Prognostic value of different N1 lymph node zones in pN1M0 non-small cell lung cancer: a systematic review and meta-analysis.

ABSTRACT: The IASLC lymph node map grouped the lymph node stations into "zones" for prognostic analyses. In the N1 lymph nodes group, N1 nodes are divided into the Hilar/Interlobar zone (N1h) and Peripheral zone (N1p). There is no consensus on the different prognostic values of N1 lymph nodes in N1h and N1p. Therefore, we conducted a systematic review and meta-analysis to assess the survival difference between N1h and N1p in patients of pN1M0 NSCLC. Medline, the Cochrane Library, Embase, and the Web of science were systematically searched to identify relevant studies published up to April 4th, 2020. A retrospective and prospective cohort study comparing N1h versus N1p to the pN1M0 NSCLC was included. Hazard ratios (HRs) for OS were aggregated according to a fixed or random-effect model. Ten publications for 1946 patients of pN1M0 NSCLC were included for the meta-analysis.The 5-year OS was lower for patients with N1h (HR: 1.67, 95% CI 1.44-1.94; P < 0.001). The pooled 5-year OS in N1h and N1p were 40% and 56%, respectively. The patients in pN1M0 NSCLC have different survival according to different N1 lymph node zones involvement: patients with N1p metastasis have a better prognosis than those with N1h metastasis.

Project description:BackgroundThe N parameter in the tumor-node-metastasis (TNM) classification of lung cancer is categorized according to the location of nodal metastasis, while that for some other cancers are classified according to the size and number of metastatic foci. In lung cancer, the impact of size of nodal metastasis on prognosis is unclear. This analysis aims to examine whether it is possible to subdivide the pathological N (pN) factor based on the tumor diameter of lymph node metastases.MethodsWe studied 35 cases of adenocarcinoma and 26 cases of squamous cell carcinoma of the lung. The maximum diameter of lymph node metastasis was measured, and the relationship between the maximum diameter of lymph node metastases and prognosis was investigated.ResultsSquamous cell carcinoma cases had a significantly larger maximum tumor diameter of lymph node metastasis than adenocarcinoma cases (P=0.03). Based on receiver operating characteristic (ROC) curve results for the adenocarcinoma cases, we set 4 mm as a diameter cutoff value. The 5-year overall survival (OS) rate was 85.6% for patients with diameters ≤4 mm and 57.7% for those with diameters >4 mm; this difference was statistically significant (P=0.04). On univariate analysis using a Cox proportional hazards model, significant differences were observed in metastatic lymph node diameter (≤4 vs. >4 mm) (P=0.04), sex (P=0.04), and pathological T (pT) status (pT1 and 2 vs. pT3 and 4) (P=0.03). However, similar results were not obtained for patients with squamous cell carcinoma.ConclusionsThe tumor diameter of pathological N1 lymph node metastases impacts the prognosis of patients with lung adenocarcinoma. If pathological N1 can be further subdivided according to tumor diameter of lymph node metastases, a more accurate prognosis may become possible. However, pathological type must be considered for staging in lung cancer. While the pN parameter for most cancers considers tumor size of lymph node metastasis, this does not apply to lung cancer.

Project description:Background: The prognostic significance of the lymph node (LN) classification for small bowel neuroendocrine tumors (SBNETs) remains unknown. The aim of the present study was to evaluate and compare the prognostic assessment of different LN staging systems. Methods: Patients with SBNETs were identified from the Surveillance, Epidemiology, and End Results (SEER) database. The X-tile program was used to determine the cutoff value of the resected lymph nodes (RLNs), negative lymph nodes (NLNs), lymph node ratio (LNR), and the log odds of positive lymph nodes (LODDS). Survival analyses were performed using Kaplan-Meier curves with log-rank test. Logistic regression analysis was used to evaluate the differences between different periods. Univariate and multivariate Cox proportional hazards models were used to assess the prognostic value of different LN staging systems on cause-specific survival (CSS). The relative discriminative abilities of the different LN staging systems were assessed using the Akaike information criterion (AIC) and the Harrell consistency index (HCI). Result: A total of 3,680 patients were diagnosed with SBNETs between 1988 and 2014 from the SEER database. A significant difference over time (1988-1999 vs. 2000-2014) was seen in age (P <0.001), tumor differentiation (P <0.001), T stage (P <0.001), and RLN (P <0.001) subgroups. Multivariate Cox survival analysis identified that LN status stratified by the number of RLNs, NLNs, LNR, and LODDS all predicted CSS in patients with SBNETs (all P <0.05), whereas the number of positive lymph nodes (PLNs) failed (P = 0.452). When assessed using categorical variables, LODDS staging systems showed the best prognostic performance (HCI: 0.766, AIC: 7,575.154) in the whole population. Further analysis based on different RLNs after eliminating the missing data showed that when the RLNs are <12, the LODDS (HCI: 0.769, AIC: 1,088.731) maintained the best prognostic performance as well when the RLNs are ≥12 (HCI: 0.835, AIC: 825.692). Among patients with LNR scores of 0 or 1, there was a residual heterogeneity of outcomes that were better stratified and characterized by the LODDS. Conclusion: LODDS was a better predicator of survival when LN status was stratified as a categorical variable and should be considered when assessing the prognosis of patients with SBNETs to allow a more reliable means to stratify patient survival.
