Project description:A critical aspect of air pollution exposure models is the estimation of the air exchange rate (AER) of individual homes, where people spend most of their time. The AER, which is the airflow into and out of a building, is a primary mechanism for entry of outdoor air pollutants and removal of indoor source emissions. The mechanistic Lawrence Berkeley Laboratory (LBL) AER model was linked to a leakage area model to predict AER from questionnaires and meteorology. The LBL model was also extended to include natural ventilation (LBLX). Using literature-reported parameter values, AER predictions from LBL and LBLX models were compared to data from 642 daily AER measurements across 31 detached homes in central North Carolina, with corresponding questionnaires and meteorological observations. Data was collected on seven consecutive days during each of four consecutive seasons. For the individual model-predicted and measured AER, the median absolute difference was 43% (0.17 h(-1)) and 40% (0.17 h(-1)) for the LBL and LBLX models, respectively. Additionally, a literature-reported empirical scale factor (SF) AER model was evaluated, which showed a median absolute difference of 50% (0.25 h(-1)). The capability of the LBL, LBLX, and SF models could help reduce the AER uncertainty in air pollution exposure models used to develop exposure metrics for health studies.

Project description:The environmental burden of disease is the mortality and morbidity attributable to exposures of air pollution and other stressors. The inequality metrics used in cumulative impact and environmental justice studies can be incorporated into environmental burden studies to better understand the health disparities of ambient air pollutant exposures. This study examines the diseases and health disparities attributable to air pollutants for the Detroit urban area. We apportion this burden to various groups of emission sources and pollutants, and show how the burden is distributed among demographic and socioeconomic subgroups. The analysis uses spatially-resolved estimates of exposures, baseline health rates, age-stratified populations, and demographic characteristics that serve as proxies for increased vulnerability, e.g., race/ethnicity and income. Based on current levels, exposures to fine particulate matter (PM2.5), ozone (O?), sulfur dioxide (SO?), and nitrogen dioxide (NO?) are responsible for more than 10,000 disability-adjusted life years (DALYs) per year, causing an annual monetized health impact of $6.5 billion. This burden is mainly driven by PM2.5 and O? exposures, which cause 660 premature deaths each year among the 945,000 individuals in the study area. NO? exposures, largely from traffic, are important for respiratory outcomes among older adults and children with asthma, e.g., 46% of air-pollution related asthma hospitalizations are due to NO? exposures. Based on quantitative inequality metrics, the greatest inequality of health burdens results from industrial and traffic emissions. These metrics also show disproportionate burdens among Hispanic/Latino populations due to industrial emissions, and among low income populations due to traffic emissions. Attributable health burdens are a function of exposures, susceptibility and vulnerability (e.g., baseline incidence rates), and population density. Because of these dependencies, inequality metrics should be calculated using the attributable health burden when feasible to avoid potentially underestimating inequality. Quantitative health impact and inequality analyses can inform health and environmental justice evaluations, providing important information to decision makers for prioritizing strategies to address exposures at the local level.

Project description:This article presents an ArcGIS geodatabase of socio-demographic and physical characteristics derived from recent high resolution data sources to construct measures of population vulnerability to inundation in the 28 counties of coastal North Carolina, U.S.A. as presented in Pricope et al., 2019. The region is simultaneously densely populated, low-lying and exposed to recurrent inundation related to storms and incremental sea level rise. The data presented here can be used as a decision support tool in coastal planning, emergency management preparedness, designing adaptation strategies and developing strategies for coastal resilience. The socio-demographic data (population and housing) was derived from 228 tables at the block-group level of geography from the 2010 U.S. Census Bureau. These data were statistically analyzed, using Principal Component Analysis, to identify key factors and then used to construct a Social Vulnerability Index (SOVI) at the block-group level of geography which highlighted regions where socio-demographic characteristics such as family structure, race, housing (primarily owner vs. renter-occupied), special needs populations (e.g. elderly and group living), and household/family size play an overwhelmingly important role in determining community vulnerability from a social perspective. An index of physical exposure was developed using the National Flood Hazards Maps (available from North Carolina's Flood Risk Information System and FEMA) along with a novel building inventory dataset available from the North Carolina Department of Public Safety that contains the Finished-Floor Elevation of every structure in the state. We took advantage of the unprecedented high spatial resolution nature of the building inventory dataset to calculate an index of physical vulnerability to inundation of every block group in the 28 coastal counties relative to Base Flood elevations and identified hotspots where this intersection predisposes people to an increased risk of flooding. Here, we present the final derived dataset containing the social, physical and an integrative measure of vulnerability to flooding that can be used at multiple scales of analysis, starting with the regional, county, local, and neighborhood to identify areas of priority intervention for risk-reduction in coastal planning and emergency management preparedness as well as forward-looking adaptation strategies.

Project description:BackgroundAfter North Carolina (NC) fire inspectors detected unsafe carbon monoxide (CO) levels inside several waterpipe cafés, the state fire code was amended to include provisions regulating waterpipe cafés, adding a requirement for air ventilation. These regulations apply to new buildings constructed after 1 January 2016, but can be enforced for older buildings where there exists a distinct hazard to life. We measured air quality at a sample of waterpipe cafés before and after the starting date of this regulation and collected information on presence of air ventilation.MethodsAir quality (CO, fine particulate matter (PM2.5)) monitoring was conducted inside and outside of six waterpipe cafés in NC in September of 2015 (time 1) and September of 2016 (time 2). In addition, questionnaires were administered to managers from each waterpipe café at time 2 to determine the presence of air ventilation systems.ResultsElevated levels of CO and PM2.5 were found inside waterpipe cafés at time 1 (median CO=42 ppm; median PM2.5=379.3 µg/m3) and time 2 (median CO=65 ppm; median PM2.5=484.0 µg/m3), with no significant differences between time periods (p>0.05). Indoor levels were significantly higher than levels outside cafés at both time periods (p<0.05). All waterpipe cafés reported having an air ventilation system that was installed prior to time 1 air monitoring.ConclusionsUnsafe levels of CO and PM2.5 were observed in waterpipe cafés in NC, despite reported use of air ventilation systems. Prohibiting indoor waterpipe smoking may be necessary to ensure clean air for employees and patrons.

Project description:BackgroundThe built environment, a key component of environmental health, may be an important contributor to health disparities, particularly for reproductive health outcomes.ObjectiveIn this study we investigated the relationship between seven indices of residential built environment quality and adverse reproductive outcomes for the City of Durham, North Carolina (USA).MethodsWe surveyed approximately 17,000 residential tax parcels in central Durham, assessing > 50 individual variables on each. These data, collected using direct observation, were combined with tax assessor, public safety, and U.S. Census data to construct seven indices representing important domains of the residential built environment: housing damage, property disorder, security measures, tenure (owner or renter occupied), vacancy, crime count, and nuisance count. Fixed-slope random-intercept multilevel models estimated the association between the residential built environment and five adverse birth outcomes. Models were adjusted for maternal characteristics and clustered at the primary adjacency community unit, defined as the index block, plus all adjacent blocks that share any portion of a line segment (block boundary) or vertex.ResultsFive built environment indices (housing damage, property disorder, tenure, vacancy, and nuisance count) were associated with each of the five outcomes in the unadjusted context: preterm birth, small for gestational age (SGA), low birth weight (LBW), continuous birth weight, and birth weight percentile for gestational age (BWPGA; sex-specific birth weight distributions for infants delivered at each gestational age using National Center for Health Statistics referent births for 2000-2004). However, some estimates were attenuated after adjustment. In models adjusted for individual-level covariates, housing damage remained statistically significantly associated with SGA, birth weight, and BWPGA.ConclusionThis work suggests a real and meaningful relationship between the quality of the residential built environment and birth outcomes, which we argue are a good measure of general community health.

Project description:OBJECTIVE:To examine effects of maternity care coordination (MCC) on perinatal health care utilization among low-income women. DATA SOURCES:North Carolina Center for Health Statistics Baby Love files that include birth certificates, maternity care coordination records, WIC records, and Medicaid claims. STUDY DESIGN:Causal effects of MCC participation on health care outcomes were estimated in a sample of 7,124 singleton Medicaid-covered births using multiple linear regressions with inverse probability of treatment weighting (IPTW). PRINCIPAL FINDINGS:Maternity care coordination recipients were more likely to receive first-trimester prenatal care (p < .01) and averaged three more prenatal visits and two additional primary care visits during pregnancy; they were also more likely to participate in WIC and to receive postpartum family planning services (p < .01). Medicaid expenditures were greater among mothers receiving MCC. CONCLUSIONS:Maternity care coordination facilitates access to health care and supportive services among Medicaid-covered women. Increased maternal service utilization may increase expenditures in the short run; however, improved newborn health may reduce the need for costly neonatal care, and by implication the need for early intervention and other supports for at-risk children.

Project description:We observed elevated air pollutant concentrations, especially of ultrafine particles (UFP), black carbon (BC) and NO, across the residential neighborhood of the Boyle Heights Community (BH) of Los Angeles, California. Using an electric vehicle mobile platform equipped with fast response instruments, real-time air pollutant concentrations were measured in BH in spring and summer of 2008. Pollutant concentrations varied significantly in the two seasons, on different days, and by time of day, with an overall average UFP concentration in the residential areas of ~33 000 cm-3. The averaged UFP, BC, and NO concentrations measured on Soto St, a major surface street in BH, were 57 000 cm-3, 5.1 µg m-3, and 67 ppb, respectively. Concentrations of UFP across the residential areas in BH were nearly uniform spatially, in contrast to other areas in the greater metropolitan area of Los Angeles where UFP concentrations exhibit strong gradients downwind of roadways. We attribute this "UFP cloud" to high traffic volumes, including heavy duty diesel trucks on the freeways which surround and traverse BH, and substantial numbers of high-emitting vehicles (HEVs) on the surface streets traversing BH. Additionally, the high density of stop signs and lights and short block lengths, requiring frequent accelerations of vehicles, may contribute. The data also support a role for photochemical production of UFP in the afternoon. UFP concentration peaks (5 s average) of up to 9 million particles cm-3 were also observed immediately behind HEVs when they accelerated from stop lights in the BH neighborhood and areas immediately adjacent. Although encounters with HEV during mornings accounted for only about 6% and 17% of time spent monitoring residential areas and major surface streets, HEV contributed to about 28% and 53% of total ultrafine particles measured on the route, respectively. The observation of elevated pollutant number concentrations across the Boyle Heights community highlights how multiple factors combine to create high pollutant levels, and has important human exposure assessment implications, including the potential utility of our data as inputs to epidemiological studies.

Project description:Air quality, like weather, can affect everyone, but responses differ depending on the sensitivity and health condition of a given individual. To help protect exposed populations, many countries have put in place real-time air quality nowcasting and forecasting capabilities. We present in this paper an optimal combination of air quality measurements and model outputs and show that it leads to significant improvements in the spatial representativeness of air quality. The product is referred to as multi-pollutant surface objective analyses (MPSOAs). Moreover, based on MPSOA, a geographical mapping of the Canadian Air Quality Health Index (AQHI) is also presented which provides users (policy makers, public, air quality forecasters, and epidemiologists) with a more accurate picture of the health risk anytime and anywhere in Canada and the USA. Since pollutants can also behave as passive atmospheric tracers, they provide information about transport and dispersion and, hence, reveal synoptic and regional meteorological phenomena. MPSOA could also be used to build air pollution climatology, compute local and national trends in air quality, and detect systematic biases in numerical air quality (AQ) models. Finally, initializing AQ models at regular time intervals with MPSOA can produce more accurate air quality forecasts. It is for these reasons that the Canadian Meteorological Centre (CMC) in collaboration with the Air Quality Research Division (AQRD) of Environment Canada has recently implemented MPSOA in their daily operations.

Project description:Long-term coronavirus disease 2019 (long-COVID) refers to persistent symptoms of SARS-CoV-2 (COVID-19) lingering beyond four weeks of initial infection. Approximately 30% of COVID-19 survivors develop prolonged symptoms. Communities of color are disproportionately affected by comorbidities, increasing the risk of severe COVID-19 and potentially leading to long-COVID. This study aims to identify trends in health disparities related to COVID-19 cases, which can help unveil potential populations at risk for long-COVID. All North Carolina (NC) counties (n = 100) were selected as a case study. Cases and vaccinations per 1000 population were calculated based on the NC Department of Health and Human Services COVID-19 dashboard with reports current as of 8 October 2021, which were stratified by age groups and race/ethnicity. Then, NC COVID-19 cases were correlated to median household income, poverty, population density, and social vulnerability index themes. We observed a negative correlation between cases (p < 0.05) and deaths (p < 0.01) with both income and vaccination status. Moreover, there was a significant positive association between vaccination status and median household income (p < 0.01). Our results highlight the prevailing trend between exacerbated COVID-19 infection and low-income/under-resourced communities. Consequently, efforts and resources should be channeled to these communities to effectively monitor, diagnose, and treat against COVID-19 and potentially prevent an overwhelming number of long-COVID cases.

Project description:Birth defects are responsible for a large proportion of disability and infant mortality. Exposure to a variety of pesticides have been linked to increased risk of birth defects.We conducted a case-control study to estimate the associations between a residence-based metric of agricultural pesticide exposure and birth defects. We linked singleton live birth records for 2003 to 2005 from the North Carolina (NC) State Center for Health Statistics to data from the NC Birth Defects Monitoring Program. Included women had residence at delivery inside NC and infants with gestational ages from 20 to 44 weeks (n?=?304,906). Pesticide exposure was assigned using a previously constructed metric, estimating total chemical exposure (pounds of active ingredient) based on crops within 500 meters of maternal residence, specific dates of pregnancy, and chemical application dates based on the planting/harvesting dates of each crop. Logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals for four categories of exposure (<10(th) , 10-50(th) , 50-90(th) , and >90(th) percentiles) compared with unexposed. Models were adjusted for maternal race, age at delivery, education, marital status, and smoking status.We observed elevated ORs for congenital heart defects and certain structural defects affecting the gastrointestinal, genitourinary and musculoskeletal systems (e.g., OR [95% confidence interval] [highest exposure vs. unexposed] for tracheal esophageal fistula/esophageal atresia?=?1.98 [0.69, 5.66], and OR for atrial septal defects: 1.70 [1.34, 2.14]).Our results provide some evidence of associations between residential exposure to agricultural pesticides and several birth defects phenotypes.
