Legal provisions for enforcing social distancing to guard against COVID-19: the case of Hong Kong.

ABSTRACT: The implementation of social distancing measures through legal regulations to contain an epidemiologic outbreak has received little research attention. We reviewed and synthesized data on epidemiology, mobility trends and enforcement activities in the first 5 months of the COVID-19 epidemic in Hong Kong to examine the effectiveness of the newly enacted social distancing regulations. Data collected showed reduced patronage of retail and recreational activities during the epidemic. The regulations' enforcement could be inferred from the increase in the number of inspections, verbal warnings given to operators of scheduled premises and to people in public gatherings, but which rarely led to prosecutions. In parallel, the number of reported COVID-19 cases became stabilized. Our analyses suggested that public compliance with social distancing regulations could be maintained through promotional efforts without enforcement by prosecution. The adverse impacts of prolonged social distancing on the economy and citizens' social and psychological well-being were, however, of concern.

Project description:IntroductionThe COVID-19 pandemic has led to the implementation of social distancing measures on an unprecedented scale, but their impacts on the mental health of the general public, especially in places with strict and prolonged restrictions, are not well understood. This study explored the Hong Kong public's views on social distancing measures that were implemented approximately two years into the pandemic and examined how these perceptions influence their stay-at-home motivation and mental health based on Protection Motivation Theory (PMT).DesignA two-phase sequential exploratory mixed-methods study was conducted.SettingData were collected as part of a larger study of a universal online public health campaign in Hong Kong.ParticipantsFirst, qualitative interviews with 26 participants from diverse backgrounds explored their views on COVID-19 and social distancing measures. Subsequently, a quantitative online survey of 1025 participants from the general population examined the relationship between PMT constructs, stay-at-home motivation, and mental health.Outcome measuresMental health and PMT constructs, including protective motivation, perceived severity, perceived vulnerability, self-efficacy, response cost, and response efficacy.ResultsThe qualitative results identified three themes: 'the inescapable and unpredictable nature of COVID-19', 'stay-at-home impacts all facets of life', and 'the realities and challenges of social distancing during a pandemic'. Subsequently, quantitative findings showed that most PMT constructs, such as perceived severity (ß=0.21, 95% CI=0.15-0.27), perceived vulnerability (ß=0.24, 95% CI=0.18-0.30), self-efficacy (ß=0.27, 95% CI=0.20-0.34) and response efficacy (ß=0.09, 95% CI=0.02-0.15), were associated with stay-at-home motivation (all p<0.01), except for response cost. Higher perceived severity (ß=1.42-1.74), perceived vulnerability (ß=0.93-1.36), and response cost (ß=1.29-1.64) were associated with poorer mental health.ConclusionThis study examined the public perceptions and experiences of strict and prolonged social distancing measures two years into a global pandemic. The findings highlighted the significance of using the PMT model to understand the factors influencing stay-at-home intentions and their mental health during the pandemic. The findings provide valuable insights to guide the development and implementation of future public health interventions.

Project description:BackgroundThe impacts of non-pharmaceutical interventions (NPIs) and vaccine boosters on the transmission of the largest outbreak of COVID-19 (the fifth wave) in Hong Kong have not been reported. The outbreak, dominated by the Omicron BA.2 subvariant, began to spread substantially after the Spring Festival in February, 2022, when the temperature varied greatly (e.g. a cold surge event). Tightening social distancing measures did not succeed in containing the outbreak until later with the use of rapid antigen tests (RAT) and increased vaccination rates. Temperature has been previously found to have significant impact on the transmissibility. Understanding how the public health interventions influence the number of infections in this outbreak provide important insights on prevention and control of COVID-19 during different seasons.MethodsWe developed a transmission model incorporating stratified immunity with vaccine-induced antibody responses and the daily changes in population mobility, vaccination and weather factors (i.e. temperature and relative humidity). We fitted the model to the daily reported cases detected by either PCR or RAT between 1 February and 31 March using Bayesian statistics, and quantified the effects of individual NPIs, vaccination and weather factors on transmission dynamics.ResultsModel predicted that, with the vaccine uptake, social distancing reduced the cumulative incidence (CI) from 58.2% to 44.5% on average. The use of RAT further reduced the CI to 39.0%. Without vaccine boosters in these two months, the CI increased to 49.1%. While public health interventions are important in reducing the total infections, the outbreak was temporarily driven by the cold surge. If the coldest two days (8.5 °C and 8.8 °C) in February were replaced by the average temperature in that month (15.2 °C), the CI would reduce from 39.0% to 28.2%.ConclusionPreventing and preparing for the transmission of COVID-19 considering the change in temperature appears to be a cost-effective preventive strategy to lead people to return to normal life.

Project description:BackgroundGuided internet-based psychological interventions show substantial promise for expanding access to evidence-based mental health care. However, this can only be achieved if results of tightly controlled studies from the treatment developers can also be achieved in other independent settings. This dissemination depends critically on developing efficient and effective ways to train professionals to deliver these interventions. Unfortunately, descriptions of therapist training and its evaluation are often limited or absent within dissemination studies.ObjectiveThis study aimed to describe and evaluate a program of therapist training to deliver internet-based Cognitive Therapy for social anxiety disorder (iCT-SAD). As this treatment was developed in the United Kingdom and this study was conducted in Hong Kong with local therapists, an additional objective was to examine the feasibility, acceptability, and initial efficacy of iCT-SAD in this cultural context, based on data from a pilot case series.MethodsTraining in iCT-SAD was provided to 3 therapists and included practice of the face-to-face format of therapy under clinical supervision, training workshops, and treating 6 patients with the iCT-SAD program. Training progress was evaluated using standardized and self-report measures and by reviewing patient outcomes. In addition, feedback from patients and therapists was sought regarding the feasibility and acceptability of the program.ResultsThe training program was effective at increasing therapists' iCT-SAD knowledge and skills, resulting in levels of competence expected of a specialist Cognitive Behavioral Therapy practitioner. The 6 patients treated by the trainees all completed their treatment and achieved a mean pre- to posttreatment change of 53.8 points (SD 39.5) on the primary patient outcome measure, the Liebowitz Social Anxiety Scale. The within-group effect size (Cohen d) was 2.06 (95% CI 0.66-3.46). There was evidence to suggest that the patients' clinical outcomes were sustained at 3-month follow-up. These clinical results are comparable to those achieved by UK patients treated by the developers of the internet program. Patient and therapist feedback did not identify any major cultural barriers to implementing iCT-SAD in Hong Kong; some modest language suggestions were made to assist understanding.ConclusionsThe therapist training implemented here facilitated the successful dissemination of an effective UK-developed internet intervention to Hong Kong. The treatment appeared feasible and acceptable in this setting and showed highly promising initial efficacy. A randomized controlled trial is now required to examine this more robustly. As therapist training is critical to the successful dissemination of internet interventions, further research to develop, describe, and evaluate therapist training procedures is recommended.
