Quantification of Workload and Wellness Measures in a Women's Collegiate Volleyball Season.

ABSTRACT: The purpose of this paper was to quantify internal and external loads completed by collegiate volleyball athletes during a competitive season. Eleven players were sampled (using accelerometers and subjective wellness surveys) during the practice (n = 55) and game (n = 30) sessions over the 2019 season. Longitudinal data were evaluated for trends across the preseason, non-conference play, and conference play periods. Data were also analyzed with respect to positional groups. Longitudinal analysis of accelerometer data showed higher workload demand during practices than games. Positional group differences were most when evaluating jump count and height. Setters accrued over twice as many jumps in a practice than during a game and had similar overall jump counts in practice to attacking positions. Average team wellness values varied with time in the season, especially during times of congested travel. This is the first study to look at both game and practice workload and wellness measures in collegiate women's volleyball. The results suggest athlete monitoring can be used to understand the demands of volleyball and used in the future to enhance practice and recovery day design to optimize athlete well-being.

Project description:BackgroundThere is a relative paucity of research examining the sport-specific injury epidemiology of high school and collegiate volleyball athletes. Moreover, differences in study methodology frequently limit our ability to compare and contrast injury data collected from selected populations.HypothesisThere are differences between the injury patterns characteristic of high school and collegiate female volleyball athletes.Study designRetrospective clinical review.Level of evidenceLevel 3.MethodsWe statistically analyzed injury incidence and outcome data collected over a 4-year interval (2005-2006 to 2008-2009) by 2 similar injury surveillance systems, the National Collegiate Athletic Association's Injury Surveillance System (NCAA ISS) and the High School Reporting Injuries Online (HS RIO). We compared diagnoses, anatomic distribution of injuries, mechanisms of injury, and time lost from training or competition between high school and collegiate volleyball athletes.ResultsThe overall volleyball-related injury rate was significantly greater among collegiate athletes than among high school athletes during both competition (injury rate ratio, 2.9; 95% CI, 2.5-3.4) and practice (injury rate ratio, 3.5; 95% CI, 3.1-3.9). Collegiate athletes had a higher rate of ankle sprain, knee injury, and shoulder injury. Concussions represented a relatively high percentage of injuries in both populations (5.0% of total NCAA ISS injuries vs 4.8% of total HS RIO injuries, respectively).ConclusionThe data suggest that although similar, there were distinct differences between the injury patterns of the 2 populations. Compared with high school volleyball players, collegiate athletes have a higher rate of acute time loss injury as well as overuse time loss injury (particularly patellar tendinosis). Concussions represented a significant and worrisome component of the injury pattern for both study populations.Clinical relevanceThe injury data suggest that important differences exist in the injury patterns of female high school compared with collegiate volleyball athletes. Consideration of the specific injury patterns may be helpful in future prevention efforts.

Project description:The purpose of this study was to evaluate the effects of a competitive soccer season on biomarkers and performance metrics in order to determine the correlation between changes in biomarkers, body composition, and performance outcomes. Twenty-one Division 1 female collegiate soccer players were monitored throughout the 16-week season. Player workload was measured using heart rate and Global Position Satellite systems at all practices and games. Performance testing, including vertical jump, VO2max, and 3-repetition maximum testing for bench press, squat and deadlift, occurred prior to pre-season and immediately post-season. Blood draws occurred prior to preseason and every 4-weeks thereafter, following a game. Body composition was assessed prior to the start of season (week 0) and weeks 6, 10, 14, and 17 (post-season). Delta area under the curve was calculated for biomarkers and body composition variables to account for seasonal changes adjusted for baseline. Pearson-product moment correlations were used to assess relationships with significance set at p < 0.05. Trends were considered p ≤ 0.10. No significant time main effects were seen for anabolic biomarkers (p > 0.05). Significant time effects were seen for catabolic biomarkers throughout the season (p = 0.001). No changes in body weight, VO2max, vertical jump, and deadlift occurred. Squat and bench press improved (p = 0.01 and p = 0.02, respectively) with a decline in percent body fat (p = 0.03) and a trend for increased fat free mass (p = 0.09). Additionally, total cortisol (TCORT) negatively correlated with fat free mass (r = -0.48; p = 0.03) and positively correlated with VO2max (r = 0.47; p = 0.04). A trend was shown for a positive correlation between both TCORT and free cortisol (FCORT) and percent body fat (r = 0.39; r = 0.40; p = 0.08, respectively). IGF-1 and growth hormone positively correlated to deadlift (r = 0.57; P = 0.02 and r = 0.59; p = 0.03), whereas creatine kinase showed a trend for a positive correlation with deadlift (r = 0.49; p = 0.06). IL-6 negatively correlated with bench press (r = -0.53; p = 0.03). These findings support a relationship between biomarkers, performance outcomes, and body composition. Biomarker monitoring may be useful to detect individual player's physiological response to an athletic season and may help provide insights in efforts to optimize performance outcomes.

Project description:BackgroundBaseline testing of objective lower limb function may help clinicians make more informed return-to-sport (RTS) decisions in the event of an anterior cruciate ligament (ACL) injury. However, as these tests are based on physical performance, it is possible that they improve during the season as athletes get stronger and fitter. Hence, it may be difficult to ascertain the patient's preinjury status and have an accurate reference for comparison when determining readiness for RTS. The purpose of this study was to examine changes in common ACL RTS tests during a college soccer season to determine the most appropriate time to perform baseline testing.HypothesisHop test performance will improve across the season.Study designDescriptive laboratory; prospective cohort.Level of evidenceLevel 4.MethodsA total of 31 women's soccer players from 1 NCAA Division I university agreed to participate this study. Participants performed 4 single-leg hop tests and the 505-agility test to measure cutting speed on 3 occasions: preseason (PRE), midseason (MID), and end-of-season (END). Performance on each test was compared across days to determine whether performance increased during the season. As a secondary analysis, limb symmetry index (LSI) was also compared across the season.ResultsA total of 23 participants (age, 19.7 ± 1.3 years; height, 1.69 ± 0.07 m; weight 60.9 ± 7.2 kg) completed all 3 testing sessions during the season. Performance during PRE was better than MID and END for all hop tests (all P < 0.01). LSI did not change during the season (P value range, 0.18-0.79).ConclusionPerformance on the hop tests was best during preseason and declined thereafter, which may be reflective of cumulative fatigue.Clinical relevanceBaseline testing of RTS tests performed during preseason may provide an accurate representation of an athlete's best abilities over the course of a collegiate soccer season. Preseason testing would also enable athletic trainers to acquire baseline data for all athletes before they are injured.

Project description:ImportanceThe ideal hospitalist workload and optimal way to measure it are not well understood.ObjectiveTo obtain expert consensus on the salient measures of hospitalist workload.Design, setting, and participantsThis qualitative study used a 3-round Delphi technique between April 5 and July 13, 2022, involving national experts within and external to the field. Experts included hospitalist clinicians, leaders, and administrators, as well as researchers with expertise in human factors engineering and cognitive load theory.Main outcomes and measuresThree rounds of surveys were conducted, during which participants provided input on the salient measures of hospitalist workload across various domains. In the first round, free-text data collected from the surveys were analyzed using a directed qualitative content approach. In the second and third rounds, participants rated each measure's relevance on a Likert scale, and consensus was evaluated using the IQR. Percentage agreement was also calculated.ResultsSeventeen individuals from 14 organizations, encompassing clinicians, leaders, administrators, and researchers, participated in 3 rounds of surveys. In round 1, participants provided 135 unique qualitative comments across 10 domains, with 192 unique measures identified. Of the 192 measures presented in the second round, 6 (3%) were considered highly relevant, and 25 (13%) were considered moderately relevant. In round 3, 161 measures not meeting consensus were evaluated, with 25 (16%) considered highly relevant and 95 (59%) considered moderately relevant. Examples of measures considered highly relevant included a patient complexity score and outcome measures such as savings from hospital days avoided and clinician turnover.Conclusions and relevanceIn this qualitative study measuring hospitalist workload, multiple measures, including those quantifying work demands and the association of those demands with outcomes, were considered relevant for measuring and understanding workloads. The findings suggest that relying on traditional measures, such as productivity-related measures and financial measures, may offer an incomplete understanding of workloads and their association with key outcomes. By embracing a broader range of measures, organizations may be able to better capture the complexity and nuances of hospitalist work demands and their outcomes on clinicians, patients, and organizations.

Project description:This prospective controlled observational cohort study assessed the performance of a novel panel of serum microRNA (miRNA) biomarkers on indicators of concussion, subconcussive impacts, and neurocognitive function in collegiate football players over the playing season. Male collegiate student football athletes participating in a Division I Football Bowl Subdivision of the National Collegiate Athletic Association (NCAA) were enrolled. There were a total of 53 participants included in the study, 30 non-athlete control subjects and 23 male collegiate student football athletes. Neurocognitive assessments and blood samples were taken within the week before the athletic season began and within the week after the last game of the season and measured for a panel of pre-selected miRNA biomarkers. All the athletes had elevated levels of circulating miRNAs at the beginning of the season compared with control subjects (p < 0.001). Athletes with the lowest standard assessment of concussion (SAC) scores at the beginning of the season had the highest levels of miRNAs. The area under the curve (AUC) for predicting pre-season SAC scores were miR-195 (0.90), miR-20a (0.89), miR-151-5p (0.86), miR-505* (0.85), miR-9-3p (0.77), and miR-362-3p (0.76). In athletes with declining neurocognitive function over the season, concentrations of miRNAs increased over same period. There were significant negative correlations with miR-505* (p = 0.011), miR-30d (p = 0.007), miR-92 (p = 0.033), and (p = 0.008). The miRNAs correlating with balance problems were miR-505* (p = 0.007), miR-30d (p = 0.028), and miR-151-5p (p = 0.023). Those correlating with poor reaction times were miR-20a (0.043), miR-505* (p = 0.049), miR-30d (p = 0.031), miR-92 (p = 0.015), and miR-151-5p (p = 0.044). Select miRNAs were associated with baseline concussion assessments at the beginning of the season and with neurocognitive changes from pre to post-season in collegiate football players. Should these findings be replicated in a larger cohort of athletes, these markers could potentially serve as measures of neurocognitive status in athletes at risk for concussion and subconcussive injuries.
