Project description:Introduction: Vietnamese nail salon technicians are continuously exposed to neurotoxins linked to cognitive impairments and Alzheimer's disease. This study examined the association of occupational exposure with cognitive function and depressive symptoms among Vietnamese nail salon technicians. Methods: The sample included 155 current or former Vietnamese female nail technicians and 145 control group participants. Measures included the Montreal Cognitive Assessment (MoCA) and the Center for Epidemiologic Studies Depression Scale (CES-D). Results: Average cognitive functioning was significantly higher for the control compared to the nail technician group (mean difference = 1.2, p < 0.05). No differences were observed for depression. Multivariate findings revealed that exposure was negatively associated with cognitive functioning (β = -0.29, 95% CI: -0.53, -0.05, p < 0.05). Discussion: Nail salon work and the extent of occupational exposure were associated with lower cognitive functioning among Vietnamese nail technicians. Longitudinal research can further examine the risk for cognitive decline and dementia for this vulnerable population.

Project description:Pincer nail is a common condition characterized by excessive transverse curvature of the nail that causes pain, cosmetic problems, and functional limitation. Surgical treatments include correction of the nail bed and distal phalanges, which have shown good outcomes but are difficult to perform in general clinics because of their high invasion. Conservative treatments require special devices, which may be difficult to keep on hand in general clinics. We report two cases of pincer nails successfully treated by total nail avulsion. The patient in case 1 is a 66-year-old woman. She had a pincer nail on her right third toe that was painful. Total nail avulsion was performed on the nail, and taping was performed after surgery. Three months later, a new flattened nail grew. The curvature index was improved from 2.43 to 1.10. The patient in case 2 is a 68-year-old woman. Total nail avulsion was performed on a pincer nail of the left second toe. Nineteen months later, the nail was flattened and she remained free from pain. The curvature index was improved from 3.62 to 1.32. In both cases, pain was soon relieved by total nail avulsion and there were no complications. Total nail avulsion does not require special devices, can be performed in a short time during outpatient treatment, and is relatively less invasive. It is also easily applied to small nails. Total nail avulsion is an effective treatment for pincer nails on the second and third toes. Future studies are needed to determine if it is equally effective for pincer nails on the first toe.

Project description:The tips of mammalian digits can regenerate after amputation, like those of amphibians. It is unknown why this capacity is limited to the area associated with the nail. Here we show that nail stem cells (NSCs) reside in the proximal nail matrix and that the mechanisms governing NSC differentiation are coupled directly with their ability to orchestrate digit regeneration. Early nail progenitors undergo Wnt-dependent differentiation into the nail. After amputation, this Wnt activation is required for nail regeneration and also for attracting nerves that promote mesenchymal blastema growth, leading to the regeneration of the digit. Amputations proximal to the Wnt-active nail progenitors result in failure to regenerate the nail or digit. Nevertheless, β-catenin stabilization in the NSC region induced their regeneration. These results establish a link between NSC differentiation and digit regeneration, and suggest that NSCs may have the potential to contribute to the development of novel treatments for amputees.

Project description:Onycholysis and paronychia has been associated with chemotherapy treatment for women with breast cancer. Our primary aim was to investigate the effectiveness of different topical interventions to ameliorate nail toxicity. Secondary aims were to explore the full range and severity of possible nail changes associated with taxane-based chemotherapy and the specific impact this had on quality of life, using two novel measures. This was an exploratory randomised controlled trial of three topical interventions (standard care, nail polish or specialist nail drops) for the prevention or reduction of nail changes induced by taxane-based chemotherapy. Outcomes included nail toxicity assessed at three time points (baseline, 3 weeks and 3 months post completion of chemotherapy) using two novel clinical tools (NToX-G12, NToX-QoL) and the Common Terminology Criteria for Adverse Events (CTCAE v3) and EQ-5D-5L. A total of 105 women were recruited (35 in each arm) and monitored up to three months post completion of chemotherapy. Almost 20% of patients were over the age of 60 years. There were 26 withdrawals, the majority from the nail polish arm. Residual Maximum Likelihood REML analysis indicated a significant arm, time and interaction effect for each intervention (p < 0.001). Less nail toxicity was observed in patients receiving specialist nail drops or standard care arms in comparison to those using nail polish. This study provides evidence to support clinicians' suggestions on nail care recommendations based on the patients' needs and preferences. Future investigations into comparing or combining cryotherapy and topical solutions that can support patient's decisions are warranted.

Project description:Nail technicians are exposed to volatile organic compounds (VOCs) emitted from nail products used in their daily work, which may cause adverse health effects. This study aimed to assess VOC exposure of nail technicians in the South African formal and informal sectors and to provide a task-based exposure assessment of different nail applications. Personal passive sampling was conducted on 10 formal and 10 informal nail technicians located in the northern suburbs of Johannesburg and the Braamfontein area, over 3 days. Real-time measurements were taken to determine task-based peak exposures. The number of clients serviced, working hours, type of nail application, type of ventilation, room volume, and carbon dioxide (CO2) concentrations, were also recorded. There were differences in the nail products used, the types of nail applications performed, the number of clients serviced, and breathing zones VOC concentrations of the formal and informal nail technicians. Some formal nail salons were equipped with mechanical ventilation while the informal nail salons relied on natural ventilation. CO2 concentrations were higher in the informal than the formal nail salons and increased during the course of the working day. Formal nail technicians were exposed to higher total volatile organic compounds (TVOC) concentrations than informal nail technicians, which may be due to the different nail application procedures as well as ‘background’ emissions from their co-workers—the bystander effect. Acetone was the predominantly detected VOC: the formal nail technicians were exposed to significantly higher TWA (8 h) concentrations [geometric mean (GM) 43.8 ppm, geometric standard deviation (GSD) 2.49] than were the informal nail technicians (GM 9.87 ppm, GSD 5.13). Methyl methacrylate among the informal nail technicians was measured at 89.7% detection frequency, far higher than that among the formal nail technicians (3.4%). This may be attributed to the observed popularity of acrylic nail applications in this sector. Nail applications involving soak-off gave rise to high TVOC peaks at the start of the nail application process. This is the first study to compare organic solvent exposures among formal and informal nail technicians and determine task-based peak exposures. It also brings attention to the often-overlooked informal sector of this industry.

Project description:The main purpose of this work was to determine if the use of hybrid nail polishes causes changes in concentration of the most important sulfur amino acids that build nail plate structures, cysteine and methionine. We found that the average contents of cysteine and methionine in studied samples before the use of hybrid manicure were 1275.3 ± 145.9 nmol mg-1 and 111.7 ± 23.8 nmol mg-1, respectively. After six months of hybrid manicure use, the average amount of these sulfur amino acids in studied samples were 22.1% and 36.5% lower in the case of cysteine and methionine, respectively. The average amounts of cysteine and methionine in nail plate samples after the use of hybrid manicures were 992.4 ± 96.2 nmol mg-1 and 70.9 ± 14.8 nmol mg-1, respectively. We also confirmed that in studied women the application of UV light varnishes reduced the thickness of the nail plate, from 0.50 ± 0.12 mm before to 0.46 ± 0.12 mm after the use of the hybrid manicure.

Project description:Research on human nail tissue has been limited by the restricted access to fresh specimen. Here, we studied transcriptome profiles of human nail units using polydactyly specimens. Single-cell RNAseq with 11,541 cells from 4 extra digits revealed nail-specific mesenchymal and epithelial cell populations, characterized by RSPO4 (major gene in congenital anonychia) and SPINK6, respectively. In situ RNA hybridization demonstrated the localization of RSPO4, MSX1 and WIF1 in onychofibroblasts suggesting the activation of WNT signaling. BMP-5 was also expressed in onychofibroblasts implicating the contribution of BMP signaling. SPINK6 expression distinguished the nail-specific keratinocytes from epidermal keratinocytes. RSPO4+ onychofibroblasts were distributed at close proximity with LGR6+ nail matrix, leading to WNT/β-catenin activation. In addition, we demonstrated RSPO4 was overexpressed in the fibroblasts of onychomatricoma and LGR6 was highly expressed at the basal layer of the overlying epithelial component, suggesting that onychofibroblasts may play an important role in the pathogenesis of onychomatricoma.

Project description:Yellow nail syndrome (YNS; OMIM 153300, ORPHA662) is a very rare disorder that almost always occurs after 50 years of age but a juvenile or familial form has also been observed. YNS is diagnosed based on a triad associating yellow nail discoloration, pulmonary manifestations (chronic cough, bronchiectasia, pleural effusion) and lower limb lymphedema. Chronic sinusitis is frequently associated with the triad. YNS etiology remains unknown but a role of lymphatic impairment is usually evoked. YNS is more frequently isolated but may be associated in rare cases with autoimmune diseases, other clinical manifestations implicating lymphatic functions or cancer and, hence, is also considered a paraneoplastic syndrome. YNS management is symptomatic and not codified. YNS can resolve spontaneously. Oral vitamin E alone or even better when associated with triazole antifungals may achieve partial or total disappearance of nail discoloration. Pleural effusion can be treated surgically, with decortication/pleurectomy or pleurodesis. Antibiotic prophylaxis is prescribed for bronchiectasia with chronic sputum production. Lymphedema treatment is based on low-stretch bandages and the wearing of elastic compression garments combined with skin care, exercises and, as needed, manual lymph drainage.

Project description:BackgroundNail-bed injuries are the most common hand injury in children. Surgical dogma is to replace the nail plate after repairing the nail bed. Recent evidence suggests this might increase infection rates and returns to clinic. The aim of this feasibility trial was to inform the design and conduct of a definitive trial comparing replacing or discarding the nail plate after nail-bed repair.MethodsThis study recruited participants from four hand units in the UK between April and July 2015. Participants were children under the age of 16 years with a nail-bed injury requiring surgery. They were randomized to either having the nail plate replaced or discarded after nail-bed repair. The follow-up method was also allocated randomly (postal versus clinic). Information was collected on complications at 2 weeks and 30 days, and on nail-plate appearance at 4 months using the Zook classification. Two possible approaches to follow-up were also piloted and compared.ResultsDuring the recruitment phase, there were 156 potentially eligible children. Sixty were randomized in just over 3 months using remote web-based allocation. By 2 weeks, there were two infections, both in children with replaced nail plates. The nail-replaced group also experienced more complications. There was no evidence of a difference in return rates between postal and clinic follow-up.ConclusionRecruitment was rapid and nail-bed repair appeared to have low complication and infection rates in this pilot trial. The findings have led to revision of the definitive trial protocol, including the mode and timing of follow-up, and modification of the Zook classification.
