Best Practices: Caregiver Training Resources Derived From Remote Behavioral Service Delivery Within the Foster Care System.

ABSTRACT: In the face of COVID-19 and necessary shifts in service delivery for behavior analysts, caregiver involvement in behavioral interventions will likely increase. Resources that caregivers can consume and implement independently are critical in helping them manage behavior in their homes. This article includes antecedent and consequent behavior-management strategies that correspond with provided written instructions and video tutorials designed for caregivers. The materials presented within this article were originally produced and found effective in aiding caregivers managing behavior in their homes within the Alabama foster care system. Although individuals within this system are at a higher risk of abuse and neglect and may engage in higher levels of aberrant behavior, we are distributing this document in hopes it will help behavior analysts working across a variety of populations as they navigate changes in service delivery and adopt resources for continued care and caregiver training.

Project description:IntroductionA caesarean section is a major obstetric procedure that can save the life of mother and child. Its purpose is to protect the mother's health from the complications of childbirth and to protect the baby's health. In sub-Saharan Africa (SSA), there are major inequalities in access to caesarean sections and significant variations in practices to determine the indications for the procedure. Periodic analyses of maternal deaths have shown that more than half of maternal and new born deaths are due to suboptimal care and are therefore potentially preventable. The objective of our study is to assess the impact of health staff training under the PADISS project (to support the health system's integrated development) on the quality of CS procedures in North Kivu, by comparing two periods.Material and methodsThe populations compared were recruited from the referral hospitals in North Kivu, DRC (Democratic Republic of Congo). The first (group 1) was made up of patient files studied retrospectively for the period from 01/11/2013 to 01/01/2016. The second group (group 2), studied prospectively, comprised patient files from June 2019 to January 2020. Obstetric, maternal and foetal data were compared. Statistical analyses were performed using STATA/IC 15.0 for Windows. Univariate and multiple logistic regression was performed to determine which characteristics are associated with maternal and perinatal morbidity and mortality. A p value < 0.05 was considered statistically significant.ResultsCS frequency was approximately 17% in both study periods. We observed a CS frequency of about 34% at North Kivu provincial hospital for the two populations studied. The main indications for CS were dystocia, foetal distress and scarred uterus for both populations. In the population studied prospectively, after the implementation of health staff training, there were fewer incidence rate of dystocia, foetal distress and neonatal death, a more complete patient record, shorter hospital stay, and fewer blood transfusions but more incidence rate of scarred uterus, post-operative complications and low birth weight. Intervention had no statistically significant impact on low birth weight (OR = 1.9, p = 0.13), on neonatal mortality (OR = 0.69, p = 0.21).ConclusionOur study shows a decrease in neonatal deaths, dystocia and foetal distress, but an increase in post-operative complications, maternal deaths and cases of scarred uterus and low birth weight. However, multiple logistic regression did no support the conclusion.

Project description:Each year, millions of dogs enter thousands of animal shelters across the United States. Life in the shelter can be stressful, and one type of intervention that improves dogs' experience is human interaction, particularly stays in foster homes. Prior research has demonstrated that fostering can reduce dogs' cortisol and increase their resting activity. Despite these benefits, little is understood about the utilization of foster caregiving in animal shelters, and even less so during a crisis. On March 11, 2020, the World Health Organization deemed the coronavirus outbreak a worldwide pandemic, and subsequently a nationwide emergency was declared in the United States. Nearly all states issued stay-at-home orders to curb the spread of the virus. During this time, media outlets reported increased interest in the adoption and fostering of shelter pets. This study explores canine foster caregiving at 19 US animal shelters during the first 4 months of the COVID-19 pandemic. In our investigation, we found that shelters' utilization of foster caregiving increased from March to April 2020 but returned to initial pandemic levels by June 2020. Slightly less than two-fifths of foster caregivers were community members with no prior relationship with the shelter, and these caregivers were over four times more likely to adopt their fostered dogs than those with a pre-existing relationship to the shelter. Individuals fostering with the intention to adopt, in fact, adopted their dogs in nearly three-quarters of those instances. With regards to shelters' available resources, we found that very low-resource shelters relied more heavily on individuals with prior relationships to provide foster caregiving while very high-resource shelters more often recruited new community members. We also found that our lowest resourced shelters transferred more dogs out of their facilities while more resourced shelters rehomed dogs directly to adopters. To our knowledge, these findings represent the first in-depth reporting about dog fostering in US animal shelters and, more specifically, foster caregiving during the COVID-19 pandemic. In total, they provide greater understanding of how monetary and human resources were utilized to affect the care and ultimately, the outcomes of shelter dogs during this time.
