Conjunctivitis can be the only presenting sign and symptom of COVID-19.

ABSTRACT: Five cases of non-remitting conjunctivitis turned out to be the sole presenting sign and symptom of COVID-19. These patients tested positive on RT-PCR of naso-pharyngeal swabs and developed no fever, malaise, or respiratory symptoms throughout the course of their illness. They all fully recovered. In the current efforts to fight the spread of this virus, authors want to emphasize that atypical clinical presentations of COVID-19 can occur and a high level of suspicion should be maintained. Ocular involvement and transmission of SARS-CoV-2 should never be overlooked. In fact, conjunctival mucosae are susceptible to respiratory viruses and remain an important point of entry. For this reason, eye protection in the form of goggles or a face shield should be considered essential for all healthcare providers, even when taking care of patients who are not showing typical symptoms of COVID-19.

COVID-19 and conjunctivitis: a meta-analysis.

Project description:COVID-19 is a disease first identified in Wuhan City, Hubei Province, China, in December 2019, causes by a SARS-COV-2 virus infection. By 27 October 2020, 43,921,473 confirmed cases were reported worldwide, with 1,166,389 COVID-19 deaths. Conjunctivitis has been reported in adults and pediatric patients with COVID-19.ObjectiveThe aim of this meta-analysis is to estimate the odd Ratio (ORs) of conjunctivitis in patients with COVID-19.MethodsA systematic review and meta-analysis have been performed using the PubMed and Google Scholar literature search. The ORs of conjunctivitis in adults and pediatric patients is the outcome of this meta-analysis.ResultsThere have been 1041 articles published since the outbreak in December 2019, according to the latest literature. For the meta-analysis, 20 studies with a total of 3383 participants were included. The odds ratio (ORs) of conjunctivitis was 0.01 (95% confidence interval [CI]: 0.00-0.02). No bias has been reported.ConclusionConjunctivitis is the most common ocular manifestations reported in adults. This comprehensive meta-analysis quantifies the existing evidence linking conjunctivitis with COVID-19 and highlights the high percentage of heterogeneity that is shown in the current studies. Finally, it offers a single review article which includes all the current articles available for COVID-19 and conjunctivitis in adults and children.

Severe headache as the sole presenting symptom of COVID-19 pneumonia: A case report.

Project description:The spectrum of symptoms of COVID-19 continues to expand as more clinical observations are reported. Neurological manifestations including headache are increasingly described. However, headache as the sole presenting symptom of COVID-19 pneumonia has not been reported. We describe a patient in Tanzania who experienced severe headache for seven days before the onset of other symptoms of COVID-19 that led to her isolation, diagnosis, and treatment.

scRNAseq of Antigen Presenting Cells in severe COVID-19 patients

Project description:We isolated PBMC from healthy, moderate ( Oxygen supply < 10L/min), and severe (Oxygen supply >= 10L/min) COVID-19 patients after their admission to Intensive Care Units (ICU), at two timepoints (Day-1 and Day-4); and performed both CD14+ Monocyte enrichment followed by a Pan-DC kit to retrieve all Antigen Presenting Cell (APC) subsets from these age-matched patients. We performed single cell RNA sequencing using 10X technology on the single cell suspensions and constracted a high-resolution map of 81,643 Antigen Presenting Cells (APC) from the three COVID-19 severity groups. We were able to retrieve all the known six APC subsets and deciphered the altered pathways and ati-viral mechanisms, correlated with the disease severity.

Altered Mental Status: An Important but Overlooked Presenting Symptom of COVID-19 in Older Adults.

Project description:ObjectivesTo determine whether altered mental status (AMS) as a presenting symptom in older adults with COVID-19 is independently associated with adverse outcomes.MethodsA retrospective single center observational study of admitted patients (n = 421) age greater than 60 and a positive COVID-19 test. Outcomes included mortality, intubation, acute respiratory distress syndrome, acute kidney injury, and acute cardiac injury. Multivariate regression analysis was used to determine if presenting with AMS was associated with adverse outcomes.ResultsThere was an increased risk of mortality (RR 1.29, 95% CI 1.05-1.57), intubation (RR 1.52, 95% CI 1.09-2.12) and AKI (RR 1.42, 95% CI 1.13-1.78) in patients that presented with AMS.ConclusionsDuring a global pandemic, prognostic indicators are vital to help guide the clinical course of patients, reduce healthcare cost, and preserve life. Our study suggests that AMS can play a major role in diagnostic algorithms in older adults with COVID-19.

Project description:Study Design Case report and review of the literature. Objective To present an unusual case of conus medullaris ependymal cyst. Methods This is a case of a 41-year-old woman with a diagnosis of ependymal cyst who presented with a history of shock-like pain to the lower limbs. Results The patient underwent a successful cyst decompression. Conclusion A few cases have been reported in the literature. Including this case, there are a total of eight cases of ependymal cyst in the conocaudal segment reported.

Longitudinal symptom dynamics of COVID-19 infection.

Project description:As the COVID-19 pandemic progresses, obtaining information on symptoms dynamics is of essence. Here, we extracted data from primary-care electronic health records and nationwide distributed surveys to assess the longitudinal dynamics of symptoms prior to and throughout SARS-CoV-2 infection. Information was available for 206,377 individuals, including 2471 positive cases. The two datasources were discordant, with survey data capturing most of the symptoms more sensitively. The most prevalent symptoms included fever, cough and fatigue. Loss of taste and smell 3 weeks prior to testing, either self-reported or recorded by physicians, were the most discriminative symptoms for COVID-19. Additional discriminative symptoms included self-reported headache and fatigue and a documentation of syncope, rhinorrhea and fever. Children had a significantly shorter disease duration. Several symptoms were reported weeks after recovery. By a unique integration of two datasources, our study shed light on the longitudinal course of symptoms experienced by cases in primary care.

Symptom clusters in COVID-19: A potential clinical prediction tool from the COVID Symptom Study app.

Project description:As no one symptom can predict disease severity or the need for dedicated medical support in coronavirus disease 2019 (COVID-19), we asked whether documenting symptom time series over the first few days informs outcome. Unsupervised time series clustering over symptom presentation was performed on data collected from a training dataset of completed cases enlisted early from the COVID Symptom Study Smartphone application, yielding six distinct symptom presentations. Clustering was validated on an independent replication dataset between 1 and 28 May 2020. Using the first 5 days of symptom logging, the ROC-AUC (receiver operating characteristic - area under the curve) of need for respiratory support was 78.8%, substantially outperforming personal characteristics alone (ROC-AUC 69.5%). Such an approach could be used to monitor at-risk patients and predict medical resource requirements days before they are required.
