Project description:ObjectivesTo investigate the intra- and inter-rater reliability of the total radiomics quality score (RQS) and the reproducibility of individual RQS items' score in a large multireader study.MethodsNine raters with different backgrounds were randomly assigned to three groups based on their proficiency with RQS utilization: Groups 1 and 2 represented the inter-rater reliability groups with or without prior training in RQS, respectively; group 3 represented the intra-rater reliability group. Thirty-three original research papers on radiomics were evaluated by raters of groups 1 and 2. Of the 33 papers, 17 were evaluated twice with an interval of 1 month by raters of group 3. Intraclass coefficient (ICC) for continuous variables, and Fleiss' and Cohen's kappa (k) statistics for categorical variables were used.ResultsThe inter-rater reliability was poor to moderate for total RQS (ICC 0.30-055, p < 0.001) and very low to good for item's reproducibility (k - 0.12 to 0.75) within groups 1 and 2 for both inexperienced and experienced raters. The intra-rater reliability for total RQS was moderate for the less experienced rater (ICC 0.522, p = 0.009), whereas experienced raters showed excellent intra-rater reliability (ICC 0.91-0.99, p < 0.001) between the first and second read. Intra-rater reliability on RQS items' score reproducibility was higher and most of the items had moderate to good intra-rater reliability (k - 0.40 to 1).ConclusionsReproducibility of the total RQS and the score of individual RQS items is low. There is a need for a robust and reproducible assessment method to assess the quality of radiomics research.Clinical relevance statementThere is a need for reproducible scoring systems to improve quality of radiomics research and consecutively close the translational gap between research and clinical implementation.Key points• Radiomics quality score has been widely used for the evaluation of radiomics studies. • Although the intra-rater reliability was moderate to excellent, intra- and inter-rater reliability of total score and point-by-point scores were low with radiomics quality score. • A robust, easy-to-use scoring system is needed for the evaluation of radiomics research.

Project description:Few studies have reported the reproducibility and stability of ultrasound (US) images based radiomics features obtained from automatic segmentation in oncology. The purpose of this study is to study the accuracy of automatic segmentation algorithms based on multiple U-net models and their effects on radiomics features from US images for patients with ovarian cancer. A total of 469 US images from 127 patients were collected and randomly divided into three groups: training sets (353 images), validation sets (23 images), and test sets (93 images) for automatic segmentation models building. Manual segmentation of target volumes was delineated as ground truth. Automatic segmentations were conducted with U-net, U-net++, U-net with Resnet as the backbone (U-net with Resnet), and CE-Net. A python 3.7.0 and package Pyradiomics 2.2.0 were used to extract radiomic features from the segmented target volumes. The accuracy of automatic segmentations was evaluated by Jaccard similarity coefficient (JSC), dice similarity coefficient (DSC), and average surface distance (ASD). The reliability of radiomics features were evaluated by Pearson correlation and intraclass correlation coefficients (ICC). CE-Net and U-net with Resnet outperformed U-net and U-net++ in accuracy performance by achieving a DSC, JSC, and ASD of 0.87, 0.79, 8.54, and 0.86, 0.78, 10.00, respectively. A total of 97 features were extracted from the delineated target volumes. The average Pearson correlation was 0.86 (95% CI, 0.83-0.89), 0.87 (95% CI, 0.84-0.90), 0.88 (95% CI, 0.86-0.91), and 0.90 (95% CI, 0.88-0.92) for U-net++, U-net, U-net with Resnet, and CE-Net, respectively. The average ICC was 0.84 (95% CI, 0.81-0.87), 0.85 (95% CI, 0.82-0.88), 0.88 (95% CI, 0.85-0.90), and 0.89 (95% CI, 0.86-0.91) for U-net++, U-net, U-net with Resnet, and CE-Net, respectively. CE-Net based segmentation achieved the best radiomics reliability. In conclusion, U-net based automatic segmentation was accurate enough to delineate the target volumes on US images for patients with ovarian cancer. Radiomics features extracted from automatic segmented targets showed good reproducibility and for reliability further radiomics investigations.

Project description:The purpose of this study was to (i) evaluate the test-retest repeatability and reproducibility of radiomic features in virtual monoenergetic images (VMI) from dual-energy CT (DECT) depending on VMI energy (40, 50, 75, 120, 190 keV), radiation dose (5 and 15 mGy), and DECT approach (dual-source and split-filter DECT) in a phantom (ex vivo), and (ii) to assess the impact of VMI energy and feature repeatability on machine-learning-based classification in vivo in 72 patients with 72 hypodense liver lesions. Feature repeatability and reproducibility were determined by concordance-correlation-coefficient (CCC) and dynamic range (DR) ≥0.9. Test-retest repeatability was high within the same VMI energies and scan conditions (percentage of repeatable features ranging from 74% for SFDE mode at 40 keV and 15 mGy to 86% for DSDE at 190 keV and 15 mGy), while reproducibility varied substantially across different VMI energies and DECTs (percentage of reproducible features ranging from 32.8% for SFDE at 5 mGy comparing 40 with 190 keV to 99.2% for DSDE at 15 mGy comparing 40 with 50 keV). No major differences were observed between the two radiation doses (<10%) in all pair-wise comparisons. In vivo, machine learning classification using penalized regression and random forests resulted in the best discrimination of hemangiomas and metastases at low-energy VMI (40 keV), and for cysts at high-energy VMI (120 keV). Feature selection based on feature repeatability did not improve classification performance. Our results demonstrate the high repeatability of radiomics features when keeping scan and reconstruction conditions constant. Reproducibility diminished when using different VMI energies or DECT approaches. The choice of optimal VMI energy improved lesion classification in vivo and should hence be adapted to the specific task.

Project description:Radiomics is an emerging field using the extraction of quantitative features from medical images for tissue characterization. While MRI-based radiomics is still at an early stage, it showed some promising results in studies focusing on breast cancer patients in improving diagnoses and therapy response assessment. Nevertheless, the use of radiomics raises a number of issues regarding feature quantification and robustness. Therefore, our study aim was to determine the robustness of radiomics features extracted by two commonly used radiomics software with respect to variability in manual breast tumor segmentation on MRI. A total of 129 histologically confirmed breast tumors were segmented manually in three dimensions on the first post-contrast T1-weighted MR exam by four observers: a dedicated breast radiologist, a resident, a Ph.D. candidate, and a medical student. Robust features were assessed using the intraclass correlation coefficient (ICC > 0.9). The inter-observer variability was evaluated by the volumetric Dice Similarity Coefficient (DSC). The mean DSC for all tumors was 0.81 (range 0.19-0.96), indicating a good spatial overlap of the segmentations based on observers of varying expertise. In total, 41.6% (552/1328) and 32.8% (273/833) of all RadiomiX and Pyradiomics features, respectively, were identified as robust and were independent of inter-observer manual segmentation variability.

Project description:Objectives To explore agreement among healthcare professionals assessing eligibility for work disability benefits.Design Systematic review and narrative synthesis of reproducibility studies.Data sources Medline, Embase, and PsycINFO searched up to 16 March 2016, without language restrictions, and review of bibliographies of included studies.Eligibility criteria Observational studies investigating reproducibility among healthcare professionals performing disability evaluations using a global rating of working capacity and reporting inter-rater reliability by a statistical measure or descriptively. Studies could be conducted in insurance settings, where decisions on ability to work include normative judgments based on legal considerations, or in research settings, where decisions on ability to work disregard normative considerations. : Teams of paired reviewers identified eligible studies, appraised their methodological quality and generalisability, and abstracted results with pretested forms. As heterogeneity of research designs and findings impeded a quantitative analysis, a descriptive synthesis stratified by setting (insurance or research) was performed.Results From 4562 references, 101 full text articles were reviewed. Of these, 16 studies conducted in an insurance setting and seven in a research setting, performed in 12 countries, met the inclusion criteria. Studies in the insurance setting were conducted with medical experts assessing claimants who were actual disability claimants or played by actors, hypothetical cases, or short written scenarios. Conditions were mental (n=6, 38%), musculoskeletal (n=4, 25%), or mixed (n=6, 38%). Applicability of findings from studies conducted in an insurance setting to real life evaluations ranged from generalisable (n=7, 44%) and probably generalisable (n=3, 19%) to probably not generalisable (n=6, 37%). Median inter-rater reliability among experts was 0.45 (range intraclass correlation coefficient 0.86 to κ-0.10). Inter-rater reliability was poor in six studies (37%) and excellent in only two (13%). This contrasts with studies conducted in the research setting, where the median inter-rater reliability was 0.76 (range 0.91-0.53), and 71% (5/7) studies achieved excellent inter-rater reliability. Reliability between assessing professionals was higher when the evaluation was guided by a standardised instrument (23 studies, P=0.006). No such association was detected for subjective or chronic health conditions or the studies' generalisability to real world evaluation of disability (P=0.46, 0.45, and 0.65, respectively).Conclusions Despite their common use and far reaching consequences for workers claiming disabling injury or illness, research on the reliability of medical evaluations of disability for work is limited and indicates high variation in judgments among assessing professionals. Standardising the evaluation process could improve reliability. Development and testing of instruments and structured approaches to improve reliability in evaluation of disability are urgently needed.

Project description:PurposeAccurate lesion segmentation is a prerequisite for radiomic feature extraction. It helps to reduce the features variability so as to improve the reporting quality of radiomics study. In this research, we aimed to conduct a radiomic feature reproducibility test of inter-/intra-observer delineation variability in hepatocellular carcinoma using 3D-CT images, 4D-CT images and multiple-parameter MR images.Materials and methodsFor this retrospective study, 19 HCC patients undergoing 3D-CT, 4D-CT and multiple-parameter MR scans were included in this study. The gross tumor volume (GTV) was independently delineated twice by two observers based on contrast-enhanced computed tomography (CECT), maximum intensity projection (MIP), LAVA-Flex, T2W FRFSE and DWI-EPI images. We also delineated the peritumoral region, which was defined as 0 to 5 mm radius surrounding the GTV. 107 radiomic features were automatically extracted from CECT images using 3D-Slicer software. Quartile coefficient of dispersion (QCD) and intraclass correlation coefficient (ICC) were applied to assess the variability of each radiomic feature. QCD<10% and ICC≥0.75 were considered small variations and excellent reliability. Finally, the principal component analysis (PCA) was used to test the feasibility of dimensionality reduction.ResultsFor tumor tissues, the numbers of radiomic features with QCD<10% indicated no obvious inter-/intra-observer differences or discrepancies in 3D-CT, 4D-CT and multiple-parameter MR delineation. However, the number of radiomic features (mean 89) with ICC≥0.75 was the highest in the multiple-parameter MR group, followed by the 3DCT group (mean 77) and the MIP group (mean 73). The peritumor tissues also showed similar results. A total of 15 and 7 radiomic features presented excellent reproducibility and small variation in tumor and peritumoral tissues, respectively. Two robust features showed excellent reproducibility and small variation in tumor and peritumoral tissues. In addition, the values of the two features both represented statistically significant differences among tumor and peritumoral tissues (P<0.05). The PCA results indicated that the first seven principal components could preserve at least 90% of the variance of the original set of features.ConclusionDelineation on multiple-parameter MR images could help to improve the reproducibility of the HCC CT radiomic features and weaken the inter-/intra-observer influence.

Project description:Acute post-anoxic myoclonus (PAM) can be divided into an unfavorable (generalized/subcortical) and more favorable ((multi)focal/cortical) outcome group that could support prognostication in post-anoxic encephalopathy; however, the inter-rater variability of clinically assessing these PAM subtypes is unknown.We prospectively examined PAM patients using a standardized video protocol. Videos were rated by three neurologists who classified PAM phenotype (generalized/(multi)focal), stimulus sensitivity, localization (proximal/distal/both), and severity (Clinical Global Impression-Severity Scale (CGI-S) and Unified Myoclonus Rating Scale (UMRS)).Poor inter-rater agreement was found for phenotype and stimulus sensitivity (κ=-0.05), moderate agreement for localization (κ=0.46). Substantial agreement was obtained for the CGI-S (intraclass correlation coefficient (ICC)=0.64) and almost perfect agreement for the UMRS (ICC=0.82).Clinical assessment of PAM is not reproducible between physicians, and should therefore not be used for prognostication. PAM severity measured by the UMRS appears to be reliable; however, the relation between PAM severity and outcome is unknown.

Project description:PurposeThe aim of this paper is to describe a public, open-access, computed tomography (CT) phantom image set acquired at three centers and collected especially for radiomics reproducibility research. The dataset is useful to test radiomic features reproducibility with respect to various parameters, such as acquisition settings, scanners, and reconstruction algorithms.Acquisition and validation methodsThree phantoms were scanned in three independent institutions. Images of the following phantoms were acquired: Catphan 700 and COPDGene Phantom II (Phantom Laboratory, Greenwich, NY, USA), and the Triple modality 3D Abdominal Phantom (CIRS, Norfolk, VA, USA). Data were collected at three Dutch medical centers: MAASTRO Clinic (Maastricht, NL), Radboud University Medical Center (Nijmegen, NL), and University Medical Center Groningen (Groningen, NL) with scanners from two different manufacturers Siemens Healthcare and Philips Healthcare. The following acquisition parameter were varied in the phantom scans: slice thickness, reconstruction kernels, and tube current.Data format and usage notesWe made the dataset publically available on the Dutch instance of "Extensible Neuroimaging Archive Toolkit-XNAT" (https://xnat.bmia.nl). The dataset is freely available and reusable with attribution (Creative Commons 3.0 license).Potential applicationsOur goal was to provide a findable, open-access, annotated, and reusable CT phantom dataset for radiomics reproducibility studies. Reproducibility testing and harmonization are fundamental requirements for wide generalizability of radiomics-based clinical prediction models. It is highly desirable to include only reproducible features into models, to be more assured of external validity across hitherto unseen contexts. In this view, phantom data from different centers represent a valuable source of information to exclude CT radiomic features that may already be unstable with respect to simplified structures and tightly controlled scan settings. The intended extension of our shared dataset is to include other modalities and phantoms with more realistic lesion simulations.

Project description:IntroductionExternal radiotherapy is a major treatment for localized prostate cancer (PCa). Dose escalation to the whole prostate gland increases biochemical relapse-free survival but also acute and late toxicities. Dose escalation to the dominant index lesion (DIL) only is of growing interest. It requires a robust delineation of the DIL. In this context, we aimed to evaluate the inter-observer variability of DIL delineation.Material and methodsTwo junior radiologists and a senior radiation oncologist delineated DILs on 64 mpMRIs of patients with histologically confirmed PCa. For each mpMRI and each reader, eight individual DIL segmentations were delineated. These delineations were blindly performed from one another and resulted from the individual analysis of the T2, apparent diffusion coefficient (ADC), b2000, and dynamic contrast enhanced (DCE) sequences, as well as the analysis of combined sequences (T2ADC, T2ADCb2000, T2ADCDCE, and T2ADCb2000DCE). Delineation variability was assessed using the DICE coefficient, Jaccard index, Hausdorff distance measure, and mean distance to agreement.ResultsT2, ADC, T2ADC, b2000, T2 + ADC + b2000, T2 + ADC + DCE, and T2 + ADC + b2000 + DCE sequences obtained DICE coefficients of 0.51, 0.50, 0.54, 0.52, 0.54, 0.55, 0.53, respectively, which are significantly higher than the perfusion sequence alone (0.35, p < 0.001). The analysis of other similarity metrics lead to similar results. The tumor volume and PI-RADS classification were positively correlated with the DICE scores.ConclusionOur study showed that the contours of prostatic lesions were more reproducible on certain sequences but confirmed the great variability of prostatic contours with a maximum DICE coefficient calculated at 0.55 (joint analysis of T2, ADC, and perfusion sequences).

Project description:BackgroundRadiomics suffers from feature reproducibility. We studied the variability of radiomics features and the relationship of radiomics features with tumor size and shape to determine guidelines for optimal radiomics study.MethodsWe dealt with 260 lung nodules (180 for training, 80 for testing) limited to 2 cm or less. We quantified how voxel geometry (isotropic/anisotropic) and the number of histogram bins, factors commonly adjusted in multi-center studies, affect reproducibility. First, features showing high reproducibility between the original and isotropic transformed voxel settings were identified. Second, features showing high reproducibility in various binning settings were identified. Two hundred fifty-two features were computed and features with high intra-correlation coefficient were selected. Features that explained nodule status (benign/malignant) were retained using the least absolute shrinkage selector operator. Common features among different settings were identified, and the final features showing high reproducibility correlated with nodule status were identified. The identified features were used for the random forest classifier to validate the effectiveness of the features. The properties of the uncalculated feature were inspected to suggest a tentative guideline for radiomics studies.ResultsNine features showing high reproducibility for both the original and isotropic voxel settings were selected and used to classify nodule status (AUC 0.659-0.697). Five features showing high reproducibility among different binning settings were selected and used in classification (AUC 0.729-0.748). Some texture features are likely to be successfully computed if a nodule was larger than 1000 mm3.ConclusionsFeatures showing high reproducibility among different settings correlated with nodule status were identified.
