Retroperitoneal Fibrosis after Chronic Abscesses of Silicone Fluid Fillers in a Case of Gluteal Augmentation.

ABSTRACT: Filler injection or implantation is a progressing revolutionary subject. Although the widely available kinds in many implications are considered safe, post filler adverse events are not uncommon. These reactions range from mild reactions such as edema or erythema to detrimental reactions such as recurrent infected granuloma or vascular occlusion, which are predominantly related to non-FDA approved materials. Here, we presented a patient with a significant history of gluteal augmentation using unlicensed silicone who developed extensive retroperitoneal fibrosis complicated by deep venous occlusions and obstructive uropathy.

A rare case of aggressive pyogenic spondylitis with giant abscesses after vertebral augmentation.

Project description:Although Streptococcus constellatus (SC), an opportunistic pathogen, can cause abscesses and empyema. The SC infection after vertebral augmentation (VA) can interfere with patients' daily living activities and can be life-threatening in severe cases. A 67-year-old male complained of lumbar pain for two months. The patient underwent percutaneous vertebroplasty of the second and third lumbar vertebrae two months ago. On admission, laboratory and imaging evidence suggested infection of the second and third lumbar vertebrae with bilateral psoas major and left lumbodorsal abscesses. After three weeks of empirical anti-infective therapy, abscess removal and the second and third lumbar vertebrae fusion with iliac bone graft were performed under general anesthesia. Intraoperative pathology and next-generation sequencing (NGS) examination of the pus suggested SC infection, and oral linezolid was given for 12 weeks after surgery. The infection was eventually cured and the patient achieved satisfactory function. In conclusion, pyogenic spondylitis due to SC infection after VA is a life-threatening complication. In addition to infectious disease consultation and routine etiological screening, NGS is important to identify infection with unknown pathogens. Surgery combined with sensitive antibiotics is appropriate for patients with progressive neurological deficits.

Silicone Composites with CNT/Graphene Hybrid Fillers: A Review.

Project description:This review is dedicated to versatile silicone rubber composites based on carbon nanotube/graphene (CNT/G) hybrid fillers. Due to their unique mechanical, electrical, thermal, and biological properties, such composites have enormous potential for medical, environmental, and electronics applications. In the scope of this paper, we have explored CNT/graphene/silicone composites with a different morphology, analyzed the synergistic effect of hybrid fillers on various properties of silicone composites, and observed the existing approaches for the fabrication of hybrid composites with a seamless, assembled, and/or foamed structure. In conclusion, current challenges and future prospects for silicone composites based on CNTs and graphene have been thoroughly discussed.

Clinical characteristics of IgG4-related retroperitoneal fibrosis versus idiopathic retroperitoneal fibrosis.

Project description:Retroperitoneal fibrosis (RPF) is an uncommon condition characterized by inflammation and fibrosis in the retroperitoneal space. More than two-thirds of RPF are idiopathic, with the remaining stemed from a variety of secondary causes. It was suggested that IgG4-related RPF is a secondary form of RPF. We undertook this study to compare detailed demographic, clinical and laboratory characteristics of IgG4-related RPF and IRPF in a large Chinese cohort. We retrospectively reviewed the medical records of 132 RPF patients diagnosed at Peking University People's Hospital between March 2010 and March 2018. Among the 132 patients, the mean age at disease onset was 54.8 years. IgG4-related RPF group showed greater male predominance compared to IRPF group. IgG4-related RPF patients showed a longer interval between symptom onset and diagnosis, and allergic diseases were more common in this group. Sixty-four patients (48.4%) had lower back pain, which was more common in IRPF group than that in IgG4-related RPF patients. In terms of organ involvement, although 42 of 47 patients (89.3%) with IgG4-related RPF had other organ involvement, there were no patients in the IRPF group with other organ involvement. In addition, the serum IgG4 level, elevated eosinophils counts and IgE level were significantly higher in IgG4-related RPF patients. We described the demographic, clinical and laboratory differences between IgG4-related RPF and IRPF patients, indicating their potential differences in pathogenesis, which was of great importance to diagnose and manage the two phenotypes.

Compression Response of Silicone-Based Composites with Integrated Multifunctional Fillers.

Project description:Polydimethylsiloxane (PDMS) is known for its exceptional mechanical properties, chemical stability, and flexibility. Recent advancements have focused on developing functional PDMS composites by integrating various functional fillers, including polymers, ceramics, and metals, for advanced applications such as electronics, medical devices, and aerospace. Consequently, there is a growing need to investigate PDMS composites to achieve higher filler loadings offering enhanced mechanical performance. This study addresses this need by utilizing the high molecular weight (MW) PDMS resin we have developed, offering its high elongation capacity of up to >6500%. We incorporated boron (B), hollow glass microballoons (HGMs), and tungsten-coated hollow glass microballoons (WHGMs) into the developed high MW PDMS. The resulting composites demonstrated excellent elastic properties and significant compression resilience (35-80%) and elastic modulus (1.28-10.15 MPa) at high filler loadings (~60 vol.%). Specifically, B/PDMS composites achieved up to 67.6 vol.% of B, HGM/PDMS composites held up to 68.6 vol.% of HGM, and WHGM/PDMS composites incorporated up to 54.0 vol.% of WHGM. These findings highlight the potential of high MW PDMS for developing high-performance PDMS composites suitable for advanced applications such as aerospace, automotive, and medical devices.

Case report: IgG4-related renal disease co-existing with retroperitoneal fibrosis.

Project description:A 51-year-old woman visited our clinic for orthopnea, chest discomfort, and facial and leg edema. Acute kidney injury and pleural effusion were diagnosed and she received hemodialysis. This is a rare case of IgG4-related disease diagnosed by renal biopsy in the patient with retroperitoneal fibrosis and severe acute kidney injury, which progressed rapidly and required hemodialysis without markedly elevated serum IgG4 level and urinary tract obstruction. Immunostaining of a kidney biopsy revealed IgG4-related renal disease and magnetic resonance imaging showed retroperitoneal fibrosis in the paraspinal and presacral areas. After 3 days of glucocorticoid treatment, renal function and bilateral pleural effusion were restored. Another 2 months of glucocorticoid treatment decreased the extent of retroperitoneal fibrosis on follow-up abdominal computed tomography. IgG4-related disease is a recently recognized fibro-inflammatory condition with a dense lymphoplasmacytic infiltration of IgG4-positive plasma cells. The kidney and retroperitoneum are the frequently associated organs in IgG4-related disease. This case shows that IgG4-related disease can involve the kidney and/or other organs and be diagnosed by kidney biopsy.

Pediatric idiopathic retroperitoneal fibrosis.

Project description:Retroperitoneal fibrosis (RPF) is a very rare disease that is even more rare in the pediatric population. Even less common are idiopathic pediatric cases of retroperitoneal fibrosis, with a majority of reported pediatric retroperitoneal fibrosis cases being associated with secondary etiologies. We present an 11-year-old Caucasian female that was diagnosed with idiopathic retroperitoneal fibrosis using magnetic resonance imaging (MRI) to work-up severe bilateral hydronephrosis that was identified with retroperitoneal ultrasound. Given the uncommon nature of this serious condition, we present this case to illustrate the importance for physicians to include retroperitoneal fibrosis in the differential diagnosis of a pediatric patient presenting with obstructive urinary findings and understand the utility of using MRI to diagnosis and monitor this disease.

Al@SiO<sub>2</sub> Core-Shell Fillers Enhance Dielectric Properties of Silicone Composites.

Project description:Over the past decade, there has been significant interest in polysiloxane-based dielectric elastomers as promising soft electroactive materials. Nevertheless, the natural low permittivity of polydimethylsiloxane has limited its practical applications. In this study, we have developed silicone rubber/Al@SiO2 composites with a high dielectric constant, low dielectric loss, and high electrical breakdown strength by controlling the shell layer thickness and the content of the core-shell filler. We also investigated the dielectric behavior of the composites. The use of core-shell fillers has increased the Maxwell-Wagner-Sillars (MWS) relaxation process while reducing the dielectric loss of direct current conductance in silicone rubber composites. Moreover, the temperature dependence of the MWS relaxation time in the composites follows the Arrhenius equation. This strategy of increasing the permittivity of silicone composites through core-shell structural fillers can inspire the preparation of other high dielectric constant composites.

Influence of Herbal Fillers Addition on Selected Properties of Silicone Subjected to Accelerated Aging.

Project description:This work aims to assess the impact of the type and percentage of powdered herbs on selected properties of silicone-based composites. The matrix was an addition cross-linked platinum-cured polydimethylsiloxane. The fillers were powdered thyme and sage, which were introduced at 5, 10, and 15 wt.%. The introduced fillers differed in composition, morphology, and grain size. The grain morphology showed differences in the size and shape of the introduced fillers. The qualitative and quantitative assessment resulting from the incorporation was conducted based on tests of selected properties: density, wettability, rebound resilience, hardness, and tensile strength. The incorporation slightly affected the density and wettability of the silicone. Rebound resilience and hardness results differed depending on the filler type and fraction. However, tensile strength decreased, which may be due to the matrix's distribution of fillers and their chemical composition. Antibacterial activity evaluation against S. aureus proved the bacteriostatic properties of the composites. Accelerated aging in PBS solution further deteriorated the mechanical properties. FTIR and DSC have demonstrated the progressive aging of the materials. In addition, the results showed an overall minimal effect of fillers on the silicone chemical backbone and melting temperature. The developed materials can be used in applications that do not require high mechanical properties.

Complications and Surgical Treatment of Breast Augmentation Using Autologous Fat Transfer and Fillers.

Project description:BackgroundBreast augmentation with autologous fat grafting or hyaluronic acid injection requires minimal loss of healthy tissue. With an increasing trend of breast augmentation with these fillers, accompanying complications have also increased. Patients with complications often complain of induration, cyst formation, calcification, and infection, which require surgical treatment. We will discuss these complications and their surgical treatment through our experience of cases.MethodsThis retrospective study included 20 patients who all required surgical treatment due to breast augmentation complications such as induration, cyst formation, calcification, and infection, and who visited us between May 2007 and June 2018. The patients' ages ranged from 25 to 63, and the mean age was 39.9. The material used for breast augmentation was fat for 17 cases, and hyaluronic acid, paraffin, and silicon for one case each. The results were analyzed through plastic surgeons at our hospital.ResultsWe performed a zigzag incision in the peri-areola margin to 17 of 18 patients for complications of autologous fat grafting and hyaluronic acid injection. The one excluded patient required an adipo-fascial flap from an inframammary fold incision. For one patient with silicon injection complication and one patient with paraffin injection complication, each required mammary gland resection.ConclusionsA zigzag incision in the peri-areolar margin was useful for treating complications of breast augmentation with autologous fat grafting and hyaluronic acid injection. All cases resulted in inconspicuous fine scars, with high patient satisfaction. However, this incision was insufficient to remove injected silicon and paraffin.
