Visualizing Air Pollution: Communication of Environmental Health Information in a Chinese Immigrant Community.

ABSTRACT: This study developed and evaluated a visual approach to promoting environmental health literacy about highway pollution. The Interactive Map of Chinatown Traffic Pollution was the centerpiece of a communication approach designed to make complex scientific information about traffic-related air pollution comprehensible to Chinese immigrants with limited English proficiency. The map enabled visualization of the spatial distribution of ultrafine particles (less than 100 nanometers in diameter), a toxic and invisible form of air pollution, in Boston Chinatown. A university-community partnership enabled design of intergenerational training sessions aimed toward empowering community members to take health-promoting actions that reduce exposure to ultrafine particulate pollution. A mixed methods approach was taken to evaluation. Nine high school youth learned to use the map and then tutored adults recruited from English as a Second Language (ESL) classes and from a community workshop. Seventy-three of these adults completed a pre-post survey measuring change in three domains: pollution knowledge, attitudes toward environmental issues, and self-efficacy in using maps. Adult participants demonstrated statistically significant improvements in all three domains (Wilcoxon signed-rank test, all p < 0.01). Seventeen adults and nine youth participated in interviews. Interview participants reported adjusting daily routines to reduce exposure to pollution.

Environmental and Health Impacts of Air Pollution: A Review.

Project description:One of our era's greatest scourges is air pollution, on account not only of its impact on climate change but also its impact on public and individual health due to increasing morbidity and mortality. There are many pollutants that are major factors in disease in humans. Among them, Particulate Matter (PM), particles of variable but very small diameter, penetrate the respiratory system via inhalation, causing respiratory and cardiovascular diseases, reproductive and central nervous system dysfunctions, and cancer. Despite the fact that ozone in the stratosphere plays a protective role against ultraviolet irradiation, it is harmful when in high concentration at ground level, also affecting the respiratory and cardiovascular system. Furthermore, nitrogen oxide, sulfur dioxide, Volatile Organic Compounds (VOCs), dioxins, and polycyclic aromatic hydrocarbons (PAHs) are all considered air pollutants that are harmful to humans. Carbon monoxide can even provoke direct poisoning when breathed in at high levels. Heavy metals such as lead, when absorbed into the human body, can lead to direct poisoning or chronic intoxication, depending on exposure. Diseases occurring from the aforementioned substances include principally respiratory problems such as Chronic Obstructive Pulmonary Disease (COPD), asthma, bronchiolitis, and also lung cancer, cardiovascular events, central nervous system dysfunctions, and cutaneous diseases. Last but not least, climate change resulting from environmental pollution affects the geographical distribution of many infectious diseases, as do natural disasters. The only way to tackle this problem is through public awareness coupled with a multidisciplinary approach by scientific experts; national and international organizations must address the emergence of this threat and propose sustainable solutions.

Air pollution and lung function in the European Community Respiratory Health Survey.

Project description:BackgroundThe association of long-term air pollution and lung function has not been studied across adult European multi-national populations before. The aim of this study was to determine the association between long-term urban background air pollution and lung function levels, as well as change in lung function among European adults.MethodsForced expiratory volume in 1 s (FEV1), forced vital capacity (FVC) and the ratio thereof (FEV1/FVC) were assessed at baseline and after 9 years of follow-up in adults from 21 European centres (followed-up sample 5610). Fine particles (PM(2.5)) were measured in 2000/2001 using central monitors.ResultsDespite sufficient statistical power no significant associations were found between city-specific annual mean PM(2.5) and average lung function levels. The findings also do not support an effect on change in lung function, albeit statistical power was insufficient to significantly detect such an association.ConclusionsThe inability to refuse the null hypothesis may reflect (i) no effect of urban air pollution on lung function or (ii) inherent biases due to the study design. Examples of the latter are lack of individual-level air quality assignment, not quantified within-city contrasts in traffic-related pollution, or the heterogeneity of the studied populations and their urban environments. Future studies on long-term effects of air pollution on lung function could increase statistical power and reduce potential misclassification and confounding by characterizing exposure on the level of individuals, capturing contrasts due to local sources, in particular traffic.

Linking environmental effects to health impacts: a computer modelling approach for air pollution.

Project description:UnlabelledSTUDY OBJECTIVE AND SETTING: To develop a computer model, using a geographical information system (GIS), to quantify potential health effects of air pollution from a new energy from waste facility on the surrounding urban population.DesignHealth impacts were included where evidence of causality is sufficiently convincing. The evidence for no threshold means that annual average increases in concentration can be used to model changes in outcome. The study combined the "contours" of additional pollutant concentrations for the new source generated by a dispersion model with a population database within a GIS, which is set up to calculate the product of the concentration increase with numbers of people exposed within each enumeration district exposure response coefficients, and the background rates of mortality and hospital admissions for several causes.Main resultsThe magnitude of health effects might result from the increased PM10 exposure is small-about 0.03 deaths each year in a population of 3 500 000, with 0.04 extra hospital admissions for respiratory disease. Long term exposure might bring forward 1.8-7.8 deaths in 30 years.ConclusionsThis computer model is a feasible approach to estimating impacts on human health from environmental effects but sensitivity analyses are recommended.Relevance to clinical or professional practiceThe availability of GIS and dispersion models on personal computers enables quantification of health effects resulting from the additional air pollution new industrial development might cause. This approach could also be used in environmental impact assessment. Care must be taken in presenting results to emphasise methodological limitations and uncertainties in the numbers.

Meta-analysis of adverse health effects due to air pollution in Chinese populations.

Project description:BackgroundPooled estimates of air pollution health effects are important drivers of environmental risk communications and political willingness. In China, there is a lack of review studies to provide such estimates for health impact assessments.MethodsWe systematically searched the MEDLINE database using keywords of 80 major Chinese cities in Mainland China, Hong Kong and Taiwan on 30 June 2012, yielding 350 abstracts with 48 non-duplicated reports either in English or Chinese after screening. We pooled the relative risks (RR) per 10 ?g/m3 of particulate matter (PM10), nitrogen dioxide (NO2), sulphur dioxide (SO2) and ozone (O3).ResultsFor short-term effects, the pooled RR (p<0.05) ranges were: 1.0031 (PM10) to 1.0140 (NO2) for all-cause mortality, 1.0034 (cardiopulmonary, PM10) to 1.0235 (influenza and pneumonia, SO2) for 9 specific-causes mortality, 1.0021 (cardiovascular, PM10) to 1.0162 (asthma, O3) for 5 specific-causes hospital admissions. For birth outcomes, the RR (p<0.05) ranged from 1.0051 (stillbirth, O3) to 1.1189 (preterm-birth, SO2) and for long-term effect on mortality from 1.0150 (respiratory, SO2) to 1.0297 (respiratory, NO2). Publication bias was absent (Egger test: p=0.326 to 0.624). Annual PM10 and NO2 concentrations were inversely associated with RR of mortality (p=0.017-0.028).ConclusionsEvidence on short-term effects of air pollution is consistent and sufficient for health impact assessment but that on long-term effects is still insufficient.

Estimating the health benefit of reducing indoor air pollution in a randomized environmental intervention.

Project description:Recent intervention studies targeted at reducing indoor air pollution have demonstrated both the ability to improve respiratory health outcomes and to reduce particulate matter (PM) levels in the home. However, these studies generally do not address whether it is the reduction of PM levels specifically that improves respiratory health. In this paper we apply the method of principal stratification to data from a randomized air cleaner intervention designed to reduce indoor PM in homes of children with asthma. We estimate the health benefit of the intervention amongst study subjects who would experience a substantial reduction in PM in response to the intervention. For those subjects we find an increase in symptom-free days that is almost three times as large as the overall intention-to-treat effect. We also explore the presence of treatment effects amongst those subjects whose PM levels would not respond to the air cleaner. This analysis demonstrates the usefulness of principal stratification for environmental intervention trials and its potential for much broader application in this area.

Next-Generation Community Air Quality Sensors for Identifying Air Pollution Episodes.

Project description:Conventional regulatory air quality monitoring sites tend to be sparsely located. The availability of lower-cost air pollution sensors, however, allows for their use in spatially dense community monitoring networks, which can be operated by various stakeholders, including concerned residents, organizations, academics, or government agencies. Networks of many community monitors have the potential to fill the spatial gaps between existing government-operated monitoring sites. One potential benefit of finer scale monitoring might be the ability to discern elevated air pollution episodes in locations that have not been identified by government-operated monitoring sites, which might improve public health warnings for populations sensitive to high levels of air pollution. In the Imperial Air study, a large network of low-cost particle monitors was deployed in the Imperial Valley in Southeastern California. Data from the new monitors is validated against regulatory air monitoring. Neighborhood-level air pollution episodes, which are defined as periods in which the PM2.5 (airborne particles with sizes less than 2.5 ?m in diameter) hourly average concentration is equal to or greater than 35 ?g m-3, are identified and corroborate with other sites in the network and against the small number of government monitors in the region. During the period from October 2016 to February 2017, a total of 116 episodes were identified among six government monitors in the study region; however, more than 10 times as many episodes are identified among the 38 community air monitors. Of the 1426 episodes identified by the community sensors, 723 (51%) were not observed by the government monitors. These findings suggest that the dense network of community air monitors could be useful for addressing current limitations in the spatial coverage of government air monitoring to provide real-time warnings of high pollution episodes to vulnerable populations.

The Environmental Benefits Mapping and Analysis Program - Community Edition (BenMAP-CE): A tool to estimate the health and economic benefits of reducing air pollution.

Project description:A number of software tools exist to estimate the health and economic impacts associated with air quality changes. Over the past 15 years, the U.S. Environmental Protection Agency and its partners invested substantial time and resources in developing the Environmental Benefits Mapping and Analysis Program - Community Edition (BenMAP-CE). BenMAP-CE is a publicly available, PC-based open source software program that can be configured to conduct health impact assessments to inform air quality policies anywhere in the world. The developers coded the platform in C# and made the source code available in GitHub, with the goal of building a collaborative relationship with programmers with expertise in other environmental modeling programs. The team recently improved the BenMAP-CE user experience and incorporated new features, while also building a cadre of analysts and BenMAP-CE training instructors in Latin America and Southeast Asia.

Air pollution and population health: a global challenge.

Project description:"Air pollution and population health" is one of the most important environmental and public health issues. Economic development, urbanization, energy consumption, transportation/motorization, and rapid population growth are major driving forces of air pollution in large cities, especially in megacities. Air pollution levels in developed countries have been decreasing dramatically in recent decades. However, in developing countries and in countries in transition, air pollution levels are still at relatively high levels, though the levels have been gradually decreasing or have remained stable during rapid economic development. In recent years, several hundred epidemiological studies have emerged showing adverse health effects associated with short-term and long-term exposure to air pollutants. Time-series studies conducted in Asian cities also showed similar health effects on mortality associated with exposure to particulate matter (PM), sulfur dioxide (SO(2)), nitrogen dioxide (NO(2)) and ozone (O(3)) to those explored in Europe and North America. The World Health Organization (WHO) published the "WHO Air Quality Guidelines (AQGs), Global Update" in 2006. These updated AQGs provide much stricter guidelines for PM, NO(2), SO(2) and O(3). Considering that current air pollution levels are much higher than the WHO-recommended AQGs, interim targets for these four air pollutants are also recommended for member states, especially for developing countries in setting their country-specific air quality standards. In conclusion, ambient air pollution is a health hazard. It is more important in Asian developing countries within the context of pollution level and population density. Improving air quality has substantial, measurable and important public health benefits.

Health Effects of Air Pollution in China.

Project description:Background Rapid economic and social development in China has resulted in severe air pollution and consequent adverse impacts on society. The health effects of air pollution have been widely studied. Methods Using information from the China Health and Retirement Longitudinal Study (CHARLS) database, we established a hierarchical linear model combining pollution and socioeconomic and psychosocial variables to examine the effects of air pollution on public health in China. Local air pollution was characterized in multiple dimensions. Results The relationship of health to its determinants greatly differed between Eastern and Central/Western China. Higher education, higher income level, better life satisfaction, and long-term marriage were significantly associated with better health status among Chinese. In addition, regional healthcare resources were positively associated with the health of residents. As indicated by the hierarchical model with health as dependent variable, in Central/Western China, longest duration of good air quality in spring/summer was positively associated with health (estimated coefficient = 0.067, standard error = 0.026), while the mean Air Quality Index (AQI) in autumn/winter was inversely associated with health (estimated coefficient = -0.082, standard error = 0.031). Good air quality in the current study is defined as daily average AQI less than 35. Conclusions Duration (in days) of acceptable air quality was particularly important for improving public health. Future policies should target increased duration of good air quality while managing air pollution by controlling or decreasing severe air pollution.
