Toxic Stress in a Mid-Sized Urban Community: An Initial Needs Assessment of Families with Children in Maywood, IL.

ABSTRACT:

Background

Loyola Medical Center is located in Maywood, IL, a community that faces high rates of poverty, violence, and barriers to healthcare. These factors can contribute to toxic stress, which has been shown to negatively impact children's health.

Objectives

The goal of this project was to partner with community organizations to obtain a baseline needs assessment from families in Maywood regarding sources of toxic stress and to identify interventions of interest.

Methods

In total, 75 anonymous surveys were collected from the Loyola Outpatient Center Pediatric Clinic and a Maywood community center. Survey responses were statistically analyzed in order to determine toxic stressors most commonly impacting families in Maywood as well as interventions of most interest to the community.

Results

There were 78 respondents for a response rate of approximately 71%. The most common stressors were smoking in the home (33.3%), food insecurity (29.5%), and exposure to violence (26.9%). In this sample, Black respondents were 11.5 times more likely than non-Black respondents to report that their child was exposed to violence in the community - even after controlling for concern about their child's behavior which served as a surrogate measure of the child's exposure to toxic stress (P = 0.001). Further, those living with food insecurity were 7.40 times more likely to report that access to food and transportation vouchers were important (P < .001). For every 1-point increase in the total toxic stress score, respondents were 1.35 times more likely to report that increasing access to mental health resources was important to them, though this was not significant (P = .10).

Conclusion

The data demonstrate that toxic stressors, health risks and unmet social needs are prevalent in the Maywood community, which puts local children at risk for future adverse health outcomes. With this information, pediatricians at Loyola Medical Center can work with community organizations to allocate resources to address toxic stressors in Maywood.

Project description:BackgroundCommunity Health Navigators (CHNs) are members of a patient's care team that aim to reduce barriers in accessing healthcare. CHNs have been described in various healthcare settings, including chronic disease management. The ENhancing COMmunity health through Patient navigation, Advocacy, and Social Support (ENCOMPASS) program of research employs CHNs, who are trained to improve access to care and community resources for patients with multiple chronic diseases. With complex and demanding roles, it is essential that CHNs communicate with each other to maintain knowledge exchange and best practices. A Community of Practice (CoP) is a model of situated learning that promotes communication, dedication, and collaboration that can facilitate this communication. The objective of this study was to engage with CHNs to determine how a CoP could be implemented to promote consistency in practices and knowledge for CHNs across primary care sites.MethodsA needs assessment for a CHN CoP was conducted using sequential steps of inquiry. A preliminary focused literature review (FLR) was done to examine the ways in which other healthcare CoPs have been implemented. Results from the FLR guided the creation of an exploratory survey and group interview with key informants to understand best approaches for CoP creation. Political, economic, social, and technological (PEST) and strengths, weaknesses, opportunities, and threats (SWOT) analyses synthesized results in a comprehensive manner for strategic recommendations.ResultsThe FLR identified different approaches and components of healthcare CoPs and guided analyses of mitigatable risk factors and leverageable assets for the intervention. The survey and group interview revealed an informal and effective CoP amongst current CHNs, with preferred methods including coffee meetings, group trainings, and seminars. A well-maintained web platform with features such as an encrypted discussion forum, community resource listing, calendar of events, and semi-annual CHN conferences were suggested methods for creating an inter-regional, formal CoP.ConclusionThe study findings recognise the presence of an informal CoP within the studied CHN cohort. Implementation of a formal CoP should complement current CoP approaches and aid in facilitating expansion to other primary care centres utilizing digital communication methods, such as a comprehensive web platform and online forum.

Project description:Vegetable gardens are increasingly common in urban areas and can provide numerous societal benefits; however, contamination with potential toxic elements (PTEs) due to urbanization and industrialization is cause for concern. The present study aimed to assess the source of contamination and pollution levels in urban garden soils, as well as the health risks for adults and children consuming vegetables grown in such environments. Various types of vegetable samples and their corresponding soils from 26 community gardens were collected throughout Chengdu City in southwestern China. The results showed that leafy vegetables, particularly lettuce leaves and Chinese cabbage, had relatively higher levels of Cd (0.04 mg/kg FW) and Pb (0.05 mg/kg FW), while higher levels of As (0.07 mg/kg FW), Cr (0.07 mg/kg FW), and Hg (0.003 mg/kg FW) were found in amaranths, tomatoes, and Houttuynia cordatas, respectively. The pollution indices revealed that the vegetable purplish soils were relatively more polluted by Cd and As, and the concentrations of these metals in vegetables were correlated with their concentrations in the soils. Principal component analysis grouped the PTEs in two dimensions that cumulatively explained 62.3% of their variation, and hierarchical clustering identified two distinct clusters indicating that Cr originated from a unique source. The health risk assessment revealed that exposure to As and Cd induced the greatest non-carcinogenic risk, whereas Cr was most likely to cause cancer risks. Furthermore, contaminated vegetable consumption was riskier for children than adults. The critical factors contributing to PTE contamination in vegetable gardens were determined to be vegetable species, total soil element content, soil pH, and soil organic matter content. Overall, Cr and As pollution present the greatest concern, and community health care services must enact more effective regulatory and preventative measures for urban gardens in terms of PTEs.
