Incorrect sleeping position and eye rubbing in patients with unilateral or highly asymmetric keratoconus: a case-control study.

ABSTRACT: PURPOSE:To evaluate eye rubbing and sleeping position in patients with Unilateral or Highly Asymmetric Keratoconus (UHAKC). METHODS:Case-control study of consecutive UHAKC patients diagnosed at the Rothschild Foundation. Controls were age- and sex-matched, randomly selected refractive surgery clinic patients. Patients self-administered questionnaires regarding their family history of keratoconus, eye rubbing, and sleeping habits. All the eyes underwent a comprehensive ocular examination. Logistic regression was used to analyze univariate and multivariate data to identify risk factors for keratoconus. RESULTS:Thirty-three UHAKC patients and 64 controls were included. Univariate analyses showed that daytime eye rubbing [OR?=?172.78], in the morning [OR?=?24.3], or in eyes with the steepest keratometry [OR?=?21.7] were significantly different between groups. Allergy [OR?=?2.94], red eyes in the morning [OR?=?6.36], and sleeping on stomach/sides [OR?=?14.31] or on the same side as the steepest keratometry [OR?=?94.72] were also significantly different. The multivariate model also showed statistical significance for most factors including daytime eye rubbing [OR?=?134.96], in the morning [OR?=?24.86], in the steepest eye [OR?=?27.56], and sleeping on stomach/sides [OR?=?65.02] or on the steepest side [OR?=?144.02]. Project description:PURPOSE:To determine optimal objective, machine-derived variables and variable combinations from Scheimpflug and spectral-domain (SD) OCT imaging to distinguish the clinically unaffected eye in patients with asymmetric keratoconus (AKC) from a normal control population. DESIGN:Retrospective case-control study. PARTICIPANTS:Thirty clinically unaffected eyes with no physical findings on slit-lamp examination, no definitive abnormalities on corneal imaging, and corrected distance acuity of 20/20 or better from 30 patients with highly AKC eyes and 60 eyes from 60 normal control patients who had undergone uneventful LASIK with at least 2 years of stable follow-up (controls). METHODS:Scheimpflug and SD OCT imaging were obtained in all eyes, and receiver operating characteristic (ROC) curves were generated to determine area under the curve (AUC), sensitivity, and specificity for each machine-derived variable and variable combination. MAIN OUTCOME MEASURES:Distinguishing AKC eyes from controls as determined by AUC, sensitivity, and specificity. RESULTS:No individual machine-derived metric from Scheimpflug or SD OCT technology yielded an AUC higher than 0.75. Combining 5 Scheimpflug metrics (index height decentration [IHD], index vertical asymmetry [IVA], pachymetry apex, inferior-superior value, and Ambrosio's Relational Thickness Maximum [ARTmax]) yielded the best Scheimpflug results (AUC 0.86, sensitivity 83%, specificity 83%). Combining 11 SD OCT thickness metrics (minimum-median, temporal outer, superior nasal outer, minimum, epithelium minimum-maximum, epithelial standard deviation, superior inner, superior outer, superior temporal outer, superior nasal inner, central) yielded the best SD OCT results (AUC 0.96, sensitivity 89%, specificity 89%). Combining 13 total Scheimpflug/SD OCT metrics yielded the best results overall (AUC 1.0, sensitivity 100%, specificity 100%). The most impactful variables in combined models included epithelial thickness variability and total focal corneal thickness variability from SD OCT and anterior curvature and topometric indices from Scheimpflug technology. No posterior corneal metrics were impactful in modeling. CONCLUSIONS:Individual machine-derived metrics from Scheimpflug and SD OCT imaging poorly distinguished normal eyes from minimally affected eyes from patients with highly AKC. Combined SD OCT metrics performed better than combined Scheimpflug metrics. Combining anterior curvature and asymmetry indices from Scheimpflug with regional total thickness and epithelial thickness variability metrics from SD OCT clearly distinguished the 2 populations. Posterior corneal indices were not useful in distinguishing populations.

Project description:To investigate corneal sensitivity to selective mechanical, chemical, and thermal stimulation and to evaluate their relation to dry eye symptoms in patients with keratoconus.Corneal sensitivity to mechanical, chemical, and thermal thresholds were determined using a gas esthesiometer in 19 patients with keratoconus (KC group) and in 20 age-matched healthy subjects (control group). Tear film dynamics was assessed by Schirmer I test and by the non-invasive tear film breakup time (NI-BUT). All eyes were examined with a rotating Scheimpflug camera to assess keratoconus severity.KC patients had significatly decreased tear secretion and significantly higher ocular surface disease index (OSDI) scores compared to controls (5.3±2.2 vs. 13.2±2.0 mm and 26.8±15.8 vs. 8.1±2.3; p<0.001). There was no significant difference in NI-BUT between the two groups (KC: 9.8±4.8 vs. control: 10.7±3.8; p>0.05). The mean threshold for selective mechanical (KC: 139.2±25.8 vs. control: 109.1±24.0 ml/min), chemical (KC: 39.4±3.9 vs. control: 35.2±1.9%CO2), heat (KC: 0.91±0.32 vs. control: 0.54±0.26 ?°C) and cold (KC: 1.28±0.27 vs. control: 0.98±0.25 ?°C) stimulation in the KC patients were significantly higher than in the control subjects (p<0.001, for all parameters). No correlation was found between age and mechanical, chemical, heat or cold thresholds in the patients with KC (p>0.05), whereas in the control subjects both mechanical (r = 0.52, p = 0.02), chemical (r = 0.47, p = 0.04), heat (r = 0.26, p = 0.04) and cold threshold (r = 0.40, p = 0.03) increased with age. In the KC group, neither corneal thickness nor tear flow, NI-BUT or OSDI correlated significantly with mechanical, chemical, heat or cold thresholds (p>0.05 for all variables).Corneal sensitivity to different types of stimuli is decreased in patients with keratoconus independently of age and disease severity. The reduction of the sensory input from corneal nerves may contribute to the onset of unpleasant sensations in these patients and might lead to the impaired tear film dynamics.

Project description:ImportanceThis study determines the prevalence of unilateral vision impairment (VI) and unilateral blindness to assist in policy formulation for eye health care services.ObjectiveTo determine the prevalence and causes of unilateral VI and unilateral blindness in Australia.Design, setting, and participantsThis cross-sectional population-based survey was conducted from March 2015 to April 2016 at 30 randomly selected sites across all strata of geographic remoteness in Australia. A total of 1738 indigenous Australians 40 years or older and 3098 nonindigenous Australians 50 years or older were included.Main outcomes and measuresThe prevalence and causes of unilateral vision impairment and blindness, defined as presenting visual acuity worse than 6/12 and 6/60, respectively, in the worse eye, and 6/12 or better in the better eye.ResultsOf the 1738 indigenous Australians, mean (SD) age was 55.0 (10.0) years, and 1024 participants (58.9%) were female. Among the 3098 nonindigenous Australians, mean (SD) age was 66.6 (9.7) years, and 1661 participants (53.6%) were female. The weighted prevalence of unilateral VI in indigenous Australians was 12.5% (95% CI, 11.0%-14.2%) and the prevalence of unilateral blindness was 2.4% (95% CI, 1.7%-3.3%), respectively. In nonindigenous Australians, the prevalence of unilateral VI was 14.6% (95% CI, 13.1%-16.3%) and unilateral blindness was found in 1.4% (95% CI, 1.0%-1.8%). The age-adjusted and sex-adjusted prevalence of unilateral vision loss was higher in indigenous Australians than nonindigenous Australians (VI: 18.7% vs 14.5%; P = .02; blindness: 2.9% vs 1.3%; P = .02). Risk factors for unilateral vision loss included older age (odds ratio [OR], 1.60 for each decade of age for indigenous Australians; 95% CI, 1.39-1.86; OR, 1.65 per decade for nonindigenous Australians; 95% CI, 1.38-1.96), very remote residence (OR, 1.65; 95% CI, 1.01-2.74) and self-reported diabetes (OR, 1.52; 95% CI, 1.12-2.07) for indigenous Australians, and having not undergone an eye examination in the past 2 years for nonindigenous Australians (OR, 1.54; 95% CI, 1.04-2.27). Uncorrected refractive error and cataract were leading causes of unilateral VI in both populations (70%-75%). Corneal pathology (16.7%) and cataract (13.9%) were leading causes of unilateral blindness in indigenous Australians, while amblyopia (18.8%), trauma (16.7%), and age-related macular degeneration (10.4%) were major causes of unilateral blindness in nonindigenous Australians.Conclusions and relevanceUnilateral vision loss is prevalent in indigenous and nonindigenous Australians; however, most cases are avoidable. As those with unilateral vision loss caused by cataract and posterior segment diseases may be at great risk of progressing to bilateral blindness, national blindness prevention programs may benefit from prioritizing examination and treatment of those with unilateral vision loss.
