Mapping LGBTQ+ Youth Resource Density Across Four High HIV Prevalence Corridors in the US.

ABSTRACT: Introduction
Studies using geospatial data to understand LGBTQ+-friendly sexual health and wellness resource availability have often focused on services catered to adults. While HIV rates have increased in adolescents in recent years, few studies have explored disparities in resource access for adolescent gay and bisexual men (AGBMSM).Methods
We used geospatial data of resources (collected and verified 2017-2018) from the iReach app to understand disparities in resource access for AGBMSM within and between 4 high HIV prevalence corridors in the US.Results
AGBMSM in non-metro areas had access to fewer resources and some rural counties had no LGBTQ+ -friendly resources. Corridors comprising states with legacies of punitive laws targeting sexual and gender minorities demonstrate stark geographic disparities across the US.Conclusions
Policy-makers must understand the granularity of disparities within regions. Online resources may be able to surmount LGBTQ+ resource deserts. However, physical access to LGBTQ+ -friendly services must be improved as a fundamental strategy for reducing HIV among AGBMSM.Supplementary information

Project description:BackgroundThere are hardly any data about the incidence, risk factors and outcomes of ICU-associated A.baumannii colonisation/infection in HIV-infected and uninfected persons from resource-poor settings like Africa.MethodsWe reviewed the case records of patients with A.baumannii colonisation/infection admitted into the adult respiratory and surgical ICUs in Cape Town, South Africa, from January 1 to December 31 2008. In contrast to colonisation, infection was defined as isolation of A.baumannii from any biological site in conjunction with a compatible clinical picture warranting treatment with antibiotics effective against A.baumannii.ResultsThe incidence of A.baumannii colonisation/infection in 268 patients was 15 per 100 person-years, with an in-ICU mortality of 26.5 per 100 person-years. The average length of stay in ICU was 15 days (range 1-150). A.baumannii was most commonly isolated from the respiratory tract followed by the bloodstream. Independent predictors of mortality included older age (p?=?0.02), low CD4 count if HIV-infected (p?=?0.038), surgical intervention (p?=?0.047), co-morbid Gram-negative sepsis (p?=?0.01), high APACHE-II score (p?=?0.001), multi-organ dysfunction syndrome (p?=?0.012), and a positive blood culture for A.baumannii (p?=?0.017). Of 21 A.baumannii colonised/infected HIV-positive persons those with clinical AIDS (CD4<200 cells/mm(3)) had significantly higher in-ICU mortality and were more likely to have a positive blood culture. Conclusion In this resource-poor setting A.baumannii infection in critically ill patients is common and associated with high mortality. HIV co-infected patients with advanced immunosuppression are at higher risk of death.

Project description:IntroductionExcessive alcohol consumption leads to unfavourable outcomes in people living with HIV (PLHIV), including reduced adherence to antiretroviral therapy (ART) and engagement into care. However, there is limited information on alcohol consumption patterns among PLHIV in sub-Saharan Africa.MethodsUsing a cross-sectional approach, the Alcohol Use Disorders Identification Test (AUDIT-C) was administered to PLHIV attending HIV clinics in Côte d'Ivoire, Togo, Senegal and Zambia (2013 to 2015). Hazardous drinking was defined as an AUDIT-C score ≥4 for men or ≥3 for women, and binge drinking as ≥6 drinks at least once per month. The prevalence of binge drinking was compared to estimates from the general population using data from the World Health Organization. Factors associated with binge drinking among persons declaring any alcohol use in the past year were assessed using a logistic regression model to estimate odds ratio (OR) and their corresponding 95% confidence intervals (CI).ResultsAmong 1824 PLHIV (median age 39 years, 62.8% female), the prevalence of hazardous alcohol use ranged from 0.9% in Senegal to 38.4% in Zambia. The prevalence of binge drinking ranged from 14.3% among drinkers in Senegal to 81.8% in Zambia, with higher estimates among PLHIV than in the general population. Male sex (OR 2.4, 95% CI 1.6 to 3.7), tobacco use (OR 1.7, 95% CI 1.0 to 2.9) and living in Zambia were associated with binge drinking.ConclusionsAlcohol consumption patterns varied widely across settings and binge drinking was more frequent in HIV-positive individuals compared to the general population. Interventions to reduce excessive alcohol use are urgently needed to optimize adherence in the era of universal ART.

Project description:This meta-narrative review on mental health early intervention support for LGBTQ+ youth aimed to develop a theoretical framework to explain effective mental health support. Using the RAMESES standards for meta-narrative reviews, we identified studies from database searches and citation-tracking. Data extraction and synthesis was conducted through conceptual coding in Atlas.ti. in two stages: 1) conceptual mapping of the meta-narratives; 2) comparing the key concepts across the meta-narratives to produce a theoretical framework. In total, 2951 titles and abstracts were screened and 200 full papers reviewed. 88 studies were included in the final review. Stage 1 synthesis identified three meta-narratives - psychological, psycho-social, and social/youth work. Stage 2 synthesis resulted in a non-pathological theoretical framework for mental health support that acknowledged the intersectional aspects of LGBTQ+ youth lives, and placed youth at the centre of their own mental health care. The study of LGBTQ+ youth mental health has largely occurred independently across a range of disciplines such as psychology, sociology, public health, social work and youth studies. The interdisciplinary theoretical framework produced indicates that effective early intervention mental health support for LGBTQ+ youth must prioritise addressing normative environments that marginalises youth, LGBTQ+ identities and mental health problems.

Project description:A high-density consensus map is a powerful tool for gene mapping, cloning and molecular marker-assisted selection in wheat breeding. The objective of this study was to construct a high-density, single nucleotide polymorphism (SNP)-based consensus map of common wheat (Triticum aestivum L.) by integrating genetic maps from four recombinant inbred line populations. The populations were each genotyped using the wheat 90K Infinium iSelect SNP assay. A total of 29,692 SNP markers were mapped on 21 linkage groups corresponding to 21 hexaploid wheat chromosomes, covering 2,906.86 cM, with an overall marker density of 10.21 markers/cM. Compared with the previous maps based on the wheat 90K SNP chip detected 22,736 (76.6%) of the SNPs with consistent chromosomal locations, whereas 1,974 (6.7%) showed different chromosomal locations, and 4,982 (16.8%) were newly mapped. Alignment of the present consensus map and the wheat expressed sequence tags (ESTs) Chromosome Bin Map enabled assignment of 1,221 SNP markers to specific chromosome bins and 819 ESTs were integrated into the consensus map. The marker orders of the consensus map were validated based on physical positions on the wheat genome with Spearman rank correlation coefficients ranging from 0.69 (4D) to 0.97 (1A, 4B, 5B, and 6A), and were also confirmed by comparison with genetic position on the previously 40K SNP consensus map with Spearman rank correlation coefficients ranging from 0.84 (6D) to 0.99 (6A). Chromosomal rearrangements reported previously were confirmed in the present consensus map and new putative rearrangements were identified. In addition, an integrated consensus map was developed through the combination of five published maps with ours, containing 52,607 molecular markers. The consensus map described here provided a high-density SNP marker map and a reliable order of SNPs, representing a step forward in mapping and validation of chromosomal locations of SNPs on the wheat 90K array. Moreover, it can be used as a reference for quantitative trait loci (QTL) mapping to facilitate exploitation of genes and QTL in wheat breeding.

Project description:Wildlife corridors aim to promote species' persistence by connecting habitat patches across fragmented landscapes. Their implementation is limited by patterns of land ownership and complicated by differences in the jurisdictional and regulatory authorities under which lands are managed. Terrestrial corridor conservation requires coordination across jurisdictions and sectors subject to site-specific overlapping sources of legal authority. Mapping spatial patterns of legal authority concurrent with habitat condition can illustrate opportunities to build or leverage capacity for connectivity conservation. Streamside areas provide pragmatic opportunities to leverage existing policy mechanisms for riverine and terrestrial habitat connectivity across boundaries. Conservation planners and practitioners can make use of these opportunities by harmonizing actions for multiple conservation outcomes. We formulated an integrative, data-driven method for mapping multiple sources of legal authority weighted by capacity for coordinating terrestrial habitat conservation along streams. We generated a map of capacity to coordinate streamside corridor protections across a wildlife habitat gap to demonstrate this approach. We combined values representing coordination capacity and naturalness to generate an integrated legal-ecological resistance map for connectivity modeling. We then computed least-cost corridors across the integrated map, masking the terrestrial landscape to focus on streamside areas. Streamside least-cost corridors in the integrated, local-scale model diverged (?25 km) from national-scale least-cost corridors based on naturalness. Spatial categories comparing legal- and naturalness-based resistance values by stream reach highlighted potential locations for building or leveraging existing capacity through spatial coordination of policy mechanisms or restoration actions. Agencies or nongovernmental organizations intending to restore or maintain habitat connectivity across fragmented landscapes can use this approach to inform spatial prioritization and build coordination capacity. Article impact statement: Combined mapping of legal authority and habitat condition reveals capacity to coordinate actions along streams for clean water and wildlife.

Project description:ObjectivesModelling the potential impact of screening for chlamydia and gonorrhoea in youth and other populations in a resource-limited setting.MethodsWe extended an agent-based model of heterosexual chlamydia and gonorrhoea transmission in South Africa to investigate the impact of screening strategies in key populations including youth, patients in HIV care, pregnant women and female sex workers (FSWs). Additionally, we compared the modelled impact of a standardised screening programme to results obtained from other published mathematical models of chlamydia screening.ResultsAll strategies resulted in reductions in general and targeted population chlamydia and gonorrhoea transmission. Opportunistic screening of patients in youth and HIV care was shown to be the most effective, and FSW screening was shown to be the most efficient strategy. Differences between models could be attributed to differences in the modelled heterogeneity in sexual behaviour as well as differences in assumptions about immunity following chlamydia recovery.ConclusionsTaking modelling assumptions into account, opportunistic chlamydia and gonorrhoea screening of youth and those in HIV care represents a viable intervention for reducing sexually transmitted infections in the South African population.

Project description:Understanding the link between health and place can strengthen the design of health interventions, particularly in the context of HIV prevention. Individuals who might one day participate in such interventions - including youth - may further improve the design if engaged in a meaningful way in the formative research process. Increasingly, participatory mapping methods are being used to achieve both aims. We describe the development of three innovative mapping methods for engaging youth in formative community-based research: 'dot map' focus groups, geocaching games, and satellite imagery-assisted daily activity logs. We demonstrate that these methods are feasible and acceptable in a low-resource, rural African setting. The discussion outlines the merits of each method and considers possible limitations.

Project description:Documenting Inuit and local knowledge is critical to its consideration within policy discussions around Arctic shipping; especially considering the rapid increase in ship traffic due to reductions in sea ice and climate change. We present our unique community-based research approach which incorporated youth training, participatory mapping, qualitative focus group discussions, and verification exercises to document Inuit communities’ perspectives in Arctic Canada about Low Impact Shipping Corridors. These qualitative activities provided appropriate context and understanding around community-created maps, community-identified opportunities, concerns, and recommendations, and the policy relevance and feasibility of recommendations posed. Three activity phases were employed; 1) before engaging in in-community research, 2) during in-community research, and 3) after completing in-community research. Spatial and non-spatial data were analyzed using ArcGIS® and NVivo software, respectively. These methods and observations can inform future research initiatives, particularly transdisciplinary teams, including those involving southern-based (early career) researchers, working in Inuit Nunangat.• Methods presented here ensured that scientific processes and outputs were robust and rigorous and research was conducted in a respectful, reciprocal manner.• Only through the collaborative efforts of a transdisciplinary team could scientific rigour be attained and respect be afforded.• The approach can be easily applied to document community members’ perspectives on local priorities. Project description:BackgroundHigh fidelity carbon mapping has the potential to greatly advance national resource management and to encourage international action toward climate change mitigation. However, carbon inventories based on field plots alone cannot capture the heterogeneity of carbon stocks, and thus remote sensing-assisted approaches are critically important to carbon mapping at regional to global scales. We advanced a high-resolution, national-scale carbon mapping approach applied to the Republic of Panama - one of the first UN REDD?+?partner countries.ResultsIntegrating measurements of vegetation structure collected by airborne Light Detection and Ranging (LiDAR) with field inventory plots, we report LiDAR-estimated aboveground carbon stock errors of ~10% on any 1-ha land parcel across a wide range of ecological conditions. Critically, this shows that LiDAR provides a highly reliable replacement for inventory plots in areas lacking field data, both in humid tropical forests and among drier tropical vegetation types. We then scale up a systematically aligned LiDAR sampling of Panama using satellite data on topography, rainfall, and vegetation cover to model carbon stocks at 1-ha resolution with estimated average pixel-level uncertainty of 20.5 Mg C ha-1 nationwide.ConclusionsThe national carbon map revealed strong abiotic and human controls over Panamanian carbon stocks, and the new level of detail with estimated uncertainties for every individual hectare in the country sets Panama at the forefront in high-resolution ecosystem management. With this repeatable approach, carbon resource decision-making can be made on a geospatially explicit basis, enhancing human welfare and environmental protection.

Project description:INTRODUCTION:The burden of HIV infection in southern Africa is a public health concern with an increasing number of new infections. This study sought to investigate the predictors of HIV prevalence in Mozambique through a complex samples logistic regression and spatial mapping approach using nationally representative data. METHODS:We conducted a secondary data analysis using the 2015 Mozambique Demographic and Health Survey and AIDS Indicator Survey. The analysis performed in four stages while incorporating population survey sampling weights did the following: i) created a complex sample plan file in SPSS, ii) performed the weighted estimate of HIV prevalence, iii) performed complex sample chi-square test of independence, and then iv) performed complex sample logistic regression modeling. RESULTS:Out of 11,270 participants, 1,469 (13.0%) tested positive for HIV. The prevalence of HIV infection was higher in females (15.1%) than males (10.2%). We found that urban dwellers were more likely to be HIV-positive compared to rural dwellers (AOR: 1.70; CI: 1.27, 2.27). We observed provincial variations in HIV prevalence, with Maputo Cidade (17.4%), Maputo Provincia (22.6%), Gaza (25.2%) recording higher prevalence above the national estimate. Other independent predictors of HIV infection in Mozambique included age, education level, marital status, total lifetime sexual partners, and having had an STI in the last 12 months. CONCLUSIONS:The study revealed associations between high-risk sexual behavior and HIV infection. Results from our spatial mapping approach can help health policy makers to better allocate resources for cost-effective HIV/AIDS interventions. Pre-Exposure Prophylaxis (PrEP) campaigns among high-risk groups should be pursued to lower the reservoir of HIV among high-risk groups.
