European countries to stop the spread of the SARS-CoV-2 virus (COVID-19) pandemic have altered the economic activities and road transport in many cities. To rigorously evaluate how these measures have affected air quality in Europe, we developed Bayesian spatio-temporal (BST) models that assess changes in the surface nitrogen dioxide (NO2) and fine particulate matter (PM2.5) concentration across the continent. We fitted BST models to measurements of the two pollutants in 2020 using a lockdown indicator covariate, while accounting for the spatial and temporal correlation present in the data. Since other factors, such as weather conditions, local combustion sources and/or land surface characteristics may contribute to the variation of pollutant concentrations, we proposed two model formulations that allowed the differentiation between the variations in pollutant concentrations due to seasonality from the variations associated to the lockdown policies. The first model compares the changes in 2020, with the ones during the same period in the previous five years, by introducing an offset term, which controls for the long-term average concentrations of each pollutant during 2014–2019. The second approach models only the 2020 data, but adjusts for confounding factors. The results indicated that the latter can better capture the lockdown effect. The measures taken to tackle the virus in Europe reduced the average surface concentrations of NO2 and PM2.5 by 29.5% (95% Bayesian credible interval: 28.1%, 30.9%) and 25.9% (23.6%, 28.1%), respectively. To our knowledge, this research is the first to account for the spatio-temporal correlation present in the monitoring data during the pandemic and to assess how it affects estimation of the lockdown effect while accounting for confounding. The proposed methodology improves our understanding of the effect of COVID-19 lockdown policies on the air pollution burden across the continent. Graphical abstract Unlabelled Image
