Narrative meaning making and integration: Toward a better understanding of the way falling ill influences quality of life.

ABSTRACT: Falling seriously ill is often experienced as a life event that causes conflict with people's personal goals and expectations in life and evokes existential questions. This article presents a new humanities approach to the way people make meaning of such events and how this influences their quality of life. Incorporating theories on contingency, narrative identity, and quality of life, we developed a theoretical model entailing the concepts life event, worldview, ultimate life goals, experience of contingency, narrative meaning making, narrative integration, and quality of life. We formulate testable hypotheses and describe the self-report questionnaire that was developed based on the model.

Shared Decision-Making with Parents of Acutely Ill Children: A Narrative Review.

Project description:Shared decision-making (SDM) has mostly been used with adults and parents in the primary care setting, and there is limited knowledge on the use of SDM with parents of acutely ill children. The objective of this study was to review the literature on SDM with parents in the management of acutely ill children.We searched MEDLINE, SCOPUS, PsycINFO, the Cochrane Library, and ClinicalTrials.gov for English language studies published from the time of database inception to February, 2017. Study eligibility criterion was use of SDM with parents for children aged 18 years or younger with an acute medical problem.We identified 2 ongoing clinical trials and 10 published studies that met inclusion criteria: 2 using hypothetical SDM scenarios, 1 mixed methods study, and 7 intervention studies. Only 1 study compared an SDM intervention with usual care in a randomized controlled trial. The limited literature shows that parents of acutely ill children have differing preferences for testing and/or treatment, and that they generally want the opportunity to express those preferences through an SDM process. Use of SDM often results in acutely ill children undergoing fewer and/or less intensive testing or treatment, although the effect on outcomes is unclear.Parents welcome participation in SDM for management decisions with their acutely ill child. Further investigation is needed to determine how best to implement SDM with parents of acutely ill children and to assess the effect of SDM on outcomes.

Past Actions as Self-Signals: How Acting in a Self-Interested Way Influences Environmental Decision Making.

Project description:In the last few decades, awareness of environmental issues has increased significantly. Little has changed, however, in human activities contributing to environmental damage. Why is it so difficult for us to change our behavior in a domain that is clearly so important to the future of humanity? Here we propose and test the possibility that self-signaling, the way we view ourselves based on our past behaviors, is one of the factors contributing to the difficulty of taking environmental action. In three experiments, we show that previous self-interested thoughts or behaviors serve as important signals that hinder the likelihood of acting in line with an individual's reported concern for the environment. This study not only helps explain the gap between environmental awareness and action, but also suggests alternative strategies for policymakers and environmental agencies to promote proenvironmental behavior.

An Electrophysiological Abstractness Effect for Metaphorical Meaning Making.

Project description:Neuroimaging studies show that metaphors activate sensorimotor areas. These findings were interpreted as metaphors contributing to conceptual thought by mapping concrete, somatosensory information onto abstract ideas. But is sensorimotor information a necessary constituent of figurative meaning? The present study employed event-related potentials (ERPs) in a divided visual field paradigm with healthy adults to explore the role of sensorimotor feature processing in the comprehension of novel metaphors via the electrophysiological concreteness effect. Participants read French, novel adjective-noun expressions that were either metaphorical ("fat sentence") or literal ("fat hip"). While literal expressions evoked a typical concreteness effect, an enhanced frontal negativity during right hemisphere (RH) as opposed to left hemisphere (LH) presentation, metaphors showed no such sign of sensorimotor feature processing. Relative to literals, they evoked a sustained frontal negativity during LH presentation and similar amplitudes during RH presentation, but both of these effects were the greater the more abstract the metaphors were. It is the first time such an electrophysiological abstractness effect is reported, just the opposite of a concreteness effect. It is particularly noteworthy that ERPs evoked by metaphors were not contingent on figurativeness, novelty, meaningfulness, imageability, emotional valence, or arousal, only on abstractness. When compared with similarly novel literal expressions, metaphors did not evoke a typical N400 and did not activate the RH either. The findings shed new light on the neurocognitive machinery of figurative meaning construction, pervasive in everyday communication. Contrary to embodied cognition, the conceptual system might be organized around abstract representations and not sensorimotor information, even for lush, metaphorical language.

Estimating smile intensity: A better way.

Project description:Both the occurrence and intensity of facial expressions are critical to what the face reveals. While much progress has been made towards the automatic detection of facial expression occurrence, controversy exists about how to estimate expression intensity. The most straight-forward approach is to train multiclass or regression models using intensity ground truth. However, collecting intensity ground truth is even more time consuming and expensive than collecting binary ground truth. As a shortcut, some researchers have proposed using the decision values of binary-trained maximum margin classifiers as a proxy for expression intensity. We provide empirical evidence that this heuristic is flawed in practice as well as in theory. Unfortunately, there are no shortcuts when it comes to estimating smile intensity: researchers must take the time to collect and train on intensity ground truth. However, if they do so, high reliability with expert human coders can be achieved. Intensity-trained multiclass and regression models outperformed binary-trained classifier decision values on smile intensity estimation across multiple databases and methods for feature extraction and dimensionality reduction. Multiclass models even outperformed binary-trained classifiers on smile occurrence detection.

Gepants - a long way to cure: a narrative review.

Project description:Calcitonin gene-related peptide (CGRP) is probably the most potent vasodilator in cerebral circulation. Forty years after its discovery, the new CGRP-targeted therapy monoclonal antibodies, and the small molecule gepants, are now available for clinical practice. While randomized controlled trials and real-world experience consistently demonstrated the high efficacy and tolerability of monoclonal antibodies, limited evidence is available to characterize gepants fully. Depending on pharmacokinetics, these CGRP receptor antagonists can be used for acute (ubrogepant, rimegepant, and the not yet approved zavegepant) or preventive (atogepant and rimegepant) migraine treatment. Randomized placebo-controlled trials demonstrated gepants efficacy in treating acute attacks to obtain 2 h pain freedom in about 20% of patients and pain relief in about 60%, while up to 60% of treated patients with episodic migraine may experience a 50% reduction in monthly migraine days. The most common treatment-related emergent adverse events were gastrointestinal (nausea, constipation) for the acute or preventive use. No vascular or hepatic concerns have emerged so far. More studies are ongoing to investigate gepant tolerability and safety also if associated with monoclonal antibodies targeting CGRP and other therapeutic classes. Gepants are also under investigation to treat other painful and non-painful conditions. Real-life studies are necessary to confirm the trials' findings and investigate more practical clinical aspects.

Toward a universal decoder of linguistic meaning from brain activation.

Project description:Prior work decoding linguistic meaning from imaging data has been largely limited to concrete nouns, using similar stimuli for training and testing, from a relatively small number of semantic categories. Here we present a new approach for building a brain decoding system in which words and sentences are represented as vectors in a semantic space constructed from massive text corpora. By efficiently sampling this space to select training stimuli shown to subjects, we maximize the ability to generalize to new meanings from limited imaging data. To validate this approach, we train the system on imaging data of individual concepts, and show it can decode semantic vector representations from imaging data of sentences about a wide variety of both concrete and abstract topics from two separate datasets. These decoded representations are sufficiently detailed to distinguish even semantically similar sentences, and to capture the similarity structure of meaning relationships between sentences.

Better sleep, better life? How sleep quality influences children's life satisfaction.

Project description:PurposeTo assess the association between children's sleep quality and life satisfaction; and to evaluate the underlying mechanisms of this relationship.MethodsThree pediatric cohorts in the National Institutes of Health (NIH) Environmental influences on Child Health (ECHO) Research Program administered Patient-Reported Outcome Measurement Information System (PROMIS®) parent-proxy measures to caregivers (n = 1111) who reported on their 5- to 9-year-old children's (n = 1251) sleep quality, psychological stress, general health, and life satisfaction; extant sociodemographic data were harmonized across cohorts. Bootstrapped path modeling of individual patient data meta-analysis was used to determine whether and to what extent stress and general health mediate the relationship between children's sleep quality and life satisfaction.ResultsNonparametric bootstrapped path analyses with 1000 replications suggested children's sleep quality was associated with lower levels of stress and better general health, which, in turn, predicted higher levels of life satisfaction. Family environmental factors (i.e., income and maternal mental health) moderated these relationships.ConclusionChildren who sleep well have happier lives than those with more disturbed sleep. Given the modifiable nature of children's sleep quality, this study offers evidence to inform future interventional studies on specific mechanisms to improve children's well-being.

Decision-making during obstetric emergencies: A narrative approach.

Project description:This study aims to explore how physicians make sense of and give meaning to their decision-making during obstetric emergencies. Childbirth is considered safe in the wealthiest parts of the world. However, variations in both intervention rates and delivery outcomes have been found between countries and between maternity units of the same country. Interventions can prevent neonatal and maternal morbidity but may cause avoidable harm if performed without medical indication. To gain insight into the possible causes of this variation, we turned to first-person perspectives, and particularly physicians' as they hold a central role in the obstetric team. This study was conducted at four maternity units in the southern region of Sweden. Using a narrative approach, individual in-depth interviews ignited by retelling an event and supported by art images, were performed between Oct. 2018 and Feb. 2020. In total 17 obstetricians and gynecologists participated. An inductive thematic narrative analysis was used for interpreting the data. Eight themes were constructed: (a) feeling lonely, (b) awareness of time, (c) sense of responsibility, (d) keeping calm, (e) work experience, (f) attending midwife, (g) mind-set and setting, and (h) hedging. Three decision-making perspectives were constructed: (I) individual-centered strategy, (II) dialogue-distributed process, and (III) chaotic flow-orientation. This study shows how various psychological and organizational conditions synergize with physicians during decision-making. It also indicates how physicians gave decision-making meaning through individual motivations and rationales, expressed as a perspective. Finally, the study also suggests that decision-making evolves with experience, and over time. The findings have significance for teamwork, team training, patient safety and for education of trainees.

Three more steps toward better science.

Project description:Science has striven to do better since its inception and has given us good philosophies, methodologies and statistical tools that, in their own way, do reasonably well for purpose. Unfortunately, progress has also been marred by historical clashes among perspectives, typically between frequentists and Bayesians, leading to troubles such as the current reproducibility crises. Here I wish to propose that science could do better with more resilient structures, more useful methodological tutorials, and clearer signaling regarding how much we can trust what it produces.
