THE STRATIFIED MICRO-RANDOMIZED TRIAL DESIGN: SAMPLE SIZE CONSIDERATIONS FOR TESTING NESTED CAUSAL EFFECTS OF TIME-VARYING TREATMENTS.

ABSTRACT: Technological advancements in the field of mobile devices and wearable sensors have helped overcome obstacles in the delivery of care, making it possible to deliver behavioral treatments anytime and anywhere. Here, we discuss our work on the design of a mobile health smoking cessation intervention study with the goal of assessing whether reminders, delivered at times of stress, result in a reduction/prevention of stress in the near-term, and whether this effect changes with time in study. Multiple statistical challenges arose in this effort, leading to the development of the stratified micro-randomized trial design. In these designs, each individual is randomized to treatment repeatedly at times determined by predictions of risk. These risk times may be impacted by prior treatment. We describe the statistical challenges and detail how they can be met.

Sample size considerations for micro-randomized trials with binary proximal outcomes.

Project description:Micro-randomized trials (MRTs) are a novel experimental design for developing mobile health interventions. Participants are repeatedly randomized in an MRT, resulting in longitudinal data with time-varying treatments. Causal excursion effects are the main quantities of interest in MRT primary and secondary analyses. We consider MRTs where the proximal outcome is binary and the randomization probability is constant or time-varying but not data-dependent. We develop a sample size formula for detecting a nonzero marginal excursion effect. We prove that the formula guarantees power under a set of working assumptions. We demonstrate via simulation that violations of certain working assumptions do not affect the power, and for those that do, we point out the direction in which the power changes. We then propose practical guidelines for using the sample size formula. As an illustration, the formula is used to size an MRT on interventions for excessive drinking. The sample size calculator is implemented in R package MRTSampleSizeBinary and an interactive R Shiny app. This work can be used in trial planning for a wide range of MRTs with binary proximal outcomes.

Estimating Moderated Causal Effects with Time-varying Treatments and Time-varying Moderators: Structural Nested Mean Models and Regression with Residuals.

Project description:Individuals differ in how they respond to a particular treatment or exposure, and social scientists are often interested in understanding how treatment effects are moderated by observed characteristics of individuals. Effect moderation occurs when individual covariates dampen or amplify the effect of some exposure. This article focuses on estimating moderated causal effects in longitudinal settings where both the treatment and effect moderator vary over time. Effect moderation is typically examined using covariate by treatment interactions in regression analyses, but in the longitudinal setting, this approach may be problematic because time-varying moderators of future treatment may be affected by prior treatment-for example, moderators may also be mediators-and naively conditioning on an outcome of treatment in a conventional regression model can lead to bias. This article introduces to sociology moderated intermediate causal effects and the structural nested mean model for analyzing effect moderation in the longitudinal setting. It discusses problems with conventional regression and presents a new approach to estimation that avoids these problems (regression-with-residuals). The method is illustrated using longitudinal data from the PSID to examine whether the effects of time-varying exposures to poor neighborhoods on the risk of adolescent childbearing are moderated by time-varying family income.

Sample size/power calculation for stratified case-cohort design.

Project description:The case-cohort (CC) study design usually has been used for risk factor assessment in epidemiologic studies or disease prevention trials for rare diseases. The sample size/power calculation for a stratified CC (SCC) design has not been addressed before. This article derives such result based on a stratified test statistic. Simulation studies show that the proposed test for the SCC design utilizing small sub-cohort sampling fractions is valid and efficient for situations where the disease rate is low. Furthermore, optimization of sampling in the SCC design is discussed and compared with proportional and balanced sampling techniques. An epidemiological study is provided to illustrate the sample size calculation under the SCC design.

Sample size considerations for stepped wedge designs with subclusters.

Project description:The stepped wedge cluster randomized trial (SW-CRT) is an increasingly popular design for evaluating health service delivery or policy interventions. An essential consideration of this design is the need to account for both within-period and between-period correlations in sample size calculations. Especially when embedded in health care delivery systems, many SW-CRTs may have subclusters nested in clusters, within which outcomes are collected longitudinally. However, existing sample size methods that account for between-period correlations have not allowed for multiple levels of clustering. We present computationally efficient sample size procedures that properly differentiate within-period and between-period intracluster correlation coefficients in SW-CRTs in the presence of subclusters. We introduce an extended block exchangeable correlation matrix to characterize the complex dependencies of outcomes within clusters. For Gaussian outcomes, we derive a closed-form sample size expression that depends on the correlation structure only through two eigenvalues of the extended block exchangeable correlation structure. For non-Gaussian outcomes, we present a generic sample size algorithm based on linearization and elucidate simplifications under canonical link functions. For example, we show that the approximate sample size formula under a logistic linear mixed model depends on three eigenvalues of the extended block exchangeable correlation matrix. We provide an extension to accommodate unequal cluster sizes and validate the proposed methods via simulations. Finally, we illustrate our methods in two real SW-CRTs with subclusters.

Rasch fit statistics and sample size considerations for polytomous data.

Project description:BackgroundPrevious research on educational data has demonstrated that Rasch fit statistics (mean squares and t-statistics) are highly susceptible to sample size variation for dichotomously scored rating data, although little is known about this relationship for polytomous data. These statistics help inform researchers about how well items fit to a unidimensional latent trait, and are an important adjunct to modern psychometrics. Given the increasing use of Rasch models in health research the purpose of this study was therefore to explore the relationship between fit statistics and sample size for polytomous data.MethodsData were collated from a heterogeneous sample of cancer patients (n = 4072) who had completed both the Patient Health Questionnaire - 9 and the Hospital Anxiety and Depression Scale. Ten samples were drawn with replacement for each of eight sample sizes (n = 25 to n = 3200). The Rating and Partial Credit Models were applied and the mean square and t-fit statistics (infit/outfit) derived for each model.ResultsThe results demonstrated that t-statistics were highly sensitive to sample size, whereas mean square statistics remained relatively stable for polytomous data.ConclusionIt was concluded that mean square statistics were relatively independent of sample size for polytomous data and that misfit to the model could be identified using published recommended ranges.

RNA-seq preprocessing and sample size considerations for gene network inference.

Project description:BackgroundGene network inference (GNI) methods have the potential to reveal functional relationships between different genes and their products. Most GNI algorithms have been developed for microarray gene expression datasets and their application to RNA-seq data is relatively recent. As the characteristics of RNA-seq data are different from microarray data, it is an unanswered question what preprocessing methods for RNA-seq data should be applied prior to GNI to attain optimal performance, or what the required sample size for RNA-seq data is to obtain reliable GNI estimates.ResultsWe ran 9144 analysis of 7 different RNA-seq datasets to evaluate 300 different preprocessing combinations that include data transformations, normalizations and association estimators. We found that there was no single best performing preprocessing combination but that there were several good ones. The performance varied widely over various datasets, which emphasized the importance of choosing an appropriate preprocessing configuration before GNI. Two preprocessing combinations appeared promising in general: First, Log-2 TPM (transcript per million) with Variance-stabilizing transformation (VST) and Pearson Correlation Coefficient (PCC) association estimator. Second, raw RNA-seq count data with PCC. Along with these two, we also identified 18 other good preprocessing combinations. Any of these algorithms might perform best in different datasets. Therefore, the GNI performances of these approaches should be measured on any new dataset to select the best performing one for it. In terms of the required biological sample size of RNA-seq data, we found that between 30 to 85 samples were required to generate reliable GNI estimates.ConclusionsThis study provides practical recommendations on default choices for data preprocessing prior to GNI analysis of RNA-seq data to obtain optimal performance results.

Sample size considerations and predictive performance of multinomial logistic prediction models.

Project description:Multinomial Logistic Regression (MLR) has been advocated for developing clinical prediction models that distinguish between three or more unordered outcomes. We present a full-factorial simulation study to examine the predictive performance of MLR models in relation to the relative size of outcome categories, number of predictors and the number of events per variable. It is shown that MLR estimated by Maximum Likelihood yields overfitted prediction models in small to medium sized data. In most cases, the calibration and overall predictive performance of the multinomial prediction model is improved by using penalized MLR. Our simulation study also highlights the importance of events per variable in the multinomial context as well as the total sample size. As expected, our study demonstrates the need for optimism correction of the predictive performance measures when developing the multinomial logistic prediction model. We recommend the use of penalized MLR when prediction models are developed in small data sets or in medium sized data sets with a small total sample size (ie, when the sizes of the outcome categories are balanced). Finally, we present a case study in which we illustrate the development and validation of penalized and unpenalized multinomial prediction models for predicting malignancy of ovarian cancer.

Sample Size for Joint Testing of Indirect Effects.

Project description:This paper presents methods to calculate sample size for evaluating mediation by joint testing of both links in an indirect pathway from exposure to mediator to outcome. Calculations rely on simulations of the underlying data structure, with testing of the two links performed under the simplifying assumption that the two test statistics are asymptotically independent. Simulations show that the proposed methods are accurate. Continuous and binary exposures and mediators, as well as continuous, binary, count, and survival outcomes are accommodated, along with over-dispersion of count outcomes, design effects, and confounding of the exposure-mediator and mediator-outcome relationships. An illustrative example is provided, and a documented R program implementing the calculations is available online.

Sample size estimation for stratified individual and cluster randomized trials with binary outcomes.

Project description:Individual randomized trials (IRTs) and cluster randomized trials (CRTs) with binary outcomes arise in a variety of settings and are often analyzed by logistic regression (fitted using generalized estimating equations for CRTs). The effect of stratification on the required sample size is less well understood for trials with binary outcomes than for continuous outcomes. We propose easy-to-use methods for sample size estimation for stratified IRTs and CRTs and demonstrate the use of these methods for a tuberculosis prevention CRT currently being planned. For both IRTs and CRTs, we also identify the ratio of the sample size for a stratified trial vs a comparably powered unstratified trial, allowing investigators to evaluate how stratification will affect the required sample size when planning a trial. For CRTs, these can be used when the investigator has estimates of the within-stratum intracluster correlation coefficients (ICCs) or by assuming a common within-stratum ICC. Using these methods, we describe scenarios where stratification may have a practically important impact on the required sample size. We find that in the two-stratum case, for both IRTs and for CRTs with very small cluster sizes, there are unlikely to be plausible scenarios in which an important sample size reduction is achieved when the overall probability of a subject experiencing the event of interest is low. When the probability of events is not small, or when cluster sizes are large, however, there are scenarios where practically important reductions in sample size result from stratification.
